Coronavirus disease 2019 (COVID-19) which is caused by severe acute respiratory distress syndrome virus (SARS-CoV-2) continues to affect people all around the world. This study aimed to compare the SARS-CoV-2 viral shedding time between patients diagnosed with hematologic diseases (HD) and a control group. A total of 110 patients were enrolled in this retrospective study; 55 patients with a diagnosis of HD and 55 sex and comorbidity matched controls without a diagnosis of HD, who caught COVID-19 at the same period. While promising results have been achieved in the treatment of childhood, the optimal initial treatment for adult acute lymphoblastic leukemia (ALL) has not yet been defined. Hyper-CVAD has become a widely used treatment for approximately 2 decades in the treatment of adult ALL. We conducted a retrospective analysis of 30 patients treated with Hyper-CVAD at two centers between 2014 and 2020. In all (n=30) patients treated with Hyper-CVAD, complete response (CR) rate was 86.7%, induction mortality was 10%, refractory disease was 3.3%, the median overall survival (OS) was 38 months (95% CI 7.78–68.2 months), the median disease-free survival (DFS) was 29 months (95% CI 9–49 months), the 2-year OS rate was 56.5%, and the 2-year DFS was rate 56.7%. For standard risk (n=12) ALL patients, the median OS was 20 months (95% CI 0–43 months), and median DFS was 7 months (95% CI 0–25 months). For high risk (n=18) ALL patients, the median OS was 38 months (95% CI 0–76 months), and median DFS was not reached. These results indicate that Hyper-CVAD regimen should be considered as an option for induction treatment of adult ALL patients who are newly diagnosed and eligible for allo-HSCT., Çocukluk çağı akut lenfoblastik lösemi (ALL) tedavisinde umut verici sonuçlar elde edilirken, yetişkin için optimal indüksiyon tedavisi henüz belirlenememiştir. Hyper-CVAD kemoterapi rejimi, yetişkin ALL tedavisinde yaklaşık yirmi yıldır yaygın olarak kullanılan bir tedavi haline gelmiştir. 2014 ve 2020 yılları arasında iki merkezde Hyper-CVAD rejimi ile tedavi edilen 30 hastanın retrospektif analizini gerçekleştirdik. Hyper-CVAD ile tedavi edilen hastalarda (n=30), tam yanıt oranı (TY) %86.7, indüksiyon mortalitesi %10, refrakter hastalık %3.3, medyan genel sağkalım (GS) 38 ay (%95 CI 7.78-68.2 ay), medyan hastalıksız sağkalım (HS) 29 ay (% 95 CI 9-49 ay), 2 yıllık GS oranı % 56.5 ve 2 yıllık HS oranı % 56.7 olarak saptadık. Standart risk (n=12) ALL hastalar için, medyan GS 20 ay (%95 CI 0-43 ay) ve medyan HS 7 ay (%95 CI 0-25 ay) saptadık. Yüksek riskli (n=18) ALL hastalar için, medyan GS 38 ay iken (%95 CI 0-76 ay), medyan HS’a ulaşılamadı. Bu sonuçlar, Hyper-CVAD rejiminin yeni tanı konmuş ve allojenik kök hücre transplantasyonu için uygun olan ALL hastalarının indüksiyon tedavisi için bir seçenek olarak değerlendirilmesi gerektiğini göstermektedir.

Amaç: Allojenik kök hücre nakli (AKHN), birçok hematolojik malignite için, graft-versus-host hastalığı (GVHH) gibi bazı hayatı tehdit edici risklerine rağmen halen tek küratif tedavi yöntemidir. GVHH post-transplant siklofosfamid uygulamasıyla birlikte haploidentik kök hücre nakli yapılan hastalarda dahi kontrol altına alınabilmektedir. Çalışmamızın amacı da tam uyumlu kardeş donörden yapılan nakillerde post-transplant siklofosfamid kullanımının GVHH üzerine olan etkinliğini standart immünosüpresif tedaviyle karşılaştırmaktır. Gereç ve Yöntem: Yüksek riskli hematolojik malignite tanısı olup tam uyumlu kardeş donörden AKHN yapılan hastalar çalışmaya alındı. Post- transplant siklofosfamid tedavisi alan hastalar, immünosüpresif tedavi olarak ayrıca takrolimus ve mikofenolat mofetil aldı; diğer kolda ise hastalar metotreksat ve siklosporin ile standart immünosüpresif tedavi aldı. Çalışmanın primer sonlanım noktası şiddetli akut GVHH gelişimi sıklığı olarak belirlendi. Bulgular: Çalışmaya toplam 40 hasta alındı. Standart immünosüpresif kolunda yalnızca 3 hastada şiddetli (grade 3-4) GVHH görülürken, post- transplant siklofosfamid alan grupta hiç görülmedi. Hastaların 1 yıllık progresyonsuz sağkalım ve genel sağkalım oranları standart immünosüpresif tedavi alanlar için %54 ve %62, post-transplant siklofosfamid alan hastalar için %78,8 ve %93,3 olarak izlendi. Sonuç: Post-transplant siklofosfamid, ucuz, kullanılabilir ve klinik olarak etkili bir GVHH profilaksi ajanı olarak kök hücre naklinde kullanılabilir.

Objective: The cases of diffuse large B-cell lymphoma, (DLBCL not otherwise specified (NOS)) which immunohistochemicallyexhibit MYC and BCL2 expressions are defined as double-expressor lymphomas (DELs). This study aimed to assess the prognosticimpact of DEL and the expressions of other proteins that may have role in tumorogenesis.Materials and Methods: In this study, 90 tumor samples from patients diagnosed with DLBCL NOS were evaluated retrospectively.Immunoexpressions of MYC, BCL2, activation-induced cytidine deaminase (AID), uracil-DNA glycosylase (UNG) and DNAmismatch repair proteins including MLH1, MSH2, MSH6 and PMS2 were analyzed.Result: Eleven cases (12.2%) which exhibited ≥40% MYC and ≥50% BCL2 immunexpressions were classified as DEL DLBCL. Patientswith MYC positivity displayed lower overall survival rate than MYC negative cases. A trend of lower overall survival was observed inthe double-expressor lymphoma group, however, this was not proven to be statistically significant. Significant relationship betweenAID, UNG and p53 immunexpressions with double-expressor lymphoma or overall survival was not detected. The correlationbetween immunexpressions of p53 and MYC was observed. The loss of expression of mismatch repair proteins was not observed inany cases.Conclusion: In this study, a relationship between low overall survival and MYC expression is detected. However, our result does notdemonstrate that double-expressor lymphoma can be associated with poor outcomes.
