Abstract

Non-Down-syndrome-related acute megakaryoblastic leukemia (non-DS-AMKL) is a rare form of leukemia that can present with a variety of initial symptoms, including fever, rash, bruising, bleeding, or other more clinically challenging symptoms. Herein, we describe a 19-month-old female patient who presented with left lower extremity pain and language regression who was diagnosed with AMKL, not otherwise specified (NOS), on the basis of peripheral blood and bone marrow analysis, as well as cytogenetic and molecular diagnostic phenotyping. Of note, in addition to this patient's karyotype showing trisomy 3, a fusion between CBFA2T3 (core-binding factor, alpha subunit 2, translocated to, 3) on chromosome 16 and GLIS2 (GLIS family zinc finger protein 2), also on chromosome 16, was observed. Patients with AMKL who have trisomy 3 with CBFA2T3::GLIS2 fusions are rare, and it is not known if the co-occurrence of these abnormalities is coincidental or biologically related. This highlights the continued need for further expansion of genetic testing in individuals with rare disease to establish the groundwork for identifying additional commonalities that could potentially be used to identify therapeutic targets or improve prognostication.

Abstract

Background: Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage. We did subgroup analyses of the RESTART trial to explore whether these brain imaging features modify the effects of antiplatelet therapy.Methods: RESTART was a prospective, randomised, open-label, blinded-endpoint, parallel-group trial at 122 hospitals in the UK that assessed whether starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. For this prespecified subgroup analysis, consultant neuroradiologists masked to treatment allocation reviewed brain CT or MRI scans performed before randomisation to confirm participant eligibility and rate features of the intracerebral haemorrhage and surrounding brain. We followed participants for primary (recurrent symptomatic intracerebral haemorrhage) and secondary (ischaemic stroke) outcomes for up to 5 years (reported elsewhere). For this report, we analysed eligible participants with intracerebral haemorrhage according to their treatment allocation in primary subgroup analyses of cerebral microbleeds on MRI and in exploratory subgroup analyses of other features on CT or MRI. The trial is registered with the ISRCTN registry, number ISRCTN71907627.Findings: Between May 22, 2013, and May 31, 2018, 537 participants were enrolled, of whom 525 (98%) had intracerebral haemorrhage: 507 (97%) were diagnosed on CT (252 assigned to start antiplatelet therapy and 255 assigned to avoid antiplatelet therapy, of whom one withdrew and was not analysed) and 254 (48%) underwent the required brain MRI protocol (122 in the start antiplatelet therapy group and 132 in the avoid antiplatelet therapy group). There were no clinically or statistically significant hazards of antiplatelet therapy on recurrent intracerebral haemorrhage in primary subgroup analyses of cerebral microbleed presence (2 or more) versus absence (0 or 1) (adjusted hazard ratio [HR] 0·30 [95% CI 0·08–1·13] vs 0·77 [0·13–4·61]; pinteraction=0·41), cerebral microbleed number 0–1 versus 2–4 versus 5 or more (HR 0·77 [0·13–4·62] vs 0·32 [0·03–3·66] vs 0·33 [0·07–1·60]; pinteraction=0·75), or cerebral microbleed strictly lobar versus other location (HR 0·52 [0·004–6·79] vs 0·37 [0·09–1·28]; pinteraction=0·85). There was no evidence of heterogeneity in the effects of antiplatelet therapy in any exploratory subgroup analyses (all pinteraction>0·05).Interpretation: Our findings exclude all but a very modest harmful effect of antiplatelet therapy on recurrent intracerebral haemorrhage in the presence of cerebral microbleeds. Further randomised trials are needed to replicate these findings and investigate them with greater precision.Funding: British Heart Foundation.

Abstract

Nature deficit occurs when people, especially children, spend less time outdoors. Nature deficit often results in a wide range of behavioral issues, and people living in urban areas are more apt to have nature deficit. The literature suggests potential relationships of nature deficit to commitments toward sustainability. Expanding the awareness of the perceived connectivity of greenspaces, nature deficit, and attitudes about sustainability are important. Geospatial technologies, such as Geographic Information Systems and remote sensing, can accurately map green spaces in urban and suburban areas. This chapter describes some of the tools and techniques used to map these areas. Mapping green spaces in urban areas can be as simple as setting a threshold of vegetation index values to determine greenspace. In this chapter we use remote sensing data and techniques to map out vegetation and greenspaces throughout the Las Vegas metropolitan area in 1985 and 2014. Specifically, the percentage of greenspace in Las Vegas has decreased by about 2.5 % in 29 years. Studies like this can help planners, geographers, and others to understand the spatial dimensions of urban greenspace, and help them to promote vegetation throughout entire urban areas – especially those areas that are deemed to not have adequate greenspace. However, further research needs to be conducted on how residents in desert areas, like Las Vegas, perceive nature (it is green?), how the desert urban landscape influences people, the contributions of desert landscape exposure to perceptions of environmental sustainability, and potential outcomes from contact with nature different in desert environments.

Abstract

Two important content areas associated with informal environmental science programs are ecology/natural science topics and awareness of environmental problems/issues. This study attempted to evaluate which of these content areas may provide a more optimum learning experience. A quantitative analysis was conducted on two field trips to a science center that represented an ecological oriented program and an environmental issue presentation. Two variables that were chosen as indicators of program success—knowledge retention and attitude change—are outcomes that have been found prevalent in informal, environmental science education. These programs were administered and evaluated during the 1996/1997 school year at the Paul H. Douglas Environmental Science Center at Indiana Dunes National Lakeshore. The results of this study showed significant gains in science related knowledge following both the ecology and issue oriented treatments. The data indicate that the focus of the program (ecology or issues) did not significantly alter the way students responded to the knowledge section of the evaluation instrument. Results showed little impact on students' affect toward park site or related subject matter following either presentation type. Authors recommend multiple research methods to better evaluate affect changes following an informal environmental science experience.

Abstract

The objective of this paper is to report and analyze the results of two nominal group sessions that sought to determine the social science research needs and priorities of the Harpers Ferry Center (HFC), a unit in the National Park Service (NPS) dedicated to the creation of interpretive media, as perceived by HFC personnel and individual NPS site interpretive personnel, respectively. The Nominal Group Technique, or NGT (Delbecq & Van de Ven, 1971), is a collective decision-making technique that has been used widely within recreation research to determine and prioritize concerns of practitioners, including research agenda items (Ewert, 1990). This study indicates that those involved in different aspects of interpretation have different perspectives on social science needs. Not only do such groups need to understand each other's perspectives and contributions better, but this study also suggests that their collaboration is critical to the crafting of well-rounded social science research agendas in interpretation.

Abstract

An important goal for environmental interpretation is to attempt to change a constituent's knowledge, attitude, and/or behavior toward the park site and beyond. This study evaluated the impact of two different interpretive experiences on elementary students’ environmental knowledge, attitude, and behavior. Each program represented a major variable associated with the attitude/behavior change goals supported by interpreters. These programs were administered and evaluated during the 1995–96 school year at the Paul H. Douglas Environmental Education Center at Indiana Dunes National Lakeshore. The first interpretive experience, offered during the fall, was dedicated to ecological information, whereas the second program was conducted in the spring and was based on environmental issues associated with the site. Results of this quantitative analysis show significant gains in knowledge and little impact on attitude/behavior. The authors recommend multiple research methods to better evaluate effect and behavior changes following an interpretive experience.
