Abstract

AbstractWomen’s sexual and reproductive health plays an important role in fulfilling human rights. Even during Covid-19 pandemic, it is important to fulfill sexual and reproductive health rights. However, many human rights violations occur among minority groups, especially women in cases of abortion. During pandemic, based on data The National Population and Family Planning Agency (BKKBN) of Indonesia, there has been a decrease by 40% in contraceptive use and an increase in unwanted pregnancies by 15–30%. The number of unwanted pregnancies will increase and make abortion an option for every woman. Social restrictions in Covid-19 situation create challenges and vulnerabilities for women, which has had a significant impact from the pandemic in fulfilling social justice. This article aims to analysis how abortion during Covid-19 is handled based on a human rights approach and how Indonesia as a state to fulfills its responsibilities in overcoming the problem of abortion during Covid-19. The research method used qualitative study by combining doctrinal legal research and empirical legal research. It was concluded that the rise in abortion cases during the Covid-19 pandemic shows that the state has not been able to fulfill the state’s responsibility to fulfill, to respect and to protect human rights, especially women’s sexual and reproductive health rights.

Abstract

Background: The diagnostic accuracy of colon capsule endoscopy (CCE) depends on a well-cleansed bowel. Evaluating the cleansing quality can be difficult with a substantial interobserver variation. Objectives: Our primary aim was to establish a standard of agreement for bowel cleansing in CCE based on evaluations by expert readers. Then, we aimed to investigate the interobserver agreement on bowel cleansing. Design: We conducted an interobserver agreement study on bowel cleansing quality. Methods: Readers with different experience levels in CCE and colonoscopy evaluated bowel cleansing quality on the Leighton–Rex scale and Colon Capsule CLEansing Assessment and Report (CC-CLEAR), respectively. All evaluations were reported on an image level. A total of 24 readers rated 500 images on each scale. Results: An expert opinion-based agreement standard could be set for poor and excellent cleansing but not for the spectrum in between, as the experts agreed on only a limited number of images representing fair and good cleansing. The overall interobserver agreement on the Leighton–Rex full scale was good (intraclass correlation coefficient (ICC) 0.84, 95% CI (0.82–0.85)) and remained good when stratified by experience level. On the full CC-CLEAR scale, the overall agreement was moderate (ICC 0.62, 95% CI (0.59–0.65)) and remained so when stratified by experience level. Conclusion: The interobserver agreement was good for the Leighton–Rex scale and moderate for CC-CLEAR, irrespective of the reader’s experience level. It was not possible to establish an expert-opinion standard of agreement for cleansing quality in CCE images. Dedicated training in using the scales may improve agreement and enable future algorithm calibration for artificial intelligence supported cleansing evaluation. Trial registration: All included images were derived from the CAREforCOLON 2015 trial (Registered with The Regional Health Research Ethics Committee (Registration number: S-20190100), the Danish data protection agency (Ref. 19/29858), and ClinicalTrials.gov (registration number: NCT04049357)).

Agar penyelenggaraan pemerintahan daerah dapat mewujudkan efisiensi dan efektifitas meningkatkan pembangunan dan kesejahteraan masyarakat, sebagaimana diamanatkan a/eh UU No.32 Tahun 2004 tentang Pemerintahan Daerah, pemerintah wajib melakukan pembinaan dan pengawasan terhadap penyelenggaraan pemerintahan daerah. Dalam kenyataannya, begitu banyak institusi yang merasa berkepentingan melaksanakan pengawasan terhadap penyelenggaraan pemerintahan daerah yang perlu kiranya diketahui efektifitasnya. Untuk itu perlu penelitian pelaksanaan pembinaan dan pengawasan dalam rangka meningkatkan sinergitas dan efisiensi penyelenggaraan pemerintahan daerah. Metode penelitian digunakan metode survey untuk dapat menggambarkan fenomena dan mengungkapkan hubungan antar variabel. Hasil penelitian menunjukkan perlu suatu kebijakan untuk mensinergikan seluruh jajaran pengawasan, kejaksaan, kepolisian, KPK, dan lain-lain yang diatur dengan peraturan perundang-undangan.

Abstract

Abstract The accurate measurement of agricultural productivity and its consequential effects on the welfare of farmers plays a pivotal role in economies heavily dependent on agriculture. This study delves into the repercussions of employing standard weights and measures on both productivity and welfare in the Ruvuma Region of Tanzania. Employing a cross-sectional design, primary data were collected from 557 households selected at random from six districts within Ruvuma region. Utilizing negative binomial and multiple regression models, the study scrutinizes the impact of standard weights and measures on maize productivity and farmer welfare. The findings elucidate a noteworthy correlation between the adoption of standard units and amplified maize productivity and cash income. Farmers utilizing standard measurements in maize transactions exhibited a 19.5% higher yield per acre and a 14.5% improvement in welfare compared to those adhering to traditional measuring methods. Influential factors such as sex, the quantity of maize produced, and land size significantly influenced the adoption of standard units. Recommending empowerment initiatives through government agencies and pertinent organizations, this study advocates for targeted training and sensitization to redirect small-holder farmers from traditional measuring practices. Such endeavours align with Sustainable Development Goal 2, contributing to the overarching aim of achieving zero hunger by 2030.

A concurrent vitamin B12 deficiency is not well described and may be related to a rare gastrointestinal manifestation of sarcoid and Lofgren’s syndrome. We describe a case of a 57-year-old male presented with migratory polyarthritis, erythemic nodules, edema of his legs and fever. His laboratory tests showed anemia with a profound vitamin B12 deficiency. Imaging demonstrated bilateral hilar adenopathy. Pathology revealed non-necrotizing granulomas consistent with sarcoidosis. The patient was started on prednisone and vitamin B12 supplements with improvement of his complaints and vitamin B12 levels. Sarcoidosis can manifest in many extrapulmonary organs, including the gastrointestinal tract, resulting in nutritional deficiencies, such as vitamin B12 deficiency. Treatment of these nutritional deficiencies includes treatment with steroids, as well as vitamin supplementation. We suggest this case to be a rare manifestation of gastrointestinal involvement in Lofgren syndrome; however, a biopsy from the GI tract was not performed to confirm the diagnosis. An informed consent was obtained from the patient. Abstract

Video capsule endoscopy (VCE) of the small-bowel has been proven to accurately diagnose small-bowel inflammation and to predict future clinical flares among patients with Crohn’s disease (CD). In 2017, the panenteric capsule (PillCam Crohn’s system) was introduced for the first time, enabling a reliable evaluation of the whole small and large intestines. The great advantage of visualization of both parts of the gastrointestinal tract in a feasible and single procedure, holds a significant promise for patients with CD, enabling determination of the disease extent and severity, and potentially optimize disease management. In recent years, applications of machine learning, for VCE have been well studied, demonstrating impressive performance and high accuracy for the detection of various gastrointestinal pathologies, among them inflammatory bowel disease lesions. The use of artificial neural network models has been proven to accurately detect/classify and grade CD lesions, and shorten the VCE reading time, resulting in a less tedious process with a potential to minimize missed diagnosis and better predict clinical outcomes. Nevertheless, prospective, and real-world studies are essential to precisely examine artificial intelligence applications in real-life inflammatory bowel disease practice.

Abstract

This study aimed to: i) compare the number of decisions made in official matches between youth academy and professional soccer players and ii) compare the quality and speed of decision-making between youth academy and professional players. In experiment 1 we assessed 12 soccer matches (7 professional and 5 academy matches) through notational analysis. In experiment 2 we assessed 138 soccer players from professional (n = 42) and academy (n = 96) levels. The TacticUP® was used to assess game reading and decision-making skills. Results indicated that professional players make around 56% more decisions than youth academy players in official matches of their respective tournaments. In addition, professional players are quicker in making decisions for all match situations: with the ball (offensive phase), without the ball (offensive and defensive phases), near and distant from the ball. Based on the results, we verified that both in situ (official match) and in vitro (controlled test) situations the speed of decision-making was a distinguishing feature of performance between professional and youth academy players. Abstract

Background: Video capsule endoscopy (VCE) has been proven to accurately diagnose small-bowel inflammation and predict flares among patients with quiescent Crohn’s disease (CD). However, data regarding its predictive role in this population over an extended follow-up are scarce. Objectives: To predict clinical exacerbation and to assess the yield of Lewis score in identifying CD patients with future clinical exacerbation during an extended follow-up (>24 months). Design: A post hoc analysis study. Methods: Adult patients with quiescent small-bowel CD who were followed with VCE, inflammatory biomarkers and magnetic resonance enterography in a prospective study (between 2013 and 2018). We extracted extended clinical data (up to April 2022). The primary composite outcome (i.e. clinical exacerbation) was defined as intestinal surgery, endoscopic dilation, CD-related admission, corticosteroid administration, or biological/immunomodulator treatment change during follow-up. Results: Of the 61 patients in the study [median age 29 (24–37) years, male 57.4%, biologic treatment 46.7%], 18 patients met the primary outcome during an extended follow-up [median 58.0 (34.5–93.0) months]. On univariable analysis, complicated [hazard ratio (HR) 7.348, p = 0.002] and stricturing disease phenotype (HR 5.305, p = 0.001) were associated with higher risk for clinical exacerbation during follow-up. A baseline VCE middle small-bowel segment Lewis score (midLS) ⩾ 135 identified patients with future exacerbation [AUC (area under the curve) 0.767, 95% confidence interval (CI) 0.633–0.902, p = 0.001, HR 6.317, 93% negative predictive value], whereas the AUC of the conventional Lewis score was 0.734 (95% CI: 0.589–0.879, p = 0.004). Sensitivity analysis restricted to patients with either complicated ( n = 34) or stricturing ( n = 26) disease phenotype revealed that midLS still predicted clinical exacerbation during follow-up (AUC 0.747/0.753, respectively), in these patients. Conclusion: MidLS predicts treatment failure in quiescent CD patients (median follow-up of 5 years) independently of disease phenotype.

Background: Deep learning techniques can accurately detect and grade inflammatory findings on images from capsule endoscopy (CE) in Crohn’s disease (CD). However, the predictive utility of deep learning of CE in CD for disease outcomes has not been examined. Objectives: We aimed to develop a deep learning model that can predict the need for biological therapy based on complete CE videos of newly-diagnosed CD patients. Design: This was a retrospective cohort study. The study cohort included treatment-naïve CD patients that have performed CE (SB3, Medtronic) within 6 months of diagnosis. Complete small bowel videos were extracted using the RAPID Reader software. Methods: CE videos were scored using the Lewis score (LS). Clinical, endoscopic, and laboratory data were extracted from electronic medical records. Machine learning analysis was performed using the TimeSformer computer vision algorithm developed to capture spatiotemporal characteristics for video analysis. Results: The patient cohort included 101 patients. The median duration of follow-up was 902 (354–1626) days. Biological therapy was initiated by 37 (36.6%) out of 101 patients. TimeSformer algorithm achieved training and testing accuracy of 82% and 81%, respectively, with an Area under the ROC Curve (AUC) of 0.86 to predict the need for biological therapy. In comparison, the AUC for LS was 0.70 and for fecal calprotectin 0.74. Conclusion: Spatiotemporal analysis of complete CE videos of newly-diagnosed CD patients achieved accurate prediction of the need for biological therapy. The accuracy was superior to that of the human reader index or fecal calprotectin. Following future validation studies, this approach will allow for fast and accurate personalization of treatment decisions in CD.

Abstract

This paper aims to offer new evidence as to how sub-related party transactions (RPTs) can be related to corporate governance for Indonesia's business group. We address an ongoing theoretical tension and some recent research in the RPTs literature by focusing on revenue, expenses, loans, and receivables. Business groups are classified by size or market capitalization. This paper examines whether RPTs in the business group relate with domestic/foreign shareholders, independent board/commissioner, and firm size as controlling factors. The business groups were selected through purposive sampling that met the analysis criteria with their typology in the population of business groups listed on IDX. We used panel data analysis for four models. This relationship is more pronounced than some recent research for business group firms and firms with more highly concentrated foreign ownership regarding the effect RPTs on revenue, expenses, loans, and receivables. Related to the controlling variable, firm size shows a significant effect on every sub RPTs. The results may imply that foreign ownership exploits Indonesia with expenses such as cross-border transactions of capital goods, intangible property (royalty), intra-firm services, and the cost of debt. Therefore, there is a need for a balanced interest for government and business in Indonesia via foreign direct investment with corporate governance implementation and adaptive regulation.

Methods and results A prospective single‐centre cohort study of HTx recipients and LVAD‐supported patients who received a two‐dose SARSCoV‐2 mRNA vaccine (BNT162b2, Pfizer‐BioNTech). Whole blood for anti‐spike IgG (S‐IgG) antibodies were drawn at 6 months after the first vaccine dose. S‐IgG data at 6 weeks were available for a subgroup of HTx recipients. S‐IgG ≥ 50 AU/mL were interpreted positive. The cohort included 53 HTx recipients and 18 LVAD‐supported patients. The median time from HTx or LVAD implantation to the 1st vaccine dose was 90 (IQR 30, 172) months and 22 (IQR 6, 78) months, respectively. The seropositivity rates of S‐IgG antibodies and their titre levels in HTx recipients and LVAD‐supported patients were 45% and 83% respectively, (P = 0.006), and 35 (IQR 7, 306) AU/mL and 311 (IQR 86, 774) AU/mL, respectively, (P = 0.006). Reduced SARSCoV‐2 vaccine immunogenicity in HTx recipients was associated with older age [odds ratio (OR) 0.917 confidence interval (CI 0.871, 0.966), P = 0.011] and with the use of anti‐metabolites‐based immunosuppressive regimens [OR 0.224 (CI 0.065, 0.777), P = 0.018]. mTOR inhibitors were associated with higher immunogenicity [OR 3.1 (CI 1.01, 9.65), P = 0.048]. Out of 13 HTx recipients who were S‐IgG seropositive at 6 weeks after the first vaccine dose, 85% remained S‐IgG seropositive at 6 month follow‐up. Conclusions At 6 months post‐vaccination, S‐IgG immunogenicity in HTx recipients is low, particularly in older HTx recipients and in those treated with anti‐metabolites drugs.
