1. 3D margin assessment predicts local tumor progression after ablation of colorectal cancer liver metastases

Abstract

Objective To determine the feasibility and prognostic value of 3D measuring of the ablation margins using a dedicated image registration software.Methods This retrospective study included 104 colorectal liver metastases in 68 consecutive patients that underwent microwave ablation between 08/2012 and 08/2019. The minimal ablation margin (MM) was measured in 2D using anatomic landmarks on contrast enhanced CT(CECT) 4–8 weeks post-ablation, and in 3D using an image registration software and immediate post-ablation CECT. Local tumor progression (LTP) was assessed by imaging up to 24 months after ablation. A blinded interventional radiologist provided feedback on the possibility of additional ablation after examining the 3D-margin measurements.Results The 3D-margin assessment was completed in 79/104 (76%) tumors without the need for target manipulation. In 25/104 (24%) tumors, manipulation was required due to image misregistration. LTP was observed in 40/104 (38.5%) tumors: 92.5% vs 7.5% for those with margin

4. MRI of frozen tissue demonstrates a phase shift

Abstract

While temperature mapping is desired during cryosurgery for prostate cancer treatment, an effective approach for this purpose is still needed. We have demonstrated a phase shift with temperature in our in-vivo canine experiments and ex-vivo tissue sample experiments within the frozen tissue. The phase shift is much larger (~0.7 degree/°C with an echo time (TE) of 0.1 ms at 0.5T) in magnitude than that predicted by conventional proton resonant frequency shift (0.008 degree/°C). It shows little dependence on the TEs of used and thus is not due to a frequency change, though frequency dependent phase shift has been observed near the frozen tissue. This phase shift varies monotonically with temperature within the frozen tissue, and therefore may be potentially used as a novel temperature mapping approach in cryoablation applications.

Abstract

In this work a more time efficient approach for tissue assessment with MR‐ARFI is proposed. During HIFU treatments gadolinium‐free assessment of treated tissue is highly desirable. MR‐ARFI allows measuring tissue displacement in the focal spot. Therefore raster‐scanning the ultrasound focal spot through the tissue of interest can give information about tissue stiffness in the “probed” area. To enhance efficiency of such ultrasound “probing,” we replaced a conventional point focus with a line focus that allows greater area coverage during MR‐ARFI acquisitions. This approach was tested in a phantom and in ex vivo bovine muscle. The results of the study showed that the line focus produces a fairly uniform line shape focal spot in both temperature and displacement maps. Using one line focus position, the displacement maps obtained in the muscle tissue well depicted the difference in displacement in the area where lesion was created. Abstract

In MRgFUS, visualization of the focal spot has relied on “test spots” of a small temperature rise. MR acoustic radiation force imaging (MR‐ARFI) could be used instead for visualization, without the need to a temperature rise. The goal of this study was to develop a method for rapid focal spot localization method, based on MR‐ARFI. In addition, a temperature sensitive alteration of the sequence was proposed to measure any potential temperature rise during MR‐ARFI sonications. An EPI pulse sequence was modified to incorporate displacement encoding repeated bipolar gradients. The pulse sequence triggered the FUS system to emit 23 ms ultrasound pulses during the encoding gradients. This imaging method was tested in a gel phantom and porcine tissue on a 3T GE MRI scanner equipped with an InSightec FUS transducer. Abstract

High intensity focused ultrasound (HIFU) is a promising technique for noninvasive treatment of liver tumors. To safely monitor HIFU, accurate temperature measurements must be acquired. However, respiratory motion impairs the performance of conventional MR‐guided PRF‐thermometry sequences, leading to inaccurate temperature measurements and locations. We developed an MR imaging system to minimize these with the following key elements: imaging at 3.62 frames/sec and utilizing real‐time referenceless thermometry processing. A spatially saturated, spectral‐spatially excited flyback echo planar readout pulse sequence that allows rapid temperature acquisition was developed. The spatial‐spectral pulse ignores lipids, and other off‐resonance effects are mitigated with linear delays of the EPI readout. The saturation pulse restricts the field of view in the phase encode direction, allowing for increased resolution without aliasing.
