Nanofibers are investigated to be superiorly applicable in different purposes such as drug delivery systems, air filters, wound dressing, water filters, and tissue engineering. Herein, polyacrylonitrile (PAN) is thermally treated for autocatalytic cyclization, to give optically active PAN-nanopolymer, which is subsequently applicable for preparation of nanofibers through solution blow spinning. Whereas, solution blow spinning is identified as a process for production of nanofibers characterized with high porosity and large surface area from a minimum amounts of polymer solution. The as-prepared nanofibers were shown with excellent photoluminescence and microbicide performance. According to rheological properties, to obtain spinnable PAN-nanopolymer, PAN (12.5-15% wt/vol, honey like solution, 678-834 mPa s), thermal treatment for 2-4 h must be performed, whereas, time prolongation resulted in PAN-nanopolymer gelling or rubbering. Size distribution of PAN-nanopolymer (12.5% wt/vol) is estimated (68.8 ± 22.2 nm), to reflect its compatibility for the production of carbon nanofibers with size distribution of 300-400 nm. Spectral mapping data for the photoluminescent emission showed that, PAN-nanopolymer were exhibited with two intense peaks at 498 nm and 545 nm, to affirm their superiority for production of fluorescent nanofibers. The microbial reduction % was estimated for carbon nanofibers prepared from PAN-nanopolymer (12.5% wt/vol) to be 61.5%, 71.4% and 81.9%, against S. aureus, E. coli and C. albicans, respectively. Even though epidural blood patch (EBP) is thought to be the definitive treatment for severe cases of postdural puncture headache (PDPH), it may be accompanied by complications like adhesion arachnoiditis, and cauda equina syndrome, especially if the injection is repeated. The sphenopalatine ganglion (SPG) block is a new minimally invasive technique for the treatment of PDPH, with variable results according to the clinical situation and deployed approach. Chitosan is produced by deacetylating the abundant natural chitin polymer. It has been employed in a variety of applications due to its unique solubility as well as its chemical and biological properties. In addition to being biodegradable and biocompatible, it also possesses a lot of reactive amino side groups that allow for chemical modification and the creation of a wide range of useful derivatives. The physical and chemical characteristics of chitosan, as well as how it is used in the food, environmental, and medical industries, have all been covered in a number of academic publications. Chitosan offers a wide range of possibilities in environmentally friendly textile processes because of its superior absorption and biological characteristics. Chitosan has the ability to give textile fibers and fabrics antibacterial, antiviral, anti-odor, and other biological functions. One of the most well-known and frequently used methods to create nanofibers is electrospinning. This technique is adaptable and effective for creating continuous nanofibers. In the field of biomaterials, new materials include nanofibers made of chitosan. Numerous medications, including antibiotics, chemotherapeutic agents, proteins, and analgesics for inflammatory pain, have been successfully loaded onto electro-spun nanofibers, according to recent investigations. Chitosan nanofibers have several exceptional qualities that make them ideal for use in important pharmaceutical applications, such as tissue engineering, drug delivery systems, wound dressing, and enzyme immobilization. The preparation of chitosan nanofibers, followed by a discussion of the biocompatibility and degradation of chitosan nanofibers, followed by a description of how to load the drug into the nanofibers, are the first issues highlighted by this review of chitosan nanofibers in drug delivery applications. The main uses of chitosan nanofibers in drug delivery systems will be discussed last.

The primary goal of this study is to prepare chitosan nanoparticles (CSNPs) by the ionic gelation method via the treatment of chitosan (0.2 wt.%) with tripolyphosphate (0.2 wt.%) ultrasonically for 45 min. FT-IR spectroscopy and TEM images were used to characterize and validate CSNP production. Cellulosic materials with different concentrations of CSNPs have better antibacterial and colouring characteristics. The treated cellulosic fabrics were analyzed by FT-IR spectroscopy, SEM, and thermogravimetric analysis. Colourimetric data measurements expressed in K/S values were used to evaluate the impact of CSNPs on the dyeing affinity of cellulosic materials. In addition, antibacterial activity against bacteria and fungi was tested on the treated cellulosic fabrics. According to the K/S values, cellulosic textiles treated with CSNPs (0.3 wt.%) had a better affinity for acid dyeing. These textiles also offer better antibacterial properties and are more resistant to washing, light, and rubbing. A cytotoxicity study found that CSNPs give cellulosic materials antibacterial and acid dyeing properties, which is good for the environment.

Background: Non-steroidal anti-inflammatory drugs are often used as part of multimodal analgesia to control postoperative pain. This randomized, controlled, double-blinded, non-inferiority study aimed to compare the postoperative analgesic effects of intravenous ibuprofen versus ketorolac in children undergoing open unilateral lower abdominal surgery. The authors hypothesized that postoperative analgesia produced by intravenous ibuprofen would be non-inferior to that of intravenous ketorolac., Methods: Sixty-six children aged 2 to 8 years who were scheduled to undergo unilateral lower abdominal surgery, were recruited. Patients in the ibuprofen group received 10mg/kg/6h intravenous ibuprofen. Patients in the ketorolac group were given 0.5mg/kg/6h intravenous ketorolac. The primary outcome measure was 24-h postoperative morphine consumption. The secondary outcome measures were postoperative pain score, the incidence of early postoperative fever and the incidence of ibuprofen and ketorolac adverse effects including pain during drug infusion, vomiting, epigastric pain and allergic reaction., Results: Fifty-nine patients completed the study (30 ibuprofen, 29 ketorolac). There was no significant difference (P=0.305) in the mean (SD) 24-h postoperative morphine consumption (μ/kg) between intravenous ibuprofen, 16.00 (5.31), and ketorolac, 14.65 (4.61). The reported pain scores were similar in both groups. The incidence of postoperative fever was significantly lower (p=0.039) in the ibuprofen group (3%) than the ketorolac group (20%). Facile and green sol-gel method was used to synthesize carbon-containing titania nanopowder, and diethanolamine (DEA) was used as the in situ carbon source. The titania gel was heat treated at temperatures ranging from 300 to 700 °C. X-ray diffraction (XRD), thermal analysis, and Raman spectroscopy reported no crystalline phase at <325 °C. Crystallization of the anatase phase with traces of brookite phases was observed at T > 325 °C, followed by a transformation to anatase/rutile in the range of 400 °C < T ≤ 650 °C. Finally, the complete phase transformation to the rutile phase occurs at temperatures of T > 650 °C. High-resolution electron microscopy (HREM) micrographs confirm the coexistence of anatase and rutile nanocrystals and amorphous carbon clusters in the composite samples. Chemical element analysis via X-ray photoelectron spectroscopy (XPS) indicated nonstoichiometry in the O/Ti ratio, the presence of (Ti 3+ ) oxidation state, and elemental carbon. Thermogravimetric (TG) measurements are the most accurate method to measure the carbon content in samples. UV-vis spectroscopy demonstrated considerable enhancement in the optical absorption properties and electronic structure of prepared samples compared to the pure anatase and rutile. This enhancement is strongly correlated with the structure and composition of prepared samples and consequently depends on the preparation method as well as conditions. Background: Surgical repair of congenital inguinal hernia results in significant postoperative discomfort and pain. The aim of the current study was to evaluate the pre-emptive analgesic efficacy of a transversalis fascia plane (TFP) block after pediatric inguinal herniorrhaphy., Methods: Forty-four patients aged 12 to 60 months who underwent unilateral inguinal herniorrhaphy were enrolled. Four patients were excluded, and the remaining were allocated to the control group and the TFP block group. In the TFP block group, 0.4 mL/kg bupivacaine 0.25% was instilled in the plane between the transversus abdominis and transversalis fascia, while in the control group 0.9% saline was used instead of bupivacaine. The collected data were the total dose of paracetamol consumed during the first 12 h postoperatively, the postoperative Face, Leg, Activity, Cry, Consolability (FLACC) pain score, time to first use of rescue analgesia, number of patients required additional postoperative analgesics, and parents' satisfaction., Results: The median paracetamol consumption was significantly lower in the TFP block group than in the control group, and FLACC pain scores were significantly lower for all study times in the TFP block group with higher parental satisfaction scores than those for the control group. The number of patients who required additional analgesics was significantly lower in the TFP block group than in the control group., Conclusions: The use of a TFP block decreases postoperative analgesic consumption and postoperative pain intensity after pediatric inguinal herniorrhaphy. Future studies with larger sample size are required to evaluate the actual complications rate of TFP block.

Context: Minimally invasive therapeutic modalities have been used to relieve refractory pain of knee osteoarthritis (OA)., Objective: The main objective of the study was to compare the adductor canal block (ACB) with combined ACB and infiltration between the popliteal artery and the posterior capsule of the knee (I-PACK) in patients suffering knee OA pain., Patients and Methods: Fifty-six patients were randomly allocated into two equal groups: Group I received ultrasound-guided ACB with 10 mL of 0.125 bupivacaine plus 40 mg methylprednisolone And Group II received ultrasound-guided ACB with 10 mL of 0.125 bupivacaine plus 40 mg methylprednisolone and I-PACK block using same volume and concentration as ACB., Results: Group II showed a statistically significant lower value of visual analog and Western Ontario and McMaster Universities scores at all follow-up intervals compared to Group I., Conclusion: Combined ACB and I-PACK block provide more effective analgesia and better functional outcome compared to the ACB alone., Competing Interests: There are no conflicts of interest., (Copyright: © 2019 Anesthesia: Essays and Researches.)

Context: Pain resulting from failed back surgery syndrome (FBSS) is generally resistant to physiotherapy and pharmacological treatment., Objective: The aim of this study is to evaluate the effect of adding calcitonin to local anesthetic and steroids during ultrasound-guided caudal epidural injection for patients suffering from FBSS., Patients and Methods: Fifty-six patients were randomly allocated into two equal groups. All patients underwent ultrasound-guided caudal epidural injection. Group A received 40 mg (1 mL) methylprednisolone +9 mL lidocaine 0.5% + 1500 iu hyaluronidase in 10 mL normal saline, whereas Group B received 40 mg (1 mL) methylprednisolone +9 mL of 0.5% lidocaine + 1500 iu hyaluronidase in 5 mL normal saline + 50 iu calcitonin in 5 mL normal saline., Results: A statistically significant decrease in visual analog scale, Oswestry Disability Index, and analgesic consumption was recorded in Group B as compared to Group A at 1, 2, and 3 months interval. Context: Some pediatric supraglottic airway devices have not been evaluated in a large perspective for blind intubation although they are validated as a conduit for fiberoptic-guided intubation., Objective: The objective of this study was to compare the success rate of blind technique with correction maneuvers versus fiberoptic-guided intubation through the air-Q laryngeal airway in pediatric patients., Patients and Methods: One hundred and twenty-six pediatric patients undergoing elective surgery under general anesthesia were randomized into two equal groups. The air-Q was placed in all patients as a conduit for tracheal intubation. Group B patients underwent blind intubation with correction maneuvers. Group F patients underwent fiberoptic-guided intubation., Results: There was no significant difference between the groups regarding the success rate of intubation or postextubation complications. Group F patients showed a significantly longer time to intubation and total time of the trial. Furthermore, Group F patients showed a significantly higher level of pre- and postintubation heart rate compared to Group B patients., Conclusion: Blind tracheal intubation in pediatric patients through the air-Q with correction maneuvers could be a good alternative for fiberoptic-guided intubation with stable hemodynamics and shorter time till intubation., Competing Interests: There are no conflicts of interest.

Context: Erector spinae plane (ESP) block is a promising technique in the field of pediatric postoperative analgesia considering its safety and simplicity., Objective: The objective of the study is to compare the efficacy of ultrasound (US)-guided ilioinguinal/iliohypogastric nerve (IIN) block and ESP block for postoperative analgesia after pediatric unilateral inguinal hernia repair., Patients and Methods: Sixty patients randomized into two equal groups. ESP group received US-guided ESP block, and IIN group received US-guided IIN block. Block in both groups was done with 0.5 mL/kg 0.125 bupivacaine + fentanyl 1 μg/mL injectate. Primary outcome is to compare both groups regarding time to first analgesic request. Secondary outcomes include evaluation of postoperative FLACC score, number of patients requiring rescue analgesic, number of rescue analgesic doses, parental satisfaction, incidence of postoperative vomiting, and motor weakness., Results: IIN group showed a significantly higher FLACC score at 4 and 6 h, significantly higher number of rescue medication doses and number of patients needed rescue analgesia. The ESP group carries a significantly higher parental satisfaction and a significantly longer time to first rescue analgesic compared to IIN group., Conclusion: The findings suggest that US guided ESP block resulted in a more effective and longer duration of postoperative analgesia following a pediatric unilateral inguinal hernia repair compared to IIN block., Competing Interests: There are no conflicts of interest.

Purpose: To compare dexamethasone (DA) to dexmedetomidine (DE) as adjuvants for bupivacaine during infraorbital nerve block (IONB) in a randomized clinical trial., Patients and Methods: One hundred patients were randomized into two equal groups. By the guidance of ultrasound, the IONB was performed bilaterally in all patients, using 1 mL of the solution on each side. In DA group, 1 mL of 0.5% bupivacaine was added to 0.1 mg.kg-1 DA and diluted to 2 mL in DE group; 1 mL of 0.5% bupivacaine was added to 0.5 μg.kg -1 DE and diluted to 2 mL. The primary outcome was time to first rescue analgesic. The secondary outcomes included face legs activity cry consolability (FLACC) score rating during the first 24 h, hemodynamic variables, the incidence of postoperative vomiting, sedation score, and parent satisfaction., Results: During the first 24 h postoperatively, the DE group showed a significantly lower FLACC score and a longer time to first analgesic request compared to DA group. Sedation and parent satisfaction scores were significantly higher while heart rate and mean blood pressure were significantly lower in DE group compared to DA group. There were no differences in other secondary outcomes., Conclusion: The use of DE as an adjuvant to bupivacaine in IONB for cleft lip repair resulted in lower pain score and more prolonged duration of analgesia compared to DA., Competing Interests: There are no conflicts of interest.

This research work focuses on the development of a one-bath functional finishing procedure for imparting durable multifunctional properties such as easy care, soft-hand, antibacterial and/or ultra violet (UV) protection to cotton/wool and viscose/wool blends using diverse finishing combinations and formulations. In this study finishing agents such as reactant resin, silicon softeners, 4-hydroxybenzophenone, triclosan, and pigment colorant were selected using magnesium chloride/citric acid as a mixed catalyst and the pad-dry microwave fixation technique. The results reveal that enhancement in the imparted functional properties are governed by type of the finished substrate as well as nature and concentration of finishing formulation components. The finished fabrics still retained high level of functionalities even after 15 consecutive laundering. Surface morphology and composition of selected samples were investigated using scan electron microscope (SEM) and energy-dispersive X-ray spectroscopy (EDX) analysis. The mode of interactions was also investigated. Practical applications for multifunctionlization of cellulose/wool blended fabrics are possible using these sorts of proper finishing formulations and unique finishing application method., (Copyright © 2014 Elsevier Ltd.
