6. Menopausal women and physical activity

Abstract

Background: Physical inactivity is well documented as being a key component in many non-communicable diseases of middle age and when combined with menopausal effects, finding approaches to encourage healthy lifestyle choices can be difficult. Purpose: To explore underlying factors for activity and inactivity in middle age women. Methods: This study explored the attitudes and perceptions of nine women aged 40-54 in London UK, who self-reported as exercisers or non-exercisers, to exercise participation at this life transition. The study used a grounded theory approach with theoretical sampling and semi-structured interviews. The study followed O���Brien et al.���s Standards for reporting qualitative research. Results: Four themes emerged: ���Being middle-aged���, ���perceptions of physical activity levels���, ���being an exerciser��� and ���health messaging���. Findings show that: Personal physical activity identities are relative to an individual���s previous levels of activity and measured through internal judgements of fitness improvement; participants who tried to participate in physical activity found it difficult to find appropriately targeted fitness classes and the reductive nature of health messaging meant all participants thought campaigns were not meant for them but for others. Conclusions: The mismatch of physical activity identities offers a wider understanding than objective measures alone for personal activity improvement; age-appropriate classes and population specific health messaging may improve adherence. These finding have relevance to bodies concerned with improving health outcomes in this population. Abstract

HIV-associated wasting, characterized by progressive loss of lean body mass and bodyweight, remains a significant problem in the era of highly active antiretroviral therapy. Loss of body cell mass, a component of lean body mass, is associated with decreased survival. Somatropin (recombinant human growth hormone) derived from mammalian cells (Serostim) is the only US FDA-approved treatment indicated to increase lean body mass, bodyweight, and physical endurance in HIV-associated wasting.Somatropin 0.1 mg/kg/day administered subcutaneously for 12 weeks effectively increased work output, bodyweight, and lean body mass, and improved health-related quality of life, compared with placebo, and had a generally manageable tolerability profile in a large randomized study in patients with HIV-associated wasting.Potential areas for further research include determination of longer-term efficacy and tolerability, the cost effectiveness of treatment, the optimal somatropin dosage, management of patients after 12 weeksʼ therapy, and whether maintenance strategies might exist to maintain accrued lean body mass with lower doses of somatropin. Nevertheless, indications to date are that somatropin is likely to have an important role in the treatment of patients with HIV-associated wasting. [ABSTRACT FROM AUTHOR]

25. A Letter From Darier to Bowen on the Naming of Bowen's Disease

Abstract

John T. Bowen described the disease that later came to bear his name in 1912.1 At the time, he was the first Edward Wigglesworth Professor of Dermatology at Massachusetts General Hospital, Harvard Medical School, Boston, having succeeded James C. White.2 He was 54 years old. Bowen reported two cases and gave the disease a descriptive title "chronic atypical epithelial proliferation."Bowen's name was applied to this disease at the suggestion of Jean Darier, who was head of a service at l'Hôpital St Louis in Paris.3 Darier had previously described keratosis follicularis, subsequently known as Darier's disease or Darier-White disease, in 1889.4 In June 1914, Darier wrote to Bowen about his experience with two patients he recognized as having the condition described by Bowen, and gingerly suggested that Bowen's name be attached to the disease.
