Introduction: To compare the effectiveness of tadalafil alone and in combination with dapoxetine in patients with premature ejaculation. Methods: In this study, 120 patients treated at the urology clinic for premature ejaculation were evaluated prospectively. IELT and IIEF forms of patients were filled before this study, and their urological examinations were performed. Patients that were being treated were divided into two groups. Patients in group 1 received only 5 mg tadalafil, whereas patients in group 2 received a combination of dapoxetine 30 mg and tadalafil 5 mg. Patients with retained stents who have not followed up for a long time are difficult cases for urologists. They may be admitted to the hospitals because of several complaints like flank pain, pyelonephritic attacks, stent migration, encrustation, calcification and even renal failure by means of obstruction. Rarely,long standing stents may become fragmented. In the present case, a 81 year-old woman was admitted to our inpatient clinics with initial complaints of dysuria and flank pain. Her history revealed that she had been performed ureterorenoscopy for the removal of ureteral stone and placed double-j stent 2 years ago. After subsequent evaluations, previously forgotten and fragmented stent has been determined to be the cause of complaints of the patient., Takibe gelmeyen hastalarda içerde bırakılmış olan stentler ürologlar için zorlu olgulardır. Hastalar tekrarlayan böğür ağrısı, piyelonefrit atakları, stent migrasyonu, enkrustasyonu, taşlaşması ve böbrek yetmezliğiyle sonuçlanacak obstrüksiyon nedenleri ile başvurabilirler. Nadiren uzun yerleştirme süreleri stentlerin parçalanmalarına yol açabilir. 81 yaşındaki kadın hasta dizüri ve yan ağrısı nedeniyle kliniğe kabul edildi. Özgeçmişinde bir dış merkezde 2 yıl önce üreterorenoskopi ile üreter taşı alındığı, çift J kateter takıldığı bildirildi. Yapılan tetkikler sonrası hastada unutulmuş ve kopuk stentin bu yakınmalara yol açtığı belirlendi.

42-year-old male admitted to the emergency department with penile swelling and hematoma after a sexual intercourse. The patient had gone under surgical exploration. There was not any problem in the integrity of tunica albugenia. The superficial dorsal vein which was ruptured was found and ligated. Postoperative sexual function and erection was completely normal. Rupture of the superficial dorsal vein of the penis occurs rare, but it must be kept in mind in the diagnosis of penile fracture in clinical conditions such as lack of sudden detumescence, painless penile swelling and hematoma., 42 yaşında erkek hasta ilişki sonrası penil şişlik ve hematom nedeni ile acil servise başvurdu. Hasta opere edildi. Tunika albüginea bütünlüğü tam idi. Rüptüre süperfisial dorsal ven bulundu ve ligate edildi. Postoperatif seksüel fonksiyon ve ereksiyon tamamen normal idi. Penisin süperfisial dorsal ven ruptürü nadir görülür ancak ağrısız şişlik ve hematom olması, ani detümesans olmaması gibi klinik durumlarda penil fraktür ayırıcı tanısında akılda tutulması gerekir.

Purpose: We aimed to present the data on totally tubeless nephrolitotomy (PNL) outcomes undergone in our clinic. Materials and Methods: We retrospectively analyzed 140 patients undergone totally tubeless PNL in our clinic between January 2010 and December 2012. Results: Mean patient age was 47,84 (5-73). Mean operative time was 88,8 (45-110) minutes where mean stone area was 375,3 (110-175) mm2 . 92,8% was our stone free rate. Remaining 10 patients had undergone ureterorenoscopy for ureteral stones. Two patients having non-opaque stones had residual kidney stones and treated by SWL. 8 patients (%5,7) needed blood transfusion, 2 patients needed perirenal hematoma and treated conservatively. No patients needed additional treatment or encountered vascular complications. One patient encountered serozal colon injury and treated conservatively. Conclusion: With regard to patient comfort, shorter hospital stay, low analgesic requirement, totally tubeless PNL gained popularity. We believe totally tubeless PNL may be used in suitable cases. Key Words: Kidney Stone, Totally Tubeless, Nephrolithotomy, Amaç: Bu çalışmada tam tüpsüz perkutan nefrolitotomi (PNL) uygulamasıyla ilgili deneyimlerimiz aktarılmıştır. Materyal-Metod: Ocak 2010-Aralık 2012 tarihleri arasında böbrek taşı olan 140 olguda uygulanan tam tüpsüz PNL yönteminin sonuçları incelendi. Bulgular: Hastaların yaş ortalaması 47,84(5-73) idi. Ortalama operasyon süresi 88.8(45-110) dk, taş alanı ise 375.2(110-175) mm² idi. Taşsızlığa ulaşma oranımız KÖRF dahil 130(%92.8) olarak hesaplandı. PNL uyguladığımız hastalarımızdan 1 tanesinde serozal kolon yaralanması 2 hastada postoperatif perirenal hematom izlendi. Sonuç: Sonuç olarak uygun vakalarda tam tüpsüz PNL, günümüzde böbrek taşı tedavisinde taşsızlığa ulaşma noktasında standart PNL kadar güvenle uygulanabileceğini düşünmekteyiz.

Giriş: Uluslararası Prostat semptom skoru International Prostate Symptom Score IPSS güncel üroloji pratiğinde en sık kullanılan, tedavi kararının verilmesinde etkili semptom skorlarından biridir. Bu çalışmada farklı sebeplerle polikliniğe başvuran hastalara önce IPSS formu verilerek tamamlaması istenmiş takiben iki farklı merkezde eğitimlerine devam eden 2. yıl asistanı ve yine 2 farklı merkezde görevli uzman doktorlar eşliğinde farklı zamanlarda doldurulan IPSS sorgulama formuna verilen cevaplar değerlendirilerek aralarındaki farklar tartışılmıştır. Gereç ve Yöntemler: İki farklı merkeze Kasım 2011-Ağustos 2012 tarihleri arasında başvuran 50 yaş üzerindeki erkek hastalara uluslararası prostat semptom skoru formu verilerek doldurmaları istendi.Aynı sorgulama hastaya 2. yıl asistanı ve uzman doktor tarafından farklı zamanlarda uygulandı. Bulgular: Sorgulama formunu 3 farklı şekilde tamamlayan en az ilkokul mezunu toplam 143 hasta değerlendirmeye alındı. Hastaların ortalama yaşı 55,6 50-71 olarak saptandı. Çalışmada hastaların ortalama skoru 14,3±6.93 olarak hesaplanırken, asistan eşliğinde ortalama skor 17,2±6,16; uzman doktor eşliğinde sorgulama ile 18,0±5,72 olarak hesaplandı. Amaç: Perkütan nefrolitotomide preoperatif hidronefroz derecesinin operasyon süresine, kılavuz teli üretere atma süresine, floroskopi süresine, irrigasyon sıvısı kulanım miktarına, taşsızlığa ve kanamaya olan etkisini araştırdık.Gereç ve Yöntemler: Ocak 2009 – Aralık 2012 tarihleri arasında hastanemiz üroloji kliniğinde toplam 405 hastaya PNL operasyonu uygulandı. Hidronefrozu olmayan olgular Grade 0 Grup 1’e, düşük dereceli hidronefrozu olan olgular ise Grup 2’ye, ileri derece hidronefrozu olan olgular Grup 3’e dahil edildi. Bu çalışmada üç grubun operasyon süreleri, uygun access sağlanıp vizyon oluşturulana kadar geçen süre, floroskopi süreleri, operasyon öncesi ve sonrası hematokrit değerlerindeki değişim, taşsızlık oranları açısından karşılaştırıldi.Bulgular: Hasta sayıları Grup 1’de 120, Grup 2’de 160, Grup 3’de ise 125 idi. Gruplardaki hastaların yaş ortalaması Grup 1 de 47,76±15,14 yıl, Grup 2 de 42,31 ±15,06 yıl ve Grup 3’de ise 49,05±11,50 yıl olarak hesaplandı.
