The striking recurrence of the door image in Kafka’s works was highlighted in an article published in Buenos Aires in 1944 by Carlos Sandelín. This text was plagiarized years later by the Salvadorean writer Mario Hernández Aguirre. The present essay proves this misappropriation, analyses the characteristics and relevance of Sandelín’s thesis and concludes with a contribution of its own: the recognition of the concept of threshold as the center of Kafkaesque aesthetics.

En Uruguay existe una desigual distribución de médicos entre la capital y el resto del país, determinando que los pacientes deban ser referidos a centros asistenciales fuera del área en la que residen. El Proyecto ECHO (del inglés: Extension for Community Healthcare Outcomes) busca mejorar el acceso a atención especializada para poblaciones rurales mediante la utilización de tecnologías de la comunicación, democratizando el conocimiento. Objetivo: evaluar los resultados en lo referente a competencias y habilidades profesionales en los participantes de las teleclínicas ECHO sobre cáncer ginecológico en Uruguay. Método: evaluación retrospectiva de impacto con línea de base y línea de impacto mediante un censo vía web. Período: setiembre 2020- mayo/2021. Se relevaron 22 variables, 14 indicadores dependientes que miden autopercepciones sobre incremento de capacidades en forma retrospectiva a partir de una escala Likert de 5 valores y, 8 independientes que abordan aspectos generales de la población. Teniendo en cuenta que no hay una distribución normal se aplica el test no paramétrico de Wilkoxon. Resultados: se obtuvieron 36 respuestas. Los resultados, muestran que todos los pares tienen una significación bilateral, pudiendo afirmar que existe una diferencia significativa entre las capacidades previas y posteriores a la participación en el programa ECHO. Conclusiones: se objetiva una mejora en la autopercepción de las capacidades de los participantes luego de la implementación de las teleclínicas ECHO.

Objetivo: Evaluar tres diferentes grados de corrección en el tratamiento quirúrgico de la displasia del desarrollo de la cadera (DDH) inveterada mediante modelos de elementos finitos basados en tomografía computarizada. Método: Se desarrollaron tres modelos tridimensionales de elementos finitos de resultados posoperatorios hipotéticos, basados en tres tomografías computarizadas tridimensionales de un paciente pediátrico diagnosticado de displasia del desarrollo de la cadera luxada inveterada: uno con el índice acetabular de la cadera contralateral (CLAT), otro basado en un modelo biomecánico teórico de Bombelli (BMB) y otro recreando el posoperatorio real (PO) del paciente. Resultados: Los esfuerzos en la cadera afectada fueron mayores que en la cadera no afectada. El CLAT mostró el mayor esfuerzo y la menor zona de carga. Por el contrario, el BMB mostró el menor esfuerzo y las mayores zonas de carga. Conclusiones: La propuesta basada en el BMB dio los mejores resultados en cuanto a la distribución de los esfuerzos sobre la cadera, mientras que la peor fue el CLAT. Cualitativamente, estimando la estabilidad y la amplitud de movimiento de la cadera, el caso PO se consideró el mejor.

Glass ionomer cements (GICs) are currently the first-choice materials in minimally invasive dentistry and are widely used in paediatric dentistry. The objective of this study was to evaluate the influence of viscosity and mixing on porosity and the relationship between the porosity and microhardness of GICs. Nine GICs were used: EQUIA® Forte Fil, Ionostar®Plus, 3MTMKetacTM Universal AplicapTM, Riva Light Cure HV®, Riva Silver®, Riva Protect®, VitremerTM, VitrebondTM Plus and Activa BioActive RestorativeTM. Samples were prepared from each material and images were obtained by scanning electron microscopy (SEM). Using software, the number and diameter of pores, area, and percentage of area they occupied were measured. After SEM analysis, the samples were used to make surface microhardness (SMH) measurements. Ionostar® Plus, EQUIA® Forte Fil and Riva Light Cure HV® had the highest number of pores. Riva Protect® had the largest pores and the highest percentage of pore-occupied area, while Activa BioActive RestorativeTM and VitrebondTM Plus had a smaller pore size and a smaller percentage by area. Riva Silver® and EQUIA® Forte Fil showed the highest microhardness values and VitrebondTM Plus and VitremerTM the lowest. In conclusion, when the cement viscosity is low, the type of mixture determines the degree of porosity, but has less influence when viscosity is high. There was no correlation between microhardness and the porosity of GICs.

Background: Evidence from clinical practice on the effects of switching from emtricitabine/tenofovir disoproxil fumarate (F/TDF) to emtricitabine/tenofovir alafenamide (F/TAF)-based triple-therapy (TT) regimens on renal parameters is limited. Objective: This retrospective analysis evaluated the effects on renal function of switching from F/TDF to F/TAF-based TT regimens with no change in third agent among people living with HIV (PLWH). Methods: Data were from a multicenter Spanish PLWH cohort. Patients with a baseline estimated glomerular filtration rate (eGFR-EPI) measurement, ≥1 follow-up measurement, ≥30 days treatment with F/TAF, and who switched from F/TDF to F/TAF with no change in third agent were included. Multivariate mixed linear models were used to evaluate change from baseline over time in eGFR-EPI. eGFR-EPI changes before and after switch were analyzed in a matched patient subgroup. Cystic fibrosis (CF) is a genetic and multisystemic disease that requires a high therapeutic demand for its control. The aim of this study was to assess therapeutic adherence (TA) to different treatments to study possible clinical consequences and clinical factors influencing adherence. This is an ambispective observational study of 57 patients aged over 18 years with a diagnosis of CF. The assessment of TA was calculated using the Medication Possession Ratio (MPR) index. These data were related to exacerbations and the rate of decline in FEV1 percentage. Compliance was good for all CFTR modulators, azithromycin, aztreonam, and tobramycin in solution for inhalation. The patients with the best compliance were older; they had exacerbations and the greatest deterioration in lung function during this period. The three variables with the highest importance for the compliance of the generated Random Forest (RF) models were age, FEV1%, and use of Ivacaftor/Tezacaftor. This is one of the few studies to assess adherence to CFTR modulators and symptomatic treatment longitudinally. CF patient therapy is expensive, and the assessment of variables with the highest importance for a high MPR, helped by new Machine learning tools, can contribute to defining new efficient TA strategies with higher benefits.

Desde los inicios del cine, el contenido musical ha desempeñado un papel fundamental. La música y las canciones son una parte esencial de los productos cinematográficos, especialmente en determinados géneros. El presente artículo realiza un análisis contrastivo de un corpus de doce películas del género de animación infantil y no infantil, musical, comedia romántica y suspense en torno a la modalidad y las estrategias de traducción empleadas según los elementos que priman en cada canción, tales como la rima, la fidelidad o el significado. Los resultados muestran una tendencia al doblaje de canciones en películas destinadas a público infantil, subtitulado de canciones en películas destinadas a público no infantil y no traducción de canciones de la banda sonora no originales.

BackgroundSince 1996, the standard of care (SOC) therapy for HIV treatment has consisted of a backbone of two nucleoside analogue reverse transcriptase inhibitors (NRTI) paired with a third agent. Use of two-drug combinations (2DC) has been considered for selected patients to avoid toxicities associated with the use of NRTIs. This study aimed to compare the real-world outcomes of integrase strand transfer inhibitor (INSTI)-containing triple therapy (TT) to dolutegravir- (DTG) and/or boosted protease inhibitor (bPI)-based 2DC in a large Spanish cohort of HIV patients.MethodsA retrospective analysis was performed using data from the VACH cohort, a prospective multicentre Spanish cohort of adult HIV patients. All treatment experienced patients initiating a TT of an INSTI combined with two NRTIs or a 2DC-containing DTG and/or a bPI between 01/01/2012 and 01/06/2017 were included. The unit of analysis was patient-regimens. The overall sample analysis was complemented with two sub-analyses. The first sub-analysis focused on patients treated with a backbone plus DTG compared to those treated with DTG+ one other antiretroviral. The second sub-analysis focused on patients with HIV RNAResultsOverall 7,481 patients were included in the analysis, contributing to 9,243 patient-regimens. Patient characteristics at baseline differed among groups, with the 2DC group being significantly older and having a higher proportion of women, a longer time on ART and a higher number of previous virologic failures. Median (95% Confidence Interval [C.I.]) time to switch was 2.5 years (2.3, 2.7) in 2DC group versus 2.9 years (2.7, 3.0) in TT. Adjusted hazard ratios (95% C.I.) for discontinuation due to any reason, virologic failure and toxicity in the 2DC vs TT group were 1.29 (1.15; 1.44), 2.06 (1.54; 2.77) and 1.18 (0.94; 1.48), respectively. Results were consistent in the two sub-analyses.ConclusionIn this analysis, time to discontinuation and probability of remaining free of virologic failure were significantly higher in patients on INSTI-based TT compared to DTG- and/or bPI-containing 2DC, with no differences in toxicity.

Current antifungal chemotherapy against the prevalent basidiomycete Cryptococcus neoformans displays some drawbacks. This pathogenic fungus is refractory to echinocandins, whereas conventional treatment with amphotericin B plus 5-fluorocytosine has a limited efficacy. In this study, we explored the potential cryptococcal activity of some natural agents. After conducting a screening test with a set of propolis from different geographical areas, we selected an extract from China, which displayed a certain cytotoxic activity against C. neoformans, due to this extract being cheap and easily available in large amounts. The combination of this kind of propolis with carnosic acid in a 1:4 ratio induced a stronger fungicidal effect, which occurred following a synergistic pattern, without visible alterations in external cell morphology. Furthermore, several carnosic acid–propolis formulations applied onto preformed biofilms decreased the metabolic activity of the sessile cells forming biofilms. These data support the potential application of mixtures containing these two natural extracts in the design of new antifungal strategies in order to combat opportunistic infections caused by prevalent pathogenic fungi.

Background: Leptospirosis is a worldwide zoonotic infection, and its management needs to be refined. This study aims to discern which antibiotic would be the best option to treat leptospirosis disease and analyze the efficacy of chemoprophylaxis regimens to prevent this illness. Methods: systematic review and meta-analysis on the efficacy of antibiotic treatment and chemoprophylaxis of leptospirosis in humans. Results: Ten clinical trials compared an antibiotic treatment with placebo or other antibiotic treatments in leptospirosis (the most recent one was published in 2007). The meta-analysis shows no effect of penicillin treatment on mortality compared to placebo (OR 1.65; 95% CI 0.76–3.57; p = 0.21). There are no differences between penicillin and cephalosporins or doxycycline. Penicillin does not reduce the time of defervescence (MD-0.16; 95% CI (−1.4) –1.08; p = 0.80) nor hospital stay (MD 0.15; 95% CI (−0.75)–1.06; p = 0.74). Besides, the data did not demonstrate any effectiveness of the use of penicillin in terms of the incidence of oliguria/anuria, the need for dialysis treatment, time to creatinine normalization, incidence of jaundice, or the liver function normalization time. Eight trials have assessed prophylactic treatment against leptospirosis with different strategies. A weekly dose of 200 mg of doxycycline does not show benefit versus placebo regarding the number of new cases of symptomatic leptospirosis (OR 0.20; 95% CI 0.02–1.87; p = 0.16). A single dose of doxycycline at exposure to flood water could have a beneficial effect (OR 0.23; 95% CI 0.07–0.77; p = 0.02). None of the other chemoprophylaxis regimens tested have shown a statistically significant effect on the number of new symptomatic cases. Conclusion: There is no evidence that antibiotics are a better treatment than placebo regarding mortality, shortening of fever, liver and kidney function, or reduction in the hospital stay. On the other hand, neither doxycycline nor penicillin, nor azithromycin have shown statistically significant differences in preventing symptomatic infection. Well-designed clinical trials, including other antibiotics such as quinolones or aminoglycosides, are urgently needed to improve our understanding of the treatment for this infection, which continues to be a neglected disease.

In Spain there is over a million people with hearing disability and their integration in information society is a challenge. Especially in the field of accessible cinema, where only three systems are available: the mobile application Whatscine, the magnetic loop system and subtitling screen that follows a color-coding to identify the characters and contextual information.The objective of this study is to recognize their situation in our country focusing on the integration of different methods in conventional cinema rooms. These include delving into the adaptation methods, the correct signposting, adapted films, social inclusion or exclusion of spectator in cinemas and reviewing their own experience after viewing two available titles using two of the three options outlined: Whatscine and subtitling screen.

A experiência aqui apresentada refere-se ao tema geral das novas tecnologias e configurações sociais na área da Saúde, a partir de um projeto desenvolvido na zona rural do departamento de Salto, Uruguai, entre 2010 e 2011. Tomando esta experiência interdisciplinar como ponto de partida, exporemos algumas reflexões sobre o desenvolvimento de multimídia na interseção Comunicação e Saúde no momento do impacto do Plano CEIBAL nas comunidades do meio rural, os desafios que esta conjuntura nos proporciona como comunicadores quanto ao desenvolvimento de projetos interdisciplinares na comunidade, e a articulação das novas tecnologias. Introducción: las parasitosis intestinales constituyen un problema de salud pública, por la elevada prevalencia que muestran, donde los manipuladores de alimentos parasitados, se convierten en potenciales fuentes de infección, pues algunas formas parasitarias se transmiten directamente de la fuente de infección al susceptible, por vía fecal-oral. A esto se suma el incremento en el consumo de comida fuera del núcleo familiar, en residentes de zonas urbanas y rurales. El objeto es recolectar información para que las autoridades sanitarias implementen programas de salud acordes a cada región. Métodos: estudio descriptivo, de campo y transversal. Fueron 64 los manipuladores de alimentos, pero la muestra fue de 50. Se aplicó un cuestionario sobre prácticas higiénicas en la manipulación de alimentos, se determinó nivel socioeconómico y prevalencia de parasitosis por ensayos coproparasitológicos. Resultados: fueron 44 mujeres y 6 hombres; un 52% pertenecía al estrato IV; la mayoría conocía las prácticas higiénicas en la manipulación de alimentos, pero un 26% tenía alguna parasitosis intestinal; los parásitos más frecuentes fueron: E. nana (41,2%) y B. hominis (38,7%); el monoparasitismo fue del 54%. Conclusión: en la presencia y frecuencia de una enfermedad parasitaria intestinal intervienen diversos factores. Por lo tanto, en manipuladores de alimentos, la educación ajustada al entorno, en buenas prácticas de higiene, es básica para prevenir la transmisión.

In questi due testi di Cesare Pavese ci troviamo con delle chiare ekphrasis pittoriche che contribuendo alla semplicità stilistica della prosa pavesiana ci parlano della grande allusività poetica dei suoi testi più lirici ed elegiaci.

Este trabajo, partiendo de la importancia que la piedad, el amor y la historia ocupan tanto en la obra de Elsa Morante como en el pensamiento de María Zambrano, trata de poner de relieve el peso que la «razón poética» zambraniana desempeña en la creación artística de la escritora italiana, sobre todo en La Storia (1974), en Aracoeli (1982) y en sus diferentes ensayos críticos, contenidos en Pro e contro la bomba atomica (1987). La pensadora malagueña a lo largo de su exilio en Roma (1953-1964) ejerció las veces de maestra y guía para la escritora italiana y le sirvió de fuente de inspiración filosófica, en parte gracias a la mediación de Araceli Zambrano, amiga personal de Elsa Morante.

Este artículo se basa en el estudio semántico cognitivo del léxico «amoroso», de la retórica «erótica» y de las paremias misóginas que hacen apología del uso de la violencia contra las mujeres. Al estudiar la metáforas eróticas y sexuales de «nuestra vida cotidiana», se ha pretendido sacar a la luz el profundo sustrato androcéntrico y patriarcal de nuestra retórica «amorosa», dentro del cual se esconde siempre una visión violenta y sádica de la sexualidad.

Los equipos multidisciplinares, son una estrategia sugerida hace ya más de un decenio por grupos canadienses. Los pacientes tratados en un ámbito multidisciplinar demuestran un mejor conocimiento de la enfermedad renal crónica y mejores parámetros bioquímicos. Los objetivos fueron, aumentar los conocimientos, en nuestros pacientes con enfermedad renal crónica avanzada y su familia, sobre su enfermedad, su correcto cuidado y sobre el tratamiento renal sustitutivo, a través de la educación en grupo. Valorar si la información aumenta el nivel de ansiedad de los pacientes. Valorar el grado de satisfacción con la intervención educación en grupo. Estudio retrospectivo que contó con 19 pacientes, asistentes a los dos cursos. Se realizaron dos cuestionarios al inicio y al final del curso sobre nivel de conocimientos y el nivel de ansiedad (Inventario de Ansiedad Estado-Rasgo). Al final de cada edición, se realizó un cuestionario de evaluación. Las variables estudiadas fueron el sexo, edad, nivel de estudios y asistencia de un familiar. El nivel de conocimientos alcanzó una diferencia estadísticamente significativa entre ambos cuestionarios (inicial-final) (p = 0,013). Los resultados obtenidos en la evaluación del nivel de ansiedad mostraron un ligero incremento en la ansiedad. El cuestionario de satisfacción mostró una valoración muy positiva de la intervención. Podemos concluir, que la mayoría de los pacientes han aumentado sus conocimientos sobre enfermedad renal crónica avanzada y técnicas de sustitución, han demostrado una alta satisfacción con la experiencia de esta intervención y la mayoría han experimentado un ligero aumento de ansiedad.

Il saggio studia il famoso racconto buzzatiano: I topi alla luce della psicoanalisi freudiana e dell’esperienza esistenziale dell’autore veneto, allo scopo di dimostrare la polisemica e ricca valenza ermeneutica dei roditori all’interno della narrazione. La presenza simbolica dei topi, legata a una chiara catena intertestuale, antica e moderna, struttura all’interno del rècit onorico di Buzzati, l’angoscia «edipica» di uno dei suoi testi più perturbanti.

Hepatitis C virus (HCV) shows a great geographical diversity reflected in the high number of circulating genotypes and subtypes. The response to HCV treatment is genotype specific, with the predominant genotype 1 showing the lowest rate of sustained virological response. Virally encoded enzymes are candidate targets for intervention. In particular, promising antiviral molecules are being developed to target the viral NS3/4A protease and NS5B polymerase. Most of the studies with the NS5B polymerase have been done with genotypes 1b and 2a, whilst information about other genotypes is scarce. Here, we have characterized the de novo activity of NS5B from genotypes 1 to 5, with emphasis on conditions for optimum activity and kinetic constants. En la asignatura Oftalmología los procederes diagnósticos se basan en exámenes instrumentados que requieren un entrenamiento especial y sistemático para la adquisión de las habilidades. La evaluación sistemática y el examen práctico, basados en la observación, constituyen las vías fundamentales para constatar su apropiación. Con el fin de hacer más certeros los juicios y unificar criterios, se diseñaron sobre la base de la teoría de la actividad, listas de comprobación para 3 de las habilidades que se incluyen en el programa de la asignatura Oftalmología en 5to. año de Medicina. Se incluyen además los criterios de calificación.In the Ophthalmology subject the diagnostic procedures are based on tests designed in such a way that require a special and systematic training for the acquisition of abilities. The systematic evaluation and the practical test, which are based on obervation, are the fundamental ways to verify their suitability. In order to make judgments more accurate and to unify criteria, checking lists for 3 of the abilities included in the program of Ophthalmology subject in the 5th year of Medicine were made on the basis of the theory of activity. The marking criteria were also taken into consideration
