A two-year-old boy was diagnosed with Miller Fisher syndrome (MFS). MFS is a neurological disease that commonly runs a benign course in children. The boy developed sporadic pinkeye, fevers, rash and upper respiratory infections, which eventually resolved. He was vaccinated with Hemophilus influenzae type b vaccine. When his mother noticed that his left eye was drifting two days later, he was examined at an emergency room. He was presumed to have an infection of the middle ear and was treated for that. The next day he had difficulty walking. His coordination was impaired and his reflexes were slowed. His pupils remained fixed and dilated. Over the next few days he became weaker and was put on a ventilator. Brain scans were normal, but electromyography revealed dysfunction indicative of MFS. He was treated with plasmapheresis, and recovered completely within six months.
