BackgroundBRCA1/2 mutation carriers are recommended to undergo risk-reducing salpingo-oophorectomy (RRSO) at 35 to 45 years of age. RRSO substantially decreases ovarian cancer risk, but at the cost of immediate menopause. Knowledge about the potential adverse effects of premenopausal RRSO, such as increased risk of cardiovascular disease, osteoporosis, cognitive dysfunction, and reduced health-related quality of life (HRQoL), is limited. ObjectiveThe aim of this study is to assess the long-term health effects of premenopausal RRSO on cardiovascular disease, bone health, cognitive functioning, urological complaints, sexual functioning, and HRQoL in women with high familial risk of breast or ovarian cancer. MethodsWe will conduct a multicenter cross-sectional study with prospective follow-up, nested in a nationwide cohort of women at high familial risk of breast or ovarian cancer. A total of 500 women who have undergone RRSO before 45 years of age, with a follow-up period of at least 10 years, will be compared with 250 women (frequency matched on current age) who have not undergone RRSO or who have undergone RRSO at over 55 years of age. Participants will complete an online questionnaire on lifestyle, medical history, cardiovascular risk factors, osteoporosis, cognitive function, urological complaints, and HRQoL. A full cardiovascular assessment and assessment of bone mineral density will be performed. Blood samples will be obtained for marker analysis. Cognitive functioning will be assessed objectively with an online neuropsychological test battery. ResultsThis study was approved by the institutional review board in July 2018. In February 2019, we included our first participant. As of November 2020, we had enrolled 364 participants in our study. ConclusionsKnowledge from this study will contribute to counseling women with a high familial risk of breast/ovarian cancer about the long-term health effects of premenopausal RRSO. Objective: To study the impact of premenopausal risk‐reducing salpingo‐oophorectomy (RRSO), compared with postmenopausal RRSO, on urinary incontinence (UI) ≥10 years later. Design: Cross‐sectional study, nested in a nationwide cohort. Setting: Multicentre in the Netherlands. Population: 750 women (68% BRCA1/2 pathogenic variant carriers) who underwent either premenopausal RRSO (≤45 years, n = 496) or postmenopausal RRSO (≥54 years, n = 254). All participants were ≥55 years at the time of the study. Methods: Urinary incontinence was assessed by the urinary distress inventory‐6 (UDI‐6); a score ≥33.3 indicated symptomatic UI. The incontinence impact questionnaire short form (IIQ‐SF) was used to assess the impact on women's health‐related quality of life (HR‐QoL). Differences between groups were analysed using regression analyses adjusting for current age and other confounders. Main outcome measures: Differences in UDI‐6 scores and IIQ‐SF scores between women with a premenopausal and a postmenopausal RRSO. Results: Women in the premenopausal RRSO group had slightly higher UDI‐6 scores compared with women in the postmenopausal RRSO group (P = 0.053), and their risk of symptomatic UI was non‐significantly increased (odds ratio [OR] 2.1, 95% confidence interval [95% CI] 0.93–4.78). A premenopausal RRSO was associated with a higher risk of stress UI (OR 3.5, 95% CI 1.2–10.0) but not with urge UI. The proportions of women with a significant impact of UI on HR‐QoL were similar in the premenopausal and postmenopausal RRSO groups (10.4% and 13.0%, respectively; P = 0.46). Conclusions: More than 15 years after premenopausal RRSO, there were no significant differences in overall symptomatic UI between women with a premenopausal and those with a postmenopausal RRSO. Aims: Heart failure outcomes remain poor despite advances in therapy. The European Society of Cardiology Heart Failure III Registry (ESC HF III Registry) aims to characterize HF clinical features and outcomes and to assess implementation of guideline‐recommended therapy in Europe and other ESC affiliated countries. Methods: Between 1 November 2018 and 31 December 2020, 10 162 patients with chronic or acute/worsening HF with reduced, mildly reduced, or preserved ejection fraction were enrolled from 220 centres in 41 European or ESC affiliated countries. The ESC HF III Registry collected data on baseline characteristics (hospital or clinic presentation), hospital course, diagnostic and therapeutic decisions in hospital and at the clinic visit; and on outcomes at 12‐month follow‐up. These data include demographics, medical history, physical examination, biomarkers and imaging, quality of life, treatments, and interventions – including drug doses and reasons for non‐use, and cause‐specific outcomes. Conclusion: The ESC HF III Registry will provide comprehensive and unique insight into contemporary HF characteristics, treatment implementation, and outcomes, and may impact implementation strategies, clinical discovery, trial design, and public policy. Objective: To examine the effect of a premenopausal risk-reducing salpingo-oophorectomy (RRSO) in women at increased risk of ovarian cancer on objective and subjective cognition at least 10 years after RRSO.Design: A cross-sectional study with prospective follow-up, nested in a nationwide cohort.Setting: Multicentre in the Netherlands.Population or Sample: 641 women (66% BRCA1/2 pathogenic variant carriers) who underwent either a premenopausal RRSO ≤ age 45 (n = 436) or a postmenopausal RRSO ≥ age 54 (n = 205). All participants were older than 55 years at recruitment.Methods: Participants completed an online cognitive test battery and a questionnaire on subjective cognition. We used multivariable regression analyses, adjusting for age, education, breast cancer, hormone replacement therapy, cardiovascular risk factors and depression.Main Outcome Measures: The influence of RRSO on objective and subjective cognition of women with a premenopausal RRSO compared with women with a postmenopausal RRSO.Results: After adjustment, women with a premenopausal RRSO (mean time since RRSO 18.2 years) performed similarly on objective cognitive tests compared with women with a postmenopausal RRSO (mean time since RRSO 11.9 years). However, they more frequently reported problems with reasoning (odds ratio [OR] 1.8, 95% confidence interval [95% CI] 1.1–3.1) and multitasking (OR 1.9, 95% CI 1.1–3.4) than women with a postmenopausal RRSO. This difference between groups disappeared in an analysis restricted to women of comparable ages (60–70 years).Conclusions: Reassuringly, approximately 18 years after RRSO, we found no association between premenopausal RRSO and objective cognition.

Microorganisms give an early and integrated measure of soil functioning. In particular, soil microbial respiration is recommended for monitoring soil quality. The present study aims to determine the capacity of Albit® (poly-β-hydroxybutyrate, PHB) to reduce the detrimental effects of pesticides on soil microbial respiration. The effects of three conventional pesticides (deltamethrin, dithianon, and difenoconazole) on basal respiration (BR) and substrate-induced respiration (SIR) were assessed in the presence and absence of Albit®. The studied pesticides caused negative impacts on soil functioning, reducing BR and SIR. Applications of Albit® increased BR and SIR, and both BR and SIR were kept similar to the control when pesticides were applied with Albit®. PHB, an active ingredient of Albit®, is known to increase beneficial microflora in the rhizosphere due to its regulatory activity on indigenous microorganisms. Thus, more studies should be carried out under different edaphoclimatic conditions to study the benefits of Albit® applications along with pesticides in order to mitigate their side effects on soil microbial functioning.

- Abstract
AIMS Heart failure outcomes remain poor despite advances in therapy. The European Society of Cardiology Heart Failure III Registry (ESC HF III Registry) aims to characterize HF clinical features and outcomes and to assess implementation of guideline-recommended therapy in Europe and other ESC affiliated countries. METHODS Between 1 November 2018 and 31 December 2020, 10 162 patients with chronic or acute/worsening HF with reduced, mildly reduced, or preserved ejection fraction were enrolled from 220 centres in 41 European or ESC affiliated countries. The ESC HF III Registry collected data on baseline characteristics (hospital or clinic presentation), hospital course, diagnostic and therapeutic decisions in hospital and at the clinic visit; and on outcomes at 12-month follow-up. These data include demographics, medical history, physical examination, biomarkers and imaging, quality of life, treatments, and interventions - including drug doses and reasons for non-use, and cause-specific outcomes. CONCLUSION The ESC HF III Registry will provide comprehensive and unique insight into contemporary HF characteristics, treatment implementation, and outcomes, and may impact implementation strategies, clinical discovery, trial design, and public policy.

Purpose To evaluate and compare the outcomes of live surgery (LS) and no-live surgery (NLS) on cataract surgery with implantation of different types of intraocular lenses (IOLs). Methods Retrospective, contralateral eye, case series of patients that underwent cataract surgery in live or non-live view during two consecutive editions of national meetings. Both eyes of the same patients were implanted with the same IOL, one in LS and the other in NLS. Results 108 eyes of 54 patients, aged between 50 and 82 (72 ± 8.2) and implanted with different types of IOLs, were reviewed. Both eyes in each patient were well matched at baseline in terms of intraocular biometric characteristics, corneal curvature and endothelial cell density (ECD) (p > 0.05). There are no statistically significant differences between the biometric and topographic parameters, aberrometric data and the loss of ECD in the post-operative outcomes (p > 0.05). However, comparing the different types of IOLs, there is a significant loss of ECD in eyes implanted with a toric IOL during LS (p = 0.0014 and p = 0.04, in 2017 and 2018 edition respectively). Conclusions In this series of live cataract surgery, eyes operated in LS or NLS have comparable outcomes, underlying the importance and the benefits of live view in terms of medical education and the low-risk of complications. Nevertheless, we have found a significant increase in ECD loss in patients implanted with toric IOLs during LS.

BACKGROUND: Allergic rhinitis is a common IgE-mediated disease. Multiple mechanisms are involved in the regulation of IgE synthesis, and cytokines produced by immune cells play an important role in this process. In addition, the study of the features of immunological reactivity in seasonal allergic rhinitis is of interest both for getting new data about the pathogenesis of the disease and optimization of the treatment. AIMS: This study aimed to determine the cytokine status and hematological parameters in patients with seasonal allergic rhinitis outside the exacerbation period. MATERIALS AND METHODS: This study included 43 adult patients with seasonal allergic rhinitis (remission stage) and compared them with 47 healthy individuals. Patients with perennial symptoms and/or sensitization to perennial allergens were excluded. Complete blood cell count and serum interleukin (IL)-4, IL-5, IL-6, IL-8, IL-10, IL-18, monocyte chemoattractant protein-1 (MCP-1), total immunoglobulin (Ig) E, and eosinophil cationic protein levels were measured. RESULTS: The remission stage in patients with seasonal allergic rhinitis is characterized by higher serum levels of total IgE and IL-8 compared with the healthy group. The normal serum total IgE level of patients with seasonal allergic rhinitis in remission is associated with activation of the monocytemacrophage link (increased serum levels of MCP-1, IL-6, IL-8, and absolute and relative numbers of monocytes). Increased serum total IgE level is associated with the predominant signs of the Th2 phenotype of the immune response (increased serum levels of IL-4 and IL-5). CONCLUSIONS: The immune reactivity of patients with natural remission of seasonal allergic rhinitis is characterized by Th2 phenotype features if the serum level of IgE is increased and features of activation of monocytemacrophage system if the level of IgE is normal.

- Abstract
Objectives: Few studies have analyzed factors associated with delirium subtypes. In this study, we investigate factors associated with subtypes of delirium only in patients with dementia to provide insights on the possible prevention and treatments. Design: This is a cross-sectional study nested in the “Delirium Day” study, a nationwide Italian point-prevalence study. Setting and Participants: Older patients admitted to 205 acute and 92 rehabilitation hospital wards. Measures: Delirium was evaluated with the 4-AT and the motor subtypes with the Delirium Motor Subtype Scale. Dementia was defined by the presence of a documented diagnosis in the medical records and/or prescription of acetylcholinesterase inhibitors or memantine prior to admission. Results: Of the 1057 patients with dementia, 35% had delirium, with 25.6% hyperactive, 33.1% hypoactive, 34.5% mixed, and 6.7% nonmotor subtype. There were higher odds of having venous catheters in the hypoactive (OR 1.82, 95% CI 1.18-2.81) and mixed type of delirium (OR 2.23, CI 1.43-3.46), whereas higher odds of urinary catheters in the hypoactive (OR 2.91, CI 1.92-4.39), hyperactive (OR 1.99, CI 1.23-3.21), and mixed types of delirium (OR 2.05, CI 1.36-3.07). We found higher odds of antipsychotics both in the hyperactive (OR 2.87, CI 1.81-4.54) and mixed subtype (OR 1.84, CI 1.24-2.75), whereas higher odds of antibiotics was present only in the mixed subtype (OR 1.91, CI 1.26-2.87). Conclusions and Implications: In patients with dementia, the mixed delirium subtype is the most prevalent followed by the hypoactive, hyperactive, and nonmotor subtype. Motor subtypes of delirium may be triggered by clinical factors, including the use of venous and urinary catheters, and the use of antipsychotics. Future studies are necessary to provide further insights on the possible pathophysiology of delirium in patients with dementia and to address the optimization of the management of potential risk factors.

- Abstract
Background: Vulvar Paget disease is an extremely rare skin disorder, which is most common in postmenopausal women. Most vulvar Paget disease cases are noninvasive; however, it may be invasive or associated with an underlying vulvar or distant adenocarcinoma. The current treatment of choice for noninvasive vulvar Paget disease is wide local excision, which is challenging because of extensive intraepithelial spread and may cause severe morbidity. Recurrence rates are high, ranging from 15% to 70%, which emphasizes the need for new treatment options. Imiquimod, a topical immune response modifier, has been shown to be effective in a few studies and case reports, and is a promising new treatment modality. Objective: To prospectively investigate the efficacy, safety, and effect on quality of life of a standardized treatment schedule with 5% imiquimod cream in patients with noninvasive vulvar Paget disease. Study Design: The Paget Trial is a multicenter prospective observational clinical study including 7 tertiary referral hospitals in the Netherlands. A total of 24 patients with noninvasive vulvar Paget disease were treated with topical 5% imiquimod cream 3 times a week for 16 weeks. The primary efficacy outcome was the reduction in lesion size at 12 weeks after the end of treatment. Secondary outcomes were safety, clinical response after 1 year, and quality of life. Safety was assessed by evaluation of adverse events and tolerability of treatment. Quality of life was investigated with 3 questionnaires taken before, during, and after treatment. Results: Data were available for 23 patients, 82.6% of whom responded to therapy. A complete response was reported in 12 patients (52.2%), and 7 patients (30.4%) had a partial response. A histologic complete response was observed in 10 of the 12 patients with a complete response. Patients experienced side effects such as fatigue (66.7%–70.9%) and headaches (16.7%–45.8%), and almost 80% needed painkillers during treatment. Eight

BACKGROUND: Allergic diseases and gastrointestinal tract diseases can influence on the natural course of each other. AIM: To study the sensitization profile in patients with comorbidity of seasonal allergic rhinitis (SAR) and upper gastrointestinal tract inflammatory diseases (UGITID). MATERIALS AND METHODS: 112 adult residents of Volgograd city suffering from SAR but without perennial symptoms and sensitization to indoor allergens have been included in the study. 31/112 patients had H. pylori- negative and 38/112 H. pylori- positive UGITID. Control group consisted of 43/112 patients without gastrointestinal diseases. Skin prick-testing with 3 groups of pollen allergens have been carried out. RESULTS: 75.9% of patients were sensitized to weed pollen; sensitization to grass pollen was in 1.5–3 times less, and 10.5% of patients (4/38) had positive tests with birch pollen. The sensitization to quinoa and ragweed in SAR and H. pylori«–» -UGITID patients was comparable with control group but less common with wormwood, sumpfweed allergens. The sensitization in SAR patients and H. pylori«+» -UGITID was similar to the control group, but positive SPT with sunflower and corn allergens were rare then in control group. CONCLUSION: Weed pollen allergens prevail in sensitization spectrum of adult Volgograd residents with SAR. Sensitization to goose-foot and ragweed is common less in SAR patients and H. pylori«–» -UGITID but to graminea grass and birch pollen is more often. It may be supposed that the UGITID are predisposing factors to the sensitization to cross-reacting plant food allergens. At the same time supposed immunomodulating action of H. pylori make the differences between AR patients with H. pylori«+» -UGITID or H. pylori«–» -UGITID minor.

The diketoximes of 4,4'-diacetyldiphenyl oxide and 4,4'-diacetyldiphenyl sulfide were synthesized, the possibility of their use for the synthesis of simple and complex polyesters and polypyrroles, as well as the ability to complex with nickel salts. Organic chemistry used oximes in the 19th century, afterward, as an analytical reagent in the 20th century. Currently, oximes are superior to carbonyl compounds and alcohols in the variety of reactions and the widespread use in synthetic chemistry. Oximes easily turn into other classes of organic compounds – amines, cyano and nitro compounds, carbonyl compounds, heterocyclic compounds, including pyrroles, imidazoles, pyridines, they can be used as suitable compounds with salts of Cu (II), Ni (II), Zn (II), Cd (II), and Co (II). Oximes with complex heterocyclic and aromatic substituents are used as intermediates in fine organic synthesis. It should be noted that oximes have a wide spectrum of biocidal action and are used as anti-inflammatory, antiviral, antimicrobial agents, insecticides. Their versatile biological activity is considered in the preparation of antidepressant, sedative, vasodilator and other drugs [1].

The concept of the basic guaranteed income for citizens is widely discussed by scientists and politicians. The pandemic forced authorities around the world to implement punctual payments to the whole population or some categories (such as families with children, compensation of losses for businesses at lockdown, etc.). These subsidies raised again the question of the future of labor: if routine work is accomplished by robots and bots, computers and neuron-networks, what activities stay for humans? The corporate loyalty of employees was highly appreciated during last centuries. But the pandemic introduced remote regime of work for the masses of specialists, the lockdown for services sector business and the unemployment for the professionals who lost their stable jobs, showed the necessity to discuss the content of the labor contract. The article represents the results of the exploratory interview with 43 employers (mainly from small and medium enterprises) to identify the core symptoms of the evolution of labor. The answers on labor contract’ essence, given by employers, allowed researchers to raise hypotheses about the new features of the labor market: if the most appreciated content of labor is the creative contribution, how the interaction between business and labor should be organized? During the pandemic, the rupture widened rapidly between the successful part of population who taken the functions of their own autonomous value creation process, and the other non-privileged part who lost their integration into societal and, particularly, economic system.

Media exposure regarding male sexual dysfunction and the growing number of viable treatment alternatives for erectile dysfunction has resulted in increasing numbers of men seeking clinical appointments and treatment for the condition, which has previously been considered taboo. Because these problems usually arise within the context of relationships, some investigators have alluded to increased rates of sexual dysfunction among the partners of these men. Also, since general practitioners, gynaecologists, geriatrists, and urologists are also seeing female patients for evaluation of various types of dysfunction, certain groups of these women with underlying chronic conditions have been noted to have high rates of concomitant sexual dysfunction. Physicians who have good rapport with these patients are in a privileged position to help with these intimate problems, which are often difficult for patients to discuss. Therefore, it is of extreme importance that these professionals become knowledgeable about and comfortable with the initial evaluation and possible treatment of female sexual dysfunction.A intensa exposição pela mídia do tema disfunção sexual masculina e o número crescente de tratamentos disponíveis para a disfunção erétil têm resultado num número crescente de homens que procuram consulta e tratamento clínico para este problema, antes considerado como "tabu". Uma vez que o problema geralmente surge no contexto do relacionamento, alguns investigadores aludem às taxas crescentes de disfunção sexual nas parceiras destes homens. Além disso, clínicos, ginecologistas, geriatras e urologistas igualmente estão avaliando pacientes do sexo feminino quanto a vários tipos de disfunção. Em alguns grupos de mulheres com condições crônicas subjacentes, também têm sido observado um aumento nas taxas de disfunção sexual concomitante. Médicos que tenham estabelecido uma boa relação médico-paciente estão em posição única para ajudar com este problema, que em virtude de sua própria característica de intimidade, geralmente é de difícil abordagem para os pacientes. Desta forma, é de extrema importância que estes profissionais tenham conhecimento e sintam-se à vontade para iniciar a avaliação e possivelmente o tratamento da disfunção sexual feminina.

BACKGROUND: Vulvar Paget disease is an extremely rare skin disorder, which is most common in postmenopausal women. Most vulvar Paget disease cases are noninvasive; however, it may be invasive or associated with an underlying vulvar or distant adenocarcinoma. The current treatment of choice for noninvasive vulvar Paget disease is wide local excision, which is challenging because of extensive intraepithelial spread and may cause severe morbidity. Recurrence rates are high, ranging from 15% to 70%, which emphasizes the need for new treatment options. Imiquimod, a topical immune response modifier, has been shown to be effective in a few studies and case reports, and is a promising new treatment modality.OBJECTIVE: To prospectively investigate the efficacy, safety, and effect on quality of life of a standardized treatment schedule with 5% imiquimod cream in patients with noninvasive vulvar Paget disease.STUDY DESIGN: The Paget Trial is a multicenter prospective observational clinical study including 7 tertiary referral hospitals in the Netherlands. A total of 24 patients with noninvasive vulvar Paget disease were treated with topical 5% imiquimod cream 3 times a week for 16 weeks. The primary efficacy outcome was the reduction in lesion size at 12 weeks after the end of treatment. Secondary outcomes were safety, clinical response after 1 year, and quality of life. Safety was assessed by evaluation of adverse events and tolerability of treatment. Quality of life was investigated with 3 questionnaires taken before, during, and after treatment.RESULTS: Data were available for 23 patients, 82.6% of whom responded to therapy. A complete response was reported in 12 patients (52.2%), and 7 patients (30.4%) had a partial response. A histologic complete response was observed in 10 of the 12 patients with a complete response. Patients experienced side effects such as fatigue (66.7%-70.9%) and headaches (16.7%-45.8%), and almost 80% needed painkillers during treatment. Eight patients (34.8%) adjusted the treatment protocol to 2 applications a week, and 3 patients (13.0%) stopped treatment because of side effects after 4 to 11 weeks. Treatment improved quality of life, whereas a slight, temporary negative impact was observed during treatment. Two patients with a complete response developed a recurrence within 1 year after treatment. Follow-up showed 6 patients with a noninvasive recurrence after a median of 31 months (14-46 months) after the end of treatment.CONCLUSION: Topical 5% imiquimod cream can be an effective and safe treatment alternative for noninvasive vulvar Paget disease, particularly when compared with treatment with surgical excision.

- Abstract
PURPOSE The Groningen International Study on Sentinel nodes in Vulvar cancer (GROINSS-V)-II investigated whether inguinofemoral radiotherapy is a safe alternative to inguinofemoral lymphadenectomy (IFL) in vulvar cancer patients with a metastatic sentinel node (SN). METHODS GROINSS-V-II was a prospective multicenter phase-II single-arm treatment trial, including patients with early-stage vulvar cancer (diameter < 4 cm) without signs of lymph node involvement at imaging, who had primary surgical treatment (local excision with SN biopsy). Where the SN was involved (metastasis of any size), inguinofemoral radiotherapy was given (50 Gy). The primary end point was isolated groin recurrence rate at 24 months. Stopping rules were defined for the occurrence of groin recurrences. RESULTS From December 2005 until October 2016, 1,535 eligible patients were registered. The SN showed metastasis in 322 (21.0%) patients. In June 2010, with 91 SN-positive patients included, the stopping rule was activated because the isolated groin recurrence rate in this group went above our predefined threshold. Among 10 patients with an isolated groin recurrence, nine had SN metastases > 2 mm and/or extracapsular spread. The protocol was amended so that those with SN macrometastases (> 2 mm) underwent standard of care (IFL), whereas patients with SN micrometastases (<= 2 mm) continued to receive inguinofemoral radiotherapy. Among 160 patients with SN micrometastases, 126 received inguinofemoral radiotherapy, with an ipsilateral isolated groin recurrence rate at 2 years of 1.6%. Among 162 patients with SN macrometastases, the isolated groin recurrence rate at 2 years was 22% in those who underwent radiotherapy, and 6.9% in those who underwent IFL (P = .011). Treatment-related morbidity after radiotherapy was less frequent compared with IFL. CONCLUSION Inguinofemoral radiotherapy is a safe alternative for IFL in patients with SN micrometastases, with minimal morbidity. For patients wit

Introduction Minimal important differences (MIDs) are useful for interpreting changes or differences in health-related quality of life scores in terms of clinical importance. There are currently no MID guidelines for the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire core 30 (EORTC QLQ-C30) specific to ovarian cancer. This study aims to estimate MIDs for interpreting group-level change of EORTC QLQ-C30 scores in ovarian cancer. Methods Data were derived from four EORTC published trials. Clinical anchors for each EORTC QLQ-C30 scale were selected using correlation strength and clinical plausibility. MIDs for within-group change and between-group differences in change over time were estimated via mean change method and linear regression respectively. For each EORTC QLQ-C30 scale, MID estimates from multiple anchors were summarized via weighted-correlation. Distribution-based MIDs were also examined as supportive evidence. Results Anchor-based MIDs were determined for deterioration in 7 of the 14 EORTC QLQ-C30 scales assessed, and in 11 scales for improvement. Anchor-based MIDs for within-group change ranged from 4 to 19 (improvement) and − 9 to −4 (deterioration). Between-group MIDs ranged from 3 to 13 (improvement) and − 11 to −4 (deterioration). Generally, absolute anchor-based MIDs for most scales ranged from 4 to 10 points. Conclusions Our findings will aid interpretation of EORTC QLQ-C30 scores in ovarian cancer and inform sample size calculations in future ovarian cancer trials with endpoints that are based on EORTC QLQ-C30 scales.

- Abstract
BACKGROUND BRCA1/2 mutation carriers are recommended to undergo risk-reducing salpingo-oophorectomy (RRSO) at 35 to 45 years of age. RRSO substantially decreases ovarian cancer risk, but at the cost of immediate menopause. Knowledge about the potential adverse effects of premenopausal RRSO, such as increased risk of cardiovascular disease, osteoporosis, cognitive dysfunction, and reduced health-related quality of life (HRQoL), is limited. OBJECTIVE The aim of this study is to assess the long-term health effects of premenopausal RRSO on cardiovascular disease, bone health, cognitive functioning, urological complaints, sexual functioning, and HRQoL in women with high familial risk of breast or ovarian cancer. METHODS We will conduct a multicenter cross-sectional study with prospective follow-up, nested in a nationwide cohort of women at high familial risk of breast or ovarian cancer. A total of 500 women who have undergone RRSO before 45 years of age, with a follow-up period of at least 10 years, will be compared with 250 women (frequency matched on current age) who have not undergone RRSO or who have undergone RRSO at over 55 years of age. Participants will complete an online questionnaire on lifestyle, medical history, cardiovascular risk factors, osteoporosis, cognitive function, urological complaints, and HRQoL. A full cardiovascular assessment and assessment of bone mineral density will be performed. Blood samples will be obtained for marker analysis. Cognitive functioning will be assessed objectively with an online neuropsychological test battery. RESULTS This study was approved by the institutional review board in July 2018. In February 2019, we included our first participant. As of November 2020, we had enrolled 364 participants in our study. CONCLUSIONS Knowledge from this study will contribute to counseling women with a high familial risk of breast/ovarian cancer about the long-term health effects of premenopausal RRSO. Agricultural use of pesticides has greatly increased worldwide over the last several decades, affecting soil microorganisms. Microbial basal respiration and substrate-induced respiration rates are commonly used to assess the detrimental effects of pesticides on soil quality. The goal of the present study was (1) to compare the impact of different pesticides on soil microbial respiration under field conditions, and (2) to characterize the recovery time of soil microbial respiration after pesticide application. The following pesticides were used in the present study: chlorpyrifos, phosalone, dimethoate (organophosphorus insecticides), λ-cyhalothrin (pyrethroid insecticide), and kresoxim-methyl (fungicide). The application of all the pesticides at commercial doses led to a decrease in soil microbial respiration. The inhibition of basal respiration and substrate-induced respiration rate decreased in the following order: chlorpyrifos > phosalone > dimethoate > λ-cyhalothrin ≈ kresoxim-methyl. Among all the pesticides assessed, chlorpyrifos showed the highest toxicity as well as the highest persistence. Several of the observed results differed greatly from previous studies; thus, local assessments are highly advisable. Given that environmental concerns can be a key decision factor for pesticide selection, assessment of different pesticides—such as undertaken in this study—could help farmers to choose the most appropriate pesticide.
