25. Risk Factors Associated with Simultaneous Use of Alcohol and Prescription Opioids Among Young Adults in Michigan

Abstract

Purpose:Alcohol can have serious side effects alone and can enhance the side effects of prescription opioids in unpredictable and dangerous ways. This study aims to identify risk factors for simultaneous use of alcohol and prescription opioids among young adults. Methods: Demographic characteristics, substance use, mental well-being, other substance-related factors, and simultaneous use of alcohol and prescription opioids were utilized to run multiple logistic regression analysis (N= 1751; aged 18–25). Results:Mental well-being (OR= 0.971, p= .003) and education level (OR= 0.383, p< .001) were associated with a lower likelihood of simultaneous use. Knowing someone who had a fatal overdose (OR= 2.443, p< .001), binge drinking (OR= 1.065, p= .012), and older age (OR= 1.250, p< .001) were associated with a greater likelihood of simultaneous use. Conclusion:The risk and protective factors identified in the current study point to specific areas for intervention to reduce simultaneous use. Further efforts are needed to minimize the increasing polysubstance-involved overdose mortality among young adults.

Abstract

Tweet: The authors discuss harm reduction strategies and associated outcome metrics in relation to the ongoing opioid crisis. Opioid overdose deaths and opioid-related harms have reached unprecedented levels, particularly as compounds such as xylazine and fentanyl have infiltrated the drug supply. Harm reduction strategies are especially relevant in this climate, as they aim to reduce drug-related harms independently of whether an individual chooses to enter treatment or abstain from opioid use. Assessment of harm reduction strategies should consider targets beyond substance use. The purpose of this narrative review is to summarize existing harm reduction strategies in the United States and elsewhere and to describe how outcomes of these strategies could be assessed. Strategies designed to reduce opioid-related harms include syringe exchange programs, overdose education and naloxone distribution, supervised consumption sites, injectable opioid agonist treatment, safer supply programs, drug checking, and decriminalization. Outcomes besides abstinence that could be considered include reductions in substance use patterns that are associated with a high likelihood of negative consequences (e.g., solitary use and polydrug use), reductions in severity of opioid use disorder or early remission of opioid use disorder, improved quality of life and psychological health, improvements in community functioning including linkage and adherence to treatment, and reductions in the incidence of infectious disease. As opioid overdose deaths and related harms continue to rise, harm reduction strategies have become critically important. Harm reduction strategies for opioid use disorder should be considered in the context of outcomes that extend beyond measuring drug use, to capture benefits such as reduced mortality and greater well-being and quality of life. Abstract

Substance use rates are higher among transgender and gender-diverse people than cisgender people. Rates of substance use are also higher among young adults compared with other age groups. However, little research examines substance use among transgender and gender-diverse young adults. This study examines factors associated with binge drinking and cannabis use among transgender and gender-diverse young adults in Michigan. Participants (N = 78; ages 18–25) completed an online survey that included demographics and substance use and related characteristics. Race, stimulant misuse, and sedative misuse were associated with more frequent binge drinking. Older age and past-year pain reliever misuse were associated with less frequent cannabis use. Past-year stimulant use and using cannabis alone were associated with more frequent cannabis use. These findings suggest that additional marginalized identities may increase the risk for maladaptive substance use. Future research should examine substance use behaviors and correlates to inform interventions among this population. [ABSTRACT FROM AUTHOR]
