Abstract

The review analyzes the etiological and pathogenetic factors (including immunopathogenesis factors) of chronic obstructive pulmonary disease (COPD) and metabolic syndrome (MS), cites data on clinical and pathogenetic characteristics of combined pathology, emphasizes the commonality of risk factors and individual links of pathogenesis in syntropy.The clinical and pathogenetic features of the comorbidity of COPD and MS are closely related to the severity of chronic “metabolic” inflammation induced by elements of adipose tissue. Functional and biochemical disorders recorded in metabolic syndrome (insulin resistance, hyperglycemia, dyslipidemia) are considered as factors contributing to dysfunction of the components of innate and adaptive immunity.The review formulates a number of unresolved issues of the pathogenesis of comorbid pathology, the study of which is necessary to search for the mutual aggravating effect of COPD and MS mechanisms. In view of the clinical and laboratory metabolic syndrome equivalents variety, the authors emphasize the relevance of future studies of the pathogenetic features of chronic inflammation associated with the comorbidity of the main components of metabolic syndrome and COPD, to develop effective methods of prevention and pathogenetic therapy of comorbid pathology.

Abstract

Squamous cell carcinoma (SCC), one of the most common forms of lung cancer, shows accelerated progression and aggressive growth and usually is observed at advanced stages. SCC originates from morphological changes in the bronchial epithelium that occur during chronic inflammation: basal cell hyperplasia, squamous metaplasia, and dysplasia I-III. However, the process is not inevitable; it can be stopped at any stage, remain in the stable state indefinitely and either progress or regress. The reasons and mechanisms of different scenarios of the evolution of premalignant lesions in the respiratory epithelium are not fully understood. In this review, we summarized the literature data (including our own data) regarding genetic, epigenetic, transcriptomic and proteomic profiles of the premalignant lesions and highlighted factors (environmental causes, inflammation, and gene polymorphism) that may govern their progression or regression. In conclusion, we reviewed strategies for lung cancer prevention and proposed new models and research directions for studying premalignant lesions and developing new tools to predict the risk of their malignant transformation.

Abstract

BACKGROUND: The efficacy and safety of benralizumab, an interleukin-5 receptor alpha-directed cytolytic monoclonal antibody, for the prevention of exacerbations in patients with moderate to very severe chronic obstructive pulmonary disease (COPD) are not known.METHODS: In the GALATHEA and TERRANOVA trials, we enrolled patients with COPD (at a ratio of approximately 2:1 on the basis of eosinophil count [≥220 per cubic millimeter vs. RESULTS: In GALATHEA, the estimates of the annualized exacerbation rate were 1.19 per year (95% confidence interval [CI], 1.04 to 1.36) in the 30-mg benralizumab group, 1.03 per year (95% CI, 0.90 to 1.19) in the 100-mg benralizumab group, and 1.24 per year (95% CI, 1.08 to 1.42) in the placebo group; the rate ratio as compared with placebo was 0.96 for 30 mg of benralizumab (P = 0.65) and 0.83 for 100 mg of benralizumab (P = 0.05). In TERRANOVA, the estimates of the annualized exacerbation rate for 10 mg, 30 mg, and 100 mg of benralizumab and for placebo were 0.99 per year (95% CI, 0.87 to 1.13), 1.21 per year (95% CI, 1.08 to 1.37), 1.09 per year (95% CI, 0.96 to 1.23), and 1.17 per year (95% CI, 1.04 to 1.32), respectively; the corresponding rate ratios were 0.85 (P = 0.06), 1.04 (P = 0.66), and 0.93 (P = 0.40). At 56 weeks, none of the annualized COPD exacerbation rate ratios for any dose of benralizumab as compared with placebo reached significance in either trial. Types and frequencies of adverse events were similar with benralizumab and placebo.CONCLUSIONS: Add-on benralizumab was not associated with a lower annualized rate of COPD exacerbations than placebo among patients with moderate to very severe COPD, a history of frequent moderate or severe exacerbations, and blood eosinophil counts of 220 per cubic millimeter or greater (Funded by AstraZeneca [GALATHEA and TERRANOVA] and Kyowa Hakko Kirin [GALATHEA]; GALATHEA and TERRANOVA ClinicalTrials.gov numbers, NCT02138916 and NCT02155660.).

Abstract

Background. Nowadays little data related to the hemostatic system and fibrinolysis in patients with chronic obstructive pulmonary disease (COPD) are available. This is due to the lack of standardized methods for studying the hemostasis system, as well as to the lack of a single functional test that allows the evaluation of the complete fibrinogenesis cycle in whole blood. The aim of our study was to develop a functional test capable of analyzing the blood gas composition in the “point-of-care test” method for the evaluation of the hemostatic potential in patients with COPD, based on a standardized test stimulus, which is tissue hypoxia. The current level of clinical and laboratory diagnostics requires personification and research of the hemo-coagulation system in real time (point-of-care test), which allows low-frequency piezotromboelastography(NVTEG) to be performed. Materials and methods. NVTEG was chosen to estimate the state of the hemocoagulation system. Ten patients with COPD and 10 healthy volunteers were examined. Hypoxia was selected as a standardized test stimulus. Hypoxia conditions were caused by smoking one standard cigarette (composition: resin 10 mg/cig., nicotine 0,7 mg/cig., CO 10 mg/cig.). The degree of tissue hypoxia was assessed with the GASTAT-navi blood gas analyzer. Results. The study has shown that in response to the standard test stimulus, which is the tissue hypoxia caused by smoking of a standardized cigarette, two types of haemostatic potential reaction were detected both in patients with COPD and healthy volunteers. The first type of reaction – “hypercoagulation” – is characterized by the formation of chronometric and structural hypercoagulation at all stages of fibrinogenesis and increased coagulation activity by 25–30% compared with the response in healthy individuals. The second type of reaction – “hypocoagulation” – is characterized by the formation of chronometric and structural hypocoagulation, a decrease in coagulation activity by 25–30% compared with the response in healthy individuals. Conclusion. Test stimulus, which acts as tissue hypoxia, causes a uniform spectrum of changes in the blood gas composition and hemocoagulation system in both healthy volunteers and patients with COPD. The possibility of online assessment of all stages of fibrinogenesis makes it possible to stratify patients with COPD by type of reaction, which is certain to have an important diagnostic and prognostic value and in the future will allow a more personified approach for choosing the treatment tactics.

Abstract

To investigate the impact of smoking on the air exhaled by patients with chronic obstructive pulmonary disease (COPD) and asthmatics, by applying photoacoustic spectroscopy.The exhaled air absorption spectra (EAAS) were analyzed in healthy volunteers and patients with COPD and asthmatics, by applying an ILPA-1 CO2 laser photoacoustic gas analyzer. The procedure based on the calculation of an integrated estimate (IE) of the state of the object was used to assess the findings.Comparison of the IE of EAAS in COPD patients and non-smoking healthy individuals showed that spectra of the compounds, the formation of which was associated with smoking, were recorded in the range of wavelengths corresponding to the 10R branch of CO2 laser generation. This also provided evidence indicating that the exhaled air of asthmatics differed from that of both smoking and non-smoking healthy individuals. The calculations yielded the threshold values of EAAS IE in the range of wavelengths corresponding to the 10P branche of CO2 laser generation, which made it possible to distinguish non-smoking healthy individuals from asthmatics and COPD patients in 94 and 89% of cases, respectively.The investigation has confirmed that smoking substantially impacts the composition of the air exhaled by healthy individuals. It has been shown that the use of reference groups formed from non-smoking healthy individuals can improve the accuracy of photoacoustic spectroscopy in detecting COPD and asthma. A further development in this direction will open up new prospects for a new method to diagnose COPD and asthma.Цель исследования. Изучить влияние курения на воздух, выдыхаемый больными хронической обструктивной болезнью легких (ХОБЛ) и бронхиальной астмой (БА), с применением оптико-акустической спектроскопии. Материалы и методы. Проведен анализ спектров поглощения выдыхаемого воздуха (СПВВ) у здоровых добровольцев и больных ХОБЛ и БА с помощью лазерного оптико-акустического газоанализатора ILPA-1 на основе CO2-лазера. Для оценки результатов использовали методику, основанную на вычислении интегральной оценки (ИО) состояния объекта. Результаты. Сравнение ИО СПВВ больных ХОБЛ и некурящих здоровых лиц показало, что в диапазоне длин волн, соответствующем ветви 10R генерации CO2-лазера, регистрируются спектры соединений, образование которых связано с курением. Кроме того, получены данные об отличии выдыхаемого воздуха больных БА от выдыхаемого воздуха как курящих, так и некурящих здоровых лиц. На основе расчетов получены пороговые значения ИО СПВВ в диапазоне длин волн, соответствующем ветви 10P генерации CO2-лазера, которые позволяют отличать некурящих здоровых лиц от больных БА и ХОБЛ в 94 и 89% случаев соответственно. Заключение. Подтверждено, что курение существенно влияет на состав воздуха, выдыхаемого здоровыми лицами. Показано, что использование референтной группы, сформированной из некурящих здоровых лиц, позволяет повысить точность при выявлении ХОБЛ и БА методом оптико-акустической спектроскопии. При дальнейшей разработке данного направления открываются перспективы получения нового метода для диагностики ХОБЛ и БА.

Abstract

COPD is widespread airways disease of multifactorial nature. It is well known that geneteic factors play significant role in COPD pathogenesis. Infection is one of important COPD exogenous etiological factors. Previous study did not detected association of NRAMP1 gene polymorphisms with COPD development. Nevertheless we supposed that this COPD candidate gene may play role in COPD clinical signs Study objectives: To inverstigate the association of D543N, 1465-85G/A and 469+14G/C polymorphisms of NRAMP1 gene with COPD clinical presentation. Materials and methods: We examined 122 COPD patients (mean age 57±12). Average duration of disease is 17±12. DNA was extracted from venous blood. Polymorphisms were detected by polymerase chain reaction followed by restriction digestion. Results: The linkage of age of disease onset with 1465-85G/A polymorphisms of NRAMP1 gene was proved: mean age of disease onset is 34 and is associated with GG genotype, while GA and AA genotypes are associated with latest manifestation–at 43 (p=0,022). D543N polymorphisms are associated with modification of proteases inhibitors α1 PI and α2 MG activity in sputum. Carriers of homo-and heterozygote A allele of NRAMP1 gene condition have reliable decrease of α2 MG activity in sputum during exacerbation and increase of α1 PI in sputum during remission as compared with GG genotype carriers. Carriers of GC and CC genotypes of 469-14G/C polymorphic variant of NRAMP1 gene have increase of α1 PI activity in serum during COPD exacerbation (p Conclusion: Time of COPD manifestation is determined of 1465-85G/A polymorphisms of NRAMP1 gene. Inhibitors of proteases activity is determined of D543N and 469-14G/C polymorphisms of this gene.

Abstract

Accurate diagnosis of different bronchopulmonary diseases is important in clinical practice. This study involved 20 healthy volunteers and 77 patients with bronchopulmonary diseases, including chronic obstructive pulmonary disease (COPD), bronchial asthma, pulmonary tuberculosis, and community-acquired pneumonia. The absorption spectrum of exhaled air samples was recorded on an intra-cavity photo-acoustic gas analyzer (ILPA-1, Special Technologies, Ltd., Russia) with photo-acoustic detectors and CO2 laser with a tuning range from 9.2 to 10.8 μm. In conclusion, analysis of the Mahalanobis distance-based absorption spectral profiles of breath air from bronchopulmonary patients and healthy volunteers allows the formulation of a preliminary diagnosis.

Abstract

One of the main non-respiratory functions of the lungs is participating in regulation of hemostasis. Development and progression of COPD affects on coagulation and anticoagulation systems, leads to thrombosis. The aim of our research is evaluation of hemostatic potential of patients with COPD in comparison with healthy volunteers. The research involved 10 smoking patients with COPD and 10 healthy non-smoking volunteers. Evaluation of hemostasis system was carried out using low-frequency piezoelectric thromboelastography (LPTEG), before smoking one cigarette and after. This method of thromboelastography allows us to perform realtime snapshot analysis of all components of hemostasis in patient whole blood. Healthy volunteers did not have obvious defects of hemostatic potential before smoking. All patients with COPD had hypercoagulable type of hemostatic potential characterized by increased thrombin activity, and marked activation of anticoagulation and fibrinolysis. Hypercoagulation at all stages of hemostasis was observed after one time smoking of healthy volunteers. Whereas patients with COPD had thrombin activity increased in 1.2 in comparison before smoking one cigarette and 1.8 in comparison with healthy volunteers. A shift to hypercoagulation of the hemostatic potential was detected for 10 % of healthy volunteers and 10 % of patients with COPD after smoking of one cigarette. Low-frequency piezoelectric thromboelastography allows to determine defects of hemostatic potential by patient with COPD and to perform monitoring of changes in the hemostatic potential.

The results of application of the joint use of laser photoacoustic spectroscopy and chemometrics methods in gas analysis of exhaled air of patients with chronic respiratory diseases (chronic obstructive pulmonary disease and lung cancer) are presented. The absorption spectra of exhaled breath of representatives of the target groups and healthy volunteers were measured; the selection by chemometrics methods of the most informative absorption coefficients in scan spectra in terms of the separation investigated nosology was implemented.

The results of the joint use of laser photoacoustic spectroscopy and chemometrics methods in gas analysis of exhaled air of patients with respiratory diseases (chronic obstructive pulmonary disease, pneumonia and lung cancer) are presented. The absorption spectra of exhaled breath of all volunteers were measured, the classification methods of the scans of the absorption spectra were applied, the sensitivity/specificity of the classification results were determined. It were obtained a result of nosological in pairs classification for all investigated volunteers, indices of sensitivity and specificity.

Abstract

Pulmonary diseases diagnostics always occupies one of the key positions in medicine practices. A large variety of high technology methods are used today, but none of them cannot be used for early screening of pulmonary diseases. We discuss abilities of methods of IR and terahertz laser spectroscopy for noninvasive express diagnostics of pulmonary diseases on a base of analysis of absorption spectra of patient’s gas emission, in particular, exhaled air. Experience in the field of approaches to experimental data analysis and hard-ware realization of gas analyzers for medical applications is also discussed.

Abstract

It is important to identify patients with chronic obstructive pulmonary disease (COPD) and lung cancer in the early stages of the disease. The method of laser opto-acoustic gas analysis, in this case, can act as a promising tool for diagnostics. The material for this study were the gas emission samples collected from patients and healthy volunteers – samples of exhaled air, swabs from teeth and cheeks. A set of material was formed three groups: healthy volunteers, patients with COPD, lung cancer patients. The resulting samples were analyzed by means of laser opto-acoustic gas analyzers: with intracavity location detector (ILPA-1), with extracavity location detector (LGA-2). Presentation of the results in an easy to visual form was performed using the method of elastic maps, based on the principal component analysis. The results of analysis show potentialities of usage of laser optoacoustic spectroscopy application to assess the status of patients with chronic obstructive pulmonary disease and lung cancer.

Abstract

To reveal the tissue, cellular, and molecular predictors leading to the development of chronic obstructive pulmonary disease (COPD) in smokers on the basis of a morphometric analysis of bronchial biopsy specimens.A cross-sectional study was conducted in 115 smokers aged 40 to 60 years (58.3±3.24 years) with a male predominance of 90.9%. Morphological examination of bronchial biopsy specimens was first made in the smoking patients with and without COPD.The smokers with COPD versus those without this condition were found to have a statistically significant increase in a number of indicators: the specific volume (SV) of the surface epithelium (p=0.017), SV of basal epitheliocytes (p=0.008), the height of the epithelium (p=0.001), and the thickness of the basal membrane (p=0.006) due to impaired regeneration processes in the bronchial epithelium and to fibrosis of the lamina propria of the bronchi with a concurrent increase in the total number of fibroblasts. The group of smokers with COPD, unlike the comparison group, showed signs of vascular remodeling and microcirculatory disorders as the increased connective tissue volumetric density of the lamina propria of the bronchi with predominant perivascular localization. This was followed by reductions in the relative volume of capillaries (p=0.016), in the SV of micropinocytic vesicles (p=0.005), and the size of Weibel-Palade bodies (p=0.004) in the endotheliocytes. In the COPD patients, the total density of cell infiltrate per mm2 of the lamina propria of the bronchi was statistically significantly (p0.001) greater than that in the comparison group at the expense of neutrophils, lowly and moderately granulated basophils, macrophages, plasma cells with a simultaneous rise in their interepithelial forms.Compensatory structural changes without signs of bronchial wall remodeling were recorded in the smokers without signs of bronchial disease. The smokers with the examined nosological entity were observed to have morphological signs of hemodynamic disorders, as well as perivascular fibrosis, atrophy, and squamous cell metaplasia of the bronchial epithelial lining.Цель исследования. Выявить тканевые, клеточные и молекулярные предикторы, приводящие к развитию хронической обструктивной болезни легких (ХОБЛ) у курильщиков, на основании морфометрического анализа бронхобиоптатов. Материалы и методы. Выполнено одномоментное обследование 115 курильщиков в возрасте от 40 до 60 лет (58,3±3,24 года), среди которых преобладали мужчины (90,9%). Впервые проводили морфологическое исследование биоптатов слизистой оболочки бронхов (СОБ) у курящих пациентов с ХОБЛ и без нее. Результаты. У курильщиков с ХОБЛ по сравнению с группой курильщиков без ХОБЛ обнаружено статистически значимое увеличение ряда показателей: удельный объем (УО) покровного эпителия (р=0,017), УО базальных эпителиоцитов (р=0,008), высота эпителия (р=0,001) и толщина базальной мембраны (р=0,006) за счет нарушения процессов регенерации эпителия бронхов и фиброза собственной пластинки (СП) СОБ с одновременным увеличением общего числа фибробластов. В группе курильщиков с ХОБЛ в отличие от группы сравнения выявлены признаки сосудистого ремоделирования и расстройств микроциркуляции в виде увеличения объемной плотности соединительной ткани СП СОБ с преимущественной периваскулярной локализацией. Это сопровождалось снижением относительного объема капилляров (р=0,016), уменьшением УО микропиноцитозных везикул (р=0,005) и размера секреторных гранул Вейбеля-Палади (р=0,004) в эндотелиоцитах. У пациентов с ХОБЛ суммарная плотность клеточного инфильтрата в 1 мм2 СП СОБ статистически значимо (р0,001) превышала показатели группы сравнения за счет нейтрофилов, низко- и умеренно гранулированных базофилов, макрофагов, плазмоцитов с одновременным увеличением количества их межэпителиальных форм. Заключение. У курильщиков без признаков болезни в бронхах регистрировались структурные изменения компенсаторного характера без признаков ремоделивания бронхиальной стенки. У курящих больных с исследуемой нозологией наблюдались морфологические признаки нарушения гемодинамики, периваскулярный фиброз, атрофия и плоскоклеточная метаплазия эпителиальной выстилки бронхов.

Abstract

A human exhaled air analysis by means of infrared (IR) laser photoacoustic spectroscopy is pre- sented. Eleven healthy nonsmoking volunteers (control group) and seven patients with chronic obstructive pul- monary disease (COPD, target group) were involved in the study. The principal component analysis method was used to select the most informative ranges of the absorption spectra of patients' exhaled air in terms of the separation of the studied groups. It is shown that the data of the profiles of exhaled air absorption spectrum in the informative ranges allow identifying COPD patients in comparison to the control group. © 2015 Society of
