2. Initial and continued adherence to wearing appropriate footwear in people with diabetic foot disease: results of a pilot study

Background: For the prevention and healing of diabetic foot ulcers, appropriate footwear or medical devices are utilized. However, initial and continued adherence of patients with diabetes-related foot problems in using such therapeutic means, is not satisfactory. bjective: To explore initial and continued adherence to wearing appropriate footwear in people with diabetic foot disease. Methods: A cross-sectional study was conducted, from March to November 2016, at general and special hospitals in Athens, Greece. Forty-one outpatients with type 1 and 2 Diabetes Mellitus, with medical recommendation for wearing appropriate footwear, participated. A structured quantitative interview guide and medical measuring instruments were used. For data analysis, descriptive and inferential statistic methods were applied using the IBM SPSS 24 software package. Results: Suitable for diabetes footwear were worn from the 56.1% of participants. The 27.3% of risk patients wore their preventive shoes ≥60% (≥9.6 hours) of daytime. The patients with active foot ulcers wore appropriate footwear at a lesser percentage than the ones at risk (44.4% versus 78.6%, χ2(1) =4.36, p=0.037; OR 4.58, 95% CI 1.04-20.24, p=0.045) and the initially adherent group had significantly subordinate Visual Analogue Scale score in relation to the satisfaction from the footwear price (Mdn=5.00) than the one that was not initially adherent (Mdn=8.00), U=97.00, z=-2.36, p=0.019, r=-.38. Conclusion: For the enhancement of initial and continued adherence in wearing appropriate footwear, health care professionals could pay more attention to the education of patients with active foot ulcers, underling the importance of using right shoes. Additionally, they could provide patients with multiple price options concerning their footwear (e.g. by suggesting effective and affordable products).

5. Cryosurgery as a Single Agent and in Combination with Intralesional Corticosteroids Is Effective on Young, Small Keloids and Induces Characteristic Histological and Immunohistological Changes: A Prospective Randomized Trial

Background: As the pathogenesis of keloids is poorly understood, there is no sound biological basis of keloid management. Few controlled therapeutic studies have been published, and recurrences are a major reason for treatment failure. Objective: To detect efficacy and safety of cryosurgery regimens on keloids and the occurring biological changes caused by the treatment. Methods: This prospective randomized study compared efficacy and tolerability as well as histological/immunohistochemical effects of liquid nitrogen contact cryosurgery as a single regimen (group A) and combined with intralesional corticosteroids (group B) on young (2) keloids in 40 patients (2-sided effect, α-error 1%, power 95%). Results: Marked flattening of the lesions was achieved by both regimens. Median lesional volumes decreased from 106 to 7 mm3 in group A (p = 0.001) and from 138 to 6 mm3 in group B (p < 0.0001; ns, between groups). Good to excellent responses were registered in 83.3 and 90% of patients in groups A and B, respectively, by evaluating the lesional volume, in 80 and 95% of patients by the physician’s evaluation and in 95% of patients in either group by the patient’s assessment. Follow-up of 6–36 months revealed no further significant changes. Cryosurgery was generally well tolerated, with minor pain during treatment not requiring (27.5%) or requiring local anaesthesia (5%) – but not analgesics –, and hypopigmentation (25%). Histological examination showed increased vessel number and lumen dilatation after treatment in group B and reduction of rete ridge length in both groups with more prominent changes in group A. Tenascin C staining demarcated keloids from normal skin before therapy, while after therapy the entire treated tissue was labelled. Interferon-γ expression was significantly decreased after therapy both regarding positively stained cells and intensity in both groups. Conclusion: Cryosurgery without and with intralesional corticosteroids is effective and safe on young, small keloids not only as a destructive physical procedure, but also by inducing biochemical and immunological scar rejuvenation.

Psoriasis and leg ulcers have a marked impact on the patient's quality of life and represent a life-long burden for affected patients. The aim of this study is to compare the quality of life, anxiety and depression, self-esteem, and loneliness in patients with psoriasis and leg-ulcer patients. Eighty patients with leg ulcers, eighty patients with psoriasis, and eighty healthy controls were included in this study. The quality of life, depression and anxiety, loneliness of the patient, and self-esteem were assessed using the Dermatology Life Quality Index (DLQI), Hospital Anxiety and Depression Scale (HADS), the UCLA loneliness Scale (UCLA-Version 3), and the Rosenberg's Self-esteem Scale (RSES), respectively. The DLQI score among patients with psoriasis was 12.74 ± 4.89 and leg ulcer patients was 13.28 ± 2.57. The patients with psoriasis presented statistically significant higher anxiety (9.87 ± 4.56) than both leg ulcer patients (8.26 ± 2.82) and controls (6.45 ± 1.89), while leg ulcer patients also presented higher anxiety than controls. Regarding self-esteem, although there were no significant differences between the patients with psoriasis (15.25 ± 3.20) and the ones with leg ulcers (15.89 ± 2.93), they both presented statistically significant lower self-esteem scores than control group (18.53 ± 3.04). The patients with psoriasis presented statistically significant higher levels of loneliness and social isolation (46.18 ± 6.63) compared to leg ulcer patients (43.73 ± 5.68) than controls (42.49 ± 3.41). Psoriasis and leg ulcers are long-term skin diseases associated with significant impairment of the patient's quality of life, anxiety, and self-esteem, which are frequently under-recognized.

Chronic leg ulcers are a public health problem that can have a significant impact on the patient's physical, socioeconomic and psychological status. The aim of this study is to evaluate the quality of life, anxiety and depression, self-esteem and loneliness in patients suffering from leg ulcers. A total of 102 patients were enrolled in the study. The quality of life, anxiety and depression, self-esteem and loneliness of the patient were assessed using the Dermatology Life Quality Index (DLQI), Hospital Anxiety and Depression Scale (HADS), Rosenberg's Self-esteem Scale (RSES) and the UCLA Loneliness Scale (UCLA-Version 3), respectively. The mean DLQI score was 13·38 ± 2·59, suggesting a serious effect on the quality of life of patients. Those with leg ulcers had statistically significant higher scores according to the HADS-total scale (P = 0·031) and HADS-anxiety subscale (P = 0·015) compared with healthy volunteers. Moreover, a statistically significant difference was found between the two groups concerning the UCLA-scale (P = 0·029). Female patients presented with a higher score of anxiety (P = 0·027) and social isolation (P = 0·048), and worse quality of life (P = 0·018) than male patients. A severe quality of life impairment was documented, reflecting a significant psychosocial impact on patients with leg ulcers.

Bariatric surgery aims at weight reduction of severely obese patients. The Roux-en-Y technique is one of the most common bariatric procedures and is occasionally accompanied by nutrient insufficiencies and metabolic changes. According to the literature, skin architecture and immunity change after bariatric surgery and may lead to inflammation and increased susceptibility to pathogens. Additionally, vitamin and mineral deficiencies frequently develop in these patients and affect the skin's defense mechanisms, possibly contributing to dermatological complications. Knowledge and recognition of skin changes after bariatric surgery make an important asset for the dermatologist and help in the proper treatment of these patients. We report 2 cases of infectious skin lesions where vitamin and trace element deficiencies have possibly contributed to their persistence and resistance to traditional treatments.

A significant positive and high correlation between DLQI and FDLQI scores (r = 0.71, p 0.001) was documented, while DLQI contributed significantly to the prediction of FDLQI (standardized β = 0.71, p 0.001). Our study results indicate that the QoL of the family was also affected by the patient's condition of chronic leg ulcers and clearly associated with that of the patients.

A very significant health issue worldwide with great impact at personal, professional and social levels, is non-healing ulcers with massive cost in terms of human and material resources. A chronic ulcer is characterized by a persistent inflammation associated with high oxidative stress. A very significant health issue worldwide with great impact at personal, professional and social levels, is non-healing ulcers with massive cost in terms of human and material resources. A chronic ulcer is characterized by a persistent inflammation associated with high oxidative stress. Significant pre-clinical wound healing data of an ointment containing isopod Ceratothoa oestroides (sea lice) oil extract combined with gentamycin, polymyxin B and acetic acid (formula A) leaded to a clinical trial realized to Andreas Syggros Hospital of Cutaneous & Venereal Diseases. The trial was conducted in comparison with a formula containing hyaluronic acid and silver sulfadiazine (formula B). Adult patients with venous or diabetic foot ulcers meeting the required criteria, randomized to receive daily dressing containing either formula A or formula B. Every 2 weeks, patients were clinically evaluated for wound healing progress according to Bates Jensen or PUSH criteria. Measurements of pH, temperature, wound area, oxygen saturation and glucose blood levels were regularly obtained. Formula A demonstrates remarkable wound healing activity.
