Left bundle branch block (LBBB) is a frequent finding in patients with heart failure (HF), particularly in those with dilated cardiomyopathy (DCM). LBBB has been commonly described as a consequence of DCM development. However, a total recovery of left ventricular (LV) function after cardiac resynchronization therapy (CRT), observed in patients with LBBB and DCM, has led to increasing acknowledgement of LBBB-induced dilated cardiomyopathy (LBBB-iDCM) as a specific pathological entity. Its recognition has important clinical implications, as LBBB-iDCM patients may benefit from an early CRT strategy rather than medical HF therapy only. At present, there are no definitive diagnostic criteria enabling the universal identification of LBBB-iDCM, and no defined therapeutic approach in this subgroup of patients. This review compiles the main findings about LBBB-iDCM pathophysiology and the current proposed diagnostic criteria and therapeutic approach. Resumo: O bloqueio de ramo esquerdo (BRE) é encontrado frequentemente em doentes com insuficiência cardíaca (IC), particularmente naqueles com miocardiopatia dilatada (MCD). O BRE tem sido descrito sobretudo como uma consequência no decorrer da história natural de alguns doentes com MCD. No entanto, a observação de uma recuperação total da função do ventrículo esquerdo (FVE) após terapia de ressincronização cardíaca, tem contribuído para o reconhecimento crescente da MCD induzida por BRE como uma entidade patológica específica. Esta definição tem implicações clínicas importantes, uma vez que os doentes com este diagnóstico podem beneficiar de uma estratégia precoce de terapia de ressincronização, além da terapêutica médica estabelecida para a IC. Até ao momento, não existem critérios de diagnóstico específicos que permitam uma identificação universal de doentes com MCD induzida por BRE, nem uma abordagem terapêutica definida. Esta revisão sumariza a evidência existente acerca das principais caraterísticas da fisiopatologia da MCD induzida por BRE, bem como os critérios de diagnóstico que têm sido propostos para a sua identificação e estratégias de abordagem terapêutica.

Introduction and objectives: Cardiovascular diseases (CVD) and cancer are some of the most recognized causes of mortality and morbidity worldwide. Cancer is the second leading cause of death in heart failure (HF) populations. Recent studies have hypothesized that HF might promote the development and progression of cancer. We aim to analyze and discuss the most recent evidence on the relationship between HF and cancer development. Methods: From inception to November 2022, we searched PubMed, Web of Science and ClinicalTrials.gov for relevant articles on patients with HF and a subsequent cancer diagnosis that reported outcomes of overall and site-specific cancer incidence, or mortality. Results: Of 2401 articles identified in our original search, 13 articles met our criteria. Studies reporting risk rate estimates were summarized qualitatively. Studies reporting hazard ratios (HRs), or relative risks were combined in a meta-analysis and revealed that HF was associated with an increased overall cancer incidence with a HR=1.30 (95% CI: 1.04–1.62) compared with individuals without HF. Subgroup analyses by cancer type revealed increased risk for lung cancer (HR=1.87; 95% CI: 1.28–2.73), gastrointestinal cancer (HR=1.22; 95% CI: 1.03–1.45), hematologic cancer (HR=1.60; 95% CI: 1.23–2.08) and female reproductive cancer (HR=1.67; 95% CI: 1.27–2.21). Mortality from cancer was higher in HF patients compared with non-HF subjects with a HR=2.17 (95% CI: 1.23–3.84). Conclusions: Our systematic review and meta-analysis revealed that HF may result in a subsequent increase in cancer incidence as well as in cancer-related mortality. The most common cancer subtypes in HF patients were lung, female reproductive system, and hematologic cancers. Further research is needed to understand this association better and to provide the best cardiological and oncological care. Resumo: Introdução e objetivos: As doenças cardiovasculares (DCV) e o cancro representam algumas das mais reconhecidas causas de morbimortalidade a nível mundial, sendo o cancro a segunda principal causa de morte nos doentes com insuficiência cardíaca (IC). Estudos recentes admitem a hipótese de que a IC poderia promover o desenvolvimento e a progressão da doença maligna. O nosso objetivo com este estudo é analisar e discutir a evidência mais recente que relacione a insuficiência cardíaca com o desenvolvimento do cancro. Métodos: Procedemos a uma pesquisa na PubMed, WebofScience e ClinicalTrials.gov de artigos publicados até novembro de 2022 que incluíam doentes com história de IC que tenham sido posteriormente diagnosticados com cancro, tendo como resultado incidência global ou específica de cada tipo cancro ou mortalidade por cancro. Resultados: Dos 2401 artigos identificados na pesquisa original, 13 artigos cumpriram os nossos critérios. Os estudos que descreviam algum tipo de estimativa de risco foram resumidos qualitativamente. Os estudos com resultados relativos a hazard ratios (HR), ou riscos relativos foram combinados numa meta-análise, mostrando um aumento da incidência global do cancro dos doentes com IC em comparação com indivíduos sem IC, com um HR = 1,30 (95%CI:1,04-1,62). A análise de subgrupos por tipo de cancro revelou um risco aumentado no cancro do pulmão (HR = 1,87;95%CI:1,28-2,73), nos cancros gastrointestinais (HR = 1,22;95%CI:1,03-1,45), nos cancros hematológicos (HR = 1,60;95%CI:1,23-2,08) e nos cancros do sistema reprodutivo feminino (HR = 1,67;95%CI:1,27-2,21). A mortalidade por cancro foi mais elevada em doentes com IC em comparação aos indivíduos sem IC com um HR = 2,17 (95%CI:1,23-3,84). Conclusões: Esta revisão sistemática e meta-análise revelou que em doentes com IC verifica-se um aumento da incidência e mortalidade por cancro. Os subtipos de cancro mais incidentes nos doentes com IC foram os cancros do pulmão, sistema reprodutor feminino e hematológicos. São necessários estudos mais aprofundados nesta área de modo a melhor compreender esta associação e para poder prestar o melhor cuidado a esta população do ponto de vista cardíaco e oncológico.

Hypertrophic cardiomyopathy (HCM) is the most common inherited cardiac disease. Recently, a connection has been observed between the presence of first-degree atrioventricular block (FDAVB) and cardiovascular outcomes, although the pathophysiology of this association remains poorly understood. Considering the period 2000-2023, we retrospectively included HCM patients at sinus rhythm at the first appointment and sought possible interactions of FDAVB (defined as PR interval >200 ms) with different clinical and imaging variables and with the occurrence of cardiovascular events, including atrial fibrillation (AF). A total of 97 patients were included, of whom 57 (58.8%) were men, with a mean age of 51±19 years, and 14 (14.4%) had FDAVB. During a median of 4.29 (P25 1.92, P75 7.67) years of follow-up, 35 cardiovascular events occurred, including 13 de novo diagnoses of AF, 8 hospitalizations due to heart failure, 8 new-onset strokes, 4 myocardial infarctions, and 2 implantations of cardio defibrillators in secondary prevention; no HCM-related death occurred. We did not find any association between outcomes and the presence of FDAVB. The role of FDAVB as a prognostic marker in HCM patients requires further investigation. Cardiac magnetic resonance (CMR) is an established tool for risk stratification in several cardiomyopathies, and its role in muscular dystrophies (MuD) looks promising. We sought to assess how CMR performs in predicting cardiac events in a real cohort of MuD patients. A prospective single-center study with the enrollment of consecutive adult MuD patients referred to cardiac screening from 2012 to 2018 with the collection of clinical and CMR data. During follow-up (FUP), major adverse cardiac events were considered a composite of device implantation, ventricular tachycardia (VT), hospitalization due to heart failure, and death. Sixty-five patients were included (mean age of 32±16, 51% female); the majority had myotonic dystrophy (34; 52.3%); most were asymptomatic (60; 92.3%) and at sinus rhythm (64; 98.5%). Familial hypercholesterolemia (FH) is a common genetic disorder of lipid metabolism caused by pathogenic/likely pathogenic variants in LDLR, APOB, and PCSK9 genes. Variants in FH-phenocopy genes (LDLRAP1, APOE, LIPA, ABCG5, and ABCG8), polygenic hypercholesterolemia, and hyperlipoprotein (a) [Lp(a)] can also mimic a clinical FH phenotype. We aim to present a new diagnostic tool to unravel the genetic background of clinical FH phenotype. Biochemical and genetic study was performed in 1,005 individuals with clinical diagnosis of FH, referred to the Portuguese FH Study. A next-generation sequencing panel, covering eight genes and eight SNPs to determine LDL-C polygenic risk score and LPA genetic score, was validated, and used in this study. FH was genetically confirmed in 417 index cases: 408 heterozygotes and 9 homozygotes. Cascade screening increased the identification to 1,000 FH individuals, including 11 homozygotes. FH-negative individuals (phenotype positive and genotype negative) have Lp(a) >50 mg/dl (30%), high polygenic risk score (16%), other monogenic lipid metabolism disorders (1%), and heterozygous pathogenic variants in FH-phenocopy genes (2%). Heterozygous variants of uncertain significance were identified in primary genes (12%) and phenocopy genes (7%). Overall, 42% of our cohort was genetically confirmed with FH. In the remaining individuals, other causes for high LDL-C were identified in 68%. Hyper-Lp(a) or polygenic hypercholesterolemia may be the cause of the clinical FH phenotype in almost half of FH-negative individuals. A small part has pathogenic variants in ABCG5/ABCG8 in heterozygosity that can cause hypercholesterolemia and should be further investigated. This extended next-generation sequencing panel identifies individuals with FH and FH-phenocopies, allowing to personalize each person’s treatment according to the affected pathway.

Introdução: Face aos desafios sociais e de saúde de uma população em envelhecimento, é necessário a atualização das políticas sociais e de saúde na Europa (WHO, 2020; Rudnicka, 2020). O paradigma do Envelhecimento Saudável (HA) sublinha a necessidade de inovação nos sectores educativo, de saúde e social e a devida formação dos profissionais implicados. Objectivos: O projecto Strategic Innovative Educational Network for Healthy Ageing (SIENHA) - www.sienha.eu, visa desenvolver educação a nível superior e o desenvolvimento de competências em estudantes e profissionais dos sectores de saúde e social na Europa. Este projeto envolveu uma cooperação académica multilateral ao longo de 36 meses entre sete Instituições de Ensino Superior (IES) europeias, e foi financiado pelo programa ERASMUS+. Material e Métodos: Foram estabelecidos quatro principais resultados intelectuais: 1) Identificação de um quadro de competências profissionais em HA; 2) Desenvolvimento de um currículo de ensino superior; 3) Criação de um kit de ferramentas para inovação e investigação em HA; e 4) Produção de uma ferramenta de disseminação, um Manual em Envelhecimento Saudável. Com base numa revisão exploratória, foram estabelecidas listas de competências fundamentais e desenvolvido as estratégias de avaliação e o programa de estudos. Foi também elaborado um kit de ferramentas com vista à aplicação de investigação de implementação e estratégias pedagógicas inovadoras. Resultados: O projeto culminou na identificação de competências relevantes, na conceção de um kit de ferramentas baseado nas melhores práticas e na criação de um currículo educacional centrado no envelhecimento saudável ao longo da vida. Este currículo foi concebido para melhor preparar os profissionais de saúde e sociais, incluindo fisioterapeutas, promovendo a saúde e bem-estar ao longo da vida. Conclusões: O projeto SIENHA proporciona uma abordagem integrada e estratégica para enfrentar os desafios do envelhecimento saudável na Europa, trazendo inovações na formação e prática profissional. Esta colaboração pode representar um avanço significativo para a melhoria da formação e preparação de profissionais nos sectores de saúde e social na Europa.

Introdução: A Etapa I do projeto de internacionalização Pontes Atlânticas, do Núcleo Académico de Fisioterapia (NAFisio) da Rede Académica das Ciências da Saúde da Lusofonia (RACS), iniciado no ano letivo 21/22, com segunda edição em 22/23, promove a partilha de perspetivas sobre o contexto educativo e profissional da Fisioterapia entre estudantes de diferentes instituições de ensino superior (IES) membros da RACS. Através da criação de equipas de estudantes de diferentes IES, são realizadas reuniões on-line para partilha das suas realidades, considerando a sua experiência enquanto alunos e futuros profissionais. Objetivos: Identificar o grau de satisfação e impacto da atividade Pontes Atlânticas – Etapa 1 nos alunos participantes no projeto no ano letivo 22/23 e comparar com os resultados do ano letivo 21/22. Material e Métodos: Estudo observacional, descritivo e comparativo, por questionário on-line, disponibilizado por email, no final da atividade dos anos letivos 21/22 e 22/23. A amostra no ano letivo 21/22 foi constituída por 86 estudantes dos 103 participantes iniciais (Portugal, Brasil, Moçambique, Cabo Verde, Angola e São Tomé e Príncipe) e em 22/23 por 41 estudantes de 76 participantes (Portugal, Cabo Verde, Angola e Brasil). Resultados: Considerando e comparando, respetivamente, os resultados obtidos em 21/22 e 22/23: 93% e 83% dos alunos afirmaram que os objetivos eram claros e atingíveis, e que os materiais de suporte foram suficientes; 70% e 51% reportaram que o contacto com os pares e o planeamento foi fácil; 94% e 71% referiu que a atividade ajudou a compreender o contexto nacional/internacional do ensino e da prática da Fisioterapia; e 89% e 80% dos estudantes afirmaram que atividade foi importante para o desenvolvimento de competências pessoais e profissionais, e que deve continuar a ser realizada. Conclusões: Com os resultados obtidos, concluímos que no ano letivo 22/23 a participação diminuiu e algumas das dimensões também sofreram um decréscimo, assumindo-se como um desafio a ser analisado pelo NAFisio, por forma a encontrar estratégias para o ultrapassar. No entanto, consideramos que é um projeto a ter continuidade, uma vez que 80% dos alunos afirmaram que a atividade foi importante para o desenvolvimento de competências pessoais e profissionais.

Introdução: O desenvolvimento tecnológico e a Inteligência artificial (IA) estão a transformar a forma de ensino-aprendizagem, no ensino superior. Prova disso é o aumento da literatura existente sobre o tema (Chu et al, 2022). A maioria dos estudos incide sobre as características dos instrumentos, do seu uso, benefícios e implementação. A importância da perceção dos estudantes é reconhecida na literatura (Chan & Hu, 2023), contudo desconhece-se o que pensam os estudantes de Fisioterapia (FT). Objetivo: Adaptação do questionário “Use, and acceptability, of digital health technologies in musculoskeletal physical therapy: A survey of physical therapists and patients” dos autores Merolli et al (2022), criado para profissionais e para uma população de estudantes de fisioterapia, considerando dimensões semelhantes. Material e Métodos: a construção e validação do inquérito por questionário, até dezembro de 2023. Adaptado de Merolli et al (2022) e complementado com informação de outros instrumentos utilizados nos estudos onde se avalia a utilização da IA em FT. O questionário a criar será composto por duas partes: uma relativa à identificação do sujeito e outra relativa às questões da utilização da IA, sendo utilizadas questões fechadas, num formato tipo Likert de cinco pontos. Seguir-se-ão as etapas de um estudo de construção e validação de instrumentos e posteriormente adaptação cultural e linguística, tendo em conta a variabilidade de países que fazem parte da NaFisio. Esta será contemplada nas fases das versões de consenso, painel de DElphi e no pré-teste. Após esta etapa ocorrerá a validação das propriedades psicométricas do instrumento. A aplicação destes questionários será efetuada de acordo com os princípios da declaração de Helsínquia, após submissão e aprovação pela comissão de ética. Resultados: Os resultados previstos serão um questionário em suporte digital valido para avaliação da perceção dos estudantes de Ft sobre IA, considerando as dimensões: Instrumentos de IA mais utilizados; Benefícios e riscos do seu uso; Dificuldades na sua utilização; Utilizações mais frequentes da IA durante o estudo. Posteriormente esperamos obter de cerca de 1000 respostas de estudantes de Ft dos países lusófonos, onde poderemos calcular estatisticamente o autoconhecimento dos instrumentos de GEnAI, disponíveis, quais os mais utilizados, as mais frequentes aplicações e o valor intrínseco dado à IA e ao seu futuro na aprendizagem. Conclusões: Este é o primeiro estudo sobre a opinião dos estudantes de Ft dos países lusófonos, com a possibilidade de extensão a outros países. Os resultados deste estudo poderão fornecer uma compreensão da perceção dos estudantes fisioterapia, sobre a IA, o que poderá permitir uma melhor integração e adaptação desta no ensino de modo, eficaz, crítico e ético. Permitirá ainda conhecer a opinião dos estudantes relativo ao facto de o desenvolvimento tecnológico ser transversal influenciando o ensino da FT assim como a prática clínica. Estes parecem estar a sofrer transformações que se pretendem provar se são facilitadoras ou não de uma melhor prática clínica.

Introdução: A Síndrome de Guillain Barré (SGB) é uma doença autoimune que afeta o sistema nervoso periférico e, carateriza-se por uma paralisia ascendente simétrica, hiporreflexia ou arreflexia. Na população pediátrica, estima-se uma taxa de incidência de 0,4-0,6/100 000, que embora inferior à população adulta, é considerada a causa mais comum de paralisia flácida aguda. O tratamento da SGB é caracterizado por tratamento farmacológico e Fisioterapia. A intervenção do Fisioterapeuta é crucial nas diferentes fases da doença, tanto na prevenção de complicações secundárias, como na maximização da funcionalidade e na autonomia funcional, decorrentes dos défices neurológicos residuais. Objetivos: Descrever a aplicação de um plano de intervenção centrado no treino funcional dirigido para a tarefa motora Sit-to-Stand (STS) e analisar os resultados obtidos na mobilidade funcional e nas Atividades de Vida Diária (AVDs). Material e Métodos: O caso em análise refere-se a uma criança do sexo feminino, de 9 anos de idade, com o diagnóstico médico de status pós-síndrome Guillian-Barré, ocorrido no dia 20 de janeiro de 2023, do qual resultou um quadro de tetraparésia. Apresenta os seguintes problemas: restrição na prática dos hobbies (dançar e brincar com os amigos e com o animal de estimação), dificuldades nas AVDs (vestir/despir e higiene pessoal), por limitações na mobilidade funcional (no leito, nas transferências, no desempenho do Sit-to-Stand e da marcha), devido a défice no controlo postural e no equilíbrio, presença de dor neuropática e mecânica à extensão de ambos os joelhos, fraqueza muscular nos membros superiores e inferiores e na Core. Resultados e Discussão: Após 60 sessões de fisioterapia, foram observados resultados na intensidade da dor, de 8/10 para 0/10 na escala Wong-Baker e no equilíbrio, que se refletiram no desempenho das 4 fases do STS e das AVDs. Verificou-se uma melhoria de 5/56 para 14/56 na Escala de Equilíbrio Pediátrica (EEP), no tempo da execução do teste Five Times Sit-to-Stand, e na Medida de independência funcional (MIF) que passou de 88 para 44 segundos e de 67/126 para 84/126, respetivamente. Os resultados obtidos revelaram-se positivos tanto a nível da mobilidade funcional, como no desempenho das AVDs.

Hypertrophic cardiomyopathy (HCM) is one of the most common inherited cardiac diseases, defined as a left ventricular wall thickness of ≥15 mm, in the absence of other causes of abnormal ventricular loading. A major hallmark of this disease is the presence of left ventricular outflow tract obstruction, which develops in up to three quarters of patients, referred to as obstructive hypertrophic cardiomyopathy. Current treatment is offered to symptomatic patients, based on the presence of documented left ventricular obstruction, aimed at reducing symptoms and disease progression. This is achieved through pharmacological empirical therapy, surgery, alcohol ablation and/or pacing. Mavacamten is a first-in-class allosteric inhibitor of cardiac myosin that promises to provide clinicians with targeted therapy for these patients. The aim of this review is to provide a general overview of the modern approach to the diagnosis and management of HCM, as well as to integrate all the current knowledge on mavacamten, in anticipation of a future change in the treatment algorithm of patients with HCM. Resumo: A miocardiopatia hipertrófica (MCH) é uma das doenças cardíacas hereditárias mais frequentes, sendo definida por um espessamento da parede ventricular esquerda ≥15 mm, na ausência de outras causas de sobrecarga ventricular. Um dos principais problemas associados a esta doença é a obstrução do trato de saída do ventrículo esquerdo, presente em até ¾ dos doentes, designando-se nesta circunstância por miocardiopatia hipertrófica obstrutiva. Atualmente, os principais objetivos da terapêutica destes doentes são a redução dos sintomas e a redução da progressão da doença. Estes objetivos são atingidos através de terapia farmacológica empírica, intervenção cirúrgica, ablação septal alcoólica e/ou pacing. O Mavacamten é o primeiro inibidor alostérico da miosina cardíaca, que vem permitir uma terapia mais dirigida para estes doentes. O objetivo desta revisão é fornecer uma visão geral da atual abordagem diagnóstica e de tratamento da MCH, bem como integrar todo o conhecimento atual sobre o Mavacamten, antecipando uma futura mudança no algoritmo de tratamento de doentes com MCH.

Resumo: Nos últimos anos, tem sido crescente o reconhecimento das causas genéticas das doenças cardiovasculares resultado dos significativos progressos das técnicas laboratoriais. Este conhecimento tem permitido a identificação de «novos» fenótipos e a subclassificação das síndromes clínicas, tendo impacto nas decisões terapêuticas e no aconselhamento genético que é facultado às famílias.No presente documento descreve‐se o «estado da arte» relativamente às principais recomendações para testes genéticos nas doenças cardiovasculares, pretendendo‐se providenciar uma ferramenta útil de consulta para cardiologistas e outros profissionais envolvidos na prestação nos cuidados de saúde a doentes com cardiopatias hereditárias e respetivas famílias. Abstract: In recent years, the importance of genetic causes of cardiovascular diseases has been increasingly recognized, as the result of significant advances in molecular diagnosis techniques. This growing knowledge has enabled the identification of new phenotypes and the subclassification of clinical syndromes, impacting the therapeutic approach and genetic counseling offered to affected families.This paper describes the state of the art of genetic testing in the main cardiovascular diseases, aiming to provide a useful tool to help cardiologists and other health professionals involved in the care of individuals with hereditary heart diseases and their families.

Introduction and Objectives: Dilated cardiomyopathy (DCM) is a myocardial disease that can progress to a terminal stage, requiring heart transplantation. In this work we aim to contribute to knowledge of genetic variants in adult patients undergoing heart transplantation due to end-stage DCM, reporting the results obtained in our single-center tertiary hospital series using target next-generation sequencing (NGS). Methods and Results: Genetic variants were screened in 15 genes, preselected based on variants previously identified in DCM patients. Thirteen unrelated patients were included, nine (69%) male, mean age at diagnosis 33±13 years, eight (62%) with familial DCM. Nine genetic variants were identified in six (46%) patients: five in LMNA, two in LBD3, one in TNNT2 and one in TCAP. These variants were new in most patients. The majority were classified as of uncertain significance. Two patients were double and triple heterozygotes in the LBD3 and LMNA genes, respectively. Conclusion: Our results highlight the potential of NGS in the genetic characterization of DCM patients. LMNA is one of the most frequently mutated genes and should be included in all target gene assessments of end-stage DCM patients until more data are available. Resumo: Introdução e objetivos: A miocardiopatia dilatada é uma doença miocárdica que pode evoluir para um estádio terminal, requerendo transplante cardíaco. Neste trabalho, pretendemos contribuir para o conhecimento das variantes genéticas presentes em pacientes adultos submetidos a transplante cardíaco, descrevendo os resultados obtidos utilizando técnicas de sequenciação de ADN de nova geração. Métodos e resultados: Variantes genéticas foram pesquisadas em 15 genes pré-selecionados com base em variantes previamente identificadas em pacientes com miocardiopatia dilatada. Foram incluídos 13 pacientes não relacionados, nove (69%) do sexo masculino, com idade média na altura do diagnóstico de 33±13 anos, oito (62%) com doença familiar. Foram identificadas nove variantes genéticas em seis (46%) pacientes: LMNA-5, LBD3-2, TNNT2-1 e TCAP- 1. A maioria das variantes genéticas foi classificada como de significado incerto. Dois pacientes eram heterozigotos duplos e triplos nos genes LBD3 e LMNA, respetivamente. Conclusões: Os nossos resultados reforçam o potencial das novas tecnologias de sequenciação na caracterização genética de doentes com miocardiopatia dilatada. Até que mais dados estejam disponíveis, o gene LMNA é um dos mais frequentemente envolvidos e deverá ser incluído na avaliação de pacientes com miocardiopatia dilatada em fase terminal. Introduction: Dilated cardiomyopathy (DCM) is a disease of the heart muscle characterized by ventricular dilatation and impaired systolic function. Familial forms account for 30-50% of cases. Autosomal dominant inheritance is the predominant pattern of transmission. Causal genetic variants have been identified in several genes and molecular diagnosis has implications for genetic counseling and risk stratification. Objective: We aimed to estimate the frequency of genetic variants and the molecular basis of DCM in Portugal. Methods: We performed a multicenter study of unrelated patients, recruited between 2013 and 2014. Variants in 15 genes were screened using PCR with direct sequencing (next-generation sequencing with at least 30-fold coverage combined with Sanger sequencing). Results: A total of 107 patients were included, 64 (60%) men, mean age at diagnosis 38±13 years, with 48 (45%) familial cases. In total, 31 rare variants in eight genes (mainly in MYBPC3, TNNT2 and LMNA) were identified, in 28 patients (26%). Only four variants had been previously described in association with DCM, 11 with hypertrophic cardiomyopathy, and nine variants were novel. Four variants were likely pathogenic and the remainder were of uncertain significance. We found no major differences in the main clinical and imaging characteristics between patients with or without rare variants and patients with likely pathogenic variants. Conclusions: Our results reflect the complexity and diversity of DCM genetics. For better interpretation of the pathogenicity of the variants found and their causative roles in DCM, molecular cascade screening of families is imperative. Further insight into genotype-phenotype correlations and risk stratification is desirable. Resumo: Introdução: A miocardiopatia dilatada (MCD) é uma doença do músculo cardíaco caracterizada por dilatação ventricular e compromisso da função sistólica. As formas familiares são responsáveis por 30 a 50% dos casos. O padrão de hereditariedade predominante é o autossómico dominante. Variantes genéticas causais foram identificadas em vários genes e o diagnóstico molecular tem implicações para o aconselhamento genético e estratificação de risco. Objetivo: Avaliar a base molecular da MCD em Portugal. Métodos: Estudo multicêntrico de doentes não relacionados, recrutados entre 2013 e 2014. Foram analisados 15 genes, através da técnica de PCR com sequenciação direta (NGS com pelo menos uma cobertura de 30 vezes combinada com sequenciação de Sanger). Resultados: Foram incluídos 107 pacientes, 64 (60%) homens, idade média ao diagnóstico de 38 ± 13 anos, com 48 (45%) casos familiares. Foram identificadas 31 variantes raras, em oito genes, (principalmente MYBPC3, TNNT2 e LMNA) em 28 pacientes (26%). Apenas quatro variantes tinham sido previamente descritas em associação com MCD, 11 com miocardiopatia hipertrófica e nove variantes eram novas. Quatro variantes foram classificadas como provavelmente patogénicas e as restantes de significado incerto. Não encontrámos diferenças significativas nas principais características clínicas e imagiológicas entre doentes com ou sem variantes raras e doentes com variantes provavelmente patogénicas. Conclusões: Estes resultados refletem a complexidade e diversidade genética da MCD. Para uma melhor interpretação da patogenicidade das variantes e potencial causalidade, o rastreio molecular das famílias é imperativo. Uma visão mais aprofundada das correlações genótipo-fenótipo e da estratificação de risco é desejável. Introduction: Biomarkers in dilated cardiomyopathy (DCM) reflect various pathobiological processes, including neurohormonal activation, oxidative stress, matrix remodeling, myocyte injury and myocyte stretch. We assessed the role of biomarkers in clinical and echocardiographic parameters and in left ventricular (LV) reverse remodeling (LVRR). Methods: In this prospective study of 50 DCM patients (28 men, aged 59±10 years) with LV ejection fraction (LVEF) 10 U in LVEF after optimal medical therapy. Results: Baseline LVEF was 25.4±9.8% and LV end-diastolic diameter (LVEDD)/body surface area (BSA) was 34.2±4.5 mm/m2. LVRR occurred in 34% of patients within 17.6±15.6 months. No correlation was found between B-type natriuretic peptide (BNP), 25-hydroxyvitamin D (25(OH)D), CA-125, high-sensitivity C-reactive protein (hs-CRP), lipoprotein(a) [Lp(a)], noradrenaline, adrenaline, renin or aldosterone and LVRR. Patients in NYHA class III or IV, with pulmonary congestion or ankle edema, had higher CA-125, cystatin C, BNP and hs-CRP levels (p10 U da LVEF, após a terapêutica médica otimizada. Amyloidosis is a group of disorders characterised by the accumulation of extracellular deposits of insoluble protein aggregates. Clinical management depends on the accurate identification of the amyloid precursor and underlying cause. We describe a rare case of apolipoprotein A-IV cardiac amyloidosis, the diagnosis of which required mass spectrometry-based proteomic analysis.

BackgroundIn 2015, Singapore had the first and only reported foodborne outbreak of invasive disease caused by the group B Streptococcus (GBS; Streptococcus agalactiae). Disease, predominantly septic arthritis and meningitis, was associated with sequence type (ST)283, acquired from eating raw farmed freshwater fish. Although GBS sepsis is well-described in neonates and older adults with co-morbidities, this outbreak affected non-pregnant and younger adults with fewer co-morbidities, suggesting greater virulence. Before 2015 ST283 had only been reported from twenty humans in Hong Kong and two in France, and from one fish in Thailand. We hypothesised that ST283 was causing region-wide infection in Southeast Asia.Methodology/principal findingsWe performed a literature review, whole genome sequencing on 145 GBS isolates collected from six Southeast Asian countries, and phylogenetic analysis on 7,468 GBS sequences including 227 variants of ST283 from humans and animals. Although almost absent outside Asia, ST283 was found in all invasive Asian collections analysed, from 1995 to 2017. It accounted for 29/38 (76%) human isolates in Lao PDR, 102/139 (73%) in Thailand, 4/13 (31%) in Vietnam, and 167/739 (23%) in Singapore. ST283 and its variants were found in 62/62 (100%) tilapia from 14 outbreak sites in Malaysia and Vietnam, in seven fish species in Singapore markets, and a diseased frog in China.ConclusionsGBS ST283 is widespread in Southeast Asia, where it accounts for a large proportion of bacteraemic GBS, and causes disease and economic loss in aquaculture. If human ST283 is fishborne, as in the Singapore outbreak, then GBS sepsis in Thailand and Lao PDR is predominantly a foodborne disease. However, whether transmission is from aquaculture to humans, or vice versa, or involves an unidentified reservoir remains unknown. Creation of cross-border collaborations in human and animal health are needed to complete the epidemiological picture.

This study analyzes the immediate effects of wearing a Therasuit on sagittal plane lower limb angular displacements during gait in children with unilateral spastic cerebral palsy (US-CP). Seven participants (median age = 7.00 years; ranging from 5.83 to 9.00 years) with US-CP, levels I and II of the Gross Motor Function Classification System, were assessed with kinematic gait analysis in three different conditions: (A) Baseline; (B) Therasuit without elastics and (C) Therasuit with elastics. Significant improvements were observed at the hip joint of both lower limbs during most of the gait cycle in participants wearing a Therasuit, including a decrease in the flexion pattern at the initial contact and swing phase in both lower limbs, and an increase in the extension pattern in the paretic lower limb during the stance phase. At the knee joint in the paretic lower limb, significant differences were found between the baseline and Therasuit with elastics conditions on the knee angle at initial contact, and between baseline and both Therasuit conditions on the flexion angle at swing phase. However, the inter-individual variability in kinematic patterns at the knee joint was high. At the ankle joint, decreased plantar flexion at initial contact and increased dorsiflexion during stance and swing phases were observed at the Therasuit with elastics condition, helping to correct the equinus-foot in the paretic lower limb during the whole gait cycle. The Z-values showed large effect sizes particularly for most of the angular hip variables in both lower limbs and for the angular ankle variables in the paretic lower limb. The Therasuit seems to have some positive immediate effects on gait kinematics in children with spastic unilateral cerebral palsy by providing a more functional and safer gait pattern. Future investigations with larger samples are recommended to further support these findings.

Background. Sudden cardiac death (SCD) risk stratification in dilated cardiomyopathy (DCM) has been based on left ventricular ejection fraction (LVEF), even though SCD may occur with LVEF > 35%. Family history of unexplained SCD, especially in the young, raises concern about potential inheritable risk factors. It remains largely unknown how genetic tests can be integrated into clinical practice, particularly in the selection of implantable cardioverter defibrillator (ICD) candidates. We aimed to assess the diagnostic yield of genetic testing in DCM patients with a class I recommendation for ICD implantation, based on current guidelines. Methods. We included ambulatory stable adult patients with idiopathic or familial DCM with previously implanted ICD. Molecular analysis included 15 genes (LMNA, MYH7, MYBPC3, TNNT2, ACTC1, TPM1, CSRP3, TCAP, SGCD, PLN, MYL2, MYL3, TNNI3, TAZ, and LDB3) using next-generation sequencing. Results. We evaluated 21 patients, 12 (57%) males and 9 (43%) with familial DCM, including 3 (14%) with a family history of premature unexplained SCD. Mean age at DCM diagnosis was 40 ± 2 years, and mean age at ICD implantation was 50 ± 12 years. LVEF was 27 ± 9%, and LV end-diastolic diameter was 65 ± 7 mm. Genetic variants were found in six (29%) patients, occurring in 5 genes: TPM1, TNNT2, MYH7, PLN, and MYBPC3. The majority were classified as variants of uncertain significance. Family history of SCD was present in both patients with PLN variants. Conclusion. In patients with DCM and ICD, genetic variants could be identified in a significant proportion of patients in several genes, highlighting the potential role of genetics in DCM SCD risk stratification.

Introduction: Cardiac remodeling is manifested as changes in size, shape and function of the heart. We studied the prevalence, prognosis and predictors of left ventricular reverse remodeling (LVRR) in idiopathic dilated cardiomyopathy (IDCM) after optimized medical therapy. Methods: A total of 113 IDCM patients were followed for 7.1±5.6 years. LVRR was defined as an increase of 10 units in ejection fraction (EF) and decrease in left ventricular diastolic diameter (LVDD), in the absence of resynchronization therapy. Results: Baseline EF was 27±8% and LVDD index was 37.1±6.3 mm/m2. LVRR occurred in 34.5% within 22.6 months. Final EF was 47.5±10.1%, LVDD index was 30.2±3.9 mm/m2. A adição afeta um grupo díspar de indivíduos que tendem a ser desacreditados. Rejeitando esta visão, desenvolveu-se um projeto comunitário de educação não-formal de adultos. O público-alvo da intervenção foi um grupo de residentes numa comunidade em processo de tratamento por problemas de adição e/ou distúrbios cmportamentais. Esta investigação centrou-se na transformação do quotidiano e desenvolvimento pessoal/social dos utentes, por forma a melhorar o seu bem-estar. Para esse efeito, implementaram-se atividades agrupadas por áreas temáticas. O projeto alerta para a importância de intervenções similares. É necessário conhecer o contexto real e as pessoas para uma intervenção eficaz e valorizada.

AbstractThe analysis of the movement patterns of children with spastic diplegia (SD) during the process of standing up can contribute to a better understanding of postural control. The purpose of this study was to describe the movement patterns during this task in children with SD and typical development and to analyze the differences according to their age group. Participated 40 children (38-154 months), 20 children with SD and 20 children with typical development. The participants were instructed to lie down in a supine position and quickly stand up (10 trials). Motor task sessions were videotaped and subsequently analyzed. Children with SD had more asymmetrical and less efficient movement patterns in the Upper Limbs (UL), Axial Region (AR) and Lower Limbs (LL). The oldest group of children with SD did not have more mature and efficient movement patterns, and the oldest children with typical development have more mature and efficient movement patterns in the UL and AR.

Left ventricular noncompaction is an unusual but increasingly recognized cardiomyopathy, the etiology of which is still not definitely established. Clinical presentation includes a wide spectrum of scenarios, including heart failure, thromboembolism and malignant arrhythmias, with half of deaths occurring suddenly. Early detection of LVNC is therefore essential to prevent sudden cardiac death. To our knowledge, this is the first report of the presence of cardiac sympathetic nervous dysfunction, assessed by 123iodine-metaiodobenzylguanidine myocardial scintigraphy, in a patient with LVNC, preserved left ventricular systolic function and exercise-induced nonsustained ventricular tachycardia. This finding may be related to the increased arrhythmic risk observed in this cardiomyopathy, giving a new insight into the pathophysiology of LVNC. Resumo: O miocárdio não-compactado (MNC) é uma miocardiopatia que, embora rara, tem sido gradualmente mais descrita. O espectro de apresentação clínica é variável, incluindo insuficiência cardíaca, fenómenos tromboembólicos e arritmias, sendo que metade das mortes ocorre subitamente (MSC). Por conseguinte, o diagnóstico precoce e correto do MNC é fundamental para a prevenção da MSC. O presente artigo ilustra pela primeira vez a presença de disfunção neuronal simpática, avaliada por cintigrafia cardíaca com 123I-metaiodobenzilguinadina, num doente com MNC, função sistólica ventricular esquerda preservada e taquicardia não sustentada induzida pelo esforço. A presença de alterações na actividade adrenérgica cardíaca pode estar relacionada com o maior risco arrítmico característico desta miocardiopatia, podendo contribuir para o melhor esclarecimento da sua patofisiologia. Resumo: Introdução: O diagnóstico da Síndrome de Marfan (SM) depende fundamentalmente de uma avaliação clínica multidisciplinar. O seu diagnóstico molecular, através da identificação de mutações no gene FBN1, pode permitir estabelecer um diagnóstico definitivo mesmo perante fenótipos atípicos ou «incompletos» e reconhecer precocemente portadores assintomáticos. Objectivos: O presente trabalho teve como objectivo principal avaliar a frequência e o tipo de mutações no gene FBN1, numa população de doentes com SM, referenciados a um centro hospitalar de cuidados terciários, com cirurgia torácica. Métodos: A nossa amostra incluiu 30 indivíduos com SM (provenientes de 14 famílias), que foram avaliados em consulta de Cardiologia, Reumatologia e Oftalmologia. Em todos os casos foi efectuada a pesquisa de mutações no gene FBN1 a partir de ADN obtido de amostras de sangue periférico, utilizando a técnica de amplificação por Polymerase Chain Reaction e posterior sequenciação génica. Resultados: Identificámos 12 mutações distintas nas 14 famílias estudadas. Destas, apenas duas estavam previamente descritas na literatura, sendo as restantes 10, novas mutações. Encontrámos mutações missense em 36% dos casos e mutações conduzindo à formação de codões de terminação prematura em 50% dos casos. Conclusões: Este trabalho constitui a primeira descrição de resultados de análise genotípica em doentes portugueses com SM, de que temos conhecimento. Com este trabalho, realçamos a importância de uma avaliação clínica multidisciplinar e da utilidade da pesquisa de mutações no gene FBN1 em casos seleccionados. Ao descrever 10 novas mutações, contribuímos ainda para a ampliação do espectro de variantes do gene da FBN1 associadas à SM. Abstract: Introduction: The diagnosis of Marfan syndrome (MFS) depends on a multidisciplinary clinical evaluation. Molecular study to identify mutations in the FBN1 gene can establish a definitive diagnosis even with atypical or «incomplete» phenotypes and enable earlier diagnosis in asymptomatic patients. Objectives: The aim of the present work was to evaluate the frequency and type of FBN1 gene mutations in a population of Marfan syndrome patients referred to a tertiary care center with cardiothoracic surgery. Methods: Our sample included 30 individuals with MFS (from 14 families), evaluated in cardiology, rheumatology and ophthalmology consultations. In all patients, DNA was extracted from a peripheral blood sample and mutation screening of the entire coding sequence of the FBN1 gene was then performed, using the polymerase chain reaction. Results: We identified 12 different mutations in the 14 families studied. Of these, only two had been previously described in the literature, while the other 10 were found to be new mutations; 36% of patients carried a missense mutation and 50% carried a mutation leading to a premature termination codon. Conclusions: To the best of our knowledge this is the first genotypic description of Portuguese patients with MFS. In this study, we highlight the need for comprehensive clinical evaluation of these patients and the value of FBN1 mutation analysis in selected cases. By describing 10 new mutations, we have also helped broaden the spectrum of known FBN1 mutations associated with MFS. This study aimed to evaluate the usefulness of the Xpert MTB/RIF assay for the rapid direct detection of M. tuberculosis complex (MTBC) strains and rifampicin resistance associated mutations in a resource-limited setting such as Guinea-Bissau and its implications in the management of tuberculosis (TB) and drug resistant tuberculosis, complementing the scarce information on resistance and genotypic diversity of MTBC strains in this West African country.This cross-sectional prospective study included 100 consecutive TB patients with positive acid-fast smears at two months of anti-tuberculosis treatment or in a re-treatment situation, between May and December 2012. Resistance to rifampicin was detected using the GeneXpert system and the Xpert MTB/RIF assay. MTBC isolates obtained with the BACTEC MGIT 960 system were tested for susceptibility to first- and second-line anti-tuberculosis drugs. Overall, the prevalence of multidrug-resistant tuberculosis (MDR-TB) was found to be 9 cases. Of these, 67% (6 patients) of confirmed MDR-TB cases had no past history of TB treatment and 33% (3 patients) were previously treated cases. Extensively drug-resistant TB was not found. Molecular typing of the MDR-TB strains revealed recent transmission patterns of imported MDR strains.The Xpert MTB/RIF assay was reliable for the detection of rifampicin resistant MTBC strains directly from sputum samples of patients undergoing first-line treatment for two months, being more trustworthy than the simple presence of acid-fast bacilli in the smear. Its implementation is technically simple, does not require specialized laboratory infrastructures and is suitable for resource-limited settings when a regular source of electricity and maintenance is available as well as financial and operation sustainability is guaranteed by the health authorities. A high prevalence of MDR-TB among patients at risk of MDR-TB after two months of first-line treatment was found, in support of the WHO recommendations for its use in the management of this risk group.

Fabry disease is a rare X-linked lysosomal storage disorder caused by mutations in the alpha-galactosidase gene. The most frequent cardiac presentation of Fabry disease is cardiomyopathy characterized by left ventricular (LV) hypertrophy, usually concentric.Heart disease in affected females tends to be clinically recognized later than in males and cardiac complications are the most frequently reported cause of death in females with Fabry disease. There are few data regarding the association between Fabry disease and LV noncompaction. We report a case of a 30-year-old asymptomatic woman, heterozygous for a nonsense alpha-galactosidase gene mutation (p.R220X), who presented LV noncompaction on cardiac magnetic resonance imaging, without LV wall hypertrophy. Histopathological examination of myocardial fragments showed marked deposition of glycosphingolipids in cardiomyocytes, confirming the diagnosis of Fabry cardiomyopathy. Based on this finding, the patient was proposed for enzyme replacement therapy. This case illustrates the role of endomyocardial biopsy in the clarification of doubtful or atypical findings related to cardiac Fabry disease, even in heterozygous women, and corroborates the contention that Fabry disease should be included in the differential diagnosis of LV hypertrabeculation/noncompaction. Resumo: A doença de Fabry é uma doença rara de armazenamento lisossómico, ligada ao cromossoma X, causada por mutações no gene da α – galactosidase. A apresentação cardíaca mais frequente é uma miocardiopatia caracterizada por hipertrofia ventricular esquerda geralmente concêntrica.Nas mulheres afetadas a doença cardíaca tende a ser clinicamente reconhecida mais tardiamente do que em homens e as complicações cardíacas são a causa mais frequente de morte reportada em mulheres com doença de Fabry. Existem poucos dados sobre a associação entre a doença de Fabry e a não compactação do ventrículo esquerdo (VE). Reportamos o caso de uma mulher assintomática, com 30 anos de idade, heterozigota para uma mutação nonsense do gene da α–galactosidase gene (p.R220X) que apresentava critérios de VE não compactado na ressonância magnética cardíaca, sem hipertrofia das restantes paredes ventriculares. O exame histopatológico de fragmentos do miocárdio mostrou deposição acentuada de glicoesfingolípidos nos cardiomiócitos, corroborando o diagnóstico de miocardiopatia de Fabry. Com base nestes achados, foi proposto o início de terapia de substituição enzimática. Este caso ilustra o papel da biópsia endomiocárdica no esclarecimento de achados duvidosos ou atípicos relacionados com a doença de Fabry cardíaca, mesmo em mulheres heterozigotas, e corrobora a afirmação de que a doença de Fabry deve ser incluída no diagnóstico diferencial da hipertrabeculação/não compactação do VE. Resumo: Os autores apresentam um caso clínico de um homem de 68 anos com clínica de insuficiência cardíaca predominantemente direita, no contexto de estenose aórtica grave associada a hipertensão pulmonar. Concomitantemente, foi diagnosticada anemia que, após estudo endoscópico, se concluiu ser secundária a angiodisplasias intestinais, tendo sido feito o diagnóstico de síndrome de Heyde.Após cirurgia de substituição valvular houve resolução do quadro, com melhoria dos sintomas de insuficiência cardíaca prévia e estabilização dos valores de hemoglobina.Com este caso pretende‐se mostrar a necessidade do conhecimento de associações menos frequentes na estenose aórtica grave para uma atuação terapêutica imediata e adequada. Abstract: The authors present the case of a 68‐year‐old man with predominantly right heart failure in the context of severe aortic stenosis associated with pulmonary hypertension. Anemia was diagnosed which, after endoscopic study, was considered to be secondary to angiodysplasia and a diagnosis of Heyde syndrome was made.After valve replacement surgery the patient's heart failure improved and hemoglobin levels stabilized.We present this case to show the need to recognize less common associations of severe aortic stenosis, in order to provide immediate and appropriate treatment. Post-cardiac injury syndrome (PCIS) is an inflammatory process involving the pericardium secondary to cardiac injury. It can develop after cardiac trauma, cardiac surgery, myocardial infarction, and, rarely, after certain intravascular procedures. We report a rare case of an iatrogenic cardiac rupture followed by PCIS with delayed inflammatory pericardial effusion after pacemaker implantation. A comprehensive literature review on this topic is provided. Resumo: A síndrome pós-lesão cardíaca (SPLC) corresponde a um processo inflamatório envolvendo o pericárdio, secundário à lesão cardíaca. Pode desenvolver-se após traumatismo cardíaco, cirurgia cardíaca, enfarte agudo do miocárdio, e, raramente, após alguns procedimentos intravasculares. Os autores apresentam o caso invulgar de uma rotura cardíaca iatrogénica após implantação de um pacemaker, seguida pelo desenvolvimento de um derrame pericárdico retardado, inflamatório, correspondendo a SPLC. A propósito do referido caso clínico, é efetuada uma revisão compreensiva da literatura acerca desta entidade. We present a case of a patient with known complete congenital atrioventricular block (CAVB) since the age of 7 years old that developed dilated cardiomyopathy ten years after VVI-R pacemaker implantation. He presented severe biventricular dysfunction and was symptomatic despite optimal medical therapy. Cardiac resynchronization therapy was used, and he showed clinical and electrocardiographic improvement a month later.

The prognosis for patients with chronic coronary artery disease and severe left ventricular dysfunction is poor, despite advances in different therapies. The assessment of myocardial viability has become an important aspect of the diagnosis, prognosis and management of patients with ischemic cardiomyopathy. Patients with left ventricular dysfunction, with a substantial amount of severely ischemic myocardium are at highest risk, and are likely to benefit from coronary revascularization. Patients with predominantly scar tissue should be treated medically. Multiple imaging techniques have been developed to assess viable and nonviable myocardium by evaluating perfusion, cell membrane integrity, glucose metabolism, fibrosis and contractile reserve. PET FDG-F18, myocardial perfusion scintigraphy (with (201)Tl and (99m)Tc), dobutamine stress echocardiography and more recently magnetic resonance have been extensively evaluated for assessment of viability and prediction of clinical outcome after coronary revascularization. In general, nuclear imaging techniques have a higher sensitivity for the detection of viability, whereas techniques evaluating contractile reserve have higher specificity (with lower sensitivity). Magnetic resonance has a high diagnostic accuracy for assessment of the transmural extent of myocardial scar tissue. The aim of this article is to review the role of Nuclear Medicine in assessing myocardial viability and risk stratification in patients with advanced left ventricular dysfunction, and to compare it with other imaging modalities.

Coronary heart disease is the main cause of death in developed countries. At least 25% of coronary patients have sudden death or nonfatal myocardial infarction without prior symptoms. Risk stratification identifies patients at risk for coronary events. Evaluations should begin with patient's clinical history, but some patients benefit from noninvasive cardiac imaging evaluation. The aim of this paper is to review the main indications for noninvasive cardiac imaging in risk assessment of asymptomatic patients.
