1. The influence of home environment and wound care practice on surgical site infection among post cesarean section women in Dodoma- Tanzania

Abstract

Background: Post caesarean section surgical site infections continue to be the problem of wound management that contribute to high maternal morbidity. This study assessed the prevalence of post caesarean section surgical site infections and the influence of home environment and wound care practices at home on surgical site infections among post cesarean section women in Dodoma region-Tanzania. Methods: This was a hospital-based analytical cross-sectional study involved 183 randomly selected post caesarean section women during postnatal clinic visit within two weeks post discharge. Wound assessment and isolation of microbes were used to confirm the infection, while questionnaire was used to assess the wound care practices and characteristics of home environment. Logistic regression models were employed, and the level of significance was set at p ≤ 0.05. Results: The prevalence of post caesarean surgical site infections was 20.8 %, and the common isolated microbes were Staphylococcus aureus (52.6 %). Most isolates were sensitive to ciprofloxacin (81.5 %). Women with poor wound care practices had 6 times greater odds of developing surgical site infections (AOR = 5.95; p-value 0.004). The home environment condition did not influence the development of surgical site infections but it was related with good wound care practices. Conclusion: The post cesarean section surgical site infections rate after being discharged is high in Dodoma and it is directly influenced by the poor wound care practices and indirectly by the unconducive home environment. Efforts need to be directed to ensure women adhere to infection prevention measures when caring for their wound at home while taking into consideration their physical home environment they are living in.

5. ptx3a+ fibroblast/epicardial cells provide a transient macrophage niche to promote heart regeneration

Abstract

Summary: Macrophages conduct critical roles in heart repair, but the niche required to nurture and anchor them is poorly studied. Here, we investigated the macrophage niche in the regenerating heart. We analyzed cell-cell interactions through published single-cell RNA sequencing datasets and identified a strong interaction between fibroblast/epicardial (Fb/Epi) cells and macrophages. We further visualized the association of macrophages with Fb/Epi cells and the blockage of macrophage response without Fb/Epi cells in the regenerating zebrafish heart. Moreover, we found that ptx3a+ epicardial cells associate with reparative macrophages, and their depletion resulted in fewer reparative macrophages. Further, we identified csf1a expression in ptx3a+ cells and determined that pharmacological inhibition of the csf1a pathway or csf1a knockout blocked the reparative macrophage response. Moreover, we found that genetic overexpression of csf1a enhanced the reparative macrophage response with or without heart injury. Altogether, our studies illuminate a cardiac Fb/Epi niche, which mediates a beneficial macrophage response after heart injury.

6. hapln1a + cells guide coronary growth during heart morphogenesis and regeneration

Abstract

Abstract Although several tissues and chemokines orchestrate coronary formation, the guidance cues for coronary growth remain unclear. Here, we profile the juvenile zebrafish epicardium during coronary vascularization and identify hapln1a + cells enriched with vascular-regulating genes. hapln1a + cells not only envelop vessels but also form linear structures ahead of coronary sprouts. Live-imaging demonstrates that coronary growth occurs along these pre-formed structures, with depletion of hapln1a + cells blocking this growth. hapln1a + cells also pre-lead coronary sprouts during regeneration and hapln1a + cell loss inhibits revascularization. Further, we identify serpine1 expression in hapln1a + cells adjacent to coronary sprouts, and serpine1 inhibition blocks vascularization and revascularization. Moreover, we observe the hapln1a substrate, hyaluronan, forming linear structures along and preceding coronary vessels. Depletion of hapln1a + cells or serpine1 activity inhibition disrupts hyaluronan structure. Our studies reveal that hapln1a + cells and serpine1 are required for coronary production by establishing a microenvironment to facilitate guided coronary growth.

Non-PR, IFPRI1; D.1 Agriculture's role in national development strategy; E Building Resilience; E.1 Policies, institutions and investments for resilient social systems; G Cross-cutting gender theme; CRP2; D Transforming Agriculture; COP, EPTD; DSGD; PHND; PIM, CGIAR Research Program on Policies, Institutions, and Markets (PIM), Using a participatory rural appraisal approach, a series of qualitative studies were conducted in four countries facing negative impacts of climate change—Bangladesh, Ethiopia, Kenya and Mali—in order to determine men’s and women’s perceptions of climate change, adaptive approaches, and the degree to which assets and group participation play a role in adaptation strategies. Similarities were found across countries in terms of perceptions of climate change, impacts, and strategies for adaptation. Farmers and pastoralists, groups heavily dependent on natural resources, are starkly aware of and impacted by subtle climatic changes, and those with a stronger asset base were better able to adapt to changes and shocks.

Abstract

Placenta Percreta with invasion of the urinary bladder is a rare condition, which carries a high morbidity and mortality risk for mother and fetus. We present a case which illustrates some of the complexities of perioperative management.Anaesthetic considerations include those of the pregnant patient and fetus, potential need for massive transfusion, and possible development of coagulopathy. Ideally, these cases should be diagnosed early enough in pregnancy to allow a multidisciplinary team approach to the management of mother and fetus.The perioperative care of a patient with placenta previa is complicated but, with a multidisciplinary approach, may be successful.

Abstract

Non-PR, IFPRI1; CRP2; G Cross-cutting gender theme, PHND; EPTD; PIM, CGIAR Research Program on Policies, Institutions, and Markets (PIM), Climate change poses great challenges for poor rural people in developing countries, most of whom rely on natural resources for their livelihoods and have limited capacity to adapt to climate change. It has become clear that even serious efforts to mitigate climate change will be inadequate to prevent devastating impacts that threaten to erode or reverse recent economic gains in the developing world. Individuals, communities, and policymakers must adapt to a new reality and become resilient to the negative impacts of future climate changes. Research has demonstrated that assets, broadly defined to include natural, physical, financial, human, social, and political capital, play a fundamental role in increasing incomes, reducing vulnerability, and providing pathways out of poverty. Assets are essential to poor peoples’ ability to cope with climatic shocks and to adapt to the long-term impacts of climate change.

Abstract

CAPES - Coordenação de Aperfeiçoamento de Pessoal de Nível Superior No Brasil, a atuação do enfermeiro na Estratégia Saúde da Família tem sido de fundamental importância para a ampliação e consolidação do modelo de atenção à saúde. Reconhecendo a diversidade cultural existente no país, incluindo nesse cenário a população quilombola, a prática do enfermeiro requer a abrangência de saberes normativos, teóricos, políticos, éticos e morais. A as reflexões sobre valores morais se apresentam na complexidade do trabalho do enfermeiro, que remetem a prática de responsabilidade compartilhada entre os sujeitos em um contexto delimitado. Os devem considerar os valores daqueles a quem prestam o cuidado e a construção da moralidade em comunidades quilombolas. Parte-se do pressuposto de que o enfermeiro conduz sua prática considerando a diversidade cultural e sócio-histórica da população, uma vez que os modos de viver incorporam diferentes sentidos e significados de saúde. O objetivo do estudo foi compreender a prática enfermeiros da equipe da Estratégia Saúde da Família em comunidades quilombolas, sob o prisma da ética feminista. Trata-se de um estudo de caso único e integrado, qualitativo. A coleta de dados foi realizada no período de fevereiro a junho de 2018 mediante entrevista individual, entrevista coletiva e observação. O cenário foi a Estratégia Saúde da Família em cinco municípios da região metropolitana de Belo Horizonte/MG. Os participantes foram a totalidade (sete) enfermeiros e 59 quilombolas. Os dados foram analisados à luz da Análise de Conteúdo com auxílio do software Atlas.ti 8.0. Os resultados indicaram que a prática será efetiva quando construída nas relações, dentro do contexto social, por meio do (re)conhecimento das questões sociais e culturais, bem como as formas de vida de indivíduo/família/coletividade. O estudo evidenciou as características dos quilombos e o vínculo de parentesco, os quais envolvem expressões/manifestações cultural (música, dança) e religiosa dos antepassados, associadas à escravidão e à afrodescendência. Ademais, evidenciou-se dificultadores no que concerne à concretização da prática, como: a distância da comunidade à unidade de saúde para as comunidades rurais; ausência de agente comunitário de saúde próximo às comunidades; a sobrecarga de trabalho e o volume grande de população adstrita. Este trabalho permite uma reflexão sobre as particularidades de grupos sociais, bem como a formação de profissionais para atuarem na Estratégia Saúde da Família, especialmente no que se refere ao desenvolvimento de competência cultural, moral e política e adoção de postura crítica e responsável para desenvolver suas atividades com grupos minoritários. In Brazil, the role of nurses in the Family Health Strategy has been of fundamental importance for the expansion and consolidation of the health care model. Recognizing the cultural diversity that exists in the country, including in this scenario the quilombola population, the practice of nurses requires the coverage of normative, theoretical, political, ethical and moral knowledge. Thus, reflections on moral values are presented in the complexity of the nurse's work, which refers to the practice of shared responsibility among the subjects in a limited context. Thus, nurses must consider the values of those to whom they provide care and the construction of morality in quilombola communities. It is assumed that the nurse conducts his practice considering the cultural and socio-historical diversity of the population since the ways of living incorporate different senses and meanings of health. The objective of the study was to understand the practice of nurses from the Family Health Strategy team in quilombola communities, under the prism of feminist ethics. This is a single, integrated, qualitative case study. Data collection was carried out from February to June 2018 through individual interviews, group interviews and observation. The scenario was the Family Health Strategy in five municipalities in the Metropolitan Region of Belo Horizonte-MG. Participants were all (seven) nurses and 59 quilombolas. The data were analyzed in the light of Content Analysis with the aid of the Atlas.ti 8.0 software. The results indicated that the practice will be effective when built on relationships, within the social context, through (re) knowledge of social and cultural issues, as well as the individual/family/community life forms. The study showed the characteristics of quilombos and the relationship of kinship, which involve cultural (music, dance) and religious expressions/manifestations of ancestors, associated with slavery and Afro-descent. In addition, there were difficulties regarding the implementation of the practice, such as the distance from the community to the health unit for rural communities; absence of a community health agent close to the communities; work overload, a large volume of population attached. The relevance of the theme in terms of discussing the practice in quilombola communities in the context of the FHS is emphasized. This work allows a reflection on the particularities of social groups, as well as the training of professionals to work in the Family Health Strategy, especially with regard to the development of cultural, moral and political competence and the adoption of a critical and responsible posture to develop their activities with minority groups.
