Cardiac myxoma is the most commonly diagnosed benign cardiac tumor. It is usually located in the left atrium and typically arises from the foramen ovale in approximately 75% of the cases, in the right atrium in 23%, and in the ventricles in only 2%. Symptoms depend on its size, mobility, and location. Neurological complications are seen in 20% to 25% of patients. Herein we present a case of right ventricular myxoma with patent foramen ovale which was complicated by stroke. The mass was surgically excised and patent foramen ovale was closed. Background: Trauma is a critical public health issue, particularly impacting the young and causing significant financial and moral losses. Approximately one-third of trauma-related hospitalizations and 20-25% of trauma-related deaths are caused by serious chest injuries. We aimed to determine the prognostic value of changing pediatric global troponin levels in the emergency department after trauma. Materials and Methods: Patients who applied to the emergency department with complaints of trauma between January 1, 2015 and January 31, 2022 were evaluated. Records of pediatric patients between the ages of 0 and 18 were reviewed. The cause and severity of the trauma, electrocardiography and echocardiography results, and laboratory data (biochemical, hemogram, C-reactive protein, pro-BNP, CK-MB, troponin, etc.) of the patients were evaluated. This study is an observational and retrospective. Results: Falling from height was the most prevalent cause of injury, accounting for 43% of cases. Chest trauma was observed in 55 patients (56%). All patients with chest trauma exhibited elevated troponin levels. Troponin levels were found to be significantly higher in patients with chest trauma (p=0.011). Conclusions: Our findings highlighted the prognostic importance of cardiac troponins, especially in pediatric patients presenting with trauma. [ABSTRACT FROM AUTHOR]

Objective: Organic acidemias are intoxication-type inborn errors of the metabolism with multiple organ involvement. Patients with organic acidemia usually present in the neonatal or infantile period with high anion gap metabolic acidosis and hyperammonemia. The present study investigates the presence of congenital heart defects and secondary heart diseases in patients with organic acidemia. Material and Methods: Included in the study were 31 patients of whom 14 were diagnosed with methylmalonic acidemia (MMA), 11 with propionic acidemia and six with isovaleric acidemia. The cardiac findings of all patients included in the study were evaluated. Results: Of the sample, 63.64% were identified with accompanying congenital heart disease, with the most common diagnosis being propionic acidemia and the most common heart defects being atrial septal defects and mitral regurgitation. Conclusion: The accumulation of toxic intermediate metabolites due to enzyme deficiency is thought to be the main mechanism behind the cardiac involvement noted in organic acidemias. In the presence of unexplained deterioration, the potential for organic acidemia to accompany congenital heart disease should be kept in mind, and so it is important to screen patients with organic acidemias by echocardiography. Background/Aims: Polycythemia vera (PV) and essential thrombocythemia (ET) are chronic myeloproliferative diseases. Along with the increase in myeloproliferative cell lines, they can cause various clinical outcomes such as arterial and venous thrombosis, pulmonary hypertension, and myocardial infarction. This study was designed to evaluate the cardiac effects of these diseases via electrocardiography and echocardiography. Methods: A total of 50 patients with a diagnosis of PV, 50 patients with a diagnosis of ET, and 50 healthy individuals as a control group were included in this study. Data on patient demographics were recorded in all subjects. All patients and control subjects had electrocardiography (ECG) recordings and routine transthoracic echocardiographic examination. Pulse wave velocity (PWV) was assessed with a Holter blood pressure device. Results: In total, 50 PV patients, 50 ET patients, and 50 control group were included in the study. The demographic characteristics of the PV, ET and control groups were similar. The PR interval was significantly shorter in control subjects than in PV and ET patients (p:0.007, p:0.024). Although the measured values were within normal limits, diastolic posterior wall thickness was significantly lower in the control group compared to PV and ET patients (p:0.019, p:0.009). PWV was significantly higher in ET patients compared to the control group (p:0.012). Conclusion: In this study, evaluating the effects of PV and ET on electrocardiography and transthoracic echocardiography; ECG parameters used to predict ventricular arrhythmias (QT, QTc, Tp-Te, Tp-Te/QT) and Pulmonary Artery Pressure showed no significant change, in opposition to existing literature. Nonetheless, similar to previous publications, PV and ET were found to negatively affect the diastolic function parameters on transthoracic echocardiography. While the aortic stiffness was significantly higher in ET patients compared to the control group, no significant difference was noted between PV patients and control subjects in terms of aortic stiffness.

Objective: Ebstein’s anomaly (EA) is a rare congenital malformation associated with high perinatal mortality. In this study, we aimed to assess perinatal outcomes and factors associated with mortality in fetuses with EA. Material and Method: In this study, fetuses diagnosed with EA from 2016 to 2022 were included. Clinical information, ultrasonographic findings, and Doppler parameters were collected retrospectively. Result: A total of 14 fetuses with EA were included. Twelve patients had isolated EA, while one had unilateral renal agenesis and the other had mega cisterna magna. Amniocentesis was performed in six cases and normal results were obtained. Nine fetuses reached term pregnancy. All fetuses survived to birth. Three cases died in the first week after birth. Survivors were found to have lower tricuspid valve annulus diameter z score when compared with non-survivors (4.81±3.05 vs 6.97±1.26, respectively, p=0.05). Reversal pulmonary artery flow (2/11 vs 4/4 in survivors and non-survivors, respectively, p=0.01) and pulmonary atresia (1/11 vs 4/4 in survivors and non-survivors, respectively, p<0.01) were significantly more frequent in non-survivors. Conclusion: Perinatal mortality remains high in EA. Higher tricuspid valve annulus diameter z score, presence of reversal pulmonary artery flow, and pulmonary atresia during prenatal diagnosis are associated with poorer diagnosis. Future multicenter studies are warranted to identify risk factors and guide perinatal management. [ABSTRACT FROM AUTHOR]

12. A Speckle-Tracking Echocardiography Study: Cardiac Effect of Sleep Duration.

Objective: Sleep is a complex event involving various physiological processes. Recent studies during the last few years have shed light on the effects of sleep duration on health. We investigated the cardiac effects of sleep duration on speckle-tracking echocardiography. We divided the patients into 2 groups who slept more or less than 7 hours and evaluated all of them with transthoracic echocardiography. Methods: This study was a retrospective study was conducted to investigate the cardiac effects of sleep duration. Between June 2021 and February 2022, 129 individuals who were admitted to our hospital for routine check-up with no known chronic disease and with normal and good quality echocardiographic examination were included in the study. The patients who were eligible for the study design were called by the investigator to obtain information on average sleep duration, and two groups were defined by the self-reported sleep duration (=7 hours, >7 hours per day) and were used in the statistical analyses. Results: A total of 129 patients [=7 hours (n=80), >7 hours (n=49) per day] were enrolled in this study. All echocardiographic parameters were compared with sleep duration. Among these parameters, left ventricular (LV) global longitudinal strain (GLS) was found to be greater in the group who slept less. (-21.5±2.6 vs. -20.3±2.2 p=0.005). However, being within normal limits, LV end diastolic diameter and right heart chambers were found to be more enlarged in those who slept 7 hours or less than 7 hours. It was also observed that the right atrial volume was higher in the less sleeper group. Conclusion: As a result, GLS, which is one of the parameters showing LV function, was found to be less in healthy people with a sleep time above a certain period, and we believe that this may be a hypothesis to explain the relationship of long sleep time with cardiovascular events. 14. Early Echocardiographic Findings of Pulmonary Hypertension-specific Therapy in Children.

Objective: Echocardiography is a very useful tool for the diagnosis and evaluation of pulmonary hypertension (PH). This study was planned to investigate whether echocardiographic (ECHO) data of patients with PH are effective in the follow-up and course of treatment. Methods: A retrospective analysis of the data from 26 PH patients was performed. Analyses were performed on the data of the patients, including their demographics and ECHO findings. The ECHO measurements of the patients were labeled as 0 (beginning of the PH specific therapy), 1 (on the 15th day of the therapy), 2 (one month after the previous echocardiogram). Results: The left ventricle ejection fraction (EF) (p=0.05) and fractional shortening (FS) (p=0.038) values in ECHO2 were significantly higher than those in ECHO1 . Aortic velocity-time integral1 (VTI1 ) was significantly higher than aortic VTI0 (p=0.001; p<0.01), and tricuspid annular plane systolic excursion2 (TAPSE2 ) was significantly higher than TAPSE0 (p=0.046). Moreover, right ventricular ejection time1 (RVET1 ) was significantly higher than RVET0 (p=0.034), and left ventricular ejection time1 (LVET1 ) was significantly higher than LVET0 (p=0.003). Conclusions: This study provides information on ECHO parameters that improve during the initial stages of therapy. Based on the results of our study, even at the beginning of treatment, there were increases in right and left ventricular filling, EF, and FS. Clinical deterioration of PH can be detected early/before the clinical status of the patient worsens with detailed examinations using echocardiography. [ABSTRACT FROM AUTHOR]

15. Echocardiographic Evidence of Left Ventricular Hypertrophy in Obese Cats.

The primary objective of this study was to assess how obesity impacts the occurrence of left ventricular hypertrophy in healthy cats. The researchers evaluated the body condition score of all participating cats before conducting the study. Additionally, they performed various echocardiographic examinations, including transthoracic 2D, M-mode, and Doppler, as well as measuring systolic blood pressure. The results indicated a notable distinction in body condition scores between the non-obese and obese groups (P < .001). Moreover, significant differences were observed in terms of systolic blood pressure (SBP) and heart rate between the 2 groups (P < .001, P = .002). Echocardiographic measurements revealed statistically significant variations in left atrium and left ventricular free wall thickness between non-obese and obese cats (P < .001 and P = .003). To address obesity, the obese group underwent a lowcalorie diet, leading to improvements in echocardiographic parameters and SBP. However, these improvements did not achieve statistical significance. The study findings indicate that obesity can lead to substantial myocardial changes, resulting in left ventricular hypertrophy in cats, which has been previously reported in humans and dogs. Additionally, the study suggests that a diet intervention for obese cats might help prevent diseases like hypertrophic cardiomyopathy. [ABSTRACT FROM AUTHOR]

Apical hypertrophic cardiomyopathy (ApHCM) (Yamaguchi Syndrome) with hypertrophy of the ventricular apex constitutes 8% of the hypertrophic cardiomyopathies (HCMs). ApHCM can cause ventricular malignant dysrhythmias, atrial fibrillation, and ischemic chest pain. Definitive diagnosis is made by electrocardiography (ECG) and transthoracic echocardiography. A 73-year-old male patient was admitted to the emergency department with chest pain. The patient's vital signs were within the normal range. In the ECG, there were giant negative T wave in leads V4-5-6, 0.5 mm ST segment depression, and left ventricular hypertrophy in the inferior derivations. The left ventricular apex thickness was measured as 14 mm (reference range: 6-11). Although the HEART score was 4, the preliminary diagnosis of the patient was determined as ApHCM. Beta-blocker and antiplatelet therapy were started. The mortality and morbidity rates of ApHCM are higher among HCMs. Clinicians should be aware of such ECG and echocardiography findings to prevent possible morbidity and mortality.

18. Two Dimensional Speckle Tracking Echocardiographic Evaluation of Left Ventricle in Post-COVID-19 Patients with or without Cardiac Symptoms: A Retrospective Clinical Study.

Objective: In recent years, coronavirus disease-2019 (COVID-19) has been the primary health problem and because of the virus affinity to endothelial cells, it has become an important reason of vascular problems and cardiac injury. After mild COVID-19 infection, patients frequently attend cardiology clinics with cardiac symptoms and their primary cardiac tests are mostly normal. In this study we aimed to analyze if two-dimensional speckle tracking echocardiography shows any difference between symptomatic and asymptomatic post-COVID- 19 patients when transthoracic echocardiography parameters are normal. Material and Methods: In this retrospective single center study, 2,741 transthoracic echocardiography records were assessed and 108 post-COVID-19 patients were detected and divided into symptomatic and asymptomatic patient groups and left ventricular global longitudinal strain (LV-GLS) values were compared. Results: The number of patients with normal LV-GLS values were equal in the groups and there were 4 patients whose values were borderline in the asymptomatic group, while there was none in the symptomatic group. The number of patients with impaired GLS values in the symptomatic group were higher than the asymptomatic group (15 vs 4 patients) and the difference was statistically different (p=0.008). The average LV-GLS values were -18.88±2.50 in the asymptomatic group and -17.40±3.68 in symptomatic group but the difference was not statistically significant (p=0.098). Conclusion: More symptomatic patients than the asymptomatic ones have impaired LV-GLS values according to the results of this study. Even if it is not statistically significant, the mean LV-GLS values are also reduced in symptomatic patients after mild COVID-19 infection. [ABSTRACT FROM AUTHOR]

Material and Method: This study enrolled children under the age of 18 years who were referred with chest deformity to our pediatric cardiology unit, over a period of six years (January 2017-December 2022). Results: The mean age of the patients was 9.9 ± 5.2 years, median 11 years (0-18 years old). 89 patients with abnormal echocardiographic findings: 42 (9.56%) mitral valve prolapse, 18 (4.1%) atrial septal defect, 9 (2%) ventricular septal defect, 8 (1.82%) bicuspid without aortic valve stenosis aortic valve, 5 (1.1%) patent ductus arteriosus, 2 (0.45%) pulmonary stenosis, 2 (0.45%) great artery transposition, 2 (0.45%) hypertrophic cardiomyopathy, subaortic ridge 1 (% 0.27). Cardiac compression was present in 13.4% of the cases with pectus excavatum. 13(%3) patients were operated by a thoracic surgeon. Marfan Syndrome was diagnosed in 7 patients and Noonan Syndrome was diagnosed in 2 patients who applied to our clinic with chest deformity. Conclusion: We suggest that echocardiographic examination in patients with chest deformity is important in the diagnosis of congenital heart diseases, early diagnosis and treatment of heart compression finding.

20. Evaluation of cardiac biomarkers, electrocardiogram changes and echocardiography findings before and after exercise in paediatric athletes.

Purpose: The aim of this study was to investigate the structural and electrical changes in the heart associated with exercise in young athletes under 18 years of age and to examine the relationship between these changes and biochemical markers. Materials and Methods: In this study, 32 male paediatric athletes (five rowers, five kayakers and 22 footballers) were prospectively evaluated. Dynamic exercise training for 75 minutes was applied to all athletes. An echocardiographic evaluation was performed on the athletes at rest. An electrocardiogram (ECG) was taken before and after the exercise. Blood samples were taken from all athletes before the exercise, during the first hour of exercise and after the fourth hour of the training session to analyse the serum levels of creatine kinase myocardial band, troponin T(cTnT) and pro-brain natriuretic peptide. Results: The mean age of all athletes was 15.3±0.9 years. In the echocardiography examination, end-diastolic thickness, left ventricular end-diastolic diameter, posterior wall end-diastolic diameter, left ventricular end-systolic diameter, left ventricular mass measurements (LVM) and left ventricular mass indices (LVMI) were higher among the rowers than in the remaining groups. In the postexercise ECG, T-wave peaks were observed in 16 individuals, including all rowers, all kayakers, and six footballers. The cTnT values in the first hour and after the fourth were 0.31--0.71 ng/ml in football players, 0.10--0.27 ng/ml in rowers, and 0.04--0.15 ng/ml kayakers. An increase in cTnT levels was detected after the fourth hour of exercise in all groups. There was a correlation between T-wave peaks in the ECG taken after exercise and age, cTnT serum level and increased LVM. Conclusion: In this study, we found T-wave peaks in half of the athletes after exercise. The serum cTnT levels of young athletes may be elevated by exercise and that LVM and LVMI may be higher in athletes. Echocardiography and ECG evaluations should be performed in paediatric athletes. Objective: Heart rate variability (HRV) is a non-invasive method used to assess the status of autonomic function of the heart. The aim of this prospective study was to assess HRV and cardiac function in children with epilepsy. In addition, the effects over HRV of epilepsy type, therapy regimens, refractory and well controlled epilepsy were investigated. Material and Method: Between September 2013 and March 2014, 61 epileptic children and 53 healthy children were included in the study. All subjects were evaluated by electrocardiography, echocardiography, and 24 hours rhythm holter. Results: In the children with epilepsy, it was determined a significant reduction in all the time domain (SDNN, SDNN index, SDANN, RMSSD, pNN50 p-values 0.011, 0.001, 0.021, 0.001, 0.001, respectively) and frequency domain (LF, HF, VLF p-values 0.001, 0.001, 0.001 respectively) parameters. There were not statistically significant differences in both time and frequency domain parameters between children with partial and generalized epilepsy. There was a significant reduction in SDNN, SDNN index, SDANN, RMSSD, LF, HF, VLF (p-values 0.009, 0.008, 0.015, 0.039, 0.004, 0.010, 0.005, respectively) values in the children who underwent polytherapy regimen. Conclusion: Epilepsy is associated with suppressed HRV values indicating decreased vagal activity. Having pathological EEG findings, and polytherapy regimen associated with refractory epilepsy are effective on suppressed HRV. 24. Serum Uric Acid is Associated with Right Heart Failure in Patients with Heart Failure with Reduced Ejection Fraction: A Cross-Sectional Study.

- Abstract
Objective: Serum uric acid (UA), which is the final product of purine metabolism, is a predictor of left-sided heart failure (HF), as well as being a marker of outcomes and survival in patients with heart failure with reduced ejection fraction (HFrEF). Right-sided HF may or may not accompany HFrEF, but is associated with worse prognosis and increased morbidity and mortality when present. All available data for the association between RHF and UA are derived from patients with pulmonary arterial hypertension, and virtually no data is available for patients with HFrEF. Our aim was to understand the relationships between UA, echocardiographic markers of right ventricular systolic function and RHF in patients with HFrEF. Material and Methods: A total of 45 patients with an ejection fraction <40% and signs of HF were included. Patients were divided into tertiles according to serum UA concentration. RHF was defined according to the modified Interagency Registry for Mechanically Assisted Circulatory Support criteria. Results: Compared to patients within the lowest UA tertile, patients within the 3rd tertile had a significantly higher incidence of RHF (50.0% vs. 0.0%, Bonferroni-corrected p=0.009) and a significantly lower tricuspid annular plane systolic excursion (20.5±4.4 vs. 16.1±4.4, p=0.02). After adjusting for relevant clinical, demographic, laboratory and echocardiographic variables, serum UA remained a significant predictor of RHF [odds ratio: 2.89, 95% confidence interval (CI): 1.21-6.91, p=0.017]. For serum UA, the c-statistic for determination of RHF was 0.83 (95% CI: 0.71-0.96). Conclusion: Serum UA is associated with the occurrence of RHF in patients with HFrEF. [ABSTRACT FROM AUTHOR]

A patient suspected of Covid-19 pneumonia, in whom pneumonia lesions were first detected by chest computed tomography, was further evaluated by transthoracic echocardiography due to cardiomegaly. Transthoracic echocardiography revealed a large right atrial-related ruptured pseudoaneurysm. Contrast-enhanced computed tomography ruled out bilateral multiple lung nodules and pulmonary embolism associated with mural thrombus in the right atrial pseudoaneurysm.

The aim of this study was to investigate the short-term cardiovascular effects of medetomidine/ketamine (MED-KET) and xylazine/ketamine (XYZ-KET) anesthesia regimes and its reversal (atipamezole) in cats through using echocardiography. Twenty sexually intact female cats (weighing between 2.2 and 3.6 kg, and between 0.5 and 5.5 years of age) referred for ovariohysterectomy procedure. Electrocardiogram, noninvasive blood pressure, respiratory rate, heart rate and pulse oximetry were monitored throughout the anesthesia. Both groups showed declines in left ventricular diameter (LVD%) and left ventricular ejection fraction (EF) after anesthesia induction (T0 vs. T1, all, p<0.05); nevertheless, the reduction and starting pattern of LVD% and left ventricular EF presented important intergroup distinction. In conclusion, echocardiographic values effected significantly after anesthesia with MED-KET and XYZ-KET; and atipamezole can be useful for return to baseline values quickly and safely in cats after abdominal surgery such as ovariohysterectomy. Purpose: Acute cerebrovascular diseases is an important reason of mortality and morbidity. Aortic stiffness is a important predictor of cerebrovascular, cardiovascular (CV) and all cause death, deadly and non-deadly coronary and neurologic situations, and severe strokes. In our study, we researched the association between aortic stiffness index (ASI) and stroke seriousness in acute ischemic stroke patients. Materials and Methods: A total of 97 patients (females, 55; males, 42; 65 ± 16 years) with acute ischemic stroke were enrolled in this study. 17 patients were excluded. 80 patients were evaluated. Subjects were categorized into two groups according to the calculated NIHSS score (Group 1, NIHSS score < 16; Group 2, NIHSS score ≥ 16). Clinical characteristics, echocardiographic and laboratory parameters for all patients were evaluated. Cardiac parameters and aortic stiffness index were evaluated by two-dimensional echocardiography within 48 hours of hospitalization to the neurology clinic. Results: There were no significant differences among the groups for age, gender, hypertension, diabetes mellitus, smoking, SBP, DBP, heart rate, dyslipidemia, infarct volume, troponin, HbA1c, glucose, creatinine, LDL cholesterol were significantly higher in Group 2 patients than in Group 1 patients. LV wall thickness, aortic stiffness index, E/e’ were significantly higher in Group 2 patients than in Group 1 patients. LVEF was significantly lower in Group 2 patients than in Group 1 patients. Conclusion: ASI was significantly higher in patients with severe acute ischemic stroke. Higher ASI is associated with higher NIHSS score in patients with acute ischemic stroke

Amaç: Bu çalışmanın amacı presistolik dalga (PSD) varlığı ve non-dipper hipertansiyon arasındaki ilişkiyi araştırmaktır. Ayrıca PSD oluşumuna sol atriyum fonksiyonlarındaki değişimin etkisi olup olmadığı araştırılması amaçlanmıştır. Gereç ve Yöntem: Bu kesitsel çalışmaya 77 dipper ve 43 non-dipper hipertansiyonu olan 120 hasta dahil edildi. Dipper ve non-dipper hipertansiyon grupları, PSD varlığı ve sol atriyum fonksiyon ekokardiyografik parametreleri açısından birbirleriyle karşılaştırıldı. Bulgular: Sol atriyum aktif boşalma fraksiyonu (SAABF) (38.6 ± 2.8’ e karşı 35.1 ± 3.3) ve PSD insidansı (%69.8’ e karşı %37.7) non-dipper hipertansiyon grubunda dipper hipertansiyon grubuna göre anlamlı olarak daha yüksekti. Yaş, miyokardiyal pik erken diyastolik hız (Em), sol atriyum minimum hacmi, SAABF (odds oranı = 1.719, 95% güven aralığı = 1.367–2.162) ve PSD (odds oranı = 7.116, 95% güven aralığı = 2.063–24.548) non-dipper hipertansiyonun bağımsız öngördürücüleri olarak bulundu. Presistolik dalgası olan ve olmayanlar içinde non-dipper hipertansiyon ve dipper hipertansiyonu olan gruplar SAABF’ sine göre karşılaştırıldığında non-dipper hipertansiyon grubunda SAABF anlamlı olarak daha yüksek izlendi. Sonuç: PSD'nin varlığı hipertansiyonu hastalarında daha yüksek riskli olan non-dipper grubun kolayca saptanmasında fayda sağlayabilir. Bu hasta grubunda PSD' nin daha sıklıkla oluşumu diyastolik dolumu korumak için artmış sol atriyum kasılmasından bağımsızdır.

Early diagnosis of congenital heart diseases (CHD) is of great importance in the morbidity and mortality of the newborn. Physical examination is not always helpful in diagnosis. Screening for critical congenital heart disease with pulse oximetry is useful for early diagnosis of asymptomatic babies. In this article, 3 cases who were diagnosed with congenital heart disease due to low oxygen saturation without respiratory problems after hospitalization in the neonatal intensive care unit for different reasons were discussed. Two patients were diagnosed with pulmonary atresia with ventricular septal defect, and one patient was diagnosed with truncus arteriosus type 4. One of the patients in whom prostaglandin E1 infusion was initiated after the diagnosis was made by echocardiography died. Surgical treatment was planned for the other two patients. A baby without significant cyanosis who has low saturation with critical CHD screening may have ductus dependent heart disease, if respiratory distress is not observed when followed up in hospital, if a lung-induced ventilation-perfusion disorder is not considered, CHD should be considered first. In the second level neonatal intensive care units or when the opinion of a pediatric cardiology physician is not available, starting prostaglandin E1 infusion without waiting for ecocardiography and the patient is transfer to a center with a pediatric cardiology unit can be life-saving. - Abstract
Objective: Echocardiography is an important and pratical diagnostic tool for early diagnosis and follow-up of CHD. In our study, we aimed to evaluate the frequency, distribution of diagnoses and risk factors of cases diagnosed with CHD in the neonatal intensive care units (NICU) and neonatal services. Material and Methods: 2766 patients who were followed in NICU and neonatal services between May 2013 and May 2017 were retrospectively analyzed. 365 newborn diagnosed with CHD of 762 babies were requested pediatric cardiology consultation were included in the study. Cases diagnosed with patent foramen ovale and patent ductus arteriosus (PDA) that closed in first 3 days were excluded from study. Results: The frequency of CHD in our center was 13.2%. Most of the cases were term births, 60.5% of them were male and 30.9% were female.The most common reasons for hospitalization were respiratory diseases (28.3%). There was kinship between the parents of 19.9%.The most common reasons for cardiology consultation were murmur (27.4%), respiratory distress (15.9%) and prematurity (10.1%). Ninety six point two percent of the patients had acyanotic CHD, and 3.8% had cyanotic CHD. The most common acyanotic CHD, in order of frequency, were atrial septal defect (ASD) (40.8%), pulmonary stenosis (PS) (15.3%), PDA (13.4%), ventricular septal defect (VSD) (9.3%). The most common cyanotic CHD, in order of frequency, were hypoplastic left heart syndrome (1.1%), pulmonary atresia (0.8%) and double outlet right ventricle(0.8%). Seven point one percent of the patients had a genetic syndrome accompanying CHD, and Down syndrome was in the first place with 38.5%. Major extracardiac anomaly was found in 10.1% of the patients. While 9.3% were followed up as diabetic mothers' babies, 20.6% of them had septal hypertrophy. One-time pulse oximetry measurements were made in our study. In 72.7% of the cyanotic CHD group SatO2 was less than 90%, whereas in 76.3% of the acyanotic CHH group, SatO2 was 95% and above, and it was found to be significantly different (p <0.05). It was observed that the hypoxemia accompanying acyanotic CHD was mostly due to respiratory diseases (66.7%). While 21 patient (5.8%) were referred to other centers for further examination and treatment, 25 patient (6.8%) were mostly lost due to sepsis. Conclusion: Echocardiography seems to be an important diagnostic tool in newborns hospitalized in the NICU. Early detection of CHD will be beneficial in applying interventional procedures early and in reducing mortality and morbidity. [ABSTRACT FROM AUTHOR]

39. N-TERMINAL PROHORMONE OF BRAIN NATRIURETIC PEPTIDE IN THE DIAGNOSIS AND MANAGEMENT OF PERSISTENT PULMONARY HYPERTENSION IN NEWBORNS.

Objective: This study aimed to investigate the levels and utility of amino/N-terminal prohormone of brain natriuretic peptide (NT-proBNP) in the diagnosis of neonatal persistent pulmonary hypertension (PPHT). Materials and Methods: Infants born at ≥34 weeks of gestation were included in this retrospective cross-sectional study. In total, 33 newborns diagnosed with PPHT were included in the patient group and 50 healthy newborns were included in the control group. Patient and control groups were compared in terms of plasma NT-proBNP levels measured in the umbilical cord (UC) and at 72 hours of life. Results: The NT-proBNP levels in UC and at 72 hours were significantly higher in neonates with PPHT compared to controls (p<0.01). For the diagnosis of PPHT, UC NT-proBNP cut-off value was >2760.5 pg/ml, sensitivity was 90.7%, and specificity was 96.6% (p<0.01). For NT-proBNP at 72 hours, the cut-off value was >1414 pg/ml, sensitivity was 95.9%, and specificity was 91.2% (p<0.01). The mean UC NT-proBNP levels were 1094.7±603 pg/ml and mean NT-proBNP levels at 72 hours were 875.7±423 pg/ml in the control group. Conclusion: NT-proBNP levels are high during the initial days of life. They are useful in the diagnosis and follow-up of PPHT in newborns with hypoxemic respiratory failure and high fraction of inspired oxygen (FiO2) requirement, especially in cases where transthoracic echocardiography (TTE) cannot be performed. Amiloidoz; organ ve yumuşak dokuların hücre dışı boşluğunda amiloid fibril birikiminin neden olduğu bir hastalıktır. Öncü proteinin tipine göre amiloidoz sınıflandırması yapılır. Kalp, böbrek, karaciğer, gastrointestinal sistem ve/veya otonom sinir sistemi gibi çeşitli organlarda amiloid birikimi izlenebilmekle birlikte, prognozu en kötü olan organ tutulumu türü kardiyak amiloidozdur. Birçok amiloidoz türleri arasında hemen hemen tüm klinik kardiyak amiloidoz vakalarına, transtiretin amiloidoz (ATTR) veya hafif zincir amiloidoz (AL veya primer sistemik) neden olur. Kardiyak ATTR; kalıtsal (ATTRm) veya doğal tip (ATTRwt) olabilir. Kardiyak tutulum sıklığı ve kardiyomiyopatinin prognozu amiloidoz tipleri arasında değişkenlik gösterir ve klinik belirtiler, organ tutulum paternine bağlı olarak değişir. Değişken klinik fenotip ve genellikle özgül olmayan klinik özellikler, bu hastalıkta tanıda gecikmelere neden olabilir ve tanıda multidisipliner (hematoloji, patoloji, radyoloji, nükleer tıp, nöroloji, nefroloji, kardiyoloji, romatoloji, gastroenteroloji) yaklaşım gerekir. Gelişen tanı yöntemleri klinisyene kardiyak amiloidozda erken tanı olanağını vermektedir. Kardiyak amiloidozdan şüphelenilen hastalarda; görüntüleme yöntemleri ve gerekirse doku biyopsisi ile amiloid birikimi gösterilmelidir. Amiloidoza sebep olan öncü protein saptandıktan sonra, altta yatan hastalığın tedavisi ve semptomatik hastalarda kalp yetmezliğine yönelik tedavi uygulanmaktadır. Erken tanı ve tedavi yaklaşımları ile prognozun düzeltilebildiği gösterilmiştir. Bu derlemede, kardiyak amiloidoz hastalığı hakkında farkındalığın artırılması amaçlanmış olup, günlük pratiğimizde kullanabileceğimiz güncel tanı yöntemleri ve tedavi yaklaşımları ele alınacaktır.

Purpose: The aim of our study was to compare the right ventricular systolic and diastolic functions and pulmonary artery pressure (PAP) in obese and non-obese patients with obstructive sleep apnea syndrome (OSAS) by tissue Doppler imaging (TDI) echocardiography. Materials and Methods: This study was conducted with 69 patients, 34 obese and 35 non-obese, diagnosed moderate or severe OSAS by an overnight polysomnographic sleep study. In all patients, LV (left ventricle) and RV (right ventricle) size, left atial (LA) and RA (right atrial) dimensions, LV and RV systolic and diastolic functions and systolic PAPs were measured by M-mode, two-dimensional analysis, color flow Doppler and TDI. Results: RV diastolic dysfunction was detected in both groups; this impairment was significantly higher in the obese group (lateral tricuspid late diastolic myocardial annular zone velocity A'a: 0.13 ± 0.03 in non-obese patients and 0.11 ± 0.04 in obese patients). The mean systolic PAP was significantly higher in obese patients (31.2±5.6, 27.1±5.8, respectively) Conclusion: Obstructive Sleep Apnea Syndrome increases cardiovascular morbidity and mortality. In our study,left ventricul and right ventricul diastolic dysfunction was determined by tissue Doppler imaging in patients with moderate and severe Obstructive Sleep Apnea Syndrome. Obesity contributes to this impairment regardless of Obstructive Sleep Apnea Syndrome .

Amaç; Bu çalışmanın amacı; Obstrüktif uyku apne sendromu (OUAS) olan hastalarda, 2-boyutlu ve pulse dalga doppler (PDD) ekokardiyografi teknikleri kullanılarak SV fonksiyonlarının değerlendirilmesidir. Yöntem; Çalışmaya polisomnografi ile OUAS tanısı konulan ve henüz tedavi uygulanmamış 40 hasta ve 28 sağlıklı birey kontrol grubu olarak alındı. Hastalara M-mod, 2- boyutlu ve PDD ekokardiyografi incelemeleri yapıldı. Bulgular; Gruplar arasında M-mode ölçümleri ile elde edilen SV sistolik ve diyastolik çapları, ejeksiyon fraksiyonları açısından fark saptanmadı. OUAS’ li grupta SV kitle ve kitle indeksi ve SV diyastolik duvar kalınlıkları, kontrol grubuna göre anlamlı olarak daha fazla tespit edildi. Transmitral diyastolik doluş parametrelerinin PDD ekokardiyografi ile yapılan incelemesinde; OUAS grupta erken diyastolik doluma ait E dalga hızında, E/A oranlarında anlamlı azalma izlendi. E dalga deselerasyon zamanı ve izovolümetrik gevşeme zamanının OUAS’ lı grupta anlamlı olarak daha uzun saptandı. Apne-hipopne indeksi ile SV diyastolik parametreler arasında ilişki saptanmadı. Sonuç; OUAS, altta yatan herhangi bir kalp hastalığı olmasa da SV kitlesini etkileyen ve hastalığın şiddetinden bağımsız olarak SV diyastolik fonksiyonlarını bozan bir durumdur. Bu nedenle OSAS hastaları kardiyak açıdan da mutlaka değerlendirilmelidir.
