Currently, no treatments available for Parkinson's disease (PD) can slow PD progression. At the early stage of the disease, only a subset of individuals with PD progress quickly, while the majority have a slowly progressive form of the disease. In developing treatments that aim to slow PD progression, clinical trials aim to include individuals who are likely to progress faster, such that a treatment effect, if one exists, can be identified easier and earlier. The aim of the present study was to identify baseline predictors of clinical progression in early PD. We analyzed 12-month data acquired from the PASADENA trial Part 1 (NCT03100149, n = 76 participants who were allocated to the placebo arm and did not start symptomatic therapy) and the Parkinson's Progression Markers Initiative (PPMI) study (n = 139 demographically and clinically matched participants). By using ridge regression models including clinical characteristics, imaging, and non-imaging biomarkers, we found that Hoehn and Yahr stage and dopamine transporter single-photon emission computed tomography specific binding ratios (Dat-SPECT SBR) in putamen ipsilateral to the side of motor symptom onset predicted PD progression at the early stage of the disease. Further studies are needed to confirm the validity of these predictors to identify with high accuracy individuals with early PD with a faster progression phenotype.

Background: Currently available treatments for Parkinson's disease (PD) do not slow clinical progression nor target alpha-synuclein, a key protein associated with the disease. Objective: The study objective was to evaluate the efficacy and safety of prasinezumab, a humanized monoclonal antibody that binds aggregated alpha-synuclein, in individuals with early PD. Methods: The PASADENA study is a multicenter, randomized, double-blind, placebo-controlled treatment study. Individuals with early PD, recruited across the US and Europe, received monthly intravenous doses of prasinezumab (1,500 or 4,500 mg) or placebo for a 52-week period (Part 1), followed by a 52-week extension (Part 2) in which all participants received active treatment. Key inclusion criteria were: aged 40-80 years; Hoehn & Yahr (H&Y) Stage I or II; time from diagnosis ≤2 years; having bradykinesia plus one other cardinal sign of PD (e.g., resting tremor, rigidity); DAT-SPECT imaging consistent with PD; and either treatment naïve or on a stable monoamine oxidase B (MAO-B) inhibitor dose. Study design assumptions for sample size and study duration were built using a patient cohort from the Parkinson's Progression Marker Initiative (PPMI). In this report, baseline characteristics are compared between the treatment-naïve and MAO-B inhibitor-treated PASADENA cohorts and between the PASADENA and PPMI populations. Results: Of the 443 patients screened, 316 were enrolled into the PASADENA study between June 2017 and November 2018, with an average age of 59.9 years and 67.4% being male. Mean time from diagnosis at baseline was 10.11 months, with 75.3% in H&Y Stage II. Baseline motor and non-motor symptoms (assessed using Movement Disorder Society-Unified Parkinson's Disease Rating Scale [MDS-UPDRS]) were similar in severity between the MAO-B inhibitor-treated and treatment-naïve PASADENA cohorts (MDS-UPDRS sum of Parts I + II + III [standard deviation (SD)]; 30.21 [11.96], 32.10 [13.20], respectively). The overall PASADENA population (63.6% treatment naïve and 36.4% on MAO-B inhibitor) showed a similar severity in MDS-UPDRS scores (e.g., MDS-UPDRS sum of Parts I + II + III [SD]; 31.41 [12.78], 32.63 [13.04], respectively) to the PPMI cohort (all treatment naïve). Conclusions: The PASADENA study population is suitable to investigate the potential of prasinezumab to slow disease progression in individuals with early PD. Trial Registration: NCT03100149.

10. Temporal Muscle Thickness Correlates with Sarcopenia in Parkinson’s Disease

- Abstract
Sarcopenia is characterized by a progressive loss of muscle mass, strength, and function resulting in adverse health outcomes. Current assessment strategies are bothersome and means to simplify the diagnosis are an unmet medical need in Parkinson’s disease (PD). To evaluate temporal muscle thickness (TMT) obtained on routine cranial MRI as a surrogate marker of sarcopenia in PD patients. We correlated TMT from axial non-contrast-enhanced T1-weighted sequences of MRI close (±12 months) to an outpatient visit including sarcopenia (EWGSOP1, EWGSOP2, SARC-F), frailty (Fried’s criteria, clinical frailty scale), and disease characteristics of Parkinson’s patients (Hoehn and Yahr-scale, Movement Disorder Society-Unified Parkinson’s Disease Rating Scale, quality of life with the Parkinson’s Disease Questionnaire-8) assessments. Cranial MRI was available in 32 patients with a mean age of 73.56±5.14 years, mean disease duration of 11.46±5.66 years, and median Hoehn and Yahr stage of 2.5. The mean TMT was 7.49±2.76 (7.15) mm. Mean TMT was significantly associated with sarcopenia (EWGSOP2, p = 0.018; EWGSOP1, p = 0.023) and frailty status (physical phenotype; p = 0.045). Moreover, there were significant moderate to strong correlations between TMT measurement and appendicular skeletal muscle mass index (r: 0.437, p = 0.012), as well as handgrip strength (r: 0.561, p < 0.001). Reduced TMT seems to be a promising surrogate marker for sarcopenia (EWGSOP2) and muscle strength in this pilot study in PD patients.

Peroxisome proliferator-activated receptor gamma (PPARγ) agonists are used for the treatment of type 2 diabetes and metabolic syndrome. However, the currently used PPARγ agonists display serious side effects, which has led to a great interest in the discovery of novel ligands with favorable properties. The aim of our study was to identify new PPARγ agonists by a PPARγ pharmacophore–based virtual screening of 3D natural product libraries. This in silico approach led to the identification of several neolignans predicted to bind the receptor ligand binding domain (LBD). To confirm this prediction, the neolignans dieugenol, tetrahydrodieugenol, and magnolol were isolated from the respective natural source or synthesized and subsequently tested for PPARγ receptor binding. The neolignans bound to the PPARγ LBD with EC50values in the nanomolar range, exhibiting a binding pattern highly similar to the clinically used agonist pioglitazone. In intact cells, dieugenol and tetrahydrodieugenol selectively activated human PPARγ-mediated, but not human PPARα- or -β/δ-mediated luciferase reporter expression, with a pattern suggesting partial PPARγ agonism. The coactivator recruitment study also demonstrated partial agonism of the tested neolignans. Dieugenol, tetrahydrodieugenol, and magnolol but not the structurally related eugenol induced 3T3-L1 preadipocyte differentiation, confirming effectiveness in a cell model with endogenous PPARγ expression. In conclusion, we identified neolignans as novel ligands for PPARγ, which exhibited interesting activation profiles, recommending them as potential pharmaceutical leads or dietary supplements.

Six new iridoid glycosides (1−6) of the “Valerianatype” were isolated from leaves of Sambucus ebulus. The structures were elucidated by 1D- and 2D-NMR spectroscopy, mass spectrometry, and chemical degradation methods as 10-O-acetylpatrinoside-aglycone-11-O-[4′′-O-acetyl-α-l-rhamnopyranosyl-(1→2)-β-d-ribohexo-3-ulopyranoside] (1), 7-O-acetylpatrinoside-aglycone-11-O-[4′′-O-acetyl-α-l-rhamnopyranosyl-(1→2)-β-d-ribohexo-3-ulopyranoside] (2), 10-O-acetylpatrinoside-aglycone-11-O-[α-l-rhamnopyranosyl-(1→2)-β-d-ribohexo-3-ulopyranoside] (3), patrinoside-aglycone-11-O-[4′′-O-acetyl-α-l-rhamnopyranosyl-(1→2)-β-d-ribohexo-3-ulopyranoside] (4), 10-O-acetylpatrinoside-aglycone-11-O-[4′′-O-acetyl-α-l-rhamnopyranosyl-(1→2)-β-d-glucopyranoside] (5), and patrinoside-aglycone-11-O-2′-deoxy-β-d-glucopyranoside (6). Compounds 1−4represent the first examples of acylated iridoid diglycosides bearing the uncommon d-ribohexo-3-ulopyranosyl sugar moiety. Compound 6is the first iridoid glycoside with a 2-deoxy-d-glucopyranosyl sugar moiety.

24. Taspine: Bioactivity-Guided Isolation and Molecular Ligand−Target Insight of a Potent Acetylcholinesterase Inhibitor from Magnoliax soulangiana

- Abstract
A bioactivity-guided approach was taken to identify the acetylcholinesterase (AChE, EC 3.1.1.7) inhibitory agent in a Magnolia x soulangianaextract using a microplate enzyme assay with Ellman's reagent. This permitted the isolation of the alkaloids taspine (1) and (−)-asimilobine (2), which were detected for the first time in this species. Compound 1showed a significantly higher effect on AChE than the positive control galanthamine and selectively inhibited the enzyme in a long-lasting and concentration-dependent fashion with an IC50value of 0.33 ± 0.07 M. Extensive molecular docking studies were performed with human and Torpedo californica-AChE employing Gold software to rationalize the binding interaction. The results suggested ligand 1to bind in an alternative binding orientation when compared to galanthamine. While this is located in close vicinity to the catalytic amino acid triad, the 1−AChE complex was found to be stabilized by (i) sandwich-like -stacking interactions between the planar aromatic ligand (1) and the Trp84 and Phe330 of the enzyme, (ii) an esteratic site anchoring with the amino side chain, and (iii) a hydrogen-bonding network.

- Abstract
Because of the direct correlation of cholinergic deficit and the severity of dementia, Alzheimer's disease is preferentially treated with acetylcholinesterase (AChE) inhibitors to supplement the acetylcholine level. In this study we focused on non-alkaloid AChE inhibitors from natural sources in order to discover new lead structures. In the course of in vitro extract screening of Tyrolean plants using an enzyme assay with Ellman's reagent, the dichloromethane extract of chicory roots (Cichorium intybus L.) showed a pronounced inhibitory effect on AChE. At a concentration of 1 mg extract/ml an inhibition of 70% was measured. Based on a 3D multi-conformational molecularstructure database consisting of secondary metabolites from C. intybus known from the relevant literature, virtual screening filtering experiments were conducted using both a feature-based pharmacophore model and a docking procedure. Some low molecular weight sesquiterpenoids exhibited distinct interactions with the pharmacophore model. In order to verify the applicability of this computer-aided strategy, an activity-guided fractionation of the chicory root extract was performed, which resulted in the isolation of two sesquiterpene lactones, 8-deoxylactucin and lactucopicrin, showing significant and dose-dependent inhibitory activity on AChE (IC50 of 308.1 μM [CI95 243.9 - 405.3 μM] and 150.3 μM [CI95 100.8 - 188.1 μM], respectively). The two isolates were correctly predicted within the virtual screening process which corroborates the potential of the computer-assisted in combo screening approach for the discovery of the anti-cholinesterase compounds from C. intybus.

- Abstract
A methanolic extract of Leontodon rigens (Dryander in Aiton) Paiva & Ormonde yielded 1,10- epoxy-14-hydroxyhypocretenolide-β-D-glucopyranoside and its 6'-O-p-hydroxyphenylacetyl derivative. These compounds represent the first epoxyguaian-12,5-olide derivatives. The structures were established by means of high resolution mass spectrometry and 1-D and 2-D NMR spectroscopy. The cytotoxicity of the isolated compounds was assessed using the MTT assay and their potential to induce apoptosis by employing the annexin V-PI assay.

- Abstract
Besides the known compounds 2,4,6-trihydroxyacetophenone 4-O-ß -D-glucopyranoside and syringaresinol 4’-O-ß -D-glucopyranoside, the novel sesquiterpenoid 1,2-dehydro-3-oxocostic acid ß - D-glucopyranoside ester was isolated from Leontodon tuberosus L. and its structure established by mass spectrometry and 1D- and 2D-NMR spectroscopy. Additionally, a number of fatty and phenolic acids was identified in the crude methanolic extract by HPLC-DAD and HPLC-MS. The chemosystematic impact of the new sesquiterpenoid is discussed briefly.

Three new compounds, including a benzofuran, 1-{(2R*,3S*)-3-(β-
D -glucopyranosyloxy)-2,3-dihydro-2-[1-(hydroxymethyl)vinyl]-1-benzofuran-5-yl}ethanone (1), a lignan, [(2S,3R,4R)-4-(3,4-dimethoxybenzyl)-2-(3,4-dimethoxyphenyl)tetrahydrofuran-3-yl]methyl (2E)-2-methylbut-2-enoate (2), and a silphiperfolene-type sesquiterpene, [(1S,2Z,3aS,5aS,6R,8aR)-1,3a,4,5,5a,6,7,8-octahydro-1,3a,6-trimethylcyclopenta[c]pentalen-2-yl]methyl acetate (3), together with the known coumarins obliquin (4) and its 5-methoxy derivative 5 were isolated from the roots of Leontopodium alpinum. Another known coumarin derivative, 5-hydroxyobliquin (6), was isolated from the roots of L. leontopodioides. The structures of these compounds were established by spectroscopic studies.- Published

Two new phenylpropanoid glycosides (2'-O-acetylplantamajoside and 2'-O, 6?-O-diacetylplantamajoside), a new iridoid glycoside (10-O-(cinnamoyl)-6'-O-(desacetylalpinosidyl)-catalpol), the two known iridoid glycosides globularin and isoscrophularioside, and the known phenylpropanoid glycoside platamajoside were isolated from the methanolic extract of the underground parts of Wulfenia carinthiaca. Structure elucidations were based on high-resolution mass spectrometry and extensive 1-D and 2-D NMR spectroscopy.

From the roots of Leontopodium alpinum, four new bisabolane sesquiterpenoids, (1R*,2S*,4R*,5S*)-4-(acetyloxy)-2-[3-(acetyloxy)-1,5-dimethylhex-4-enyl]-5-methylcyclohexyl (2Z)-2-methylbut-2-enoate (1), (1R*,4S*,6R*)-4-(acetyloxy)-6-[3-(acetyloxy)-1,5-dimethylhex-4-enyl]-3-methylcyclohex-2-en-1-yl (2Z)-2-methylbut-2-enoate (2), and 3-methyl-1-{2-[(1R*,2R*,5R*,6S*)-2,5,6-tris(acetyloxy)-4-methylcyclohex-3-en-1-yl]propyl}but-2-enyl (2Z)-2-methylbut-2-enoate (3 and 4) have been isolated. The latter constituents differ from each other by the relative configurations of the chiral centers of the hexenyl side chain.

34. Structure of Tyrolobibenzyl D and Biological Activity of Tyrolobibenzyls from Scorzonera humilis

A novel tyrolobibenzyl derivative, 1?6-ß-?-apiosyl-ß-?-glucopyranosyl 4-[2-(4-hydroxyphenyl) ethyl]benzofuran-2-carboxylate 3 (tyrolobibenzyl D) was isolated from Scorzonera humilis L. and its structure established by mass spectrometry and 1D- and 2D-NMR spectroscopy. The biological activities of the new compound and related tyrolobibenzyls A-C (1-2, 4) and the semi-synthetic peracetyl derivatives of tyrolobibenzyls B (2a) and C (4a) were assessed. The results revealed no cytotoxic activity against P388 cells and neither anti-bacterial activity against Bacillus subtilis nor antifungal activity against Candida albicans and Trichophyton mentagrophytes for any of the investigated compounds. An evaluation of potential chemopreventive activity of 2, 2a, 4, and 4a also revealed no pronounced activity in any of the employed assaying systems.

- Abstract
A re-investigation of subaerial parts of Leontodon cichoraceus (Ten.) Sanguin. yielded the new germacrane- type sesquiterpenoid 15-ß-D-glucopyranosyl-8- [p-(ß-D-glucopyranosyloxy)phenylacetyl]-salonitenolide. The structure was established by APCI mass spectrometry and 1D- and 2D-NMR spectroscopy. A survey by HPLC-DAD and HPLC-MS gave no signs of this compound in 23 other taxa of Leontodon.

Tyrolobibenzyls A (2), B (3), and C (4) were isolated from Scorzonera humilis (Asteraceae) of Tyrolean origin. Compounds 2 and 3 possess a unique phenylethyl-benzofuran backbone and represent a new class of natural compounds. Structural assignments are based on 1D and 2D NMR as well as MS data. Furthermore, peracetylated derivatives of compounds 3 and 4 were synthesized. Pharmacological evaluation of these compounds by the [3H]thymidine assay showed no pronounced cytotoxicity.

The methanolic extract of subaerial parts of Leontodon autumnalis afforded four new and two known sesquiterpenoids of the guaiane type. The known compounds were identified by means of 1H and 13C NMR spectroscopy as crepidiaside A (
1 ) and B (2 ). The structures of the new compounds were determined by extensive 1D and 2D NMR experiments as 15-glucopyranosyloxy-2-oxo-guaia-3,11(13)-dien-1α,5α,6β,7α,10αH-12,6-olide (3 ); 15-glucopyranosyloxy-2-oxo-guai-3-en-1α,5α,6β,7α,10α,11βH-12,6-olide (4 ); 15-hydroxy-2-oxo-guai-3-en-1α,5α,6β,7α,10α,11βH-12,6-olide (5 ); and 15-glucopyranosyloxy-2-oxo-guaia-3,11(13)-dien-1β,5α,6β,7α,10αH-12,6-olide (6 ), respectively. HPLC−DAD and HPLC−MS analyses of crude extracts of subaerial parts of 25 different taxa of the genus Leontodon revealed that compounds1 and2 occur in all investigated members of the section Oporinia (L. autumnalis, L. croceus, L. helveticus, L. montaniformis, L. montanus, L. pyrenaicus, and L. rilaensis) and in L. duboisii from the section Kalbfussia. Compounds1 −6 are detectable neither in other investigated taxa of Kalbfussia (L. cichoraceus, L. muelleri, and L. palisae) nor in any members of the subgenus Leontodon. Compounds3 −5 occur in high amounts only in L. croceus and L. pyrenaicus and in samples of L. autumnalis from northwestern Europe. In other members of the section Oporinia, in L. duboisii as well as in samples of L. autumnalis from the Pyrenees, the Alps, the Carpathians, and southern Central Europe, these substances occur only in trace amounts; in L. montanus and its closest relatives, compounds3 −5 are not detectable at all. Compound6 is only detectable in samples of L. autumnalis, L. helveticus, L. pyrenaicus, L. rilaensis, and L. duboisii.- Published

- Abstract
Tetraferrocenylallene is synthesized in 49% yield from triferrocenylallenylium tetrafluoroborate by nucleophilic addition of 1-cuprioferrocene. Other, more conventional approaches failed, due to steric hindrance or the known reluctance of a diferrocenyl-substituted sp2 carbon to undergo condensation reactions. Side products include triferrocenyl(2-tetrahydrofuranyl)allene, 1,1,3,4,6,6-hexaferrocenylhexane-1,2,4,5-tetraene, and 1,1-bis(triferrocenylallenyl)ferrocene, indicating the involvement of allenyl radicals in the course of this reaction. X-ray crystal structure analyses show these allenes to be sterically congested cumulenes with interesting helical propeller conformations. Tetraferrocenylallene can be reversibly oxidized in three consecutive steps to the tetracation, as shown by low-temperature cyclic voltammetry. The expected nucleophilic reactivity of the central carbon of the allenic unit is hampered by the steric bulk of the metallocenyl substituents and only observed for the smallest electrophile possible, H+, which yields a rather labile tetraferrocenylallylium cation.

- Abstract
The subvalent mercury cluster [triangulo-Hg3(-dmpm)4][O3 SCF3]4 [dmpm = bis(dimethylphosphino)methane], involving the +4/3 oxidation state of mercury, was formed by reaction of [Hg2]2+ with 2 equivalents of dmpm or by reduction of a mixture of [Hg(Me2SO)6][O3SCF3]2 and dmpm in the molar ratio 2∶4 with elemental Hg. The cation consists of a Hg3 triangle [Hg–Hg 276.68(14), 280.99(14) and 295.53(14) pm] in which two edges are bridged by one dmpm ligand and the third edge is doubly bridged by a pair of dmpm ligands. The three Hg atoms and the phosphorus atoms of the singly bridging dmpm ligands are nearly in a plane. The dihedral angles of the Hg3 triangle and the mean planes formed by the doubly bridged Hg atoms and the phosphorus atoms of the two bridging dmpm ligands respectively are 61 and 46°. The complex is fluxional on the 31P NMR time-scale at 81 MHz at ambient temperature due to intramolecular exchange between the two different types of dmpm ligands.

49. Validation of home blood glucose meters with respect to clinical and analytical approaches.

OBJECTIVE: To evaluate the clinical and analytical accuracy of home blood glucose meters. RESEARCH DESIGN AND METHODS: Six blood glucose meters--Reflolux S (Boehringer Mannheim, Mannheim, Germany), One Touch II (LifeScan, Milpitas, CA), Glucocard Memory (Menarini, Florence, Italy), Precision QID (Medisense, Cambridge, U.K.), HaemoCue (HaemoCue, Angelholm, Sweden), and Accutrend alpha (Boehringer Mannheim, Mannheim, Germany)--were compared with a reference method (Beckman Glucose Analyzer II) under controlled conditions (glucose clamp technique). Validation of the blood glucose meters was accomplished by clinically oriented approaches (error grid analysis), statistical approaches (variance components analysis), and by the criteria of the American Diabetes Association (ADA), which recommend a target variability of < 5%. RESULTS: A total of 1,794 blood glucose monitor readings and 299 reference values ranging from 2.2 to 18.2 mmol/l were analyzed (705 readings < 3.89 mmol/l, 839 readings between 3.89 and 9.99 mmol/l, and 250 readings > 9.99 mmol/l). According to error grid analysis, only Reflolux S and Glucocard M had 100% of estimations within the clinically acceptable zones A and B. Assessment of analytical accuracy revealed substantial differences between the glucose meters after separation of the data into defined glycemic ranges. None of the devices met the ADA criteria. CONCLUSIONS: To evaluate accuracy of blood glucose meters, error grid analysis, as well as statistical models, are helpful means and should be performed together. Analytical performance of currently available home blood glucose meters differs substantially within defined glycemic ranges.

- Abstract
The subject of this paper is the preparation of 1,3,2-oxazaphospholidin-4-ones by reaction of phenyldi-chlorophosphine with the N-methyl amides of α-hydroxy isobutyric acid and the two chiral carboxylic acids (S) lactic acid and (R,S) mandelic acid, which leads to diastereomeric products. Reaction control by means of 31P n.m.r. demonstrates two surprising findings: the preferred generation of the thermody-namically less stable cis-isomer and an epimerization of the phosphorus chirality centre at room temperature.
