Objective This study aimed to evaluate apoptosis by assessing cleaved caspase-3 immunoexpression in hyperplastic, potentially malignant disorder (PMD), and malignant tumors in intraoral and lower lip sites. Material and Methods A retrospective study using paraffin blocks with tissues from patients with inflammatory fibrous hyperplasia (IFH), actinic cheilitis, oral leukoplakia, lower lip and intraoral squamous cell carcinoma (SCC) was performed. The tissues were evaluated by immunohistochemical analysis with anti-cleaved caspase-3 antibody. Apoptotic area index was then correlated with lesion type. Results From 120 lesions assessed, 55 (46%) were cleaved caspase-3-positive. The SCC samples (n=40) had the highest apoptotic area indices (n=35; 87.5%). Significant differences were detected between SCCs and PMDs (p=0.0003), as well as SCCs and IFHs (p=0.001), regarding caspase-3 immunopositivity. Carcinomas of the lower lip had lower apoptotic area indices than intraoral cancer (p=0.0015). Conclusions Cleaved caspase-3 immunoexpression showed differences in oral SCCs and PMDs and demonstrated a distinct role of apoptosis in carcinogenesis of intraoral and lower lip cancer. In future, the expression of cleaved caspase-3 with other target molecules in oral cancer may be helpful in delineating the prognosis and treatment of these tumors.

Methods: Tissue sections were submitted to toluidine blue staining and immunohistochemistry with antibody anti-tryptase (clone G3). Mast cells were quantitated using Image-Pro Plus software to obtain the mean number of mast cells in three regions: epithelial, superficial portion of the fibrous wall and deep portion of the fibrous wall from 20 periapical cysts, 20 dentigerous cysts (six non-inflamed and 14 inflamed) and 20 keratocystic odontogenic tumors (four non-inflamed and 16 inflamed). The objective of the present study was to evaluate the quality and intensity of the tissue response to two synthetic hydroxyapatites implanted in critical defects in the skulls of rats. Sixty animals were divided into three experimental groups: I (control), II (HA-1 = HA with 28% crystallinity) and III (HA-2 = HA with 70% crystallinity). They were sacrificed 1, 3, 6, and 9 months after implantation (n = 5 individuals per group/period). Histomorphometric analysis included i) counting of polymorphonuclear leucocytes, mast cells, macrophages and foreign body multinucleated giant cells stained with anti-lysozyme; ii) microvascular density stained with anti-Factor VIII and iii) degree of cell proliferation stained with anti-PCNA. There were no significant differences between the experimental groups in either the quality or quantity of cells in the inflammatory infiltrate, or the degree of angiogenesis and cell proliferation. We conclude that HA-1 and HA-2 are biocompatible and that the physico-chemical differences of these biomaterials did not affect cellular response.

The aims of this study were to assess the expression levels of three proteins involved in apoptosis--Bcl-2, Bcl-X, and Bax--and evaluate their relationship with clinicopathologic features and survival in oral squamous cell carcinoma (OSCC). Immunohistochemistry was used to evaluate protein expression in 53 primary OSCCs treated by radical surgery with free margins at a single institution in 1999. Histologic specimens were graded and analyzed for perineural invasion, lymphocytic infiltrate, and pattern of invasion. Digital image analysis was performed to quantify immunostaining. Survival was analyzed using the Kaplan-Meier method and Cox's proportional hazard model. The aim of this paper was to evaluate the usefulness of coupling digital image analysis with immunohistochemistry and histomorphometry data to the study of tissue response to hydroxyapatite in a model of critical size bone defect in calvaria of rats. A transosseous defect measuring 8 mm in diameter was performed with a surgical trephine in the parietal bone of 40 rats and divided into two experimental groups according to the treatment: group I (blood clot, control), group II (HA) and killed 1, 3, 6 and 9 months after implantation (n=5/group/period). The skullcaps with overlaying skin were collected and processed for paraffin embedding. The specimens were cut in the laterolateral direction into 5-µm thick semi-serial sections and stained with hematoxylin-eosin for identification and counting of polymorphonuclears cells, mastocytes, and multinucleated giant cells, MNG, or immunolabeled with anti- lysozyme, -factor VIII and –PCNA. Digital images were obtained and analyzed with the ImagePro-Plus® software for cell couting (polymorphonuclears cells, mastocytes, macrophages and MNG) and microvessel density. Image segmentation of anti-PCNA immunostaining was used for cell proliferation analysis. The digital images obtained allowed clear identification of cells of interest by through morphological aspects or immunostaining. Data recording and analysis was facilitated by the use of specific software for image processing and graphical and statistical analysis. It can be concluded that the techniques applied were usefull to identify and count cells, structures and process of interest making easier the effectiveness of hydroxyapatite in the critical size defect in rat calvaria model.

The aim of this study was to identify the presence of Helicobacter sp. in the gastric mucosa of captive marmosets (Callithrix sp.). Histologic specimens from the fundic, corpus, and antral gastric regions of six Callithrix jacchus, 12 C. kuhli, and 12 C. geoffroyi specimens were evaluated. The sections were stained with hematoxylin-eosin (H&E) and the Warthin-Starry silver impregnation method, and immunostained with rabbit anti-H. pylori polyclonal antibody. Helicobacter-like organisms (HLOs) and coccoid forms were present in silver-stained sections from 29 stomachs, whereas immunohistochemistry (IHC) tests revealed bacterial aggregates in 15 stomachs. No statistical difference relative to the presence of Helicobacter sp. was found among the gastric regions or marmoset species. Gastric lesions were found in the groups of marmosets that had positive and negative IHC results, but no correlation between inflammation and Helicobacter sp. infection was established. These findings demonstrate that marmosets are susceptible to naturally-occurring Helicobacter sp. infection, and open the way to the development of comparative studies on Helicobacter sp. infection in humans.

A leucoplasia pilosa oral (LPO) é uma das manifestações orais mais comuns na AIDS, com valor diagnóstico e prognóstico. A LPO está relacionada ao Epstein-Barr vírus (EBV), com características clínicas e histopatológicas definidas. Já existem relatos de uma fase subclínica da LPO, porém sem caracterização histopatológica. O presente trabalho teve por objetivo descrever os aspectos histopatológicos desta fase, bem como realizar uma análise comparativa entre a LPO subclínica e clínica, com a finalidade de verificar a suficiência diagnóstica dos critérios histopatológicos. Trata-se de um estudo retrospectivo de 11 casos, obtidos a partir de cinco biópsias realizadas em pacientes com lesão e da borda de seis línguas sem lesão macroscopicamente detectável, provenientes de necropsias. Foram utilizados os seguintes métodos: histopatologia, imuno-histoquímica e hibridização in situ. Os aspectos histopatológicos que caracterizaram a fase subclínica da LPO foram: ausência de paraceratose e papilomatose, acantose leve, presença de células claras e alterações nucleares (inclusão tipo Cowdry A, núcleo em "vidro fosco" e núcleo "em colar"). Houve identificação do EBV através da hibridização in situ e da imuno-histoquímica nas alterações nucleares observadas na histopatologia. Concluiu-se, com fundamentos na identificação do EBV nas alterações nucleares, que a LPO em sua fase subclínica, da mesma forma que na lesão clínica, apresenta características histopatológicas específicas e suficientes para um diagnóstico definitivo, independentemente da identificação do EBV.Oral hairy leukoplakia (OHL) is one of the most common oral manifestations of AIDS, with diagnostic and prognostic value. OHL is associated to the Epstein-Barr virus and presents clinical and histological defined characteristics. There have already been reports about a subclinical stage of OHL, although they lacked histopathologic characterization. The present study had the aim to describe the histopathological characteristics of subclinical hairy leukoplakia, as well as to carry out a comparative analysis between clinical and subclinical OHL. For that, 11 cases were analyzed - 5 biopsies from patients who presented with the lesion and 6 samples from the borders of tongues obtained in necropsies. The histopathological findings in subclinical OHL were: absence of parakeratosis and papillomatosis, mild acanthosis, ballooning cells and nuclear alterations. In situ hybridization and immunostaining were positive for EBV in the nuclear alterations identified in the histophatological analysis. Based on the identification of EBV in the nuclear alterations, it was possible to conclude that subclinical OHL, similarly to the clinical lesion, presents histopathological features that are specific and sufficient to establish the definitive diagnosis, regardless of the identification of the virus.

As doenças auto-imunes da tireóide são o resultado da quebra da auto-tolerância causada principalmente por fatores genéticos e ambientais. O retrovírus HTLV-1 tem sido implicado como um importante fator ambiental desencadeador destas doenças. Objetivando encontrar evidências da participação deste retrovírus, 47 tecidos tireoideanos parafinados de 45 pacientes com doença de Graves e dois com tireoidite de Hashimoto, juntamente com tecidos tireoideanos normais de seis tireóides obtidas após necropsia e linfonodo de paciente com infecção pelo HTLV-1, utilizados como controles negativo e positivo, respectivamente, foram estudados. Com a intenção de detectar as proteínas p19 (gag) e gp21 (env) do HTLV-1, os tecidos foram submetidos a imuno-histoquímica, utilizando-se um novo método de recuperação antigênica e anticorpos monoclonais anti-p19 e anti-gp21. Como resultado obtivemos a mediana do percentual de positividade no sexo feminino de 30% e 41%, no masculino de 27% e 52% e total de 30% e 41%, para as proteínas p19 e gp21, respectivamente. Nos tecidos tireoideanos normais, cinco foram positivos para as duas proteínas e apresentaram mediana do percentual de positividade de 17% para a p19 e 39% para a gp21. A detecção das proteínas p19 e gp21 no tecido tireoideano de pacientes com doença auto-imune da tireóide e em tecidos normais talvez indique a expressão genética de seqüências do HTLV-1 integradas ao genoma dos indivíduos estudados, ou talvez possa representar mimetismo molecular, ou reação cruzada, causada pela presença de antígenos comuns entre as proteínas p19 e gp21 do HTLV-1 e as células epiteliais foliculares da tireóide.

AIMS: To study the expression of Bcl-2, Bcl-x, as well the presence of cleaved caspase-3 in neurofibromas and malignant peripheral nerve sheath tumors. The expression of Bcl-2 and Bcl-x and the presence of cleaved caspase 3 were compared to clinicopathological features of malignant peripheral nerve sheath tumors and their impact on survival rates were also investigated. MATERIALS AND METHODS: The evaluation of Bcl-2, Bcl-x and cleaved caspase-3 was performed by immunohistochemistry using tissue microarrays in 28 malignant peripheral nerve sheath tumors and 38 neurofibromas. Immunoquantification was performed by computerized digital image analysis. CONCLUSIONS: Apoptosis is altered in neurofibromas and mainly in malignant peripheral nerve sheath tumors. High levels of cleaved caspase-3 are more common in tumors with more aggressive histological features and it is associated with lower disease free survival of patients with malignant peripheral nerve sheath tumors. A expressão de Bcl-2 e Bcl-x e a presença da caspases-3 clivada foram comparadas com as características clinicopatológicas dos tumores malignos da bainha do nervo periférico e seu impacto nas curvas de sobrevida também foi investigado. MATERIAIS E MÉTODOS: A avaliação das proteínas Bcl-2, Bcl-x e caspases-3 clivada foi realizada em 28 tumores malignos da bainha do nervo periférico e 38 neurofibromas, por imuno-histoquímica, utilizando microarrays de tecidos. A imunoquantificação foi realizada por análise computadorizada de imagem digital. CONCLUSÕES: A apoptose está alterada nos neurofibromas e principalmente em tumores malignos da bainha do nervo periférico. Altos níveis de caspase-3 clivada é mais comum em tumores com características histopatológicas mais agressivas e estão associados com baixa sobrevida livre de doença em indivíduos com tumor maligno da bainha do nervo periférico.

The deep region from all cysts showed the highest mean number of degranulated mast cells, except for non-inflamed keratocystic odontogenic tumors analyzed by immunohistochemistry.Immunohistochemical staining detected higher number of mast cells than histochemistry. The higher number of mast cells observed in inflamed lesions could indicate the participation of these cells in the inflammatory response in odontogenic lesions. The prevalence of degranulated mast cells in the deep region suggests intense activity of these cells, possibly related to growth of cystic lesions.

INTRODUÇÃO: Vários estudos têm demonstrado associação do vírus Epstein-Barr (EBV) com neoplasias malignas, inclusive genitais, em que o papilomavírus humano (HPV) é o principal vírus associado às neoplasias epiteliais benignas e malignas. OBJETIVO: Investigar a presença do EBV e do HPV em lesões genitais de ambos os sexos, em pacientes soropositivos (grupo A) ou não (grupo B) para o vírus da imunodeficiência humana (HIV). MATERIAL E MÉTODO: Selecionados 126 pacientes e 135 lesões anogenitais, sendo 67 pacientes (53%) e 75 lesões (56%) no grupo A e 59 pacientes (47%) e 60 lesões (44%) no grupo B, para análise imuno-histoquímica (IHQ) por meio dos anticorpos monoclonais antiproteína latente de membrana 1 (LMP1) e HPV (DAKO®). RESULTADOS: A análise mostrou que o número total de lesões com imunopositividade para o HPV e para a LMP1 foi maior no grupo A (32 e 35, respectivamente) quando comparado ao B (16 e seis, respectivamente). A análise estatística (nível de significância de 5%) mostrou que as proporções para o HPV não são estatisticamente significativas (z = 1,93; valor p = 0,053). Entretanto, para a LMP1, a diferença (47% no grupo A e 10% no B) é significativa (z = 4,60; valor p = 4,2×10-6). Do mesmo modo, a associação HPV-LMP1 (21% no grupo A e 7% no B) também mostrou diferença estatisticamente significativa (z = 2,38; valor p = 0,017). CONCLUSÃO: Esses resultados indicam a possibilidade de sinergismo da infecção pelo EBV e a coinfecção EBV-HPV em lesões epiteliais genitais, particularmente em pacientes soropositivos para o HIV. Entretanto, investigações com metodologia de maior especificidade e sensibilidade são necessárias para a verificação da real participação do EBV na patogênese de lesões epiteliais genitais. INTRODUCTION: Several studies have demonstrated the association between Epstein-Barr virus (EBV) and malignant neoplasias, including genital lesions, in which the human papillomavirus (HPV) is the main virus associated with both benign and malignant epithelial neoplasias. OBJECTIVE: Investigate the presence of EBV and HPV in genital lesions in HIV-infected patients (group A) or HIV non-infected patients (group B) from both genders. MATERIAL AND METHOD: We selected 126 patients and 135 anogenital lesions, comprising 67 patients (53%) and 75 lesions (56%) from group A and 59 patients (47%) and 60 lesions (44%) from group B, to histopathological and immunohistochemical analyses through latent membrane protein 1 (LMP1) monoclonal antibodies and HPV (DAKO®). RESULTS: The analysis showed that the total number of lesions with immunopositivity for HPV and for LMP1 was higher in group A (32 and 35 respectively) in comparison with B (16 and six respectively). Statistical analysis (significance level of 5%) showed that the proportions for HPV are not statistically significant (z = 1.93; value p = 0.053). However, the difference (47% in group A and 10% in B) is significant for LMP1 (z = 4.60; value p = 4.2×10-6). Similarly, the association of HPV and LMP1 (21% in group A and 7% in B) also showed a significant statistical difference (z = 2.38; value p = 0.017). CONCLUSION: The results demonstrated the possibility of synergism between EBV infection and EBV-HPV co-infection in genital epithelial lesions, mainly among HIV-infected patients. However, further investigations with a more specific and sensitive methodology are required in order to assess the real influence of EBV on the pathogenesis of genital epithelial lesions.

INTRODUÇÃO: A carcinogênese caracteriza-se como um processo multifatorial, e a inativação da proteína p53 é uma alteração genética comumente observada nos carcinomas de células escamosas de boca (CCEB). OBJETIVO: Analisar e comparar a imunoexpressão da proteína p53, por meio dos clones DO-7 e PAb-240, em CCEB com localização intrabucal e em lábio inferior. MATERIAL E MÉTODOS: Foram selecionados 40 casos de CCEB, sendo 20 de localização intrabucal e 20 em lábio inferior. Foi realizado um estudo imuno-histoquímico utilizando os anticorpos anti-p53 clone DO-7 e PAb-240. A imunoquantificação foi realizada por meio de análise digital de imagem, e os resultados, submetidos a tratamentos estatísticos. RESULTADOS: A imunoexpressão da proteína p53 foi verificada com o anticorpo DO-7 em 13 casos (65%) de carcinoma intrabucal e em 19 (95%) de carcinoma de lábio inferior. Imunorreatividade para o anticorpo PAb-240 foi observada em 9 casos (45%) de lesões intrabucais e em 15 (75%) localizados em lábio inferior. Não foram observadas, segundo o teste de Mann-Whitney, diferenças estatisticamente significativas (p > 0,05) na expressão da proteína p53 entre as duas localizações estudadas, independentemente do anticorpo avaliado. Foram identificadas, pelo teste de Wilcoxon, diferenças estatisticamente significativas entre a expressão dos clones DO-7 e PAb-240 em cada um dos grupos analisados (valor p = 0,013 - lábio inferior; valor p = 0,016 - intrabucal). CONCLUSÕES: A expressão da proteína p53 observada nos CCEB, com localizações intrabucais e labiais, sugere a ocorrência de mutações no gene TP53. As diferenças quantitativas obtidas entre os anticorpos estudados, independentemente da localização das lesões, refletem uma especificidade distinta entre os clones DO-7 e PAb-240. O desenvolvimento de mais estudos será fundamental para estabelecer o anticorpo mais adequado para proteína p53 em CCEB. BACKGROUND: Carcinogenesis is a multifactorial process and inactivation of p53 protein is a genetic change commonly observed in oral squamous cell carcinomas (OSCC). OBJECTIVES: To analyze and compare the expression of p53 protein through antibodies DO-7 and PAb-240 in OSCC samples located in the oral cavity and lower lip. MATERIAL AND METHODS: Forty cases of OSCC were selected and divided into oral cavity and lower lip groups (20 cases each). Immunohistochemical technique was performed using antibodies DO-7 and PAb-240. Quantification of the cases was performed through digital image analysis and underwent specific statistical treatments. RESULTS: Expression of p53 protein was verified with DO-7 antibody in 13 cases (65%) of oral cavity carcinomas and in 19 cases (95%) of lower lip carcinoma. PAb-240 positivity was detected in 9 cases (45%) of oral cavity lesions and in 15 cases (75%) located in the lower lip. According to Mann-Whitney test, there were no statistically significant differences between the expressions of p53 protein in both groups, regardless of the antibody used. According to Wilcoxon test, there were statistically significant differences between the expression of DO-7 antibody and PAb-240 in each of the analyzed groups (p-value = 0.013; lower lip p-value = 0.016 - oral cavity). CONCLUSIONS: The expression of p53 protein was observed both in the oral cavity and lip OSCC, which suggests the occurrence of mutations in TP53 gene. The quantitative differences between the antibodies studied, regardless of the site of the lesions, reflect different specificity between clones DO-7 and PAb-240. Further studies are required to establish the best antibody for p53 protein in oral squamous cell carcinomas.

Pheochromocytoma is a neoplasia of chromaffin cells that is very rare in children. Its signs and symptoms result from the release of catecholamines. We report the case of a child with pheochromocytoma of difficult clinical management, confirmed on the anatomicopathological study, and cured after surgical resection.

INTRODUÇÃO: Líquen plano é uma doença inflamatória crônica relativamente comum que acomete pele e membranas mucosas, separadamente ou em combinação. Freqüentemente envolve a cavidade oral, com ou sem comprometimento cutâneo concomitante. Microscopicamente, enquanto a pele apresenta aspectos bastantes característicos, a mucosa oral pode apresentar variações relacionadas às características locais da mucosa na cavidade oral. OBJETIVOS: Comparar as alterações histopatológicas do líquen plano oral (LPO) com as do líquen plano cutâneo (LPC), estudando os componentes do infiltrado inflamatório. MATERIAL E MÉTODOS: 59 biópsias (29 de LPO e 30 de LPC ) fixadas em formol e incluídas em blocos de parafina. Foi realizada análise semiquantitativa das alterações epidérmicas e dérmicas utilizando-se as colorações de hematoxilina-eosina e ácido periódico Schiff. RESULTADOS: No LPC foram observados os aspectos clássicos de ortoceratose, hipergranulose em cunha e acantose em dente de serra. Tais aspectos diferem do LPO, no qual ortoceratose e camada granulosa espessada ocorrem com menos freqüência, observando-se aquisição destas camadas nos locais onde estão normalmente ausentes. Vacuolização e apagamento do limite epitélio-conjuntivo predominaram nas duas localizações (pele e mucosa oral), e linfócitos e histiócitos foram as células inflamatórias presentes em todos os casos. CONCLUSÕES: O conjunto de alterações clássicas como ortoceratose compacta, hipergranulose em cunha, acantose em dente de serra e vacuolização da basal, associadas a linfócitos em faixa superficial é diagnóstico de LPC. Para o LPO, no entanto, devem ser consideradas as características próprias da mucosa no local biopsiado, devendo ser valorizadas alterações mais sutis da camada córnea - leve ortoceratose - e aparecimento da camada granulosa em locais onde normalmente é ausente. Aspecto em cunha da hipergranulose é de ocorrência rara no LPO. Acantose em dente de serra é achado pouco freqüente no LPO, porém vacuolização basal está sempre presente associada a infiltrado linfocitário em faixa; plasmócitos podem acompanhar linfócitos no LPO, mas estão ausentes do LPC.

Oral hairy leukoplakia (OHL) is one of the most common oral manifestations of AIDS, with diagnostic and prognostic value. OHL is associated to the Epstein-Barr virus and presents clinical and histological defined characteristics. There have already been reports about a subclinical stage of OHL, although they lacked histopathologic characterization. The present study had the aim to describe the histopathological characteristics of subclinical hairy leukoplakia, as well as to carry out a comparative analysis between clinical and subclinical OHL. For that, 11 cases were analyzed - 5 biopsies from patients who presented with the lesion and 6 samples from the borders of tongues obtained in necropsies. The histopathological findings in subclinical OHL were: absence of parakeratosis and papillomatosis, mild acanthosis, ballooning cells and nuclear alterations. In situ hybridization and immunostaining were positive for EBV in the nuclear alterations identified in the histophatological analysis. Based on the identification of EBV in the nuclear alterations, it was possible to conclude that subclinical OHL, similarly to the clinical lesion, presents histopathological features that are specific and sufficient to establish the definitive diagnosis, regardless of the identification of the virus.
