Abstract

Poverty is strongly associated with all-cause and chronic obstructive pulmonary disease (COPD) mortality. Less is known about the contribution of poverty to spirometrically defined chronic airflow obstruction (CAO) – a key characteristic of COPD. Using cross-sectional data from an asset-based questionnaire to define poverty in 21 sites of the Burden of Obstructive Lung Disease study, we estimated the risk of CAO attributable to poverty. Up to 6% of the population over 40 years had CAO attributable to poverty. Understanding the relationship between poverty and CAO might suggest ways to improve lung health, especially in low- and middle-income countries.

14. Modified Mediterranean diet score adapted to a southern Mediterranean population and its relation to overweight and obesity risk

Abstract

Objectives: The current study aimed to develop a modified Mediterranean diet (MMD) score adjusted to the southern Mediterranean countries' cultural specificities and to evaluate associations between adherence to this modified score and overweight/obesity risk in Moroccan adults. Design: Population-based cross-sectional study. Setting: Rural and urban areas of the five greatest provinces of Morocco. Participants: In total, 1516 participants were recruited between September 2009 and February 2017. Dietary assessment was obtained using a validated Moroccan FFQ. We constructed a MMD score focusing on twelve components. The MMD score ranged from 0 (no adherence to the traditional southern Mediterranean diet (MD)) to 12 (maximal adherence) and was categorised as low (scores 0-4), moderate (scores 5-7) and high (scores 8-12). Results: Among the whole population, 754 (50·5 %) were women and 738 (49·5 %) were men, and the mean age was about 55·60 ± 13·70. In total, 58 % of participants were moderately active. Regarding educational level, 50·7 % were illiterate. The prevalence of overweight and obesity was 43·3 and 8·6 %, respectively. In multivariate analyses, close adherence to MMD (scores 8-12) was associated with reduced overweight/obesity risk (OR 0·61, 95 % CI 0·44, 0·84). Conclusion: The prevalence of overweight and obesity was very high among Moroccan adults. Adherence to the traditional southern MD may help prevent overweight and obesity. © 2021 Cambridge University Press. All rights reserved.

Abstract

OBJECTIVE To present population-estimated prevalence of spirometrically confirmed chronic obstructive pulmonary disease (COPD) in adults (age ⩾40 years), living in the city of Fez, Morocco. DESIGN Following the Burden of Lung Disease (BOLD) methodology, population-based sampling plans were used for the recruitment of eligible adults. The study collected questionnaire data on respiratory symptoms, medical history, health status, exposure to risk factors for COPD and quality of life. Post-bronchodilator spirometry was performed and COPD and its stages were defined according to Global Initiative for Chronic Obstructive Lung Disease (GOLD) guidelines. RESULTS Among 768 individuals with valid data and acceptable quality post-bronchodilator spirometry results, the overall prevalence of stage 1 or higher COPD was 12.6%. The prevalence of GOLD stage 2 or higher COPD was 7.9%. Abstract

Notre étude a pour principal objectif d’estimer l’ampleur du problème des morsures comportant un risque potentiel de rage à Fès et de décrire le profil épidémiologique de 868 cas. Notre étude a été réalisée à Fès sur une période d'un an (2004) en utilisant les données d’un registre de surveillance des cas de morsure. L’âge médian des cas mordus était de 21 ans, l’étendue était de 1an à 90 ans. La majorité des patients (72%) étaient de sexe masculin. Septembre est le mois de la plus haute fréquence des morsures (n=94), l’hiver est une période de moindre risque. La majorité était causée par le chien (85.4%) et dans 95% des cas de morsure unique. siége surtout des membres inférieurs (498 cas, 64%), suivi des membres supérieurs (197 cas, 25 %). La partie du corps atteinte différait selon l'âge de la victime : 8,5 % des victimes âgées de moins de 14 ans avaient été mordues à la tête, comparativement à 2 % chez les victimes de plus de 30 ans. Abstract

L’objectif de cet article est de faire le point sur la méthodologie des essais cliniques contrôlés, les étapes de construction d’un protocole et leurs différents aspects éthiques. L’essai clinique contrôlé est un plan d’expérience dans lequel on compare de façon prospective deux groupes contemporains de patients, l’un traité par le traitement de référence (groupe contrôlé) et l’autre traité par le nouveau traitement (groupe testé). L’attribution des traitements se fait par allocation aléatoire et le suivi des patients se fait en aveugle vis à vis du traitement reçu. Ces principes constituent le cadre méthodologique de l’essai clinique contrôlé. Ce dernier ne peut pas être abordé sous son seul angle méthodologique ou statistique, d’où une sérieuse réflexion éthique à travers le monde surtout avec les rapides progrès des sciences biologiques et des techniques médicales. Au Maroc, la réglementation du médicament est régie principalement par le Ministère de la Santé qui est en charge d'assurer la veille et la sécurité sanitaire des citoyens. Il est responsable de toute la politique des médicaments au Maroc et de tous les aspects qui lui sont liés. La réalisation des essais de bio équivalence et de bio disponibilité sont deux critères occultés des textes actuellement en vigueur. Les mesures d'accompagnement de la mise en place de ces essais à savoir les principes universels en matière de protection des personnes, les Bonnes Pratiques de fabrication des Médicaments, les Bonnes Pratiques Cliniques, les Bonnes Pratiques de Laboratoire ne sont pas non plus prévu par la réglementation actuelle., Maroc Médical, Vol. 27, No 2 (2005)
