4. The Use of Platelet‐Rich Fibrin and Polypropylene Mesh in Repair of Nasal Septal Perforation.

- Abstract
Objective: In this study, we aimed to investigate the effectiveness of the use of polypropylene mesh and platelet‐rich fibrin (PRF) in nasal septal perforation (NSP) repair. Study Design: Prospective experimental study. Setting: Laboratory. Methods: Twenty‐four adult male New Zealand rabbits were used in our study. In all subjects, a 10 × 10 mm perforation was created in the septum. The subjects were divided into 3 equal groups according to the different methods used in perforation closure. The bilateral mucosal flap was used in the control group, polypropylene mesh + bilateral mucosal flap in the mesh group, and polypropylene mesh + bilateral mucosal flap + PRF in the mesh + PRF group. Results: NSP treatment success rate was found to be significantly higher in the mesh (4/6, 66.7%) and mesh + PRF (6/6, 100%) groups compared to the control group (0/6, 0%). Re‐epithelialization score was higher in the mesh + PRF group and the control group compared to the mesh group. While the necrosis, neutrophil, and abscess scores were highest in the mesh group, they were similar to the control group in the mesh + PRF group. Conclusions: While polypropylene mesh significantly increases the success rate in NSP repair, it causes severe inflammatory responses. However, when polypropylene mesh is combined with PRF, it both increases the rate of perforation closure and significantly reduces the complications associated with the use of mesh. 6. Evaluation of postoperative tongue edema with ultrasonography due to tongue depressor in adult patients undergoing tonsillectomy: A prospective, parallel-group, clinical study

Amaç: Tonsillektomi cerrahisi, Kulak Burun Boğaz (KBB) pratiğinde en sık yapılan cerrahi işlemlerden biridir. Ameliyat sırasında cerrahın görüş alanını arttırmak için kullanılan ağız açacağı ve dil basacağı, hastanın dil dokusuna yüksek basınç uygulayarak çeşitli komplikasyonlara neden olabilir. Bu çalışmanın amacı, tonsillektomi yapılan erişkin hastalarda, cerrahi esnasında kullanılan dil basacağının dil üzerinde oluşturduğu basınca bağlı olarak gelişmesi muhtemel dil ödeminin, ultrasonografi (USG) ile araştırılmasıdır.Gereç ve yöntem: Çalışmaya 18-40 yaş arası hastalar dahil edildi ve iki gruba ayrıldı. Çalışma grubu (n = 31), KBB kliniğinde tonsillektomi yapılan hastalardan oluşmakta iken kontrol grubu (n = 30), genel anestezi altında tonsillektomi dışında başka bir cerrahi yapılan, dil dokusuna herhangi bir baskı uygulanmayan ve genel anestezi süresi, tonsillektomi cerrahisinin süresine yakın olan hastalardan oluşturuldu. Her hastanın, submental koronal plan Ultrasonografi (USG) ile dil kesit alanı (TA) iki defa ölçüldü. Çalışma grubunda ilk ölçüm (TA1) endotrakeal entübasyondan hemen sonra, ancak ağız açacağı ve dil basacağı takılmadan önce yapıldı. İkinci ölçüm, (TA2) tonsillektomi cerrahisinin sonunda, dil basacağı çıkartıldıktan sonra ancak ekstübasyondan hemen önce yapıldı. Kontrol grubunda, ilk ölçüm (TA1) endotrakeal entübasyondan hemen sonra, ikinci ölçüm (TA2) ise, cerrahi bitiminde ancak ekstübasyondan hemen önce yapıldı. Dil ödemi, dil kesit alanları farkı (TA2 - TA1) olarak tanımlandı. Gruplar dil kesit alanları açısından ve dil ödemi açısından karşılaştırıldı. Bulgular: Gruplar arasında, demografik açıdan ve genel anestezi süreleri açısından fark yoktu. TA1 değerleri bakımından, tonsillektomi (3.93 ± 1.29 cm2) ve kontrol grupları (3.80 ± 1.20 cm2) arasında anlamlı bir fark bulunmadı (p = 0.212). TA2 değerleri bakımından, tonsillektomi grubu (4.63 ± 0.28 cm2) ile kontrol grubu (3.91 ± 0.23 cm2) arasında anlamlı fark bulundu (p = 0.000). Tonsillektomi grubu dil ödemi değerleri (0.90 ± 0.26 cm2), kontrol grubu değerlerine (0.11 ± 0.07 cm2) göre yüksek bulundu (p = 0.000).Sonuç: Çalışmamız ile, erişkin hastalarda gerçekleştirilen tonsillektomi cerrahisinde kullanılan ağız açacağı ve dil basacağının dil üzerinde oluşturduğu yüksek basınca bağlı olarak dil ödemi gelişmiş ve dil ödemi USG ile tespit edilmiştir., Objective: Tonsillectomy is one of the most common surgical procedures performed in Ear Nose Throat (ENT) practice. The patients in the control group (n = 30) were composed of patients who were operated on for a reason other than tonsillectomy under general anesthesia, no pressure was applied on the tongue during surgery, and the duration of general anesthesia was close to the duration of tonsillectomy surgeries. Submental coronal plane USG examination of the tongue area (TA) was performed twice for each patient. The first tongue area examination (TA1) was performed immediately after intubation but before inserting the tongue depressor in the tonsillectomy group. The second tongue area examination (TA2) was performed at the end of the tonsillectomy surgery, after removal of the tongue depressor just before extubation. In the control group, the first tongue area examination (TA1) was performed immediately after intubation, and the second tongue area examination (TA2) was performed at the end of the operation just before extubation. Tongue edema, which is thought to occur due to the high pressure applied by the tongue depressor during tonsillectomy surgeries, was calculated by the difference in tongue areas (i.e., TA2 - TA1). Results: The groups were similar in terms of demographic data and general anesthesia duration. When the mean TA1 values were taken into account between the tonsillectomy (3.93 ± 1.29 cm2) and control (3.80 ± 1.20 cm2) groups, no significant difference was found (p = 0.212). The mean TA2 value of the tonsillectomy group (4.63 ± 0.28 cm2) was greater than the control group’s TA2 value (3.91 ± 0.23 cm2) and was significantly different (p = 0.000). In addition, the mean tongue edema value of the tonsillectomy group (0.90 ± 0.26 cm2) was greater than the control group (0.11 ± 0.07 cm2) and was significantly different (p = 0.000). Conclusion: The tongue depressor used caused tongue edema in tonsillectomy surgeries in adult patients. This tongue edema appears to be the result of the pressure exerted by the tongue depressor. This study is the first to show the possible role of USG in determining tongue edema due to tongue depressor in adult patients undergoing tonsillectomy.

Background: Vernal keratoconjunctivitis (VKC) is a chronic, severe allergic disease of the conjunctiva, which mostly affects young males in early to mid-childhood. There is a high incidence of asthma, allergic rhinitis (AR) and eczema among patients with VKC. It is unknown if VKC patients have abnormal olfactory dysfunction, and if so, whether this is related to AR. Objective: Our aim was to evaluate olfactory function in children with VKC, with and without comorbid AR, in comparison to healthy children without VKC. Materials and Methods: Thirty-nine VKC patients and 32 healthy children were included in the study. After eye and ear, nose and throat (ENT) examination, acoustic rhinometry and modified Connecticut Chemosensory Clinical Research Center (CCCRC) tests were performed and the test results were compared between VKC and control groups and between VKC children with or without AR. A p value <0.05 was regarded as statistically significant. Results: The VKC group included 25 males (64.1%) with a mean age of 11.13 ± 3.22 years, while the control group included 26 males (81.3%) with a mean age of 12.50 ± 2.13 years (p > 0.05 for both age and gender). Fourteen VKC patients (35.9%) had either positive prick test or elevated serum specific IgE levels against house dust mites and pollens. Mean odor thresholds did not differ between the two groups (p = 0.084), while mean odor identification and CCCRC score were statistically significantly lower in VKC group (6.00 ± 1.02 and 6.81 ± 0.75, respectively) compared to control group (7.06 ± 0.77 and 7.5 ± 0.41, respectively) (p < 0.05, both). Thirteen VKC patients had co-associated AR (33.3%). In VKC patients with AR, mean odor threshold and identification scores were even lower (5.31 ± 0.95 and 6.23 ± 0.78, respectively) (p < 0.05, both). Conclusion: Olfactory function was found to be mildly impaired in children with VKC, especially when co-associated with AR. Therefore, loss of smell should be asked as an extraocular symptom. 19. Bilateral internal laryngoceles mimicking asthma

Laryngocele is an air-filled, abnormal dilation of the laryngeal saccule that extends upward within the false vocal fold, in communication with the laryngeal lumen. A case of 43-year-old male with bilateral internal laryngoceles, who has been treated as asthma for 4 years, is presented. The patient had dyspnea, cough, and excessive phlegm for a month and a late onset stridor. Flexible nasopharyngolaryngoscopy showed bilateral cystic enlargements of the false vocal folds and true vocal folds could not be visualized. Laryngeal CT without contrast enhancement showed bilateral internal laryngoceles. Submucosal total excision of bilateral cystic masses including parts of false vocal folds was performed. The symptoms resolved immediately after surgery. Although the incidence of internal laryngocele is rare, it should be remembered in the differential diagnosis of upper airway problems and diagnostic flexible nasopharnygolaryngoscopy is routinely indicated for airway evaluation in at-risk patients.

Nazal kavitenin lobüler kapiller hemanjiomları (Piyojenik granülom) benign bir tümör olup etyolojisi tam olarak bilinmemektedir. En çok üzerinde durulan nedenler nazal travma ve hormonal faktörlerdir. Genellikle bir pedinkül ile anterior septum mukozasından kaynaklanırlar. Tek taraflı burun tıkanıklığı ve burun kanaması en önde gelen semptomlardır. Hızlı büyümeleri ve ülseratif görünümlerinden dolayı bazen nazal maligniteler ile karıştırılabilmektedir. Bu yazıda 46 yaşında bir kadın hastada daha önceki tedavilerinden sonra iki kez nüks eden piyojenik granülom olgusunun endoskopik olarak tüm sınırları görülerek tutunduğu mukoza ve perikondrium ile birlikte çıkarılması sunuldu. Nazal piyojenik granülom olgularının cerrahi tedavilerinin endoskopik yöntemle mukoza ve perikondrium ile birlikte çıkarılmasının gerekliliği literatür eşliğinde tartışıldı., Lobular capillary hemangiomas of the nasal cavity (Pyogenic granuloma) are benign tumors and their etiology is not known exactly. Most emphasized reasons are nasal trauma and hormonal factors. They are usually pedunculated and originated from the anterior septal mucosa. Unilateral nasal obstruction and epistaxis are the most common symptoms. Due to rapid growth and ulcerative appearance, they can sometimes be confused with nasal malignancies. In this article we report the second recurrence of pyogenic granuloma after previous treatments of a 46-year-old female patient that was removed subperichondrially and subperiostially with the endoscopic visualization of all of its boundaries. In the light of the literature, we discussed the necessity of endoscopic method in surgical treatments of the nasal pyogenic granulomas in subperichondrial and subperiosteal excision of the tumor.

Sinonazal bölgenin en sık görülen anatomik varyasyonu olan konka bülloza, orta konkanın pnömatizasyonu olarak tanımlanır. Konka bülloza genellikle asemptomatik olsa da boyutuna bağlı olarak nazal septum ve osteomeatal bölge ile ilişkisine göre semptomatik hale gelebilir. En sık karşılaşılan semptomlar burun tıkanıklığı, baş ve yüz bölgesinde ağrı, koku problemleri, sinüzit, postnazal akıntı ve hipozmidir. Tanı; anterior rinoskopi, endoskopik muayene ve paranazal sinüs tomografisiyle konulur. Tedavide yeterli burun açıklığı sağlamak amacıyla endoskopik yöntemle orta konkanın parsiyel rezeksiyonu yapılır. Bu yazıda kronik baş ağrısı, burun tıkanıklığı ve koku alma bozukluğuna neden olan tek taraflı dev konka büllozası olan 34 yaşında bir kadın hasta literatür eşliğinde sunuldu., Concha bullosa, which is the most frequent anatomic variation of the sinonasal region, is defined as the pneumatization of the middle turbinate. Although concha bullosa is usually asymptomatic, it may become symptomatic according to its relationship with the nasal septum and osteomeatal region based on its size. The most frequent symptoms are nasal obstruction, pain in head and face region, olfactory problems, sinusitis, postnasal discharge, and hyposmia. Diagnosis is established by anterior rhinoscopy, endoscopic examination, and paranasal sinus tomography. To ensure sufficient nasal patency for treatment, partial resection of the middle turbinate is performed by endoscopic method. In this article, we report a 34-year-old female patient with unilateral giant concha bullosa causing chronic headache, nasal obstruction, and olfactory disorder in the light of the literature.

Amaç: Bu çalışmada, otoskleroz ameliyatı olan hastaların ameliyat yöntemi, işitme sonuçları ve komplikasyonları literatür eşliğinde değerlendirildi.Hastalar ve Yöntemler: Haziran 2010 - Haziran 2015 tarihleri arasında kliniğimizde otoskleroz tanısı konularak ameliyat edilen 50 hastanın (21 erkek, 33 kadın ort. yaş 3815 yıl; dağılım 18-67 yıl) dosyaları retrospektif olarak incelendi. Hastaların muayene bulguları, uygulanan ameliyat yöntemleri, kullanılan protezler, işitmedeki kazanç ve değişiklikler ile karşılaşılan komplikasyonlar değerlendirildi. Ameliyat öncesi ve sonrası hava-kemik yolu eşikleri karşılaştırılarak fonksiyonel kazanç oranları hesaplandı.Bulgular: Elli otoskleroz hastasının 54 kulağına stapedotomi ameliyatı uygulandı. Ameliyat sonrasında hastaların hava yolu işitme eşikleri ve hava-kemik aralıklarında istatistiksel olarak anlamlı oranda azalma saptandı. Ameliyat sonrası 46 hastada (%85.1) fonksiyonel başarı elde edildi. Hastaların birinde inkus dislokasyonu, üçünde korda timpani hasarı, birinde yüzen taban ve iki hastada da total sensörinöral işitme kaybı gözlendi. Beş hastada da geçici vertigo şikayeti gözlendi.Sonuç: Klasik yöntemle uygulanan stapedotomi, yüksek başarı oranı ve düşük komplikasyon oranları ile başarılı bir tedavi yöntemidir. Ameliyatın nadir de olsa ciddi morbiditeye neden olan komplikasyonlarının olabileceği akılda tutulmalı ve medikolegal sorunlar ile karşılaşmamak için hastalar ameliyat öncesi bilgilendirilmelidir., Objectives: This study aims to evaluate the surgical method, hearing results and complications of patients undergoing surgery for otosclerosis, in the light of literature.Patients and Methods: We retrospectively examined the records of 50 patients (21 males, 33 females; mean age 38±15 years; range 18 to 67 years) who were diagnosed with otosclerosis and underwent surgery in our clinic between June 2010 and June 2015. We evaluated the examination findings of the patients, surgical procedures applied, prostheses that were used, gains and changes in hearing, and complications. The rates of functional gain were calculated by comparing pre- and postoperative air-bone conduction thresholds.Results: Stapedotomy surgery was applied to 54 ears of 50 otosclerosis patients. Statistically significant reduction was detected in airway hearing thresholds and in air-bone gaps of patients following the surgery. After surgery, functional success was accomplished in 46 patients (85.1%). We observed incus dislocation in one patient, chorda tympani injury in three, floating base in one, and total sensorineural hearing loss in two patients. We also observed temporary vertigo complaint in five patients.Conclusion: Stapedotomy applied with the conventional method is a successful method of treatment with high success rate and low complication rates. It should be kept in mind that there may be rare complications of the surgery causing serious morbidity, and patients should be informed prior to surgery to avoid medicolegal problems.

Nazal polipozis gelişimininpatogenezi tam olarak aydınlatılamamıştır. Bu çalışmada, nazal polip patogenezindeinflamatuar hücreler, adezyon molekülleri, ara flamanlar ve kemokin reseptörlerininrollerinin incelenmesi amaçlanmıştır. Çalışmaya 35 hasta alınmıştır (Grup 1, SamterSendromu olan 10 hasta; Grup 2, Diffüz polipozisli Samter Sendromu bulgularıolmayan 10 hasta; Grup 3: Soliter nazal polipli 5 hasta; Grup 4, kontrol grubu, 10hasta). Tüm hastalardan alınan örneklerde CD4, CD8, CD62E, CD90,CD98, CD104,CD105, CD106,CD147,CXCR4, B-F45, vimentin, pansitokeratin ve MSA antijenleriekspresyonları belirlenmiştir. CD4, CD8 ve CD106 ekspresyonu oranı tüm gruplardabenzer bulunmuştur. Grup 1, Grup 2 ve Grup 3'teki CD4 eksprese eden hastaoranının kontrol grubundan daha fazla olduğu saptanmıştır. CD8 antijeni ekspreseeden hasta oranının tüm gruplarda, kontrol grubuna göre anlamlı olarak daha yüksekolduğu bulunmuştur. Grup 3 ve Grup 4'te CD147 ekspresyon oranının Grup 1 veGrup 2'den yüksek olduğu belirlenmiştir. Grup 1, Grup 2 ve Grup 3'te CD98eksprese eden hastaların oranının Grup 4'ten daha fazla olduğu bulunmuştur. Grup 1,Grup 2 ve Grup 3'te vimentin eksprese eden hastaların oranının, Grup 4'ten anlamlıolarak daha yüksek olduğu saptanmıştır. Pansitokeratin için immün işaretlemede,tüm hastalar pozitif boyanmıştır. Sonuç olarak farklı hasta gruplarında görülen nazalpoliplerdeki inflamatuar hücre içeriği, adezyon molekülleri, ara flamanlar vekemokin reseptörlerinin ekspresyonu birbirlerinden ve kontrol grubundan farklılıklargöstermektedir. Bu bulgular nazal polipozisin patogenezinde inflamatuar hücreler,adezyon molekülleri, ara flamanlar ve kemokin reseptörlerinin rollerininolabileceğini destekler niteliktedir.Anahtar kelimeler: Nazal polip, ara flamanlar, adezyon molekülleri, kemokinreseptörleri. The pathogenesis of developmentof nasal polyposis is incompletely understood. The aim of this study was theinvestigation of the role of inflammatory cells, adhesion molecules, intermediatefilaments and chemokine receptors in the development of nasal polyposis. Totally, 35patients were enrolled (Group 1, 10 patients with Samter syndrome; Group 2, 10patients with diffuse polyposis without signs of Samter syndrome; Group 3: 5patients with solitary nasal polyps; Group 4, 10 controls). Expression of CD105,CD106, CD62E, CD4 ,CD8, CXCR4, CD147, CD90,CD98,CD104, B-F45,vimentin, pancytokeratin and MSA from all patients? specimens were determined.Expression of CD4, CD8 and CD106 were similar between the groups. Ratio ofpatients expressing CD4 in Group 1, Group 2 and Group 3 were higher than thecontrols. Ratio of patients expressing CD8 antigen were significantly higher in all 3groups than the control group. Expression of CD147 in Group 3 and Group 4 wassignificantly higher than in Group 1 and Group 2. CD98 expression was higher inGroup 1, Group 2 and Group 3 than in Group 4. The ratio of patients expressingvimentin in Group 1, Group 2 and Group 3were significantly higher than in Group 4.Immunostaining for pancytokeratin was positive in all patients. In conclusion,inflammatory cell, adhesion molecule, intermediate filament and chemokine receptorprofiles in nasals polyps differ among both different patient groups and controlsubjects. Objective: Tinnitus is a common neuro-otological disease for which exact information about its cause and treatment has not been reached yet. In this study, making morphometric measurements of os temporale in patients and healthy ears of patients with unilateral tinnitus; It was aimed to compare these measurements according to gender, side, patient-health status, and bulbus jugularis levels. Methods: In the study, 30 (18 females, 12 males) patients with unilateral subjective non-pulsatile tinnitus had meatus acusticus internus (MAI) diameter, length, n. cochlearis bone canal diameter (NCKC), bulbus jugularis (BJ) diameter and level, fenestra vestibuli niche height (FVNY) os temporale computed tomography (CT) images were measured. The data obtained were compared according to gender, side, health status and bulbus jugularis levels. Results: hi the results obtained, the diameter of MAI in the sick ears was found to be statistically significantly smaller on the right side from the left side. When the ears with left and right tinnitus were compared with same person's other ear, a statistically significant difference was observed. BJ diameter was significantly different only in healthy ears with hypotympanic and mesotym-panic BJ ears. When bulbus jugularis levels were investigated, 43 hypotympanicand 17 mesotympanic BJs were seen, while no epitympanic located BJ was observed. FVNY was found to be significandy smaller in the left ears tinnitus than in the right ears tinnitus ears. When individuals with left tinnitus were compared with their healthy right ears, it was observed that the left side was significantly smaller than the right. Conclusion: Significant differences between the right and left sides of the ears with tinnitus suggest that the right and left sides may affect tinnitus in different degrees. The results obtained from the study will help to interpret the CT images of patients with tinnitus, determine the etiology, and create a treatment plan. [ABSTRACT FROM AUTHOR]

PubMed: 30339503, Muscle-flap transferring is a routine approach utilized in reconstructive operations; however, flap morbidity is often a source of post-operative difficulty. Ischemia-Reperfusion Injury (IRI) is an important contributor to the viability of flaps after transferring. The goal of this research was for assess the probable useful impacts of ozone on flap survival in a rat muscle-flap design. Materials and Methods: We examined the effects of postconditioning ozone administration on viability of pedicled composite flaps. Twenty-eight Wistar rats were randomized into four groups: sham-operated (S), ischemia-reperfusion (IR), sham-operated + ozone (O), IR + ozone (IR + O), respectively. The animals were sacrificed on the eighth day. In a general histological evaluation, flap tissues were examined with a light microscope, and apoptotic cells were counted. The Apoptotic Index (AI) was then calculated. Flap-tissue samples were sent for analyses of malondialdehyde (MDA), catalase (CAT), glutathione peroxidase (GSH-Px), superoxide dismutase (SOD), and protein carbonyl (PCO), and blood samples were sent for analyses of Total Oxidant Score (TOS), and Total Antioxidant Capacity (TAC). Data were evaluated statistically using the Kruskal–Wallis test. Results: The histomorphometric score was remarkably greater in O (p =.002). The AI was greater in IR (p =.002). WOS: 000515628000003, PubMed: 31993765, Purpose Allergic rhinitis (AR), is an IgE-mediated inflammation of the nose. Regulatory T cells (Tregs) and inflammatory cytokines have been shown to play a critical role in allergic airway inflammation. The aim of the study was to compare the levels of blood T lymphocyte subsets and IL-10, IL-17 and neopterin concentrations in serum and nasal lavage of patients with AR compared to healthy subjects. Methods The study included 38 subjects with moderate-severe AR and 36 sex- and age-matched controls. Peripheral blood CD3+, CD3+CD4+ and CD4+CD25+Foxp3 percentages were evaluated using flow cytometry. Levels of IL-10, IL-17 and neopterin were measured both in serum and nasal lavage fluid with ELISA and HPLC, respectively. Results No difference was found in the percentages of T lymphocyte subsets between the two groups (p > 0.05). Serum IL-10 levels were similar (p > 0.05), whereas nasal IL-10 was lower in AR subjects compared to control group (2.22 +/- 0.91 and 3.12 +/- 1.45 pg/ml, respectively) (p < 0.05). Mean serum and nasal IL-17 were higher in AR (107.7 +/- 79.61 and 527.36 +/- 738.7 pg/ml) than the control group (76.29 +/- 28.94 and 328.9 +/- 430.8 pg/ml) (p < 0.05 and p > 0.05). There were no significant differences in serum and nasal neopterin levels (p > 0.05). Conclusions Although there were no differences in the distribution of lymphocyte subsets between the AR and control groups, the finding of higher levels of serum and nasal IL-17 and lower levels of nasal IL-10 support the cytokine imbalance in the pathogenesis of AR., Selcuk Universitesi [15102018]
