1. Estimating the Proportion of COVID-19 Contacts Among Households Based on Individuals With Myocardial Infarction History: Cross-sectional Telephone Survey

Abstract

BackgroundAdults with cardiovascular diseases were disproportionately associated with an increased risk of a severe form of COVID-19 and all-cause mortality. ObjectiveThe aims of this study are to report the associated symptoms for COVID-19 cases, to estimate the proportion of contacts, and to describe the clinical signs and behaviors among individuals with and without myocardial infarction history among cases and contacts. MethodsA 2-week cross-sectional telephone survey was conducted during the first lockdown period in France, from May 4 to 15, 2020. A total of 668 households participated, representing 703 individuals with pre-existing cardiovascular disease in the past 2 years and 849 individuals without myocardial infarction history. ResultsHigh rates of compliance with health measures were self-reported, regardless of age or risk factors. There were 4 confirmed COVID-19 cases that were registered from 4 different households. Based on deductive assumptions of the 1552 individuals, 9.73% (n=151) were identified as contacts, of whom 71.52% (108/151) were asymptomatic. Among individuals with a myocardial infarction history, 2 were COVID-19 cases, and the estimated proportion of contacts was 8.68% (61/703), of whom 68.85% (42/61) were asymptomatic. The cases and contacts presented different symptoms, with more respiratory signs in those with a myocardial infarction history. ConclusionsThe telephone survey could be a relevant tool for reporting the number of contacts during a limited period and in a limited territory based on the presence of associated symptoms and COVID-19 cases in the households. This study advanced our knowledge to better prepare for future crises.

Abstract

What Is Known and Objective: The dispensing of clotting factor concentrates in hospital pharmacies imposes accessibility constraints on patients and their caregivers, thereby increasing the disease burden. Very few studies have addressed these issues so far in terms of individual perceptions and actual difficulties. The PHAREO study aims to report patient's perception of treatment accessibility and evaluate spatial accessibility. Methods: The PHAREO study is an observational survey based on a questionnaire specifically designed for the study purpose in collaboration with patients' representatives in the second demographic and economic French region. Results and Discussion: We collected 293 responses (participation rate of 64.1%) which show that 89.8% of respondents were either very or rather satisfied with regard to access to treatment. However, respondents reported difficulties in accessing the hospital pharmacy. The data also showed that 79.2% of respondents tended to over‐estimate travel time which was reported above their acceptable threshold for 39.2% of them. The main determinants of dissatisfaction were parental burden (OR 2.5 [1.3; 4.8], p = 0.008) and waiting time at the hospital pharmacy (OR 1.5 [1.1;2.0], p = 0.016, per 10 min increase). What Is New and Conclusion: The PHAREO study provides subjective and objective data regarding satisfaction levels of persons with haemophilia and other coagulation deficiencies, with a high representativeness rate for patients on prophylaxis (87.5%). Both respondents and hospital pharmacists pled for an evolution of the current dispensing circuit to improve access to treatment and reduce the burden for patients. Currently, the community pharmacists are apart from the dispensing circuit. The authors propose improvements in the pathway of care for patients and their caregivers by including the community pharmacists alongside the hospital pharmacists in a centralized coordination scheme. Abstract

International audience; Aim: We aimed to compare the total work incapacity (TTI) durations assigned by the emergency physician at admission, and assigned in hindsight by medical examiners and emergency physicians.Procedure: We included traffic and domestic accidents from the regional observatory for accidents and trauma (ORAT) that led to the allocation of a TTI. Three medical examiners and three emergency physicians allocated TTI durations in number of days, based on information from the ORAT. We reported the intra- and inter-physician quality based on 94 and 376 patients. The concordance was evaluated with the intra-class correlation coefficient (ICC). An agreement was observed between prescribers if the difference between their TTIs was less than or equal to 20% of the average of the assigned TTIs.Results: Concordance between medical examiners was moderate (CCI: 0.67 [0.51; 0.80]), also between emergency physicians (CCI: 0.62 [0.50; 0.72]) and between medical examiners and emergency physicians (CCI: 0.66 [0.59; 0.71]). We observed only 16% situations of agreements. Among the agreements, patients did not share common characteristics. They were usually admitted on the day of the accident and most often had head, face or neck injuries. Among the disagreement situations, patients had more frequent spinal cord injuries and were associated with a disability that did not affect normal function (IIS score 1).Conclusion: We observed a strong heterogeneity in the assessment between medical examiners and emergency physicians, leading to inequity in the judicial treatment of injuries and in the medical follow-up of victims.

Abstract

National audience; Observational studies in the absence of selection bias reflect real life practices and have fewer constraints (ethical concerns, feasibility) than randomised clinical trials. Propensity score methods are increasingly used in observational studies to adjust for confounding and perform causal inferences (14 English Pubmed occurrences in the title or the abstract in 2000, 448 in 2010 and 3388 in 2018). This method allows interpretations of effects to be closer to causal inference like in a randomised clinical trial. There is a lack of articles describing propensity scores assumptions and interpretation rules. This leads to inadequacies in their use and in their reporting in scientific articles.We propose in this article a pragmatic synthesis, for the use of clinicians, presenting the advantages and limitations associated with the use of propensity scores for causal inferences. Our objective is to give clinicians keys to understanding and interpret propensity scores. We will develop an example based on simulated data to demonstrate the creation of the propensity score, four of its uses for covariate adjustment (matching, stratification, inverse probability weighing and adjustment) and its validation as well as reporting guidelines to ensure reproducibility of results in a scientific article.

Abstract

Stroke is a sudden and brutal illness that can have serious consequences. The prevalence of the pathology is known, but its spatial incidence is less studied. The objective of this study is to describe the spatial distribution of stroke occurrences and to characterise its spatial distribution patterns in order to identify possible unequal territories. This distribution is then analysed using different territorial (socio-economic, structural, environmental) and temporal (calendar and climatic) variables in order to understand the causes of stroke over-representation in certain locations. The analyses conducted throughout this work were based on data from the Rhône Valley Emergency Network’s (RESUVal) registry of thrombolysed strokes, as well as on data from the STROKE 69 cohort in the Rhône. This work is intended to be highly operational, making it possible to propose ways of improving stroke management in the region in terms of public policies to raise awareness among populations at risk of stroke., L’accident vasculaire cérébral (AVC) est une pathologie soudaine et brutale pouvant entraîner de graves séquelles. La prévalence de la pathologie est connue, mais son incidence spatiale est moins étudiée. L’objectif de cette étude est de décrire la répartition spatiale des occurrences d’AVC et de caractériser les modèles de distribution spatiale d’AVC afin d’identifier d’éventuels territoires inégalitaires. Cette répartition est ensuite analysée à l’aide de différentes variables territoriales (socio-économiques, structurelles, environnementales) et temporelles (calendaires et climatiques) afin de comprendre les causes de la surreprésentation des AVC en certains endroits. Les analyses menées tout au long de ce travail se sont appuyées sur les données du registre des AVC thrombolysés du Réseau des urgences de la vallée du Rhône (RESUVal), ainsi que sur les données de la cohorte STROKE 69 dans le Rhône. Ce travail se veut très opérationnel, permettant de proposer des axes pour améliorer territorialement la prise en charge de l’AVC, notamment en termes de politiques publiques permettant de sensibiliser les populations à risque de survenue d’un AVC.

Abstract

France and Canada prehospital systems and care delivery in out-of-hospital cardiac arrests (OHCAs) show substantial differences. This article aims to describe the rationale, design, implementation and expected research implications of the international, population-based, France-Canada registry for OHCAs, namely ReACanROC, which is built from the merging of two nation-wide, population-based, Utstein-style prospectively implemented registries for OHCAs attended to by emergency medical services. Under the supervision of an international steering committee and research network, the ReACanROC dataset will be used to run in-depth analyses on the differences in organisational, practical and geographic predictors of survival after OHCA between France and Canada. ReACanROC is the first Europe-North America registry ever created to meet this goal. 44. Evolution of Practices in a French Trauma Centre: Decrease in Blood Transfusions and Fresh Frozen Plasma to Red Blood Cell Ratios.

Abstract

Objective: The aim of this study was to describe the evolution of transfusion practices following the introduction of tranexamic acid (TXA) and ROTEM® in a trauma resuscitation unit (TRU) from a French teaching hospital (FTH). Methods: This is a single-centre, retrospective study at a TRU from a FTH. All trauma patients aged 18 years or more and transfused with at least 4 red blood cells (RBCs) within 24 hours after trauma, from 2011 to 2016, were included. The primary objective was to analyse transfusion practices over this time period. The secondary objectives aimed at assessing differences between populations according to the fresh frozen plasma (FFP):RBC ratio applied. Results: A total of 122 patients were included. There was a significant decrease in the proportion of patients requiring at least 4 RBCs 24 hours after trauma (9% vs. 3%, P trend < .0001) as well as a decrease in the proportion of patients with a high FFP:RBC ratio (86% vs. 62% at 6 hours, P trend = .0056 and 86% vs. 56% at 24 hours, P trend = .0047). After 2013, fibrinogen was administered to more than 70% of patients and TXA to 100% of them. The observed mortality was lower than the predicted one, irrespective of FFP:RBC ratio. Conclusion: An important evolution of practices occurred including a decrease in the proportion of transfusions and use of high FFP:RBC ratios. The origin of these changes is multifactorial, likely including the systematic use of TXA and optimisation of the ROTEM protocol for fibrinogen administration. [ABSTRACT FROM AUTHOR]
