Patients with lupus anticoagulants are at high risk of systemic arterial and venous thrombosis and arterial stroke. We present an unusual case of a young man presenting inguinal pain. Doppler ultrasound revealed spermatic vein thrombosis on the left side. Hematologic workup revealed positive lupus anticoagulant. The patient was treated with therapeutic heparin.

A 68-year-old man presented with priapism and penile gangrene. The patient had no history of penis trauma or medications for erectile dysfunction. Corpus cavernosa aspiration cytology were positive for malignant cells. Total penectomy was performed. Enhanced chest and abdominal computed tomography showed a left renal tumor with pulmonary and hepatic metastases. Ultrasound-guided renal biopsy showed clear cell renal cell carcinoma.

Resume Objectifs Faire le point sur une localisation rare de faux anevrisme compliquant une maladie de Behcet et insister sur la necessite d’une surveillance vasculaire systematique dans cette maladie. Cas clinique Nous presentons le cas d’un jeune homme avec maladie de Behcet revelee par une thrombose veineuse cerebrale et qui a ete pris en charge un an plus tard en urgence pour un faux-anevrisme symptomatique de l’artere iliaque interne. Resultats Le faux anevrisme a ete traite par mise a plat avec ligature de l’artere iliaque interne, apres une cure de corticotherapie en bolus de 3 jours. Un traitement associant steroides et immunosuppresseurs a ete entrepris par la suite. Discussion L’atteinte arterielle au cours de la maladie de Behcet peut se manifester par des thromboses mais surtout par des faux anevrismes rompus ou non. L’atteinte de l’artere iliaque interne est exceptionnelle, la revue de la litterature n’ayant montre que deux cas. Le traitement de ces faux anevrismes est particulierement difficile en raison de la fragilite des parois arterielles et la necessite d’un traitement immunosuppresseur prolonge. Conclusion Cette manifestation clinique rare souligne l’importance de la surveillance clinique et radiologique vasculaire systematique des patients atteints de la maladie de Behcet.

This case illustrates a very rare localization of false aneurysm in Behçet's disease and demonstrates the importance of monitoring treated patients.The diagnosis of severe Behçet's disease was established in a young man after discovery of a cerebral venous thrombosis. One year later, the patient required emergency surgery for a symptomatic pseudoaneurysm of the internal iliac artery.Performed after a 3-day regimen of corticosteroid boluses, the pseudoaneurysm was treated by ligation of the hypogastric artery. Corticosteroids were then given for immunosuppression.Arterial involvement in Behçet's disease can be at the forefront of the clinical features and can cause potentially fatal complications. False aneurysm of the internal iliac artery is exceptional with only two cases reported in the literature. These false aneurysms occur on a fragile artery wall. Treatment requires the administration of immunosuppressive drugs.This rare clinical presentation highlights the importance of clinical and radiological monitoring in young patients with Behçet's disease.
