The networks of the brainstem and spinal cord that co-ordinate locomotion and body orientation in lamprey are described. The cycle-to-cycle pattern generation of these networks is produced by interacting glutamatergic and glycinergic neurones, with NMDA receptor-channels playing an important role at lower rates of locomotion. The fine tuning of the networks produced by 5-HT, dopamine and GABA systems involves a modulation of [Ca.sup.2+]-dependent K+ channels, high- and low-threshold voltage-activated [Ca.sup.2+] channels and presynaptic inhibitory mechanisms. Mathematical modelling has been used to explore the capacity of these biological networks. The vestibular control of body orientation during swimming is exerted via reticulospinal neurones located in different reticular nuclei. These neurones become activated maximally at different angles of tilt.

Purpose: To compare patients' psychological distress and memories from intensive care unit (ICU) treatment 4-6 weeks after ICU discharge with expectations of their relatives. Further, to explore the relationship between personality traits and ICU memories with psychological distress.Methods: A cross-sectional study of 255 patients and 298 relatives. The questionnaire included: hospital anxiety and depression scale (HADS), impact of event scale (IES), life orientation test, ICU memory tool and memory of ICU; technical procedures, pain, lack of control and inability to express needs. Relatives were assessed for their expectations of the patients' memories and psychological distress.Results: Twenty-five percent of the patients reported severe posttraumatic stress symptoms, IES-total >or= 35. The levels of anxiety and depression were significantly higher than in the general population, mean anxiety was 5.6 versus 4.2 (p < 0.001), and mean depression was 4.8 versus 3.5 (p < 0.001). Relatives expected more psychological distress and the relatives thought the patient was less able to express needs than the patients reported (p < 0.001). Higher age, unemployment, respirator treatment, pessimism, memory of pain, lack of control and inability to express needs were independent predictors of posttraumatic stress symptoms (p < 0.01).Conclusions: Psychological distress symptoms were frequent among ICU survivors. Relatives expected the patients to be more distressed after ICU treatment than the patients reported. The strongest predictors of posttraumatic stress symptoms from the ICU were memoris about pain, lack of control and inability to express needs. Background: There is a strong need for longitudinal and representative studies that focus on doctors' well-being.Methods: We conducted a longitudinal and nationwide cohort study (N = 631) of graduating medical students (T1), followed up in their first (T2), fourth (T3) and ninth (T4) postgraduate years. Response rates varied from 62 to 83%; the longitudinal response rate was 41%. We used comparison samples from a general population survey. Predictors of life satisfaction among the doctors were tested by linear regressions and repeated measures statistics.Results: Doctors of both genders had significantly lower levels of life satisfaction than those in the comparison samples. Life satisfaction was lowest at T1, improved at T2, and stayed at the same level thereafter. Adjusted predictors of life satisfaction at T4 were lower age (P = 0.03), low level of neuroticism personality trait (P = 0.02), being married/cohabiting (P < 0.001), perceived social support (P < 0.001), lower work stress (P = 0.002), negative life events (P = 0.002), and frequency of physical training (P = 0.04). Only one-fifth of the total explained variance (R (2) = 0.34) was due to personality traits, whereas half was explained by married/cohabitant status and perceived social support. Being married/cohabiting, social support and physical training remained significant when mental distress (anxiety and depressive symptoms) was included in an additional multivariate model (R (2) = 0.49). Neuroticism trait (P < 0.001) was a significant predictor in the repeated measures model, but the increase in life satisfaction from T1 to T2 was predicted by lower levels of conscientiousness trait (P = 0.009).Conclusions: Norwegian doctors were less satisfied than an educationally age-matched group of the general population. Concurrent situational factors and stress, such as social support and mental distress were of major importance. However, personality clearly predicted life satisfaction over the years. Green care is a concept that involves the use of farm animals, plants, gardens or the landscape in cooperation with health institutions for different target groups of clients. The present study aimed at examining psychiatric therapists' ( n = 60) and farmers' ( n = 15) knowledge, experience and attitudes to Green care and animal-assisted therapy (AAT) with farm animals for people with psychiatric disorders. Most respondents had some or large knowledge about Green care, but experience with Green care was generally low in both groups. Both farmers and therapists believed that AAT with farm animals could contribute positively to therapy to a large or very large extent, with farmers being significantly more positive. Most of the therapists thought that AAT with farm animals contributes to increased skills in interactions with other humans, with female therapists being more positive than males. Two-thirds of the therapists believed that AAT with farm animals to a large extent could contribute better to mental health than other types of occupational therapy. There were no differences in attitudes to AAT between psychiatrists/psychologists and psychiatric nurses. This study confirms the marked potential of offering AAT services with farm animals for psychiatric patients by documenting positive attitudes to it among psychiatric therapists. Women who participated in the hospital psychosocial support groups following breast cancer surgery were compared with non-participants. The Hospital Anxiety and Depression Scale and the EORTC quality of life questionnaire were used to measure emotional distress and quality of life at the time of diagnosis and after 12 months. The General Life Orientation Test-Revised was used to measure dispositional optimism. Of 165 women, 87% reported that they had been invited to participate, and 66% participated. The salient predictor for participating in support group was optimism (OR 0.89 CI: 95% 0.83-0.98, p=0.01). No significant difference was found between the prevalence of anxiety and depression caseness at the time of diagnosis, but after 12 months, the prevalence of anxiety caseness was significantly lower among the participants than that among the non-participants (19% vs 34%, p=0.04).In conclusion, psychosocial intervention as a component of routine breast cancer care appears to have a long-term clinical beneficial effect on anxiety. For depression and quality of life, the study is inconclusive. Self-selection may prevent patients at risk of adverse outcome to participate in support groups. [ABSTRACT FROM AUTHOR]

There is a lack of scientific studies using farm animals in animal-assisted therapy (AAT) for persons with mental disorders. This Norwegian study used video records to study the working abilities and behaviors of 35 severely ill psychiatric patients in interacting with farm animals during a three-month intervention. The patients showed higher intensity (difference score: 0.26 +/- 0.05, p < 0.0001) and exactness (difference score: 0.31 +/- 0.06, p < 0.0001) in their work at the end of the intervention, particularly patients with schizophrenia and personality disorders. The patients spent most relevant time in physical contact with the animals, feeding, cleaning, and milking cows. Among patients with affective disorders, increased intensity of work correlated significantly with increased generalized self-efficacy (rs = 0.82, p = 0.01) and decreased anxiety (rs =-0.7, p = 0.05). For the patients with schizophrenia and personality disorders no correlation was found between the behavioral parameters and the effect scores of psychiatric instruments. Applying a cognitive approach, the purpose of the present study was to expand previous research on stress-vulnerability models of depression and problem-solving deficits, as it relates to suicide attempt. Structural equation modelling, involving latent variables, was used to evaluate (a) whether low self-esteem, a low sense of self-efficacy, loneliness, and divorce constituted vulnerability factors for the development of depression; (b) whether hopelessness and suicidal ideation mediated the relationship between depression and suicide attempt; and (c) whether problem-solving deficits mediated the relationship between the vulnerability factors and suicide attempt, separate from depression/hopelessness. A total of 123 individuals, aged 18-75 years, participated in the study (72 suicide attempters and 51 psychiatric outpatients with no history of suicidal behavior). The results indicated a two path model of suicide attempt. The first path began with low self-esteem, loneliness, and separation or divorce, which advanced to depression, and was further mediated by hopelessness and suicidal ideation which led to suicide attempt. The second path developed from low self-esteem and a low sense of self-efficacy and advanced to suicide attempt, mediated by a negative appraisal of one's own problem-solving capacity, and poor interpersonal problem-solving skills. To study the association between the use of alcohol to cope with tension and hazardous drinking, and the prevalence and the predictors of such drinking behaviours. Cross-sectional surveys of two nation-wide samples of medical students, one at the beginning and one at the end of medical training. Medical students entering Norwegian medical schools in 1993 (N = 379) or graduating in 1993 and 1994 (N = 522); 55.6% of the total sample were women. Postal questionnaires including SCL-5, Perceived Medical School Stress. Use of alcohol to cope was reported by 10.5% of the students with no significant gender difference. Hazardous drinking ('binge drinking' at least 2-3 times per month) was reported by 14% of all the students, 24% among the men and 6% among the women. There was a strong association between use of alcohol to cope with tension and hazardous drinking, OR = 5.11, 95% CI (2.88-9.07) when controlling for other possible predictors. Use of alcohol to cope was also associated with increasing age, mental distress and lack of religious activity. The senior students used alcohol as a way of coping less often, but not hazardous drinking. Male gender, religious inactivity, high self-esteem and having no children were predictors of hazardous drinking. This study suggests that the use of alcohol to cope with tension is an independent risk factor connected with hazardous drinking among medical students, with no difference in prevalence between the genders. Abstract Background Up to date information on poisoning trends is important. This study reports the epidemiology of all hospitalized acute poisonings in Oslo, including mortality, follow-up referrals, and whether the introduction of over-the-counter sales of paracetamol outside pharmacies had an impact on the frequency of poisonings. Methods All acute poisonings of adults (≥16 years) treated at the five hospitals in Oslo from April 2008 to April 2009 were included consecutively in an observational cross-sectional multicentre study. A standardized form was completed by the treating physician, which covered the study aims. All deaths by poisoning in and outside hospitals were registered at the Institute of Forensic Medicine. Results There were 1065 hospital admissions of 912 individuals; 460 (50%) were male, and the median age was 36 years. The annual incidence was 2.0 per 1000. The most frequent toxic agents were ethanol (18%), benzodiazepines (15%), paracetamol (11%), and opioids (11%). Physicians classified 46% as possible or definite suicide attempts, 37% as accidental overdoses with substances of abuse (AOSA), and 16% as other accidents. Twenty-four per cent were discharged without any follow-up and the no follow-up odds were highest for AOSA. There were 117 deaths (eight in hospital), of which 75% were males, and the median age was 41 years. Thus, the annual mortality rate was 25 per 100 000 and the in-hospital mortality was 0.8%. Opioids were the most frequent cause of death. Conclusions The incidence of hospitalized acute poisonings in Oslo was similar to that in 2003 and there was an equal sex distribution. Compared with a study performed in Oslo in 2003, there has been an increase in poisonings with a suicidal intention. The in-hospital mortality was low and nine out of ten deaths occurred outside hospitals. Opioids were the leading cause of death, so preventive measures should be encouraged among substance abusers. The number of poisonings caused by paracetamol remained unchanged after the introduction of over-the-counter sales outside pharmacies and there were no deaths, so over-the-counter sales may be considered safe.
