This study aims to determine the effect of spirituality and emotional intelligence on the employees well-being with deviant behavior in the workplace as a moderator. The population in this study were nurses at Siloam Kupang Hospital, Siloam Labuan Bajo Hospital, and Siloam Bali Hospital who served in the inpatient room. The research sample was 154 respondents with a questionnaire as a data collection tool. The model in this study was analyzed using PLS (Partial Least Square). The results showed that spirituality and emotional intelligence had a positive and significant effect on employee well-being, organizational deviant behavior had no effect on employee well-being, interpersonal deviant behavior had a negative and significant impact on employee well-being, organizational deviant behavior moderated the relationship between spirituality and emotional intelligence on employee well-being, and Interpersonal deviant behavior does not moderate the relationship between spirituality and emotional intelligence on employee well-being

Introduction: Patients with triple-class-exposed relapsed/refractory multiple myeloma (TCE-RRMM) have a poor prognosis and limited treatment options. Teclistamab, a B-cell maturation antigen × CD3 bispecific antibody, was studied in patients with TCE-RRMM in the single-arm MajesTEC-1 study. To assess the relative effectiveness of teclistamab versus real-world physician’s choice of therapy (RWPC), adjusted comparisons were performed using individual patient data from MajesTEC-1 and LocoMMotion, a prospective study of patients with TCE-RRMM. Methods: An external control arm for MajesTEC-1 was created from patients in LocoMMotion (n = 248; clinical cut-off: November 2, 2021) and compared with treated patients (n = 165) from MajesTEC-1 (teclistamab 1.5 mg/kg weekly; clinical cut-off: March 16, 2022). Inverse probability weighting was used to adjust for imbalances in baseline covariates. For binary endpoints [overall response rate (ORR), very good partial response or better (≥ VGPR) rate, complete response or better (≥ CR)], relative effect of teclistamab versus RWPC was estimated with an odds ratio and relative response rate and 95% confidence interval (CI), derived from weighted logistic regression. Weighted Cox proportional hazards model was used to estimate hazard ratios (HR) and 95% CIs for time-to-event endpoints [duration of response (DOR), progression-free survival (PFS), and overall survival (OS)]. Results: After weighting, baseline characteristics were balanced across cohorts. This chapter explores the role of colonialism as it may come to translate into policy legacy from empire to colony post-independence in social policies. Specifically, we examine the role of British colonialism and its impact on the Jamaican healthcare system, casting an eye to the long-term policy conditions introduced by years of foreign rule. We pose the question: How did British colonialism impact the nature of choices and challenges confronting Jamaican policymakers as they grappled with a national response to healthcare post-independence? To answer this, we follow developments across two periods: the colonial, which concluded in 1962, and the post-colonial periods, which marked the introduction of a state-led healthcare system. Our study concludes by reflecting on the consequences of colonialism and its legacy on the healthcare policy trajectory in Jamaica.

8016 Background : Pts with TCE RRMM who have been exposed to ≥3 lines of therapy (LOT) have a poor prognosis and limited tx options. Tec is a B-cell maturation antigen × CD3 bispecific antibody being evaluated in MajesTEC-1 (NCT04557098), a single-arm, phase 1/2 study in pts with RRMM who were TCE to an immunomodulatory drug, a proteasome inhibitor, and an anti-CD38 antibody and received ≥3 LOT. Since MajesTEC-1 lacks a control arm, we assessed the comparative efficacy of tec vs tx currently used in real-world clinical practice (RWCP) by creating an external real-world control arm from LocoMMotion (NCT04035226), a prospective study of RWCP efficacy and safety outcomes in pts with TCE RRMM who received ≥3 LOT. Methods: An external control arm for MajesTEC-1 was created from pts in LocoMMotion (248 pts, clinical cutoff May 21, 2021) who met MajesTEC-1 eligibility criteria. Individual pt-level data from MajesTEC-1 were included from 150 pts treated with tec (1.5 mg/kg weekly) at a clinical cutoff of Sep 7, 2021. Inverse probability of tx weighting with average tx effect on the treated was used to adjust for imbalances in baseline covariates of prognostic significance (refractory status, International Staging System stage, time to progression on prior LOT, extramedullary disease, number of prior LOT, time since diagnosis, average duration of prior LOT, age, hemoglobin, lactate dehydrogenase, creatinine clearance, Eastern Cooperative Oncology Group performance status, gender, type of MM, and prior transplant). Comparative efficacy of tec vs RWCP was estimated for overall response rate (ORR), very good partial response (VGPR) rate, complete response or better (≥CR) rate, duration of response (DOR), progression-free survival (PFS), and overall survival (OS). For binary endpoints (ORR, VGPR rate, and ≥CR rate), relative effect of tec vs RWCP was estimated with an odds ratio, transformed into a response-rate ratio (RR) and 95% confidence interval (CI), derived from weighted logistic regression. A weighted Cox proportional hazards model was used to compute hazard ratios (HRs) and 95% CIs for time-to-event endpoints (DOR, PFS, and OS). Results: Baseline characteristics were well balanced between the 2 cohorts after reweighting the external RWCP cohort. Pts treated with tec had improved outcomes vs tx used in RWCP: ORR (RR 2.31; 95% CI 1.75–2.87; P< 0.0001), VGPR rate (RR 5.54; 95% CI 3.38–7.70; P< 0.0001), ≥CR rate (RR 91.50; 95% CI 12.66–661.43; P< 0.0001), DOR (HR 0.17; 95% CI 0.08–0.36; P< 0.0001), PFS (HR 0.47; 95% CI 0.34–0.67; P< 0.0001), and OS (HR 0.69; 95% CI 0.46–1.05; P= 0.08). Conclusions : Tec showed significantly improved efficacy over RWCP for almost all outcomes, highlighting its potential as a highly effective tx option for pts with TCE RRMM who have been exposed to ≥3 LOT.

Background : Colonization is the presence of bacteria in the intestines, skin, nose, throat or anywhere in the human body without any signs of infection but with increased risk for spreading bacteria to other patients and the emergence of new infections. Screening of colonized patients is used as part of the prevention and control of multidrug-resistant (MDR) infections caused by agents such as methicillin-resistant Staphylococcus aureus , vancomycin-resistant Enterococcus faecalis and multi-resistant Gram-negative bacteria. Material and methods: Data from 169 screened respondents hospitalized at the University Clinical Center Tuzla, between October 1, 2018, and May 1, 2019, were analyzed. Swabs were taken from nostrils, throat, axilla and groin area for all patients, and also from the anorectal area for 157 of them. Identification of MDR bacteria was done by phenotypical methods, according to the recommendations of EUCAST Clinical Breakpoint Table v.8.0, 2018. Results : Out of 169 patients, negative screening test results were found in 93 examinees (55.02%), and positive in 76 (44.97%). The largest number of patients undergoing screening was hospitalized in the Intensive Care Unit (Surgical Block), 73/169 (43.19%). The average screening time was 2.2 days or 53 hours. In 18/76 (23.68%) of patients with positive screening, prior to screening regular microbiological testing was done, and in the remaining 58/76 (76.3%) screening was performed first. Analyzing respondents with positive screening, 27 (35.53%) had negative findings during regular microbiological testing of different biological samples and for 49 (64.47%) different/same strains of MDR bacteria were isolated. Conclusion : Knowing the phenotypic profile of bacteria colonizing patients in intensive care units is a very useful tool in preventing their spread intra- and inter-hospitals.

ObjectivesIndirect comparisons via a common comparator (anchored comparisons) are commonly used in health technology assessment. However, common comparators may not be available, or the comparison may be biased due to differences in effect modifiers between the included studies. Recently proposed population adjustment methods aim to adjust for differences between study populations in the situation where individual patient data are available from at least one study, but not all studies. They can also be used when there is no common comparator or for single-arm studies (unanchored comparisons). We aim to characterise the use of population adjustment methods in technology appraisals (TAs) submitted to the United Kingdom National Institute for Health and Care Excellence (NICE).MethodsWe reviewed NICE TAs published between 01/01/2010 and 20/04/2018.ResultsPopulation adjustment methods were used in 7 percent (18/268) of TAs. Most applications used unanchored comparisons (89 percent, 16/18), and were in oncology (83 percent, 15/18). Methods used included matching-adjusted indirect comparisons (89 percent, 16/18) and simulated treatment comparisons (17 percent, 3/18). Covariates were included based on: availability, expert opinion, effective sample size, statistical significance, or cross-validation. Larger treatment networks were commonplace (56 percent, 10/18), but current methods cannot account for this. Appraisal committees received results of population-adjusted analyses with caution and typically looked for greater cost effectiveness to minimise decision risk.ConclusionsPopulation adjustment methods are becoming increasingly common in NICE TAs, although their impact on decisions has been limited to date. Further research is needed to improve upon current methods, and to investigate their properties in simulation studies.

Asupan alkohol yang berlebih dapat meningkatkan resiko hipertensi. Dampak konsumsi alkohol beragam pada setiap orang tergantung pada durasi konsumsi dan jumlah alkohol yang dikonsumsi. Tujuan penelitian ini untuk mengetahui hubungan antara kebiasaan konsumsi alkohol dengan kejadian hipertensi. Penelitian ini merupakan penelitian observasional. Subjek penelitian yaitu laki-laki yang berusia 35-59 tahun. Jumlah sampel sebanyak 98 yang dihitung dengan menggunakan rumus Lemeshow. Variabel penelitian yaitu lama konsumsi alkohol, jumlah alkohol yang dikonsumsi dan kejadian hipertensi. Analisis data menggunakan uji Spearman Rank. Hasil penelitian menunjukkan bahwa terdapat hubungan yang bermakna antara lama konsumsi alkohol dengan kejadian hipertensi pada laki-laki usia 35-59 tahun di Puskesmas Paceda kota Bitung (p= 0,000). Semakin lama konsumsi alkohol maka semakin tinggi tekanan darah (nilai r positif). Keeratan hubungan masuk kategori sedang (r= 0,435). Selanjutnya, ditemukan juga adanya hubungan yang bermakna antara jumlah konsumsi alkohol dengan kejadian hipertensi pada laki-laki usia 35-59 tahun di Puskesmas Paceda kota Bitung (p= 0,000). Semakin banyak konsumsi alkohol maka semakin tinggi tekanan darah (nilai r positif). Keeratan hubungan masuk kategori sedang (r= 0,463). Kesimpulan dari penelitian ini yaitu adanya hubungan antara kebiasaan konsumsi alkohol dengan kejadian Hipertensi. Berdasarkan hal ini maka diperlukan tindakan pengendalian konsumsi alkohol melalui kebijakan pembatasan peredaran minuman beralkohol di kota Bitung. The conclusions of this study, namely the existence of a relationship between alcohol consumption habits with occurrence of hypertension. Based on this then required control measures of alcohol consumption through policy restrictions on the circulation of alcoholic drinks in the town of Bitung.
