Bilateral medial medullary syndrome is a rare type of stroke, which results due to occlusion of the anterior spinal artery or vertebral artery or its branches. Clinical diagnosis without neuroimaging is very difficult; the characteristic brain MRI finding of “heart appearance” at DWI has been described in bilateral medial medullary infarct. We report the case of 53-year-old man with acute onset quadriparesis. MRI scan of brain, diffusion weighted (DW) imaging revealed heart shaped hyper-intensity areas in the bilateral medial medulla.

The arteria lusoria or right retro-oesophageal subclavian artery is the most common malformation of the aortic arch with a prevalence of 0.5-2.5%. It may be discovered in the face of symptoms of airway and/or oesophageal compression such as dyspnoea or dysphagia or even recurrent respiratory infections; but in most cases it is an asymptomatic condition as reported by several authors. We report the case of a 40 year old patient, in whom the thoracic CT scan performed́ in emergency following a thoracic trauma, allowed the fortuitous discovery of a retroesophageal aberrant artery.

A 29-year-old woman presented with low backache of 6 months duration. Systemic and neurological findings were unremarkable. Sphincter dysfunction was absent. CT scan (Figure: (a) sagittal view; (b) coronal reconstruction; (c) axial image) revealed a linear fat density area of thikness 3 mm decreasing in diameter caudally within the filum terminale extending from L2 to L5 vertebral level, suggesting fibrolipoma of filum terminale. The position of the conus medullaris was seen at normal level. There was no other abnormality. Fibrolipomas of the filum terminale are embryonic in origin and frequently asymptomatic. They are seen on CT and MR images as areas of fatty tissue along the filum terminale. These lipomas may result from developmental malformation that occurs during the formation of the neural tube and leads to inclusion of embryonic crests of fat cells. A fibrolipoma “infiltrates” the filum and tends to be tubular. Fibrolipoma affecting the filum terminale are rare benign tumours usually discovered at imaging and rarely symptomatic, sometimes associated with radicular pain. Low position of the conus medullaris (below L2 vertebra) is a spectrum of associated congenital abnormality that may lead to neurological, musculoskeletal, urological, or gastrointestinal abnormalities. Split cord malformation (diastematomyelia) and a dermal sinus are also possible findings.

Chondrosarcomas are frequent bone tumors, just after osteosarcomas, they represent 11 to 22% of all bone tumors. Proximal humerus localization occurs in about 9% of cases, the clinical picture is variable, imagery helps to establish the diagnosis, to give an exact lesion assessment and to guide therapeutic attitude and post treatment control. Confirmation is set by histology. We highlight through this case presentation clinical and radiological aspects of these tumors.

Nous rapportons le cas d'une patiente ayant une dysphagialusoria, c'est-a-dire une dysphagie en rapport avec un trajet aberrant de l'artere sousclaviere droite. The authors report two cases of complicated hydatid cysts of the liver, including one which was spontaneously broken in the bile ducts and the other fistulized in the right pleura. Patients presented with jaundice and biliary colic in the first case. Cough basithoracic pain, weakness, and fever in the second. Both patients were investigated by ultrasound and CT scan followed by a hydatid serology. In both cases the patients improved after surgical treatment. Will be recalled imaging aspects of these two complications.

Hydatid cyst of the liver remains a frequent parasitic disease caused by the development of the larval form of Echinococcus granulosis; it is often discovered incidentally during routine symptoms, rarely at the stage of complications. However, the diagnostic difficulties and pitfalls are numerous due to the differential diagnosis of the liver’s cystic lesions, despite the current improvement of cross-sectional imaging (ultrasound, CT and MRI). We report a case of a hydatid cyst of the liver discovered during a secondary infection, emphasizing the contribution of imaging and serology that can confirm the diagnosis before starting any therapeutic procedure.

Unicameral bone cysts (UBCs) are benign, osteolytic lesions which are often asymptomatic and are commonly seen in the proximal of humerus and femur. We report such a lesion in a eleven years old boy who presented to orthopedic clinic with a 10 months history of painful limping.Clinic, radiographic and pathologic findings and cyst aspiration evocated solitary bone cyst. Child was treated successfully with repeated intralesional steroid injection.

Introduction Physiological hypercoagulability is a known condition in pregnancy designed to limit the risk of bleeding; it may exceptionally be complicated by thrombosis of the renal vein. To the best of our knowledge, this is the third case of renal venous infarction reported in the literature. Case presentation We report the case of a 43-year-old Caucasian woman, a mother of three sons who presented with left flank pain and hematuria. The clinical investigations did not find any other cause for her thrombophilia. Conclusions Clinical onset is not specific, so it is important to evoke the diagnosis in the context of pregnancy; computed tomography angiography is the investigation of choice to set the diagnosis. It is important to know that anticoagulation therapy must be initiated as soon as possible.
