6. Terapia hormonal en la menopausia vía transdérmica: un asunto de eficacia y seguridad. Artículo de revisión

Introducción: los síntomas vasomotores (SVM) asociados a la menopausia suelen disminuir la calidad de vida de las mujeres, al generar un elocuente malestar. La terapia hormonal de la menopausia (THM) es el estándar en el tratamiento de los síntomas de la menopausia, siendo el único tratamiento que ha demostrado ser eficaz para los SVM. Objetivo: describir la eficacia y seguridad de la terapia hormonal de la menopausia vía transdérmica (THM TD) para aliviar los síntomas de la menopausia. Materiales y métodos: se hizo una revisión sistemática de la literatura en diferentes bases de datos electrónicas (CINAHL Plus, EMBASE, Scopus, Lilacs, Medline, Pubmed, entre otras), a través de términos de búsqueda estandarizados. Los desenlaces evaluados incluyeron los estudios donde se publicó cualquier tratamiento o intervención con la THM TD en mujeres tratadas por síntomas de la menopausia, porcentajes de alivio de los síntomas climatéricos y proporción de eventos adversos asociados. Resultados: se incluyeron 45 publicaciones, y convienen en aconsejar el tratamiento con la THM TD, en vista de un alivio de los síntomas climatéricos cerca del 90%, asimismo de ser más segura y potencialmente más eficaz que la terapia oral; con resultados normales en la evaluación mamográfica, y sin eventos adversos relacionados después de 5 años de seguimiento. Conclusiones: la terapia hormonal en la menopausia vía transdérmica es segura y eficaz para aliviar los síntomas de la menopausia. Al evitarse el metabolismo hepático, alcanza mejores beneficios clínicos, a la vez que minimiza la exposición de eventos adversos. Las publicaciones revisadas demuestran que la vía transdérmica es la mejor forma de tratar los síntomas de la menopausia.

7. Comparison of efficacy and safety of the new generation helixone dialyzers

Introduction: New generation helixone dialyzers has recently been developed as part of the ongoing effort to improve dialyzer hemocompatibility and avoid adverse reactions to synthetic dialyzers. This study aimed to assess the performance and albumin loss of this new dialyzer series in hemodiafiltration and compare it with the previous generation helixone series. Material and methods: A prospective study was conducted in 19 patients. Each patient underwent eight dialysis sessions with the same routine dialysis parameters; only the dialyzer varied: FX60 CorDiax, FX CorAL 60, FX600 CorDiax, FX CorAL 600, FX80 CorDiax, FX CorAL 80, FX800 CorDiax, and FX CorAL 800. The reduction ratios (RR) of urea, creatinine, ß2-microglobulin, myoglobin, kappa-free immunoglobulin light chains (κFLC), prolactin, α1-microglobulin, α1-acid glycoprotein, lambda immunoglobulin light chains (λFLC), and albumin were compared intra-individually. Dialysate albumin loss was also measured. Results: All treatments were well tolerated. The mean amount of replacement fluid ranged from 31 to 34 L. Comparison of dialysis treatments showed no differences between small molecules and even up to those the size of β2-microglobulins. Little differences were found between myoglobin, κFLC, prolactin, α1-microglobulin, and λFLC RRs, and only FX80 CorDiax was slightly superior to the others. Mean dialysate albumin losses were similar, with less than 2.5 g lost in each dialyzer. The FX80 CorDiax showed slightly higher global removal scores than the other dialyzers evaluated, except for FX CorAL 800. Conclusion: The new generation helixone dialyzers series has been updated to minimise the risk of adverse reactions, while maintaining the effectiveness and albumin loss achieved by the previous most advanced helixone generation. Resumen: Introducción: Recientemente se han desarrollado dializadores de helixona de nueva generación como parte del esfuerzo continuo para mejorar la biocompatibilidad del dializador y evitar reacciones adversas a los dializadores sintéticos. Este estudio tuvo como objetivo evaluar el rendimiento y la pérdida de albúmina de esta nueva serie de dializadores en hemodiafiltración y compararla con dializadores de helixona de la generación anterior. Material y métodos: Se realizó un estudio prospectivo en 19 pacientes. Cada paciente recibió ocho sesiones de diálisis con los mismos parámetros de diálisis de rutina; sólo varió el dializador: FX60 CorDiax, FX CorAL 60, FX600 CorDiax, FX CorAL 600, FX80 CorDiax, FX CorAL 80, FX800 CorDiax y FX CorAL 800. Los índices de reducción (RR) de urea, creatinina, ß2-microglobulina, mioglobina, cadenas ligeras kappa (κFLC), prolactina, α1-microglobulina, α1-glicoproteína ácida, cadenas ligeras lambda (λFLC) y albúmina se compararon intraindividualmente. También se midió la pérdida de albúmina del dializado. Resultados: Todos los tratamientos fueron bien tolerados. La media de líquido de reposición osciló entre 31 y 34 litros. La comparación de los tratamientos de diálisis no mostró diferencias entre moléculas pequeñas e incluso del tamaño de las β2-microglobulinas. Se encontraron pocas diferencias entre los RR de mioglobina, κFLC, prolactina, α1-microglobulina y λFLC. Las pérdidas de albúmina en el dializado fueron similares, con menos de 2,5 gramos por sesión. El dializador FX80 CorDiax mostró puntuaciones de eliminación global ligeramente más altas que el resto de los evaluados, a excepción del FX CorAL 800. Conclusión: La nueva generación de dializadores de helixona, actualizado para minimizar el riesgo de reacciones adversas, ha mantenido la eficacia y la pérdida de albúmina conseguidas por la generación de helixona previa.

This article explores the effectiveness of labor mediators in Chile, focusing on their use of strategies of mediation in compulsory mediation, based on the successful mediator questionnaire developed by Golberg (2005) and the key factors explaining why parties trust the mediator proposed by Poitras et al., (2003) and Poitras (2009). The self-descriptions were analyzed using Atlas.ti 23 software, from which the suggested category system of mediation strategies for building a good relationship with conflict parties was used. Mediators reported the use of two types of strategies, contextual and reflective in compulsory mediation. The results of this research show that the main reason for successful performances reported by mediators was due to the use of contextual and reflective strategies in two types of conflicts (rights and interest-based conflicts) in compulsory mediation in Chile. To determinate the efficacy of natural disinfectants against Candida Albicans in partial and total dental prostheses. This is a bibliographic review that focused on the search for scientific articles on disinfection of prostheses by natural methods on Scopus, SciELO and Google scholar databases. After applying the inclusion and exclusion criteria, 27 articles were selected that met the theoretical characteristics applicable to the research topic. Natural disinfectants such as mint, lemon verbena and thyme eliminate Candida albicans when exposed to concentrations of 50% for 24 hours, oregano is effective in concentrations less than 10% with the same exposure time, while muña demonstrates effectiveness in concentrations greater than 75%. The care and management of a prosthetic device is a priority, since the oral cavity is an ideal environment for the multiplication of microorganisms such as Candida albicans; It has been proven that when using mint, oregano, thyme, lemon verbena and muña with adequate concentrations they equal the action of medications such as fluconazole and doxycycline, limiting the reproduction of Candida albicans. The objective of the study was to analyze and compare the benefits of using online advertising compared to offline advertising. The aim was to understand the advantages that online advertising offered in terms of reach, interactivity, return on investment and other key factors. A comprehensive analysis of case studies, market data and current trends in online and offline advertising was carried out. Analytical tools were used to evaluate the performance of advertising campaigns in both media, as well as surveys and interviews with marketing professionals to obtain qualitative insights. The study spanned a 12-month period, with data collection from baseline to completion of the analysis. A period was selected that would allow capturing seasonal changes and trends throughout the year. The study was applied in the field of advertising and marketing, with a special focus on companies seeking to promote products or services. Case studies from different sectors were considered to obtain a broad vision applicable to various industries. The research was carried out in various markets and regions to ensure a global representation of trends. Particular attention was paid to online marketplaces such as social media platforms, search engines and specialized websites. In addition, traditional offline media, such as television, radio and print media, were considered. At the end of the study, strong conclusions were drawn about the relative benefits of online advertising compared to offline advertising. Practical recommendations were provided for businesses seeking to maximize their advertising reach and effectiveness. Objetivos: Com o avançar da pandemia e o melhor conhecimento da doença do coronavírus 2019 (COVID-19), novas maneiras de enfrentar essa enfermidade vieram à tona. Assim, propomos esta revisão sistemática para entender melhor a ação das vacinas no combate à mortalidade causada por esse vírus, em especial nos pacientes transplantados. Métodos: Nesta revisão sistemática com metanálise, os revisores analisaram 389 artigos elegíveis de acordo com os critérios utilizados e selecionaram cegamente os estudos que traziam dados sobre pacientes transplantados e mortalidade, resultando no total de 15 trabalhos inseridos no estudo. Os resultados da triagem foram adicionados a uma planilha e seus dados compilados. Resultados: Dos 15 artigos incluídos, foi possível observar uma queda significativa nos índices de mortalidade geral dos pacientes vacinados, exceto nos estudos que utilizaram o imunizante de vírus inativado. Além disso, a mortalidade dos pacientes infectados mesmo após a vacinação manteve-se próxima à do grupo dos não vacinados, assim como a necessidade de ventilação mecânica. Conclusão: A imunização dos pacientes transplantados é capaz de reduzir significativamente os índices de mortalidade dessa parcela da população. No entanto, por terem uma taxa de soroconversão reduzida, diferentes maneiras de atingir uma resposta imune adequada devem ser consideradas.

18. Evaluación de mini-espigones como contramedida para la reducción de socavación local en muros longitudinales de ríos con lecho granular usando Flow-3D y AHP

El objetivo de la investigación es evaluar una contramedida para la reducción de la socavación local en muros longitudinales en ríos con lecho granular bien gradado empleando el software Flow-3D, se selecciona la contramedida empleando el método multicriterio Analytic Hierarchy Process (AHP) y se modela numéricamente con pendientes de 2.5% y caudal de 1.68 m3/s. Mediante el método AHP se obtiene que la alternativa con mayor jerarquía son los mini-espigones, por lo cual se modelan escenarios de 0.3, 0.45 y 0.6 m de dimensión dispuestos en dirección y en contraflujo. La simulación en el software demostró que la contramedida de mini-espigones de 0.45 m de longitud dispuestos en contraflujo, presentaron una eficacia del 75 %, en comparación con los de 0.3 m y 0.6 m cuyas eficacias son 60.69 % y 51.12 % respectivamente. La investigación es un aporte significativo al planteamiento de alternativas de protección muros longitudinales.

El dinamismo y consiguiente transformación de la sociedad en la actualidad, así como el incremento poblacional amerita de una prestación de servicios públicos de calidad, los cuales se ejecuten en tiempos y plazos oportunos, por los medios vigentes; es en ese sentido que emerge la preocupación por desarrollar mecanismo y herramientas para incrementar el desempeño laboral de los colaboradores que forman parte de las instituciones. Los objetivos de este estudio son, en un primer término, determinar el tipo de relación existente entre el control interno y el desempeño laboral, así como las relaciones de las dimensiones del control interno, eficiencia y eficacia, con el desempeño laboral. Para ello, el estudio cuenta con un enfoque cuantitativo, correlacional y descriptivo, de diseño transversal y metodología hipotética-deductiva. Los resultados revelaron que existe una correlación directa, alta y significativa entre el control interno, y sus dimensiones, eficiencia y eficacia, con la variable desempeño laboral. Se concluyó que los procesos de control interno y desempeño laboral son esenciales para lograr los objetivos organizacionales.

El cambio climático como problema global requiere una respuesta urgente global. No respeta fronteras y evidencia problemas sociales, económicos, culturales y políticos, amplía las brechas de desigualdad, conflictos e inestabilidad entre los países. Una de sus causas es la deforestación, problema acuciante de Ecuador, con raíces socioeconómicas profundas vinculadas con la pobreza que requiere de una política pública de largo plazo con la participación de múltiples actores. El financiamiento climático es la piedra angular para la materialización de los acuerdos, compromisos y el cumplimiento de las metas climáticas. La investigación analiza la eficiencia de los mecanismos de financiamiento climático enfocados a REDD+ Ecuador bajo la CMNUCC. Para evaluar el impacto del financiamiento climático se realizó un análisis integral multicriterio a través del método Kipu, considerando las fases del mecanismo REDD+, criterios de eficacia, eficiencia y equidad, a través de la tasa de la deforestación, la participación de actores REDD+ y la tenencia de la tierra. Según la investigación REDD+ muestra un resultado global calificado como bueno, lo cual deja abierto las posibilidades de mejora.

Fundamento: las enfermedades cardiovasculares son la principal causa de muerte en todo el mundo. El consumo de tabaco constituye un factor de riesgo de relevancia. Existen numerosas propuestas terapéuticas desde diferentes disciplinas de la salud con el objetivo de facilitar el tránsito desde el consumo habitual hacia el abandono del tabaco. Objetivo: comparar la eficacia de intervenciones para la reducción del tabaquismo en pacientes ecuatorianos con enfermedades cardiovasculares atendidos en un hospital de Quito, Ecuador. Métodos: se realizó un estudio de tipo cuasiexperimental, antes-después sin grupo control. La población de estudio estuvo conformada por 135 pacientes de un hospital de Quito, Ecuador, con enfermedades cardiovasculares. Se evaluó la eficacia de tres intervenciones: terapia cognitivo-conductual, terapia con medicamentos y programas de apoyo. Se utilizó la prueba Q de Cochran con el software SPSS. Resultados: la terapia cognitivo-conductual fue calificada de eficaz por el 71,1 % de los pacientes; los programas de apoyo se consideraron como no eficaces en un 57,8 %. Se obtuvo un estadístico Q de Cochran igual a 21,733 (p < 0,001), que representó una diferencia significativa entre las eficacias de las intervenciones. Conclusiones: existieron diferencias sustanciales entre las eficacias de las intervenciones para la reducción del tabaquismo. La terapia cognitivo-conductual fue calificada como la más eficaz.

Introduction: alcohol-relapse rates remain high during follow-up after alcohol use treatment with proven efficacy-psychological interventions. Mindfulness-based interventions (MBI) may be an alternative but it is necessary to assess the efficacy and mechanisms of action of these interventions. Purpose: this systematic review had the aim of assess the efficacy of MBI for alcohol relapse prevention on adult population. Methodology: a clinical trials publicated since 2016 to 2021 search was done using PubMed, PsycInfo and Web of Science databases. The reporting of this systematic review was guided by the standards of the PRISMA. Statement. Results: favorable MBI results are found about craving reduction and drinking amount, as well as negative affect and stress management. IBMs can improve a higher stress tolerance through learning adaptive regulation techniques. Conclusion: MBI and mindfulness components seem to be useful for reducing specific alcohol relapse-risk factors, combined with other efficacy interventions. However, the heterogeneity of the results and methodological designs do not allow firm conclusions. [ABSTRACT FROM AUTHOR]

Objective. To assess the impact of a vaccination campaign that administered five different technologies in a middleincome country with one of the largest Covid-19 epidemics. Materials and methods. Using data from Mexico's Epidemiological Surveillance System for Viral Respiratory Disease (Sisver) and the design of the vaccine policy in Mexico as a natural experiment, we applied difference-in-differences econometric methods to assess the strategy's effectiveness on transmission, hospitalizations, and mortality rates among adults 60 to 64 years old in Mexico between April and June 2021. Results. We estimated average effectiveness levels of 60.9% against confirmed cases of Covid-19. Vaccination also decreased hospitalizations and deaths by 62.7 and 62.6%, respectively. After adjusting for vaccination coverage, we found an impact of 79.1, 80.9, and 81.3% reduction in new cases, hospitalizations, and deaths among the vaccinated. Conclusion. Despite the significant progress in our knowledge of Covid-19 vaccination effectiveness, the available evidence relies mostly on experiences from high-income countries. This study contributes to the scientific literature of Covid-19 vaccination effectiveness in a middle-income country with a multi-vaccine scheme. [ABSTRACT FROM AUTHOR]

Vasomotor symptoms (VMS) associated with menopause usually reduce the quality of life of women, generating eloquent discomfort. Menopausal hormone therapy (MHT) is the standard in the treatment of menopausal symptoms, being the only treatment that has been shown to be effective for VMS. Objective: to describe the efficacy and safety of transdermal menopausal hormone therapy (THM TD) to relieve menopausal symptoms. Methods: a systematic review of the literature was carried out in different electronic databases (CINAHL Plus, EMBASE, Scopus, Lilacs, Medline, Pubmed, among others), through standardized search terms. The outcomes evaluated included studies where any treatment or intervention with TD MHT was published in women treated for menopausal symptoms, percentages of relief of climacteric symptoms and proportion of associated adverse events. Results: 45 publications were included, and they agree in advising treatment with MHT TD, in view of a relief of climacteric symptoms close to 90%, as well as being safer and potentially more effective than oral therapy; with normal results in mammographic evaluation, and without related adverse events after 5 years of follow-up. Conclusions: transdermal hormonal therapy in menopause is safe and effective in relieving menopause symptoms. By avoiding hepatic metabolism, it achieves better clinical benefits, while minimizing the exposure to adverse events. The reviewed publications demonstrate that the transdermal route is the best way to treat menopause symptoms. [ABSTRACT FROM AUTHOR]

Interpreting scholars claim that mind mapping can be used pedagogically to enhance trainees' interpreting-specific listening skills. However, so far relevant empirical studies have been rare. A single-group post and retrospective self-assessment design was used to examine student interpreters' attitudes towards the use of mind mapping in teaching listening for interpreting and its effectiveness. Eighty-two students were involved as participants. An instruction experience questionnaire was administered at the end of the pedagogical intervention to examine their attitudes towards the mind mapping exercise. Two self-assessments of knowledge and skills were conducted at the end of the pedagogical intervention to investigate their pre-test post-test gains. One was a retrospective self-assessment about their competence before the exercise (then self-assessment) and the other was a post self-assessment about their competence after the exercise (now self-assessment). The results indicate that the participants perceived mind mapping as a positive learning experience and that it was effective in developing students' interpreting-specific listening knowledge and skills. 29. Efficacy and acceptability of music therapy for post-traumatic stress disorder: a systematic review and meta-analysis of randomized controlled trials.

Background: Music therapy is increasingly examined in randomized controlled trials (RCTs) and shows potential in treating post-traumatic stress disorder (PTSD). Objective: This systematic review and meta-analysis critically evaluates the current clinical evidence supporting the efficacy and acceptability of music therapy for PTSD. Method: RCTs comparing music therapy in addition to care as usual (CAU) versus either CAU alone or CAU combined with standard psychotherapy/pharmacotherapy for PTSD were retrieved from major English – and Chinese-language databases. Standardized mean differences (SMDs) for post-treatment PTSD symptom scores and risk differences (RDs) for retention rates upon treatment completion were calculated to assess the efficacy and acceptability of music therapy, respectively. The Cochrane risk of bias (RoB) tool 2.0 and the Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) were used to assess the RoB of included studies and certainty of the evidence, respectively. Results: Nine studies, incorporating 527 PTSD patients, were included, all with high RoB. The post-treatment PTSD symptom scores were significantly lower in the music therapy group than the inactive control group (SMD = −1.64, P < .001), but comparable between the music therapy group and the active control group (SMD = −0.28, P = .330). The retention rates did not differ significantly between the music therapy group and both control groups (RD = 0.03, P = .769; RD = 0.16, P = .829). The GRADE rated certainty level of evidence as low. Conclusions: Although meta-analytic findings suggest that music therapy is effective in reducing post-traumatic symptoms in individuals with PTSD, with its therapeutic effect comparable to that of standard psychotherapy, the low level of certainty limits its generalizability. More methodologically stringent studies are warranted to strengthen the clinical evidence for the efficacy and acceptability of music therapy for PTSD. [ABSTRACT FROM AUTHOR]

This study aimed to evaluate and determine the efficacy of a brief transdiagnostic treatment in group format for people with mild and moderate emotional disorders, and to establish correlations between the changes after treatment in the different variables. We use a single experimental group (n =11) design with pre-post measures. Participants were assessed pre- and post-intervention with the following measures: PHQ-15, PHQ-9, GAD-7, PSQW-A, RRS-B, and CERQ-18. Brief transdiagnostic therapy was found to be effective in reducing depressive symptoms (p = .005), anxious symptoms (p = .011), and worry (p = .020) with high/moderate effect sizes. In addition, significant positive correlations were found between changes in depressive symptoms and worry variables, and between changes in anxious symptoms and rumination. In conclusion, brief transdiagnostic treatments can be a treatment to consider for patients with mild and moderate emotional disorders in primary care due to their efficacy in some symptoms and their characteristics. Contexto: los implantes de testosterona pueden lograr el incremento de los niveles séricos de testosterona. Objetivo: evaluar los beneficios y riesgos de los implantes de testosterona en mujeres, mediante una revisión sistemática de la literatura. Metodología: se realizó una revisión sistemática de la literatura acerca de los beneficios y riesgos del uso de los implantes de testosterona en mujeres. La búsqueda se hizo en diferentes bases de datos electrónicas (Medline, Central, Embase, entre otras), con términos de búsqueda libres y estandarizados. En los beneficios se consideró como desenlace primario la reactivación del deseo sexual, y en los riesgos la proporción de acné, spotting, clitoromegalia e hirsutismo. Resultados: se incluyeron 79 publicaciones. No se encontraron ensayos clínicos controlados aleatorizados que evaluaran los beneficios y riesgos de los implantes de testosterona en mujeres. En escasas publicaciones observacionales retrospectivas, el uso de implantes de testosterona en mujeres, se asoció con una mejoría en varios dominios de la función sexual y la angustia personal. Los datos acerca de la eficacia de seguridad a largo plazo son escasos y la calidad de la evidencia es baja. En los riesgos y efectos adversos comunes se encontró: acné, alopecia e hirsutismo, así como eritrocitosis (policitemia), extrusión, celulitis/infección y hematomas. Conclusiones: los beneficios de los implantes de testosterona en mujeres son superados por los riesgos, y la incidencia de efectos adversos irreversibles puede ser mucho más alta de lo que se ha informado. Es necesaria una estrecha vigilancia del uso racional de los implantes de testosterona en mujeres.

Introdução As quebras de pele, ou skin tears, são consideradas lesões de etiologia traumática que podem resultar de uma variedade de forças mecânicas, como cisalhamento ou forças de atrito, trauma contuso, quedas, manuseamento inadequado, lesão provocada por equipamento ou remoção de pensos aderentes. São feridas agudas que têm alta probabilidade de se tornarem feridas crónicas complexas. Causam dor, aumentam os custos de saúde, reduzem a qualidade de vida dos utentes e são fonte de stress para a pessoa. Prolongam o tempo de internamento e aumentam o risco de morbilidade. As quebras de pele são uma problemática crescente, tornando-se fulcral investir nesta temática com o intuito de promover a sua prevenção e o tratamento adequado. Objetivo Identificar os cuidados de enfermagem e as estratégias na prevenção de quebras de pele no idoso. Métodos Foi realizado um levantamento bibliográfico de publicações no motor de busca EBSCOhost, que incorpora as bases de dados CINAHL Complete, MEDLINE Complete e Cochrane Database of Systematic Reviews e na base de dados PubMed, realizado em junho de 2022. Num total de 26 registos, foram devolvidos cinco satisfazendo os critérios de inclusão. Os estudos incluídos (n=5) foram realizados entre os anos de 2012 e 2022. Resultados Na amostra dos artigos foi possível identificar os benefícios da aplicação bidiária de produtos tópicos, cremes hidratantes e emolientes, e a relevância da administração oral durante 8 semanas de um suplemento nutricional oral na hidratação do estrato córneo e na elasticidade da pele, assim como, a importância da prevenção primária e a promoção da educação para a saúde nos cuidados à pele do idoso. Conclusão Os cuidados preventivos para evitar quebras de pele no idoso centram-se na aplicação tópica bidiária de cremes hidrantes e emolientes, nutrição (administração de suplementos nutricionais orais com 10g de peptídeos de colagénio) e hidratação adequadas; acautelar traumas cutâneos proporcionando um ambiente seguro com dispositivos adequados, e através da promoção da educação para a saúde.

37. Desenho de Estudo de um Estudo Observacional Brasileiro sobre o uso de Edoxabana em Pacientes com Fibrilação Atrial (EdoBRA)

Resumo Fundamento: Os ensaios clínicos demonstraram a segurança da Edoxabana, um anticoagulante oral não dependente de vitamina K (NOAC), e a sua eficácia na prevenção de acidente vascular cerebral e embolia sistémica em pacientes com fibrilação atrial não valvar (FANV) e também na prevenção e tratamento de tromboembolismo venoso. No entanto, pesquisas adicionais são necessárias para avaliar a segurança e a eficácia da Edoxabana em um cenário real na população brasileira. Objetivo: A fim de compreender os riscos e benefícios do uso da Edoxabana em cenários clínicos de rotina, o estudo EdoBRA está sendo conduzido para obter informações sobre a segurança e eficácia do uso da Edoxabana em pacientes não pré-selecionados com FANV no Brasil. Métodos: O estudo EdoBRA é um estudo multicêntrico, prospectivo e observacional, realizado em 36 centros no Brasil. São elegíveis para este estudo pacientes com FANV, ≥ 18 anos de idade, tratados com Edoxabana disponível comercialmente, que iniciaram o tratamento por pelo menos 14 dias e não mais do que 90 dias antes da data de inclusão no estudo, e que não estão participando de nenhum outro estudo de intervenção. Ao todo, 700 pacientes devem ser inscritos e acompanhados por um ano, com coletas de dados programadas para o período basal e 3, 6 e 12 meses após a inscrição no estudo. O objetivo primário de segurança é o sangramento clinicamente relevante (de acordo com critérios da Sociedade Internacional de Trombose e Hemostasia - ISTH), e o objetivo secundário de eficácia são desfechos cardiovasculares relevantes relacionados à FANV. Conclusão: O estudo observacional EdoBRA gerará informações adicionais relevantes sobre a Edoxabana enquanto NOAC em diversos aspectos do manejo de pacientes no atendimento clínico de rotina, como perfil de segurança e efetividade em pacientes com FANV no Brasil.

En el presente se ahondará sobre si la actividad penal del Estado -en términos de juicio y condena- resulta conveniente para la totalidad de los hechos de violencia machista contra las mujeres, o si, en determinados casos considerados leves, corresponde optar por otros mecanismos que gestionan el conflicto con mayor eficacia. En primer lugar, se hará especial hincapié al debate existente en el plano internacional sobre la aplicación de la suspensión del juicio a prueba en casos de violencia de género y, en particular, al estado de situación en la República Argentina de la mano del fallo Góngora de la Corte Suprema de Justicia. En un segundo tramo, se presentará el análisis realizado sobre siete expedientes judiciales atravesados por la violencia machista -bajo jurisdicción del Juzgado de Garantías Nº 5 de la ciudad de Mar del Plata, Provincia de Buenos Aires- y en los cuales se determinó aplicar una SJP. This work will delve into whether the criminal activity of the State -in terms of trial and sentence – is appropriate for all acts of sexist violence against women, or if, in certain cases considered mild, it is appropriate to opt for other mechanisms that manage conflict more effectively. In the first place, special emphasis will be placed on the existing debate at the international level on the application of the “probation” in cases of gender violence and, in particular, the state of the situation in the Argentine Republic at the hands of the Góngora ruling of the Supreme Court of Justice. In a second section, the analysis carried out on seven judicial files crossed by sexist violence will be presented –under the jurisdiction of the Guarantee Court No. 5 of the city of Mar del Plata, Province of Buenos Aires – and in which it was determined to apply a suspension of trial.

Governance and policy authors are concerned with the effectiveness of crisis management. In this study, the effectiveness of 27 Chinese provinces in addressing COVID‐19 is examined using the QCA approach. The contribution identifies four pathways for explaining effective crisis management and two for explaining ineffective crisis management. Strong analytical capacity is a relatively critical condition for achieving effective crisis management. Background: Posttraumatic stress disorder (PTSD) is a debilitating condition affecting millions of people worldwide. Existing treatments often fail to address the complexity of its symptoms and functional impairments resulting from severe and prolonged trauma. Electroencephalographic Neurofeedback (NFB) has emerged as a promising treatment that aims to reduce the symptoms of PTSD by modulating brain activity. Objective: We conducted a systematic review and meta-analysis of ten clinical trials to answer the question: how effective is NFB in addressing PTSD and other associated symptoms across different trauma populations, and are these improvements related to neurophysiological changes? Method: The review followed the Preferred Reporting Items for Systematic Reviews and Meta analyses guidelines. We considered all published and unpublished randomised controlled trials (RCTs) and non-randomised studies of interventions (NRSIs) involving adults with PTSD as a primary diagnosis without exclusion by type of trauma, co-morbid diagnosis, locality, or sex. Ten controlled studies were included; seven RCTs and three NRSIs with a total number of participants n = 293 (128 male). Only RCTs were included in the meta-analysis (215 participants; 88 male). Results: All included studies showed an advantage of NFB over control conditions in reducing symptoms of PTSD, with indications of improvement in symptoms of anxiety and depression and related neurophysiological changes. Meta-analysis of the pooled data shows a significant reduction in PTSD symptoms post-treatment SMD of −1.76 (95% CI −2.69, −0.83), and the mean remission rate was higher in the NFB group (79.3%) compared to the control group (24.4%). However, the studies reviewed were mostly small, with heterogeneous populations and varied quality. Conclusions: The effect of NFB on the symptoms of PTSD was moderate and mechanistic evidence suggested that NFB leads to therapeutic changes in brain functioning. Future research should focus on more rigorous methodological designs, expanded sample size and longer follow-up. Neurofeedback (NFB) was found to have moderate beneficial effects on PTSD symptoms, and positive effects on secondary outcomes such as depression and anxiety, according to a meta-analysis of seven randomised controlled trials (RCTs). The beneficial effects of NFB were observed across diverse populations, including those with different types of trauma (military and civilians) and from different ethnic backgrounds. Results suggest that modulation of alpha rhythm might be a viable NFB protocol in patients with PTSD, as changes in neurophysiological functioning, such as connectivity in the Default Mode Network (DMN) and Salience Network (SN), were observed post-NFB and were correlated with a decrease in PTSD severity. Background: Timely and effective interventions can alleviate or prevent the development of clinical symptomatology in trauma-exposed individuals. However, due to limited access to these interventions, and/or stigma around accessing mental health services, there is an unmet need. Internet-and mobile-based interventions may help to address this need. Aims: This review aims to (i) synthesise the evidence on feasibility, acceptability, and effectiveness of the 'PTSD Coach' intervention (both web-based and mobile applications) in trauma-exposed individuals; (ii) evaluate the quality of this research; and (iii) identify challenges and recommendations related to 'PTSD Coach' intervention delivery. Method: Systematic database searches were conducted (PubMed/MEDLINE, PsycINFO, EMBASE, PLoS, Web of Science, PTSDpubs, Scopus, and clinical trial databases). Review inclusion was based on predefined inclusion criteria, and study quality was assessed with the mixed methods appraisal and the risk-of-bias tools for randomised trials. Where feasible, meta-analytical pooling of intervention effects on posttraumatic stress symptoms (PTSS) was conducted. Results: Seventeen manuscripts reporting on 16 primary studies were included with the majority evaluating a self-managed PTSD Coach mobile application intervention. Most studies were conducted in higher-income countries and females were over-represented. For both platforms, satisfaction and perceived helpfulness were generally high but type of smart device operating system was identified as an influence. The pooled effect size in symptom severity in the intervention group compared to the comparison group was not significant (standardised mean difference = – 0.19) (95% CI − 0.41 to − 0.03, p =.09). Heterogeneity was not significant (p =.14; I2 = 40 %). No study was excluded based on quality assessment. Conclusion: Findings support the feasibility and acceptability of 'PTSD Coach' in trauma-exposed individuals. However, evidence on the effectiveness on PTSS remains limited. More research is still needed in low-middle-income countries, particularly those in which supported 'PTSD Coach' interventions are evaluated in larger and more diverse samples. Most studies evaluated a self-managed PTSD Coach mobile application intervention in higher-income countries. Findings generally support the feasibility and acceptability of both PTSD Coach mobile application and PTSD Coach Online in trauma-exposed individuals but the evidence on the effectiveness on posttraumatic stress symptoms remains limited.
