To conduct a systematic review about risk factors associated with non-specific low back pain (LBP) in older people. The study protocol was prospectively registered with Prospero (CRD42020191619). This systematic review with meta-analysis included cohort studies that investigated risk factors for LBP in older people. The following databases were searched up to 12 December 2020: MEDLINE (Ovid), Embase, CINAHL, SCOPUS and Web of Science. Two independent reviewers appraised methodological quality using the Critical Appraisal Checklist for Cohort Studies instrument. We identified 3939 potentially relevant publications. After removing duplicates, screening title, and abstracts, we assessed 86 publications in full text. We included the remaining 11 publications for analysis. There is strong evidence that depressive symptoms are a risk of reporting future back pain onset (I2 = 52,7%, Odds ratio 1.4, CI 1.28–1.53). Depressive symptoms are a risk factor for LBP in older people. Due to the limitations of the literature, the role of some risk factors remains unclear. An additional high-quality prospective cohort is needed to better elucidate these relationships.

Contains fulltext : 241226.pdf (Publisher’s version ) (Closed access) BACKGROUND: Olaparib, a poly (ADP-ribose) polymerase (PARP) inhibitor, has previously been shown to extend progression-free survival versus placebo when given to patients with relapsed high-grade serous or endometrioid ovarian cancer who were platinum sensitive and who had a BRCA1 or BRCA2 (BRCA1/2) mutation, as part of the SOLO2/ENGOT-Ov21 trial. The aim of this final analysis is to investigate the effect of olaparib on overall survival. METHODS: This double-blind, randomised, placebo-controlled, phase 3 trial was done across 123 medical centres in 16 countries. Eligible patients were aged 18 years or older, had an Eastern Cooperative Oncology Group performance status at baseline of 0-1, had histologically confirmed, relapsed, high-grade serous or high-grade endometrioid ovarian cancer, including primary peritoneal or fallopian tube cancer, and had received two or more previous platinum regimens. Patients were randomly assigned (2:1) to receive olaparib tablets (300 mg in two 150 mg tablets twice daily) or matching placebo tablets using an interactive web or voice-response system. Stratification was by response to previous chemotherapy and length of platinum-free interval. Treatment assignment was masked to patients, treatment providers, and data assessors. The primary endpoint of progression-free survival has been reported previously. Overall survival was a key secondary endpoint and was analysed in all patients as randomly allocated. Safety was assessed in all patients who received at least one treatment dose. This trial is registered with ClinicalTrials.gov, NCT01874353, and is no longer recruiting patients. FINDINGS: Between Sept 3, 2013 and Nov 21, 2014, 295 patients were enrolled. Patients were randomly assigned to receive either olaparib (n=196 [66%]) or placebo (n=99 [34%]). One patient, randomised in error, did not receive olaparib. Median follow-up was 65·7 months (IQR 63·6-69·3) with olaparib and 64·5 months (63·4-68·7) with placebo. Median overall survival was 51·7 months (95% CI 41·5-59·1) with olaparib and 38·8 months (31·4-48·6) with placebo (hazard ratio 0·74 [95% CI 0·54-1·00]; p=0·054), unadjusted for the 38% of patients in the placebo group who received subsequent PARP inhibitor therapy. The most common grade 3 or worse treatment-emergent adverse event was anaemia (which occurred in 41 [21%] of 195 patients in the olaparib group and two [2%] of 99 patients in the placebo group). Serious treatment-emergent adverse events were reported in 50 (26%) of 195 patients receiving olaparib and eight (8%) of 99 patients receiving placebo. Treatment-emergent adverse events with a fatal outcome occurred in eight (4%) of the 195 patients receiving olaparib, six of which were judged to be treatment-related (attributed to myelodysplastic syndrome [n=3] and acute myeloid leukaemia [n=3]). INTERPRETATION: Olaparib provided a median overall survival benefit of 12·9 months compared with placebo in patients with platinum-sensitive, relapsed ovarian cancer and a BRCA1/2 mutation. Although statistical significance was not reached, these findings are arguably clinically meaningful and support the use of maintenance olaparib in these patients. FUNDING: AstraZeneca and Merck.

Community Health Agents (CHA) are essential for the work of Primary Health Care (PHC) to be successful, but they may encounter obstacles in their performance. The goal of the study was to understand the importance of Community Health Agents in the work of territorialization in the Municipality of Itaperuna-RJ. For this, an exploratory field research was carried out with the application of a questionnaire to the CHA of the municipality, respecting the ethical aspects recommended by Resolution 466/2012. The following inclusion criteria were established: CHA aged between 18 and 60 years and working in the CEHAB or Surubi communities. For exclusion: CHA who did not reside in the studied territory. Thus, 14 professionals participated in the study, with the following results predominating: they consider that the profession makes it possible to help the community (53.8%), but this help is limited (33.3%); carry out planning of their activities (83.3%); have already acquired diseases resulting from their work (86.7%), mainly Covid-19 (33.3%); have only elementary education (83.3%); the main factor that harms the professional relationship is misinformation (58.3%); the advantage of living and working in the same territory is the proximity (66.7%) and the disadvantage is the demand from the population outside working hours (41.7%). It was concluded that this professional is essential for the promotion of improvements, but there are several challenges, such as the fact that 50% CHA interviewed say that they did not receive the proper training. El objetivo del estudio fue comprender la importancia de los Agentes Comunitarios de Salud en el trabajo de territorialización en el Municipio de Itaperuna-RJ. Para ello, se realizó una investigación de campo exploratoria con la aplicación de un cuestionario a la ACS del municipio, respetando los aspectos éticos recomendados por la Resolución 466/2012. Se establecieron los siguientes criterios de inclusión: ACS con edad entre 18 y 60 años y que trabajen en las comunidades CEHAB o Surubi. Y por exclusión: ACS que no residían en el territorio estudiado. Así, 14 profesionales participaron del estudio, con predominio de los siguientes resultados: consideran que la profesión posibilita ayudar a la comunidad (53,8%), pero esa ayuda es limitada (33,3%); realizar la planificación de sus actividades (83,3%); ya han adquirido enfermedades derivadas de su trabajo (86,7%), principalmente Covid-19 (33,3%); tienen solo educación primaria (83,3%); el principal factor que perjudica la relación profesional es la desinformación (58,3%); la ventaja de vivir y trabajar en el mismo territorio es la proximidad (66,7%) y la desventaja es la demanda de la población fuera del horario laboral (41,7%). Se concluyó que este profesional es fundamental para la promoción de mejoras, pero existen varios desafíos, como el hecho de que el 50% de los ACS entrevistados dice que no recibió la formación adecuada. Neuropathic pain (NP) is a challenge due to our limited understanding of the mechanisms that initiate and maintain chronic pain. The prelimbic division (PrL) of the medial prefrontal cortex (mPFC) is an important area of the emotional and cognitive components of pain and pharmacological systems can interact into the neocortex to elaborate the chronic pain. This work aimed to investigate the pharmacological cross-talk between synaptic neurotransmission, neuroanatomical approaches and NP conditions. A bidirectional neural tract tracer, the 3000-molecular-weight biodextran (BDA) was microinjected into the PrL cortex. The mechanical withdrawal threshold (MWT) was recorded by a von Frey test, and the effect of prelimbic cortex CB1, NMDA, and TRPV1 receptor modulation was evaluated 21 days after chronic constriction injury (CCI) of the sciatic nerve in male Wistar rats. Microinjection of a bidirectional neurotracer in the PrL cortex showed connections with the lateral division of the mediodorsal thalamic nucleus (MDL), central division of the mediodorsal thalamic nucleus (MDC), centrolateral thalamic nucleus (CL), ventromedial thalamic nucleus (VM), and the paracentral thalamic nucleus (PC). In detail, AM251, a CB1 receptor antagonist (at 50, 100 and 200 pmol) microinjections intra-PrL cortex decreased the MWT. Administrations of 6-iodonordihydrocapsaicin (6-I-CPS), a transient receptor potential vanilloid type 1 (TRPV1) antagonist, at 3 nmol and the endocannabinoid anandamide (AEA) at 50 and 100 pmol increased the MWT. AEA at 200 pmol injected in the PrL cortex decreased the MWT, and this hyperalgesic effect was blocked by 6-I-CPS at 3 nmol. The AEA (at 100 pmol) anti-allodynic effect was attenuated by AM251 (at 5 pmol). The TRPV1 selective agonist N-oleoyldopamine (OLDA) at 10 μM decreased the MWT. The blockade of the NMDA receptor with LY235959 (at 8 nmol) and 6-I-CPS (at 3 nmol) reversed the OLDA (at 10 μM) hyperalgesic effect. These findings showed that the PrL cortex sends pathways to thalamic nuclei that can mediate the nociception. We also suggest that the PrL cortex is involved in the potentiation and maintenance of mechanical allodynia by NMDA and TRPV1 receptor activation and that attenuation of this allodynia depends on CB1 receptor activation during NP.

Introdução: O envelhecimento consiste em um processo natural nos seres humanos. No entanto, as alterações fisiológicas ocorridas nessas idades podem gerar mudanças nas funções mecânicas, físicas e bioquímicas dos idosos. Por esse viés, um dos principais impasses que atinge esses indivíduos, nos dias atuais, é a síndrome demencial. Analisar os prontuários de usuários com diagnóstico de demência e os fatores associados nas Unidades Básicas de Saúde selecionadas no município de Itaperuna-RJ. Metodologia: O estudo será do tipo transversal e os prontuários dos pacientes cadastrados nas Unidades de Saúde selecionadas no município de Itaperuna-RJ serão identificados e selecionados para a pesquisa. Os resultados obtidos irão apresentar os dados numéricos sobre a pesquisa. Posteriormente, serão analisados e comparados com as pesquisas bibliográficas sobre o tema. Logo, será realizada uma análise conclusiva do estudo científico. Resultado: Foram analisados 9 artigos sobre os fatores associados no critério de inclusão e exclusão. Discussão: Nos últimos anos, houve um crescente aumento no número de indivíduos acima dos 60 anos. Com isso, há necessidade de uma maior atenção da saúde pública a esse fato. Visto que o envelhecimento acarreta enfermidades adjacentes, como doenças crônicas que estão associadas à síndrome demencial. Logo, faz-se necessário um estudo mais detalhado sobre o tema. Conclusão: Sendo assim, fica evidente que o envelhecimento requer estudos e pesquisas aprofundados por se tratar de um caso de saúde pública. Isso se deve pelo fato de tal patologia causar impacto no modo de vida dos indivíduos.

Nasal dilators were created to expand the nasal valve area. The aim of this systematic review was to verify physiological parameters associated to running performance with the use of nasal dilators. This study was registered in PROSPERO (CRD42021225795). According to the PICOS framework studies were included: Population: healthy subjects; Intervention: nasal dilators; Comparison: control group, placebo, minimal intervention, health education or other intervention; Outcomes: cardiorespiratory parameters and subjective perceptions; Study: randomized controlled trials, repeated measures or within-subjects design. The databases searched were MEDLINE, EMBASE, CENTRAL The Cochrane Library, CINAHL, SPORTDiscus, Web of Science, PEDro and Scopus. The descriptors “Running”, “Nasal Dilator”, “Randomized Controlled Trial”, and synonyms were used. The risk of bias was assessed using the PEDro scale. Random effects Der Simonian and Laird model were used. The assessment of the certainty of the evidence was carried out using the GRADE approach. Eleven articles were included. There was a difference in favour of the nasal dilator when compared to placebo for maximal oxygen uptake and rating of perceived exertion. The certainty of the evidence was very low. Future studies will probably have an impact on estimation of the effect.

The load in resistance training is considered to be a critical variable for neuromuscular adaptations. Therefore, it is important to assess the effects of applying different loads on the development of maximal strength and muscular hypertrophy. The aim of this study was to systematically review the literature and compare the effects of resistance training that was performed with low loads versus moderate and high loads in untrained and trained healthy adult males on the development of maximal strength and muscle hypertrophy during randomized experimental designs. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines (2021) were followed with the eligibility criteria defined according to participants, interventions, comparators, outcomes, and study design (PICOS): (P) healthy males between 18 and 40 years old, (I) interventions performed with low loads, (C) interventions performed with moderate or high loads, (O) development of maximal strength and muscle hypertrophy, and (S) randomized experimental studies with between- or within-subject parallel designs. The literature search strategy was performed in three electronic databases (Embase, PubMed, and Web of Science) on 22 August 2021. Results: Twenty-three studies with a total of 563 participants (80.6% untrained and 19.4% trained) were selected. The studies included both relative and absolute loads. All studies were classified as being moderate-to-high methodological quality, although only two studies had a score higher than six points. The main findings indicated that the load magnitude that was used during resistance training influenced the dynamic strength and isometric strength gains. In general, comparisons between the groups (i.e., low, moderate, and high loads) showed higher gains in 1RM and maximal voluntary isometric contraction when moderate and high loads were used. In contrast, regarding muscle hypertrophy, most studies showed that when resistance training was performed to muscle failure, the load used had less influence on muscle hypertrophy. The current literature shows that gains in maximal strength are more pronounced with high and moderate loads compared to low loads in healthy adult male populations. However, for muscle hypertrophy, studies indicate that a wide spectrum of loads (i.e., 30 to 90% 1RM) may be used for healthy adult male populations.

In this paper, a finite-difference time-domain (FDTD) formulation, based on the exponential matrix and on thin material sheet methods is developed for modeling subcellular thin graphene sheets. This formulation is validated by reproducing graphene frequency selective surfaces (FSS) known from literature. Then, we propose in this work a smart graphene FSS device. Smartness is obtained by means of a unity cell formed by a graphene ring with a graphene sheet placed in its aperture. By properly regulating the chemical potentials of the graphene elements, two frequency- tunable modus operandi are obtained: single- or dual-band rejection modes. When the device operates in its dual-band rejection mode, either of the rejection bands can be shifted individually in the frequency spectrum. Additionally, both rejection bands can also be reconfigured simultaneously.

OBJECTIVEThe objective of this study was to describe a new experimental model of hemispherotomy performed on laboratory animals.METHODSTwenty-six male young adult Wistar rats were distributed into two groups (surgery and control). The nonfluorescent anterograde neurotracer biotinylated dextran amine (BDA; 10,000 MW) was microinjected into the motor cortex area (M1) according to The Rat Brain in Stereotaxic Coordinates atlas to identify pathways and fibers disconnected after the experimental hemispherectomy. SPECT tomographic images of 99mTc hexamethylpropyleneamine oxime were obtained to verify perfusion in functioning areas of the disconnected and intact brain. A reproducible and validated surgical procedure is described in detail, including exact measurements and anatomical relationships. An additional 30 rodents (n = 10 rats per group) were divided into naïve, sham, and hemispherotomy groups and underwent the rotarod test.RESULTSCortico-cortical neural pathways were identified crossing the midline and contacting neuronal perikarya in the contralateral brain hemisphere in controls, but not in animals undergoing hemispherotomy. There was an absence of perfusion in the left side of the brain of the animals undergoing hemispherotomy. Motor performance was significantly affected by brain injuries, increasing the number of attempts to maintain balance on the moving cylinder in the rotarod test at 10 and 30 days after the hemispherotomy, with a tendency to minimize the motor performance deficit over time.CONCLUSIONSThe present findings show that the technique reproduced neural disconnection with minimal resection of brain parenchyma in young adult rats, thereby duplicating the hemispherotomy procedures in human patients.

Background Resistance training has a positive impact on functional capacity and muscle mass in the elderly. However, due to physical limitations or a simple aversion against regular exercise, a majority of the elderly do not reach the recommended exercise doses. This led us to evaluate the effect of whole-body electromyostimulation (WB-EMS), a novel, time-efficient, and smooth training technology on physical function, fat-free mass, strength, falls-efficacy, and social participation of the elderly. Methods The present study is a randomized, parallel group clinical trial approved by the Ethics Committee of our Institution. Sixty-six volunteers (age ≥ 60 years) will be recruited from the geriatric outpatient department in a tertiary hospital and primary care units and randomized into two groups: WB-EMS group or active control group (aCG). The WB-EMS or aCG protocol will consist of 16 sessions for 8 consecutive weeks, twice per week. The primary outcomes will be maximal isometric knee extension (IKE), functional lower extremity strength, fat-free mass, gait speed, and risk of falls measured before and after intervention. The secondary outcomes will be social participation and falls-efficacy assessed before and after the intervention and at three and six months of follow-up. Participant’s satisfaction with and awareness of electrical stimulation therapy will also be assessed immediately after the 8-week intervention. Discussion Patients receiving WB-EMS exercises are believed to have better outcomes than those receiving conventional, more time-consuming resistance exercises. Hence, innovative, time-efficient, joint-friendly, and highly individualized exercise technologies (such as WB-EMS) may be a good choice for the elderly with time constraints, physical limitations, or little enthusiasm, who are exercising less than the recommended amounts for impact on muscle mass, strength, and function.

No Brasil, a corrida de rua é o segundo esporte mais praticado; entretanto, pode ocasionar lesões musculoesqueléticas. Estudos sobre o tema são importantes para nortear estratégias de prevenção e intervenção. Assim, o objetivo foi investigar a prevalência de lesões e os fatores associados em corredores de rua da cidade de Juiz de Fora(MG). Trata-se de um estudo observacional do tipo transversal. A amostra foi selecionada por conveniência. Foram incluídos corredores amadores, entre 18 e 60 anos e que realizavam treinos em diferentes locais da cidade. Foram excluídos participantes com histórico de fratura e osteossíntese em membros inferiores ou que praticassem outro esporte que não fosse a musculação. Para analisar os dados, foram utilizados os testes t de Student, Mann-Whitney e X2 (α=0,05). Os participantes foram alocados em grupo lesão (GL, n=37) e grupo sem lesão (GSL, n=113). Street running is an accessible, low-cost form of exercise. However, the occurrence of musculoskeletal injuries may hinder regular practice. This study aimed at estimating the prevalence of injuries in Brazilian street runners and the associated factors. A meta-analysis of Brazilian studies was performed to investigate the prevalence and risk factors of injuries in male and female recreational street runners aged ≥18 years. We excluded systematic review studies, research conducted on professional athletes or triathletes, and duplicate articles. The following databases were used: SciELO, LILACS, PubMed, Web of Science, and Google Scholar. Keywords such as “prevalence,” “injury,” “recreational street runners,” and “Brazil” were used. Prevalence analysis was performed using the random effect model, and a funnel plot was used to assess publication bias. Then the Begg-Mazumdar and Egger tests were applied to quantify the graph results. The Prevalence Critical Appraisal Instrument was used to evaluate the methodological quality of the studies. Associated factors were analyzed with meta-regression analysis. Twenty-three studies with 3,786 runners were included in the review. The prevalence of injury was 36.5% (95% confidence interval [CI] 30.8-42.5%), and a running distance per week greater than 20 km was a predictive variable of injuries. A higher prevalence of injuries was observed in men than in women (28.3%, 95% CI 22.5-35.0%), the knee was the most affected site of injury (32.9%, 95% CI 26.7-39.6%), and muscle injuries were the most frequent type of injury (27.9%, 95% CI 18.2-40.1%). This is the first national meta-analysis conducted to investigate the prevalence of injuries in recreational street runners. Although the prevalence of injuries was moderate, caution is required in terms of the weekly duration of running. Male runners are more susceptible, and muscle and knee injuries are the most common. No entanto, a ocorrência de lesões musculoesqueléticas pode dificultar a prática regular. O objetivo deste estudo foi estimar a prevalência de lesões em corredores de rua brasileiros e os fatores associados. Foi realizada uma metanálise de estudos brasileiros para investigara prevalência e os fatores de risco de lesão em corredores de rua amadores, de ambos os sexos, com idade ≥ 18 anos. Observou-se maior prevalência de lesões em homens do que em mulheres (28,3%, IC de 95% 22,5-35,0%). O joelho foi o local mais acometido (32,9%, (IC de 95% 26,7-39,6%) e as lesões musculares foram as mais frequentes (27,9%, IC de 95% 18,2-40,1%). Esta é a primeira metanálise nacional a investigar a prevalência de lesões em corredores de rua amadores. Embora a prevalência de lesões tenha sido moderada, é preciso ter cautela em termos da duração semanal da corrida. RESUMEN La carrera de calle es una modalidad de accesible y de bajo costo. Sin embargo, la aparición de lesiones musculo esqueléticas puede dificultar la práctica regular. El objetivo de este estudio fue estimar la prevalencia de lesiones en corredores de calle brasileños y los factores asociados. Se realizó un meta-análisis de estudios brasileños que investigaron la prevalencia y los factores de riesgo de lesión en corredores de calle aficionados, de ambos sexos, con edad ≥ 18 años. Se excluyeron estudios de revisión sistemática, investigaciones con atletas profesionales o triatletas y los artículos duplicados. Las búsquedas electrónicas se realizaron en las siguientes bases de datos: SciELO, LILACS, PubMed, Web of Science y Google Académico. Se utilizaron descriptores como “prevalencia”, “lesión”, “corredores de calle aficionados” y “Brasil”. El análisis de prevalencia se realizó con el modelo de efecto aleatorio, y un gráfico de embudo se utilizó para evaluar el sesgo de publicación. A continuación, se aplicaron las pruebas Begg-Mazumdar y Egger para cuantificar los resultados del gráfico. El Prevalence Critical Appraisal Instrument se utilizó para evaluar la calidad metodológica de los estudios. Los factores asociados fueron analizados con análisis de meta-regresión. Se incluyeron 23 estudios en la revisión, totalizando 3.786 corredores, con prevalencia de lesiones del 36,5% (intervalo de confianza [IC] del 95% 30,8-42,5%), y la distancia recorrida por semana superior a 20 Km fue una variable predictiva de lesiones. Se observó mayor prevalencia de lesiones en hombres que en mujeres (28,3%, IC del 95% 22,5-35,0%). La rodilla fue el local más afectado (32,9%, IC del 95% 26,7-39,6%) y las lesiones musculares fueron las más frecuentes (27,9%, IC del 95% 18,2-40,1%). Este es el primer meta-análisis nacional en investigar la prevalencia de lesiones en corredores aficionados. Aunque la prevalencia de lesiones ha sido moderada, se requiere precaución en cuanto a la duración semanal de la carrera.
