2. Making Sense of Mental Health: A Qualitative Study of Student Counsellors

Abstract

More and more people in the UK have poor mental health. Consequently, there is a growing demand for mental health professionals such as counsellors and psychotherapists. The aim of this study was to explore how trainee counsellors make sense of mental health in general and concepts like disorder, diagnosis and treatment in particular. We employ semi-structured interviews to explore what seven UK-based trainee counsellors thought and felt about mental health and disorders and how this affects their motives and intentions in relation to practice. A four-dimensional "Folk Psychology Model" (pathologising, moralising, medicalising and psychologising) provides a useful framework to illustrate and explain tension, complexity, diversity, contradiction and confusion among trainee counsellors' views. These views reflect more general contested epistemological, and perhaps even ontological beliefs, operating within and between relevant parent disciplines and professions about the mind and mental health.

11. 'Don't Follow Them, Look at Me!': Contemplating a Haptic Digital Prototype to Bridge the Conductor and Visually Impaired Performer

Abstract

This paper reports on an exploratory research-and-development project concerning a device for conveying a conductor's gestures wirelessly to a visually impaired (blind or partially sighted) performer as a haptic signal. The research team developed this device from January to July 2017 under a University College London (UCL) Institute of Education 'seed-corn' grant. As a platform for its development, they firstly observed and analysed video footage of conductors at the Royal Academy of Music, London using Elan software to create a gestural model. Subsequently, through gaining blind end-users' feedback on the device, as well as an experiment to compare their timing using either (i) a two-dimension haptic signal or (ii) a metronomic pulsation, it is suggested that the development of technologies for this purpose should focus on the meaning the conductor intends to convey coupled with haptic signals blind end-users themselves deem suitable, rather than adopting a 'sighted perspective' in attempting faithful transference of two-dimensional captures of arm movements from one medium to another. Reasons for this assertion are explored.

Abstract

: Introduction: Interruptions in treatment pose risks for people with HIV (PWH) and threaten progress in ending the HIV epidemic; however, the COVID‐19 pandemic's impact on HIV service delivery across diverse settings is not broadly documented. Methods: From September 2020 to March 2021, the International epidemiology Databases to Evaluate AIDS (IeDEA) research consortium surveyed 238 HIV care sites across seven geographic regions to document constraints in HIV service delivery during the first year of the pandemic and strategies for ensuring care continuity for PWH. Descriptive statistics were stratified by national HIV prevalence ( Results: Questions about pandemic‐related consequences for HIV care were completed by 225 (95%) sites in 42 countries with low (n = 82), medium (n = 86) and high (n = 57) HIV prevalence, including low‐ (n = 57), lower‐middle (n = 79), upper‐middle (n = 39) and high‐ (n = 50) income countries. Most sites reported being subject to pandemic‐related restrictions on travel, service provision or other operations (75%), and experiencing negative impacts (76%) on clinic operations, including decreased hours/days, reduced provider availability, clinic reconfiguration for COVID‐19 services, record‐keeping interruptions and suspension of partner support. Almost all sites in low‐prevalence and high‐income countries reported increased use of telemedicine (85% and 100%, respectively), compared with less than half of sites in high‐prevalence and lower‐income settings. Few sites in high‐prevalence settings (2%) reported suspending antiretroviral therapy (ART) clinic services, and many reported adopting mitigation strategies to support adherence, including multi‐month dispensing of ART (95%) and designating community ART pick‐up points (44%). While few sites (5%) reported stockouts of first‐line ART regimens, 10–11% reported stockouts of second‐ and third‐line regimens, respectively, primarily in high‐prevalence and lower‐income settings. Interruptions in HIV viral load (VL) testing included suspension of testing (22%), longer turnaround times (41%) and supply/reagent stockouts (22%), but did not differ across settings. 16. 'Problematic youth' : an ethnographic study of working class culture at 'home' and within a Pupil Referral Unit

Abstract

Social anxieties surrounding the perceived deviancy of young people both within their communities and the education system have been continually reconstructed, over the past century. Burgeoning contemporary media representations and government policies which specifically draw upon the notion of an underclass (Murray: 1990) are in interplay and exacerbate public fears. Increasingly under neo-liberalism, state governance and disciplinary powers are aimed specifically at working class youth. Whilst there have been recent accounts of working-class stigmatisation through analysis of dominant discourses in political debate, policy and media, there is limited research which investigates the extent to which these representations affect the everyday lived experiences of the people to which they refer. This thesis, based on emergent themes fom ethnographic research on the 'streets' and within a Pupil Referral Unit (PRU) asse1ts that working class youths lived experience is increasingly framed by stigmatising discourses of inherited cultural deficiencies. The resources that young people draw upon in response to this experienced stigma result in further governance, regulation and restriction. Both within educational institutions and in their contested use of public and community space, young people's self-identifications, are hampered by a frustrated desire to 'belong' within communities and institutions which categorise them as deviant. The thesis focuses on the PRU as a Total Institution (Goffman: 1961), the terminus for youth moved across educational spheres and considers the meaning making of working class youth subject to policy intervention. It analyses young people's everyday experiences of marginalisation and stigmatisation focusing in on tensions that arise between young people and those policing their exile. Finally, it is suggested that young people's negotiation, contestation and reaction to the problematic discourse surrounding them, impact and affect not only their social interactions but life chances and opportunities, reflecting previous findings of cultural and social reproductions (Willis: 1977).

18. Youth movements, citizenship and the English countryside, 1930-1960

Abstract

This thesis explores the significance and meaning of the countryside within mid-twentieth century youth movements. Whilst modern youth movements have been the subject of considerable historical research, there has been little attention to the rural context within which so many of them operated. Moreover, few historians have explored youth movements into the post-Second World War period. This thesis therefore makes an original contribution both in terms of its periodisation and focus. It draws on a rich seam of archival and printed sources focusing in particular upon the Boy Scout and Girl Guide movements, the Woodcraft Folk and the Young Farmer's Club movement. The thesis examines the ways in which the countryside was employed as a space within which ‘good citizenship' could be developed. Mid-century youth movements identified the ‘problem' of modern youth as a predominantly urban and working class issue. They held that the countryside offered an effective antidote to these problems: being a ‘good citizen' within this context necessitated a respectful and mutually beneficial relationship with the rural sphere. Avenues to good citizenship could be found through an enthusiasm for outdoor recreation, the stewardship of the countryside and work on the land. Models of good citizenship were intrinsically gendered. Girls were trained for their domestic role within the home, although this was a specifically rural form of domesticity. Chapter One explores the shifting relationship between the urban public and the countryside in the mid-century and argues that the popularity of outdoor recreation developed understandings of citizenship that were directly linked to the English countryside. For youth this country-conscious citizenship could be developed in three spheres: leisure, work and the home. Chapter Two examines the approach of youth movements to youthful leisure across the mid-century and, using concern for the juvenile delinquent as a case study, argues that through physical and mental improvement the countryside could prevent misbehaviour. Parallel to this youth movements instilled an understanding of ‘good' countryside manners and encouraged members to protect the countryside from the onslaught of urban pleasureseekers. Chapter Three explores the importance of agricultural work in meanings of ‘good citizenry' arguing that for both urban and rural boys proficiency in farming, particularly in wartime, was considered an important service to the nation. Chapter Four investigates how the sphere of the home remained central to understandings of ‘good citizenship' for girls and suggests that the distinct nature of rural domesticity should be considered here. It also considers the place of youth movements within the gendered lifecycle, understandings of female deviance and issues of agency in leisure provision for girls in the mid-century. This thesis argues that, fundamentally, the mid-century period should be seen as one of continuity in the training of youth movements. The central role of the countryside in categorisations of ‘good citizenry' supports recent understandings of a rural national identity in the mid-century. Furthermore, approaches to youth were clearly divided in terms of both class and gender. Abstract

Background: Reducing avoidable harm in maternity services is a priority globally. As well as learning from mistakes, it is important to produce rigorous descriptions of ‘what good looks like’. Objective: We aimed to characterise features of safety in maternity units and to generate a plain language framework that could be used to guide learning and improvement. Methods: We conducted a multisite ethnography involving 401 hours of non-participant observations 33 semistructured interviews with staff across six maternity units, and a stakeholder consultation involving 65 semistructured telephone interviews and one focus group. Results: We identified seven features of safety in maternity units and summarised them into a framework, named For Us (For Unit Safety). The features include: (1) commitment to safety and improvement at all levels, with everyone involved; (2) technical competence, supported by formal training and informal learning; (3) teamwork, cooperation and positive working relationships; (4) constant reinforcing of safe, ethical and respectful behaviours; (5) multiple problem-sensing systems, used as basis of action; (6) systems and processes designed for safety, and regularly reviewed and optimised; (7) effective coordination and ability to mobilise quickly. These features appear to have a synergistic character, such that each feature is necessary but not sufficient on its own: the features operate in concert through multiple forms of feedback and amplification. Conclusions: This large qualitative study has enabled the generation of a new plain language framework—For Us—that identifies the behaviours and practices that appear to be features of safe care in hospital-based maternity units.

Abstract

The effects of O2 supply on the redox poise and in vivo oxygenation of cytochromes b, d, and c of Azotobacter vinelandii were explored using a modified diode-array spectrophotometer. Alterations in O2 supply resulted in changes in the absorbance attributed to cytochromes b, c and d. Moreover, three states of cytochrome d were observed in washed cell suspensions. The findings suggest that the oxygenated form of cytochrome d in the one-electron reduced cytochrome bd-type oxidase is not involved in enzyme turnover.
