19. Veno-venous extracorporeal membrane oxygenation (vv-ECMO) for severe respiratory failure in adult cancer patients: a retrospective multicenter analysis

Abstract

Purpose The question of whether cancer patients with severe respiratory failure benefit from veno-venous extracorporeal membrane oxygenation (vv-ECMO) remains unanswered. We, therefore, analyzed clinical characteristics and outcomes of a large cohort of cancer patients treated with vv-ECMO with the aim to identify prognostic factors. Methods 297 cancer patients from 19 German and Austrian hospitals who underwent vv-ECMO between 2009 and 2019 were retrospectively analyzed. A multivariable cox proportional hazards analysis for overall survival was performed. In addition, a propensity score-matched analysis and a latent class analysis were conducted. Results Patients had a median age of 56 (IQR 44-65) years and 214 (72%) were males. 159 (54%) had a solid tumor and 138 (47%) a hematologic malignancy. The 60-day overall survival rate was 26.8% (95% CI 22.1-32.4%). Low platelet count (HR 0.997, 95% CI 0.996-0.999; p = 0.0001 per 1000 platelets/[micro]l), elevated lactate levels (HR 1.048, 95% CI 1.012-1.084; p = 0.0077), and disease status (progressive disease [HR 1.871, 95% CI 1.081-3.238; p = 0.0253], newly diagnosed [HR 1.571, 95% CI 1.044-2.364; p = 0.0304]) were independent adverse prognostic factors for overall survival. A propensity score-matched analysis with patients who did not receive ECMO treatment showed no significant survival advantage for treatment with ECMO. Conclusion The overall survival of cancer patients who require vv-ECMO is poor. This study shows that the value of vv-ECMO in cancer patients with respiratory failure is still unclear and further research is needed. Abstract

Objectives: To investigate characteristics and outcomes of critically ill cancer patients with marked hyperferritinemia. Methods: A single‐center retrospective analysis comprising cancer patients with a ferritin level >10.000 μg/L treated in the intensive care unit (ICU) between 2012 and 2022 was conducted. Results: A total of 117 patients were included in the analysis. The median age was 59 years (range: 15–86 years). Females accounted for 48% of cases. 90% of patients had a hematologic malignancy. The median maximum ferritin level was 27.349 μg/L (range: 10.300–426.073 μg/L). The diagnostic criteria of septic shock were fulfilled in 51% of cases; 31% of patients had hemophagocytic lymphohistiocytosis (HLH) according to the HLH‐2004 criteria. Mechanical ventilation, renal replacement therapy and the use of vasopressors were necessary in 59%, 35% and 70% of cases, respectively. The ICU, hospital, 90‐day and 1‐year survival rates were 33.3%, 23.1%, 23.7% and 11.7%. Patients with septic shock had a worse survival than those without septic shock (p =.001); the survival of patients who fulfilled the HLH‐2004 criteria did not differ from those who did not (p =.88). Conclusion: Critically ill cancer patients with marked hyperferritinemia have poor outcomes. The present data may help to make informed decisions for this patient group. [ABSTRACT FROM AUTHOR]

Abstract

The overall prognosis of older patients with acute myeloid leukemia (AML) is dismal. Only a small subgroup experiences long‐term survival. The discrimination between patients who are candidates for potentially curative approaches and those who are not is crucial since ‐ in addition to differences in terms of AML‐directed treatment ‐ different policies concerning intensive care unit (ICU) admission and involvement of specialized palliative care (SPC) seem obvious. To shed more light on characteristics, outcomes and health care utilization of older individuals with AML, we conducted an analysis comprising 107 consecutive patients with newly diagnosed AML aged ≥70 years treated at an academic tertiary care center in Germany between 1 January 2015, and 31 December 2020. Median age was 75 years (range: 70–87 years); 45% of patients were female. The proportion of patients receiving intensive induction chemotherapy was 35%, 55% had low‐intensity treatment and 10% did not receive AML‐directed treatment or follow‐up ended before treatment initiation. At least one ICU admission was documented for 47% of patients; SPC was involved in 43% of cases. Median follow‐up was 199 days. The median overall survival (OS) was 2.5 months; the 1‐year OS rate was 16%. Among patients who died during observation, the median proportion of time spent in the hospital between AML diagnosis and death was 56%. The most common places of death were normal wards (31%) and the ICU (28%). Patients less frequently died in a palliative care unit (14%) or at home (12%). In summary, results of the present analysis confirm the unfavorable prognosis of older patients with AML despite intensive health care utilization. Future efforts in this patient group should aim at optimizing the balance between appropriate AML‐directed treatment on the one hand and health care utilization including ICU stays on the other hand. Abstract

This article examines the use of specialized palliative care (SPC) in patients with haematological malignancies (HMs) undergoing chimeric antigen receptor (CAR) T-cell therapy. The study analyzed 200 patients in Germany who received CAR T-cell therapy and compared outcomes between those with and without SPC involvement. The study found that 21.5% of patients had SPC during their treatment, with higher rates in multiple myeloma patients. The most common symptoms documented in patients with SPC were weakness, loss of appetite, and the need for assistance with daily activities. The authors recommend offering SPC to patients receiving CAR T-cell therapy due to the significant symptom burden and the likelihood of not achieving long-term survival. [Extracted from the article]

44. Airway Pressure Release Ventilation in COVID-19-Associated Acute Respiratory Distress Syndrome—A Multicenter Propensity Score–Matched Analysis.

Abstract

Background: There are limited and partially contradictory data on the effects of airway pressure release ventilation (APRV) in COVID-19-associated acute respiratory distress syndrome (CARDS). Therefore, we analyzed the clinical outcome, complications, and longitudinal course of ventilation parameters and laboratory values in patients with CARDS, who were mechanically ventilated using APRV. Methods: Respective data from 4 intensive care units (ICUs) were collected and compared to a matched cohort of patients receiving conventional low tidal volume ventilation (LTV). Propensity score matching was performed based on age, sex, blood gas analysis, and APACHE II score at admission, as well as the implementation of prone positioning. Findings: Forty patients with CARDS, who were mechanically ventilated using APRV, and 40 patients receiving LTV were matched. No significant differences were detected for tidal volumes per predicted body weight, peak pressure values, and blood gas analyses on admission, 6 h post admission as well as on day 3 and day 7. Regarding ICU survival, no significant difference was identified between APRV patients (40%) and LTV patients (42%). Median duration of mechanical ventilation and duration of ICU treatment were comparable in both groups. Similar complication rates with respect to ventilator-associated pneumonia, septic shock, thromboembolic events, barotrauma, as well as the necessity for hemodialysis were detected for both groups. Clinical characteristics that were associated with increased mortality in a Cox proportional hazards regression analysis included age (hazard ratio [HR] 1.08, 95% confidence interval [CI] 1.04-1.1; P <.001), severe acute respiratory distress syndrome (HR 2.62, 95% CI 1.02-6.7; P =.046) and the occurrence of septic shock (HR 17.18, 95% CI 2.06-143.2; P =.009), but not the ventilation mode. Interpretation: Intensive care unit survival, duration of mechanical ventilation, and ICU treatment as well as ventilation-associated complication rates were equivalent using APRV compared to conventional LTV in patients with CARDS. Abstract

High-dose chemotherapy and autologous stem cell transplantation (ASCT) can be associated with adverse events necessitating treatment on the intensive care unit (ICU). Data focusing on patients admitted to the ICU during hospitalization for high-dose chemotherapy and ASCT are scarce. We thus conducted a single-center retrospective analysis comprising 79 individuals who had high-dose chemotherapy and ASCT between 2014 and 2020 and were admitted to the ICU between the initiation of conditioning therapy and day 30 after ASCT. The median age was 57 years (range: 20-82 years); 38% of patients were female. B-cell non-Hodgkin lymphoma (34%) and plasma cell disorders (28%) were the most common indications for high-dose chemotherapy and ASCT. Sepsis represented the major cause for ICU admission (68%). Twenty-nine percent of patients required mechanical ventilation (MV), 5% had renal replacement therapy, and 44% needed vasopressors. The ICU, hospital, 90-day, and 1-year survival rates were 77.2%, 77.2%, 72.2%, and 60.3%, respectively. Stable disease or disease progression prior to the initiation of high-dose chemotherapy (p = 0.0028) and MV (p < 0.0001) were associated with an impaired survival. A total of 36 patients died during observation. The most frequent causes of death were the underlying malignancy (44%) and sepsis (39%). Taken together, the present analysis indicates a favorable overall outcome for patients admitted to the ICU during hospitalization for high-dose chemotherapy and ASCT. Thus, this patient group should not be denied admission and treatment on the ICU.

50. Behandlung des nodulär Lymphozyten-prädominanten Hodgkin Lymphoms

Abstract

Hintergrund: Das nodulär Lymphozyten-prädominante Hodgkin Lymphom (NLPHL) macht etwa 5 % aller Hodgkin Lymphome (HL) aus. Die pathologischen und klinischen Eigenschaften sowie die Therapie unterscheiden sich teilweise vom klassischen Hodgkin Lymphom (cHL). Ziel: Der vorliegende Artikel gibt einen Überblick über aktuelle Therapieoptionen beim NLPHL. Material und Methoden: Dazu wurde eine selektive Literaturrecherche durchgeführt. Ergebnisse: Im Gegensatz zum cHL exprimieren die Tumorzellen des NLPHL CD20, sind jedoch negativ für CD30. Die Therapie des NLPHL unterscheidet sich v. a. im Stadium IA ohne Risikofaktoren und in der Rezidivsituation von der des cHL. Im Stadium IA ohne Risikofaktoren werden NLPHL-Patienten mit einer alleinigen Bestrahlung behandelt. Beim rezidivierten NLPHL ist nur selten eine Hochdosischemotherapie mit autologer Stammzelltransplantation nötig. In den meisten Fällen ist eine Therapie mit einem Anti-CD20-Antikörper oder eine konventionelle Chemotherapie ausreichend. Bei NLPHL-Patienten, bei denen die Diagnose nicht im Stadium IA ohne Risikofaktoren gestellt wird, führt eine Erstlinientherapie analog dem cHL zu guten Therapieergebnissen. Auch Protokolle, die bei B‑Zell-Non-Hodgkin-Lymphomen (B-NHL) eingesetzt werden, sind wirksam. Beim NLPHL kommt es in etwa 10 % der Fälle innerhalb von 10 Jahren zu einer Transformation in ein aggressives B‑NHL. Daher ist eine histologische Sicherung bei vermutetem Rezidiv zwingend notwendig. Die Prognose des NLPHL ist grundsätzlich gut. Die 8‑Jahres-Überlebensrate liegt bei über 90 %. Schlussfolgerung: Das NLPHL stellt eine seltene Unterform des HL dar, die sich in einigen Punkten vom cHL unterscheidet. Eine stadienadaptierte Therapie führt i. d. R. zu einer langfristigen Remission. Die Therapie des NLPHL unterscheidet sich in manchen Situationen von der des cHL.
