Abstract

Bronchopulmonary dysplasia (BPD) is a multifactorial disease mainly provoked by pre- and postnatal infections, mechanical ventilation, and oxygen toxicity. In severely affected premature infants requiring mechanical ventilation, association of bacterial colonization of the lung and BPD was recently disclosed. To analyze the impact of bacterial colonization of the upper airway and gastrointestinal tract on moderate/severe BPD, we retrospectively analyzed nasopharyngeal and anal swabs taken weekly during the first 6 weeks of life at a single center inn= 102 preterm infants

Abstract

BackgroundPerinatal asphyxia and resulting hypoxic-ischemic encephalopathy is a major cause of death and long-term disability in term born neonates. Up to 20,000 infants each year are affected by HIE in Europe and even more in regions with lower level of perinatal care. The only established therapy to improve outcome in these infants is therapeutic hypothermia. Allopurinol is a xanthine oxidase inhibitor that reduces the production of oxygen radicals as superoxide, which contributes to secondary energy failure and apoptosis in neurons and glial cells after reperfusion of hypoxic brain tissue and may further improve outcome if administered in addition to therapeutic hypothermia.MethodsThis study on the effects of ALlopurinol in addition to hypothermia treatment for hypoxic-ischemic Brain Injury on Neurocognitive Outcome (ALBINO), is a European double-blinded randomized placebo-controlled parallel group multicenter trial (Phase III) to evaluate the effect of postnatal allopurinol administered in addition to standard of care (including therapeutic hypothermia if indicated) on the incidence of death and severe neurodevelopmental impairment at 24months of age in newborns with perinatal hypoxic-ischemic insult and signs of potentially evolving encephalopathy. Allopurinol or placebo will be given in addition to therapeutic hypothermia (where indicated) to infants with a gestational age36weeks and a birth weight2500g, with severe perinatal asphyxia and potentially evolving encephalopathy. The primary endpoint of this study will be death or severe neurodevelopmental impairment versus survival without severe neurodevelopmental impairment at the age of two years. Effects on brain injury by magnetic resonance imaging and cerebral ultrasound, electric brain activity, concentrations of peroxidation products and S100B, will also be studied along with effects on heart function and pharmacokinetics of allopurinol after iv-infusion.DiscussionThis trial will provide data to assess the efficacy and safety of early postnatal allopurinol in term infants with evolving hypoxic-ischemic encephalopathy. If proven efficacious and safe, allopurinol could become part of a neuroprotective pharmacological treatment strategy in addition to therapeutic hypothermia in children with perinatal asphyxia.Trial registrationNCT03162653, www.ClinicalTrials.gov, May 22, 2017.

Abstract

A gas mixture consisting of C2H2, Ar and N2 was utilized to create a plasma beam source powerful enough to deposit nitrogenated and hydrogenated amorphous carbon. The nitrogenated and hydrogenated a-C films showed unique attributes, especially the mobility of the charge carriers, optical gap and electrical conductivity. Defect density fell due to an increase in nitrogen incorporation. Moreover, the films were characterized by infrared and Raman spectroscopy and photo dermal deflection.

Abstract

Intense argon ion plating and magnetron sputtering were employed in depositing amorphous carbon thin films with high sp3 content. Transmission electron microscopy reveals the presence of graphitic regions in the thin films. The difference in the electronic and optical characteristics of the thin films and the films deposited using filtered arc and mass selected ion beam are due to variations in the microstructure of the tetrahedral amorphous carbon films.

Abstract

The effects of deposition temperature on the attributes of hydrogenated carbon films deposited from an ionized plasma beam are evaluated using two temperatures: a lower transition at around 250 degrees centigrade and a higher one at 450 degrees centigrade. Smoother and higher density, sp3 bonding and compressive stress are seen at low transition temperatures. Variances in the optical gap are observed to be wide.

37. Thermodynamic stabilization of nanocrystallinity

Abstract

Nanocrystalline materials are polycrystals made up of nanometer-sized grains separated by a network of interfaces – grain or phase boundaries – that generally make a positive contribution to the total energy of the system. Consequently, there exists a thermodynamic driving force for reducing the overall interface area, which renders such systems intrinsically unstable against coarsening. The latter process entails interface migration and the concomitant deterioration of any property enhancements effected by the ultrafine grain size. We describe a strategy for significantly reducing or even eliminating the driving force for grain growth in nanocrystalline materials via the deliberate segregation of solute atoms into the core region of boundaries. Applied to Pd – Zr solid solutions containing up to 20 at.% Zr, the strategy yields nanocrystalline specimens manifesting an unusually high thermal stability with respect to grain growth, extending to the vicinity of the melting point. Parallels are drawn between the migration of grain boundaries in segregation-stabilized systems and antiphase domain boundaries in ordered alloys.

Abstract

Objective: This observational study reports on the postnatal mortality and 30-month outcome of children who underwent fully percutaneous fetoscopic repair of myelomeningocele (MMC) at a single center in Giessen, Germany.Methods: Between October 2010 and August 2014, a total of 72 patients underwent fully percutaneous fetoscopic MMC closure at 21 + 0 to 29 + 1 (mean, 23 + 5) weeks' gestation. Of these, 52 (72%) participated in this study; however, 30-month mortality data are available for all 72 children. Children were examined at four timepoints: shortly after birth and at 3 months, 12 months and 30 months of corrected age. The patients underwent age-specific standardized neurological examinations and assessment of leg movements and ambulation at all timepoints. Cognitive and motor development were assessed using the Bayley Scales of Infant Development, second edition (BSID-II), at 30 months.Results: All 72 children survived the intrauterine procedure, however, four (5.6%) infants died postnatally (including two of the 52 comprising the study cohort). Of the 52 patients included in the study, 11.5% were delivered before the 30th week of gestation (mean, 33 + 1 weeks) and, of the survivors, 48.1% had ventriculoperitoneal shunt placement. Of the 50 infants that were alive at 30 months, independent ambulation, without orthosis, was feasible for 46%. At 30 months of follow-up, 46% of children presented with a functional level that was at least two segments better than the anatomical level of the lesion. At 30 months, 70% of the children presented with BSID-II psychomotor development index score of ≥ 70 and 80% with BSID-II mental development index score of ≥ 70.Conclusion: Intrauterine repair of MMC by percutaneous fetoscopy shows largely similar outcomes to those reported for open repair, with respect to mortality, prematurity, shunt-placement rates, motor and mental development and free ambulation. © 2020 The Authors.
