Abstract

This article describes a case of central giant cell granuloma in the right posterior region of the mandible in a 10- year-old boy. The lesion was removed by curettage and a histopathological examination was carried out. En este artículo se describe un caso de granuloma central de células gigantes en la región posterior derecha de la mandíbula en un niño de 10 años de edad. La lesión se retiró por legrado y se realizó un examen histopatológico.

Abstract

Florid osseous dysplasia (FOD) is a benign condition of the jaws in which the normal architecture of bone is replaced by a fibrous tissue containing a variable amount of bone and cementum-like tissue. This lesion is most commonly seen in middle aged black women. FOD appears as dense, lobulated masses, often symmetrically located in the mandible, rarely in the maxilla. The lesion is usually asymptomatic and benign. However, a secondary infection may occur and its treatment can be difficult and complicated. This paper reports the case of two patients. The first one is a white woman aged 65 and the second one is a black woman aged 70, both diagnosed with FOD, revealed by secondary infections. The diagnosis was based on clinical and radiographic findings, as biopsy is contraindicated. Radiological and clinical features of FOD and its management will be also discussed on the basis of recent literature. La displasia ósea florida (DOF) es una patología benigna del maxilar y mandíbula en la que se sustituye la arquitectura normal del hueso por un tejido fibroso que contiene una cantidad variable de tejido óseo y cementoide. Esta lesión es más frecuente en mujeres negras de edad media. La DOF aparece como una masa densa, lobulada, a menudo situada simétricamente en la mandíbula, rara vez en el maxilar. La lesión suele ser asintomática y benigna. Sin embargo, una infección secundaria puede ocurrir y su tratamiento puede ser difícil y complicarse. Este artículo reporta el caso de dos pacientes. La primera es una mujer blanca de 65 años y la segunda es una mujer negra de 70 años, ambas diagnosticadas con DOF, revelada por infecciones secundarias. El diagnóstico se basó en los hallazgos clínicos y radiológicos, ya que la biopsia está contraindicada. Las características clínicas y radiológicas de la DOF y su manejo son discutidos en base a la literatura reciente.

18. Central odontogenic fibroma of the maxilla.

Abstract

Central odontogenic fibroma (COF) is an uncommon tumor that accounts for 0.1% of all odontogenic tumors; it has been defined as a benign neoplasm of the jaw. Clinically, the lesion grows slowly and leads to cortical expansion. Radiologically, the most common finding is multilocular radiolucency. The lesions are associated with the crown of an unerupted molar, premolar, or incisor tooth and in some cases, with root resorption or displacement. Histologically, the lesion is characterized by mature collagen fibers and numerous fibroblasts. COF responds well to surgical enucleation with no tendency for malignancy or recurrence. We report a case of a 15-year-old female patient presented with painless swelling of the left side of the maxilla since her childhood. Radiographs revealed an expanding ill-defined radiolucency with a displacement of the adjacent tooth. The impacted tooth was pushed posteriorly. The lesion was removed surgically. 31. Lateral periodontal cyst

Abstract

Le kyste parodontal lat?ral est class? parmi les kystes odontog?nes des maxillaires (OMS 1992). Les auteurs rapportent le cas d'un kyste parodontal lat?ral chez une femme de race blanche ?g?e de 33 ans. Cliniquement, il se manifestait par une tum?faction si?geant au niveau de la r?gion maxillaire ant?rieure. Radiologiquement, il se pr?sentait par une image radioclaire si?geant entre les racines de la 12 et la 13. Apr?s un traitement chirurgical conservateur, aucun signe de r?cidive n'a ?t? rapport?. Le kyste parodontal lat?ral se manifeste par des signes cliniques, radiologiques et histologiques non sp?cifiques et pose des probl?mes de diagnostic diff?rentiel avec les kystes et les tumeurs odontog?nes b?nignes des maxillaires.The lateral periodontal cyst is classified among odontogenic cysts of the jaws (WHO 1992). The authors report the case of a lateral periodontal cyst found in a 33 year-old white woman. Clinically, it manifested by a swelling sitting in the anterior maxillary region. Radiologically, it presented by a radiolucency sitting between the roots of the 12 and the 13. After initial surgical removal, no recurrence has been reported. The lateral periodontal cyst presents non specific clinical, radiological and histological signs and creates a differential diagnosis problems with the cysts and the benign odontogenic tumors of the jaws.

32. Complete mandibular denture: stability and retention

Abstract

Les probl?mes de r?tention et d'instabilit? constituent la principale dol?ance en proth?se compl?te mandibulaire. L'am?lioration de l'?quilibre proth?tique d?pendra d'une part, de l'exploitation raisonn?e de la surface d'appui, et d'autre part de l'instauration d'un parfait ?quilibre occlusal. En fonction de la situation clinique et des conditions ?conomiques des patients, plusieurs modalit?s th?rapeutiques peuvent ?tre envisag?es afin d'am?liorer la r?tention et la stabilit? proth?tique. La technique pi?zographique constitue un excellent recours pour pallier l'instabilit? proth?tique chez de nombreux patients pr?sentant une importante tonicit? musculaire ou une alt?ration du contr?le neuromusculaire. Dans le souci de majorer l'efficacit? masticatoire, la proth?se supraradiculaire est une solution ? adopter ? chaque fois que le cas le permet. Enfin, avec le d?veloppement de l'implantologie, la proth?se supra-implantaire repr?sente une alternative fiable aboutissant ? une meilleure int?gration proth?tique par augmentation de la r?tention. Elle am?liore de ce fait la qualit? de vie des patients ?dent?s complets.The retention problem is one of the most frequent patient's complaints in complete mandibular dentures. Stability and retention of removable complete mandibular denture requires on the one hand to reproduce supporting surfaces and on the other hand to adopt the right occlusal concept. Responding to diverse clinical situations and economical conditions, many solutions to the problem of low edentulism can be envisaged. The neutral zone impression technique is useful when providing mandibular complete dentures. Its use can overcome some of the denture stability difficulties caused in some patients by a more powerful oral musculature, or in patients who have poor or altered neuromuscular control. Over denture supported by natural teeth is widely used in clinical practice and should be executed whenever the clinical conditions suggest it. The greater retention and stability of over denture in comparison with complete denture greatly improve the masticatory efficacy. The current development of implantology have proved that Implant-supported prostheses to be a great therapeutic value making easier the prosthesis integration and improving retention and patient's quality of life.
