3. Personal exposures to VOC in the upper end of the distribution - Relationships to indoor, outdoor and workplace concentrations

Abstract

Evaluation of relationships between median residential indoor, indoor workplace and population exposures may obscure potential strategies for exposure reduction. Evaluation of participants with personal exposures above median levels in the EXPOLIS study in Athens, Helsinki, Oxford and Prague illustrated that these participants frequently showed a different relationship to indoor and workplace levels than that shown by the population median. Thus, prioritization of environments for control measures based on median exposures may exclude important areas where effectively focused control measures are possible, and may therefore have little impact on the highest and most harmful exposures. Further, personal exposures at the upper end of the distribution may exceed the US EPA inhalation reference concentration (Rfc), illustrated here using hexane, naphthalene and benzene. For example upper 90th percentile personal exposures to benzene in Athens and Prague were 64 and 27 μg m-3 with peak exposures of 217 and 38 μg m-3, respectively for non-ETS exposed participants relative to an Rfc of 30 μg m-3. Strategies to reduce exposures to individual compounds, therefore, may benefit from focus on the high end of the distribution to identify activities and behaviors that result in elevated exposures. Control strategies targeting activities that lead to exposures in the upper end of the distribution would reduce the variability associated with population median values by bringing the upper end of the exposure distribution closer to median values. Thus, compliance with health-based standards would be more protective of the higher exposed fraction of the population, in whom health effects would be more expected. © 2005 Elsevier Ltd. Abstract

Background: Stents are an alternative treatment to carotid endarterectomy for symptomatic carotid stenosis, but previous trials have not established equivalent safety and efficacy. We compared the safety of carotid artery stenting with that of carotid endarterectomy. Method(s): The International Carotid Stenting Study (ICSS) is a multicentre, international, randomised controlled trial with blinded adjudication of outcomes. Patients with recently symptomatic carotid artery stenosis were randomly assigned in a 1:1 ratio to receive carotid artery stenting or carotid endarterectomy. Randomisation was by telephone call or fax to a central computerised service and was stratified by centre with minimisation for sex, age, contralateral occlusion, and side of the randomised artery. Patients and investigators were not masked to treatment assignment. Patients were followed up by independent clinicians not directly involved in delivering the randomised treatment. The primary outcome measure of the trial is the 3-year rate of fatal or disabling stroke in any territory, which has not been analysed yet. The main outcome measure for the interim safety analysis was the 120-day rate of stroke, death, or procedural myocardial infarction. Analysis was by intention to treat (ITT). This study is registered, number ISRCTN25337470. Finding(s): The trial enrolled 1713 patients (stenting group, n=855; endarterectomy group, n=858). Two patients in the stenting group and one in the endarterectomy group withdrew immediately after randomisation, and were not included in the ITT analysis. Between randomisation and 120 days, there were 34 (Kaplan-Meier estimate 4.0%) events of disabling stroke or death in the stenting group compared with 27 (3.2%) events in the endarterectomy group (hazard ratio [HR] 1.28, 95% CI 0.77-2.11). The incidence of stroke, death, or procedural myocardial infarction was 8.5% in the stenting group compared with 5.2% in the endarterectomy group (72 vs 44 events; HR 1.69, 1.

Abstract

Background: Stents are an alternative treatment to carotid endarterectomy for symptomatic carotid stenosis, but previous trials have not established equivalent safety and efficacy. We compared the safety of carotid artery stenting with that of carotid endarterectomy. Methods: The International Carotid Stenting Study (ICSS) is a multicentre, international, randomised controlled trial with blinded adjudication of outcomes. Patients with recently symptomatic carotid artery stenosis were randomly assigned in a 1:1 ratio to receive carotid artery stenting or carotid endarterectomy. Randomisation was by telephone call or fax to a central computerised service and was stratified by centre with minimisation for sex, age, contralateral occlusion, and side of the randomised artery. Patients and investigators were not masked to treatment assignment. Patients were followed up by independent clinicians not directly involved in delivering the randomised treatment. The primary outcome measure of the trial is the 3-year rate of fatal or disabling stroke in any territory, which has not been analysed yet. The main outcome measure for the interim safety analysis was the 120-day rate of stroke, death, or procedural myocardial infarction. Analysis was by intention to treat (ITT). This study is registered, number ISRCTN25337470. Findings: The trial enrolled 1713 patients (stenting group, n=855; endarterectomy group, n=858). Two patients in the stenting group and one in the endarterectomy group withdrew immediately after randomisation, and were not included in the ITT analysis. Between randomisation and 120 days, there were 34 (Kaplan-Meier estimate 4·0%) events of disabling stroke or death in the stenting group compared with 27 (3·2%) events in the endarterectomy group (hazard ratio [HR] 1·28, 95% CI 0·77-2·11). Abstract

Despite the widespread and common use of DNA-sequence data to estimate phylogenies, support or contest classifications, and identify species using barcodes, they are not commonly used as the primary or sole source of data for describing species. This is possibly due to actual or perceived pressure from peers to include morphology as the primary source of data for species descriptions. We find no compelling evidence to exclude DNA-only descriptions, or to insist that morphology always be included in a species description. It is not the data type per se that is important, but the science behind the taxonomic conclusions. Using alternative kinds of data for descriptions should not cause problems for taxonomy if links are kept with type specimens.

Abstract

Diverse indoor combustion sources contribute to the indoor air environment. To evaluate the effect of these sources on human respiratory health, we examined associations between respiratory conditions and household factors in the 2360 children's fathers (mean = 38.4 years old) and associations between lung function and household factors in 463 primary school children (mean = 8.3 years old) from Wuhan, China. Factor analysis developed new uncorrelated ‘factor’ variables. Unconditional logistic regression models or linear regression models, controlling for important covariates, estimated the respiratory health effects. Coal smoke derived from home heating (‘heating coal smoke’) was associated with high adult reporting of persistent cough, persistent phlegm, and wheeze. Cooking coal smoke was associated with physician-diagnosed adult asthma and decreased forced vital capacity (FVC), and forced expiratory volume at 1 s (FEV1) in children. The presence of any home cigarette smoker was associated with more reports of persistent cough, persistent phlegm, cough with phlegm, and bronchitis. Our study suggests that in Wuhan, there may be independent respiratory health effects of different indoor combustion sources and their exposure factors for these study populations. Practical Implications We conclude that multiple indoor air pollution sources could have adverse respiratory health effects on both children and middle-aged men in the city of Wuhan, China. These results may have implications for the Wuhan local government, the Chinese government, or other related organizations in efforts on protecting public health through regulation of indoor air pollution from indoor combustion sources. Abstract

A lot of money is being channelled into home construction financing. Buyers have not been as interested as the economic recovery would suggest because the price of the homes and interest rates have become exorbitant. Cheap and expensive houses have been selling but middle level homes have not. Not enough low-end housing has been built. Political interest in the situation has so far entailed the possibilities of subsidization and regulation but this does not constitute a responsible approach to the base of the problem. While family size has declined, housing sizes have grown larger. Land and construction costs have increased. Labor and regulatory costs are analyzed. Fannie Mae's reserve account mortgages, permitting loans at discounted rates and not requiring down payments, are analyzed. A graph of the affordability gap in housing is highlighted.

The former negativity experienced by asset-based lending is no more. Loans with underlying tangible assets have grown much more attractive. Larger banks now consider using this vehicle. Many firms are moving against their former conglomerations, with leveraged buy outs. The assessment of underlying assets is essential to successful asset-based lending. Knowledge of the borrowing industry is essential. Assets supporting loans must be controlled over the loan length. Causes of growth in asset-based lending include the dissolution of equity markets and the end of long-term debt sources. Firms are not able to turn public as easily as they had in former times.

For nearly as long as anyone can remember the nation has had a deficit budget, now the problem has reached crisis proportions. With the tax structure insufficient to needs, spending out of control due to demands of special interest groups and politicians unwilling to alienate voters, getting something done seems like an impossible task. Essentially there are only two solutions, cut spending or increase taxes. The United States, government and people, needs to determine which of two unpleasant alternatives to take and then take action. The deficit as it stands has nearly totally crowded out private credit demands, choking the free enterprise system. Some suggested solutions are the value added tax or radical cuts in spending as suggested by the Grace Commission.
