Surveillance imaging of HNSCC in patients treated with chemoradiotherapy suffers from difficulty in differentiating residual disease from radiation changes and inflammation. Thus, this study assessed ML models based on RadFs extracted from standard CT images pre- and post-chemoradiation to predict HNSCC treatment response. A retrospective analysis of HNSCC patients treated with definitive chemoradiotherapy at our institution between 2006 and 2015 was performed. Thirty-six patients with residual disease on CT scans of the soft tissue of the neck at a two- month interval-either in the primary site, nodal stations, or both-were enrolled. GTV contours from the treatment planning CT (CT1), post-treatment CT (CT2), and CT portion of the PET/CT (CT3) of the neck were exported to MatLab®, where 2D and 3D RadFs were extracted using different methods. Finally, ML models were used to identify the RadFs that predict changes and progression in HNSCC patients treated with chemoradiotherapy. SVM models using 2D RadFs, extracted from CT2, were associated with residual disease on PET/CT exams (AUC = 0.702). 2D RadFs extracted from PET/CT had moderate predictive ability to predict positive pathology for residual tumor (AUC = 0.667). NN and RF models of 3D RadFs extracted from CT2 and PET/CT had good and moderate predictive ability to predict positive pathology for residual tumor (AUC = 0.720 and 0.678, respectively). ML models using 2D and 3D RadFs derived from pre- and post-treatment CT data show promise for predicting residual tumor from radiation changes and inflammation in a small group of HNSCC cancer patients treated with chemoradiotherapy.

Imaging using nuclear medicine is one of the most common procedures in medical centers. Its great advantage is its capacity to analyze the metabolism of the patient, resulting in early diagnosis. However, quantification in nuclear medicine is complicated by many factors, including degradation due to attenuation, scattering, reconstruction algorithms, and assumed models. This project seeks to improve the accuracy and the precision of quantification in PET/CT images. We developed a framework, comprising consecutively interlinked steps initiated with the simulation of 3D anthropomorphic phantoms. These phantoms were used to generate realistic PET/CT projections by applying the Geant4 Application for Tomography Emission platform using Monte Carlo simulation. Then, a 3D image reconstruction was created, followed by an Anscombe/Wiener filter and a fuzzy connectedness segmentation process. After defining the region of interest, input activity and response activity curves were generated as excitation functions of the compartment model to enable metabolic quantification of the selected organ or structure. Finally, real PET/CT images provided by the Heart Institute of Hospital das Clinicas, School of Medicine of the University of Sao Paulo were analyzed using the method. Metabolic parameters of the three-compartment model based on the MASH anthropomorphic phantom and real PET images were computed for each of the approaches used in this project; the results were similar to the theoretically characteristic values. The three-dimensional filtering step using the Ascombe/Wiener filter was preponderant and had a high impact on the metabolic quantification process and on other important stages of the whole project.

Materials and Methods For this institutional review board-approved HIPAA-compliant retrospective study, adult patients with cirrhosis and Model for End-Stage Liver Disease (MELD) score within 3 months of initial liver CT imaging between January 3, 2006, and May 30, 2012, were identified from electronic medical records (n = 830). Purpose To compare the effectiveness of metastatic tumor response evaluation with computed tomography using computer-assisted versus manual methods. Materials and Methods In this institutional review board–approved, Health Insurance Portability and Accountability Act–compliant retrospective study, 11 readers from 10 different institutions independently categorized tumor response according to three different therapeutic response criteria by using paired baseline and initial post-therapy computed tomography studies from 20 randomly selected patients with metastatic renal cell carcinoma who were treated with sunitinib as part of a completed phase III multi-institutional study. Images were evaluated with a manual tumor response evaluation method (standard of care) and with computer-assisted response evaluation (CARE) that included stepwise guidance, interactive error identification and correction methods, automated tumor metric extraction, calculations, response categorization, and data and image archiving. A crossover design, patient randomization, and 2-week washout period were used to reduce recall bias. Comparative effectiveness metrics included error rate and mean patient evaluation time. Results The standard-of-care method, on average, was associated with one or more errors in 30.5% (6.1 of 20) of patients, whereas CARE had a 0.0% (0.0 of 20) error rate ( P < .001). The most common errors were related to data transfer and arithmetic calculation. In patients with errors, the median number of error types was 1 (range, 1 to 3). Mean patient evaluation time with CARE was twice as fast as the standard-of-care method (6.4 minutes v 13.1 minutes; P < .001). Conclusion CARE reduced errors and time of evaluation, which indicated better overall effectiveness than manual tumor response evaluation methods that are the current standard of care.

Purpose The definition of radiotherapy target volume is a critical step in treatment planning for all tumor sites. Conventional magnetic resonance imaging (MRI) pulse sequences are used for the definition of the gross target volume (GTV) and the contouring of glioblastoma multiforme (GBM) and meningioma. We propose the use of multiparametric MRI combined with radiomic features to improve the texture-based differentiation of tumor from edema for GTV definition and to differentiate vasogenic from tumor cell infiltration edema. Methods Twenty-five patients with brain tumor and peritumoral edema (PTE) were assessed. Of the enrolled patients, 17 (63 ± 10 years old, six female and 11 male patients) were diagnosed with GBM and eight (64 ± 14 years old, five female and three male patients) with meningioma. A 3 Tesla (3T) MRI scanner was used to scan patients using a 3D multi-echo Gradient Echo (GRE) sequence. After the acquisition process, two experienced neuroradiologists independently used an in-house semiautomatic algorithm to conduct a segmentation of two regions of interest (ROI; edema and tumor) in all patients using functional MRI sequences, apparent diffusion coefficient (ADC), and dynamic contrast-enhanced MRI (DCE-MRI), as well as anatomical MRI sequences—T1-weighted, T2-weighted and fluid-attenuated inversion recovery (FLAIR). Radiomic (computer-extracted texture) features were extracted from all ROIs through different approaches, including first-, second-, and higher-order statistics, both with and without normalization, leading to the calculation of around 300 different texture parameters for each ROI. Based on the extracted parameters, a least absolute shrinkage and selection operator (LASSO) analysis was used to isolate the parameters that best differentiated edema from tumors while irrelevant parameters were discarded. Results and conclusions The parameters chosen by LASSO were used to perform statistical analyses which allowed identification of the variables with the best discriminant ability in all scenarios. Receiver operating characteristic results showcase both the best single discriminator and the discriminant capacity of the model using all variables selected by LASSO. Excellent results were obtained for patients with GBM with all MRI sequences, with and without normalization; a T1-weighted sequence postcontrast (T1W+C) with normalization offered the best tumor classification (area under the curve, AUC > 0.97). For patients with meningioma, a good model of tumor classification was obtained through the T1-weighted sequence (T1W) without normalization (AUC > 0.71). However, there was no agreement between the results of both radiologists for some MRI sequences analyzed for patients with GBM and meningioma. In conclusion, a small subset of radiomic features showed an excellent ability to distinguish edema from tumor tissue through its most discriminating features.

Transarterial radioembolization using yttrium-90 microspheres is an established and effective treatment for liver malignancies. Determining response to this treatment is difficult due to the radical changes that occur in tissue as a response to radiation. Though accurate assessment of treatment response is paramount for proper patient disposition, there is currently no standardized assessment protocol. Current methods of assessment often consider changes in size, necrosis, vascularity, fluorodeoxyglucose-positron emission tomography FDG-PET metabolic activity, and diffusion using diffusion-weighted magnetic resonance imaging (DWI). Current methods of assessment require a lag time of one to two months post-treatment to determine treatment effectiveness. This delay is a hindrance to obtaining better patient outcomes, giving rise to a need to identify markers for faster determination of treatment efficacy.

To evaluate precision of a software-based liver surface nodularity (LSN) score derived from CT images. An anthropomorphic CT phantom was constructed with simulated liver containing smooth and nodular segments at the surface and simulated visceral and subcutaneous fat components. The phantom was scanned multiple times on a single CT scanner with adjustment of image acquisition and reconstruction parameters (N = 34) and on 22 different CT scanners from 4 manufacturers at 12 imaging centers. LSN scores were obtained using a software-based method. Repeatability and reproducibility were evaluated by intraclass correlation (ICC) and coefficient of variation. Using abdominal CT images from 68 patients with various stages of chronic liver disease, inter-observer agreement and test–retest repeatability among 12 readers assessing LSN by software- vs. visual-based scoring methods were evaluated by ICC. There was excellent repeatability of LSN scores (ICC:0.79-0.99) using the CT phantom and routine image acquisition and reconstruction parameters (kVp 100–140, mA 200–400, and auto-mA, section thickness 1.25–5.0 mm, field of view 35–50 cm, and smooth or standard kernels). There was excellent reproducibility (smooth ICC: 0.97; 95% CI 0.95, 0.99; CV: 7%; nodular ICC: 0.94; 95% CI 0.89, 0.97; CV: 8%) for LSN scores derived from CT images from 22 different scanners. Inter-observer agreement for the software-based LSN scoring method was excellent (ICC: 0.84; 95% CI 0.79, 0.88; CV: 28%) vs. good for the visual-based method (ICC: 0.61; 95% CI 0.51, 0.69; CV: 43%). Test–retest repeatability for the software-based LSN scoring method was excellent (ICC: 0.82; 95% CI 0.79, 0.84; CV: 12%). The software-based LSN score is a quantitative CT imaging biomarker with excellent repeatability, reproducibility, inter-observer agreement, and test–retest repeatability.

A presença da Medicina Nuclear como modalidade de obtenção de imagens médicas é um dos principais procedimentos utilizados hoje nos centros de saúde, tendo como grande vantagem a capacidade de analisar o comportamento metabólico do paciente, traduzindo-se em diagnósticos precoces. Entretanto, sabe-se que a quantificação em Medicina Nuclear é dificultada por diversos fatores, entre os quais estão a correção de atenuação, espalhamento, algoritmos de reconstrução e modelos assumidos. Neste contexto, o principal objetivo deste projeto foi melhorar a acurácia e a precisão na análise de imagens de PET/CT via processos realísticos e bem controlados. Para esse fim, foi proposta a elaboração de uma estrutura modular, a qual está composta por um conjunto de passos consecutivamente interligados começando com a simulação de phantoms antropomórficos 3D para posteriormente gerar as projeções realísticas PET/CT usando a plataforma GATE (com simulação de Monte Carlo), em seguida é aplicada uma etapa de reconstrução de imagens 3D, na sequência as imagens são filtradas (por meio do filtro de Anscombe/Wiener para a redução de ruído Poisson caraterístico deste tipo de imagens) e, segmentadas (baseados na teoria Fuzzy Connectedness). Uma vez definida a região de interesse (ROI) foram produzidas as Curvas de Atividade de Entrada e Resultante requeridas no processo de análise da dinâmica de compartimentos com o qual foi obtida a quantificação do metabolismo do órgão ou estrutura de estudo. Finalmente, de uma maneira semelhante imagens PET/CT reais fornecidas pelo Instituto do Coração (InCor) do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (HC-FMUSP) foram analisadas. Portanto, concluiu-se que a etapa de filtragem tridimensional usando o filtro Anscombe/Wiener foi relevante e de alto impacto no processo de quantificação metabólica e em outras etapas importantes do projeto em geral. The Nuclear Medicine imaging using PET modality allows evaluating the physiological condition of the heart (i.e. heart disease, perfusion, cardiac stress, physics disorders, etc.), by the use of small amounts of radioactive material. While the visual information provided by PET image is quite important and today is heavily used by the specialists, it is clear that having quantitative values of the metabolism would help to determine the real condition of the myocardium, to get a more accurate diagnosis and to apply a better treatment. Thus, we developed a framework using different tools to simulate a real PET exam focused in the heart in order to analyze the metabolic exchange in this organ. This research produced realistic PET exams by using GATE platform that performs Monte Carlo simulations, together with an anthropomorphic phantom of the whole body called MASH. This study consisted of the modeling of a commercial PET scanner with BGO detectors, where the MASH's thorax was imaged by the scanner. In order to enhance the realism of the simulation, clinical data was considered, like the quantity of radiotracer (FDG) injected, time of acquisition, and number of frames along the time, among others. Next, the projections were reconstructed using STIR's algorithms. With the aim of obtaining a better quality of the volumes and to select the specific ROI, filtering and segmentation algorithms were applied, respectively. Finally, the heart metabolic analysis was performed using a mathematical model that seeks to describe and to quantify the level of consumption and exchange of glucose in the heart through a model with three compartments. The metabolic parameters obtained were: K1=0.5690, k2=0.2266, k3=0.0718, and k4=0.0243. It was evidenced that the process of metabolic quantification using compartmental modeling is significantly relevant because of its flexibility, noninvasiveness and reliability. Hereafter, we will apply this framework under real PET images.

A Medicina Nuclear, como especialidade de obtenção de imagens médicas é um dos principais procedimentos utilizados hoje nos centros de saúde, tendo como grande vantagem a capacidade de analisar o comportamento metabólico do paciente. Este projeto está baseado em imagens médicas obtidas através da modalidade PET (Positron Emission Tomography). Para isso, foi desenvolvida uma estrutura de processamento de imagens tridimensionais PET, constituída por etapas sucessivas que se iniciam com a obtenção das imagens padrões (gold standard), sendo utilizados para este fim volumes simulados do Ventrículo Esquerdo do Coração criadas como parte do projeto, assim como phantoms gerados com o software NCAT-4D. A seguir, nos volumes simulados é introduzido ruído Poisson que é o ruído característico das imagens PET. Na sequência é executada uma etapa de pré-processamento, utilizando alguns filtros 3D tais como o filtro da mediana, o filtro da Gaussiana ponderada e o filtro Anscombe/Wiener. Posteriormente é aplicada a etapa de segmentação, processo baseado na teoria de Conectividade Fuzzy sendo implementadas quatro diferentes abordagens 3D: Algoritmo Genérico, LIFO, kTetaFOEMS e Pesos Dinâmicos. Finalmente, um procedimento de avaliação conformado por três parâmetros (Verdadeiro Positivo, Falso Positivo e Máxima Distância) foi utilizado para mensurar o nível de eficiência e precisão do processo. Constatou-se que o par Filtro - Segmentador constituído pelo filtro Anscombe/Wiener junto com o segmentador Fuzzy baseado em Pesos Dinâmicos proporcionou os melhores resultados, com taxas de VP e FP na ordem de 98,49 ± 0,27% e 2,19 ± 0,19%, respectivamente, para o caso do volume do Ventrículo Esquerdo simulado. Com o conjunto de escolhas feitas ao longo da estrutura de processamento, encerrou-se o projeto analisando um número reduzido de volumes pertencentes a um exame PET real, obtendo-se a quantificação dos volumes. The Nuclear medicine, as a specialty to obtain medical images is very important, and it has became one of the main procedures utilized in Health Care Centers to analyze the metabolic behavior of the patient. This project was based on medical images obtained by the PET modality (Positron Emission Tomography). Thus, we developed a framework for processing Nuclear Medicine three-dimensional images of the PET modality, which is composed of consecutive steps that start with the generation of standard images (gold standard) by using simulated images of the Left Ventricular Heart, such as phantoms obtained from the NCAT-4D software. Then, Poisson quantum noise was introduced into the whole volume to simulate the characteristic noises in PET images. Subsequently, the pre-processing step was executed by using specific 3D filters, such as the median filter, the weighted Gaussian filter, and the Anscombe/Wiener filter. Then the segmentation process, which is based on the Fuzzy Connectedness theory, was implemented. For that purpose four different 3D approaches were implemented: Generic, LIFO, kTetaFOEMS, and Dynamic Weight algorithm. Finally, an assessment procedure was used as a measurement tool to quantify three parameters (True Positive, False Positive and Maximum Distance) that determined the level of efficiency and precision of our process. It was found that the pair filter - segmenter formed by the Anscombe/Wiener filter together with the Fuzzy segmenter based on Dynamic Weights provided the best results, with VP and FP rates of 98.49 ± 0.27% and 2.19 ± 0.19%, respectively, for the simulation of the Left Ventricular volume. Along with the set of choices made during the processing structure, the project was finished with the analysis of a small number of volumes that belonged to a real PET test, thus the quantification of the volumes was obtained.
