1. Unexpected Gallbladder Metastasis of Clear Cell Renal Carcinoma

Abstract: Renal cell carcinoma (RCC) commonly metastasizes to areas such as the lungs, liver, bone, brain, adrenals, and lymph nodes. We present a rare case of a 59-year-old female patient with RCC metastasis to the gallbladder. The patient had undergone right nephrectomy for RCC more than 6 years prior to the metastasis. During routine follow-up, a polypoid lesion of the gallbladder was identified. Laparoscopic cholecystectomy was performed, and pathologic examination of the specimen revealed metastatic RCC. The patient was completely asymptomatic, which reinforces the importance of postoperative follow-up. Renal cell carcinoma is one of the few common malignancies for which there is good evidence of survival benefit from surgical resection of the metastatic tumours.

2. Development of a Framework for a Lean based Water and Energy Efficiency Assessment Tool

Abstract: The manufacturing industry of South Africa is the sector consuming the largest portion of the total energy consumption and second largest portion of total water consumption per annum nationally. With a significant increase in electrical energy cost in recent years, together with the reserve energy margin dropping below the minimum level required for sustainable operation of energy utilities, energy efficiency improvement is becoming imperative for organisational success as well as national economical sustainability. This paper explores selected Lean manufacturing principles and its positive effect on energy and water efficiency. Although the implementation of Lean manufacturing techniques naturally leads to the improvement of energy and water intensity, the author believes that there is even greater potential in the development of a Lean based tool which will specifically focus on the improvement of energy and water efficiency. For this purpose the value stream mapping tool was chosen as the foundation. This paper continues to explain the process undergone to develop standardised energy and water specific waste categories to be used in conjunction with the traditional Lean wastes. The study concludes by detailing the development of the tool, together with its framework for implementation and a brief discussion on the forecasting model incorporated.

4. Combining Drones and Deep Learning to Automate Coral Reef Assessment with RGB Imagery

Abstract: Coral reefs and their associated marine communities are increasingly threatened by anthropogenic climate change. A key step in the management of climate threats is an efficient and accurate end-to-end system of coral monitoring that can be generally applied to shallow water reefs. Here, we used RGB drone-based imagery and a deep learning algorithm to develop a system of classifying bleached and unbleached corals. Imagery was collected five times across one year, between November 2018 and November 2019, to assess coral bleaching and potential recovery around Lord Howe Island, Australia, using object-based image analysis. This training mask was used to develop a large training dataset, and an mRES-uNet architecture was chosen for automated segmentation. Unbleached coral classifications achieved a precision of 0.96, a recall of 0.92, and a Jaccard index of 0.89, while bleached corals achieved 0.28 precision, 0.58 recall, and a 0.23 Jaccard index score. Subsequently, methods were further refined by creating bleached coral objects (>16 pixels total) using the neural network classifications of bleached coral pixels, to minimize pixel error and count bleached coral colonies. This method achieved a prediction precision of 0.76 in imagery regions with >2000 bleached corals present, and 0.58 when run on an entire orthomosaic image. Bleached corals accounted for the largest percentage of the study area in September 2019 (6.98%), and were also significantly present in March (2.21%). Unbleached corals were the least dominant in March (28.24%), but generally accounted for ~50% of imagery across other months. Overall, we demonstrate that drone-based RGB imagery, combined with artificial intelligence, is an effective method of coral reef monitoring, providing accurate and high-resolution information on shallow reef environments in a cost-effective manner.

9. Total knee replacement in a transtibial amputee

Abstract: We present the case of a man in his 60s with a transtibial amputation (TTA) undergoing total knee replacement (TKR) for symptomatic osteoarthritis (OA). It is unusual to develop OA in the ipsilateral knee to TTA; and while it is postulated that this is because patients preferentially load their unaffected limb to protect the TTA-sided knee, there is also the ability to offload specific knee compartments through prosthetic adjustment. When planning TKR in such patients, it is important to consider several technical challenges in order to prevent a poor outcome. The literature is sparse with evidence to guide decision-making, and this case report and literature review aims to summarise our preoperative planning and intraoperative technique, which ultimately resulted in a good outcome.

10. Forging Repertories : Cathedral Music in New Spain and Its Performance

Abstract: Much engagement with the cathedral music of New Spain has been through lens of exoticism. This book challenges this view by uncovering how colonial repertories mixed European aesthetics with locally composed pieces to create canons both tailored to local liturgies and shaped by European tradition. Building upon material from the archives of Mexico City, Durango, and Puebla cathedrals, author Drew Edward Davies examines how composers, some of them priests, communicated theological doctrine through music genres. The book also offers a new understanding of cultural encounter, both by assessing how music was used for indoctrination and by rethinking stereotypes in villancicos through the lens of topic theory. Illuminating the unique mix of devotional subjects stressed in New Spain, Davies argues that topicality rather than style differentiated New Spanish musical repertory from that of Europe. Concluding with a history of the early music movement's revival of New Spanish music beginning in the 1960s, Davies suggests that exoticism and the imagination continue to shape performances in ways that may not be plausible historically, but nonetheless resonate with audiences in the contemporary world. In so doing, he invites performers and scholars alike to engage with broader repertories of New Spanish music moving forward.

Abstract: An angular analysis of the rare decay$$ {B}_s^0 $$Bs0→ ϕμ+μ−is presented, using proton-proton collision data collected by the LHCb experiment at centre-of-mass energies of 7, 8 and 13 TeV, corresponding to an integrated luminosity of 8.4 fb−1. The observables describing the angular distributions of the decay$$ {B}_s^0 $$Bs0→ ϕμ+μ−are determined in regions ofq2, the square of the dimuon invariant mass. The results are consistent with Standard Model predictions.

Abstract: This article presents differential measurements of the asymmetry between $$ {\varLambda}_b^0 $$ Λ b 0 and $$ {\overline{\varLambda}}_b^0 $$ Λ ¯ b 0 baryon production rates in proton-proton collisions at centre-of-mass energies of $$ \sqrt{s} $$ s = 7 and 8 TeV collected with the LHCb experiment, corresponding to an integrated luminosity of 3 fb−1. The $$ {\varLambda}_b^0 $$ Λ b 0 baryons are reconstructed through the inclusive semileptonic decay $$ {\varLambda}_b^0 $$ Λ b 0 → $$ {\varLambda}_c^{+} $$ Λ c + μ−$$ \overline{\nu} $$ ν ¯ μX. The production asymmetry is measured both in intervals of rapidity in the range 2.15 < y < 4.10 and transverse momentum in 2 < pT< 27 GeV/c. The results are found to be incompatible with symmetric production with a significance of 5.8 standard deviations for both $$ \sqrt{s} $$ s = 7 and 8 TeV data, assuming no CP violation in the decay. There is evidence for a trend as a function of rapidity with a significance of 4 standard deviations. Comparisons to predictions from hadronisation models in Pythia and heavy-quark recombination are provided. This result constitutes the first observation of a particle-antiparticle asymmetry in b-hadron production at LHC energies.

Abstract: We present the case of a man in his 60s with a transtibial amputation (TTA) undergoing total knee replacement (TKR) for symptomatic osteoarthritis (OA). It is unusual to develop OA in the ipsilateral knee to TTA; and while it is postulated that this is because patients preferentially load their unaffected limb to protect the TTA-sided knee, there is also the ability to offload specific knee compartments through prosthetic adjustment. When planning TKR in such patients, it is important to consider several technical challenges in order to prevent a poor outcome. The literature is sparse with evidence to guide decision-making, and this case report and literature review aims to summarise our preoperative planning and intraoperative technique, which ultimately resulted in a good outcome. Abstract: Music and dance were key elements of culture in the Ibero-American borderlands that served to reflect, shape, and express cultural and political realities. From colonizers’ perspectives, proficiency in Western musical practices equaled acculturation to Catholic norms and civilized, sedentary society. At the same time, performances of music and dance articulated changing native identities. There was no single borderlands music or dance, and evidence in the form of music manuscripts, eyewitness accounts, and musical instruments remains fragmented and scattered among periods and locations. Practices such as liturgical chant, bell ringing, religious songs, communal dance, orchestral music, and other musical arts formed integral parts of mission and urban life in the borderlands throughout the colonial period. Whereas the interior church spaces such as Durango Cathedral, were dominated by performative music genres, music making in rural and urban exterior spaces included participatory activities that produced diverse ambient soundscapes.

Abstract: In January 1761, the Royal and Pontifical University of Mexico held a poetry contest in honor of Charles III. Part of this celebration included the performance of an ode for voices and orchestra with music by Ignacio Jerusalem, the Italian-born chapel master of Mexico City Cathedral. This essay considers the words and music of Al combate, Jerusalem’s ode, in the context of international Enlightenment music genres as practiced in the Spanish world. Al combate emerges as an erudite artwork in which the Mexico City elite imagine themselves as Grecian muses in a politicized allegory that blends traditional and modern aesthetics.

Abstract: Introduction Autoinflammatory conditions can arise in patients presenting on the medical take. Patients presenting with an inflammatory response, fever and organ dysfunction are usually (and appropriately) managed as sepsis until proven otherwise. However, a suspicion should be maintained for alternative diagnoses when there is no improvement with conventional antimicrobial therapy. We present a case of a young woman presenting with polyserositis with a wide differential diagnosis including infection, autoimmunity and malignancy. She was treated successfully with colchicine. Subsequent genetic analysis at the Royal Free hospital for fever syndromes has not found a pre-described genetic polymorphism. Case description A 39-year-old woman presented to A&E with dyspnoea, night sweats, lethargy and right upper quadrant pain. Her past medical history included hypothyroidism and two years of intermittent diarrhoea. This diarrhoea was associated with a raised faecal calprotectin. Subsequent investigation with colonoscopy and MRI small bowel were normal. Other past history included pleurisy felt to be secondary to a lower respiratory tract infection aged 38 and a right knee effusion aged 15. She was born in the UK but had a Greek mother. Examination revealed reduced breath sounds in the bases which were dull to percuss, normal heart sounds and mild abdominal distension. There was no peripheral oedema. Observations showed oxygen saturations of 90% on air, BP 90/50, HR 120 and a pyrexia of 38°C. Blood results showed a raised ALT of 100 and CRP 40. She was initially treated with antibiotics for a working diagnosis of atypical pneumonia. Her respiratory failure deteriorated and a CTPA two days later showed bilateral pleural effusions, a massive pericardial effusion with impending tamponade and a right upper lobe PE. A CT CAP show moderate ascites but no evidence of malignancy or lymphoma. Pericardiocentesis showed a reactive picture with no organisms grown and normal cytopathology of the fluid. Pleural and ascitic fluid were also drained and she was commenced on LMWH for the PE. Virology, microbiological and autoimmune screens were unremarkable. CRP at this stage had risen to 78 as had the ALT to 1400. 6 days after admission, antibiotics were stopped she was commenced on colchicine 1mg bd. Within 48 hours she rapidly improved both clinically and biochemically. Discussion Since the index admission genetic analysis has shown she is HLA B51 negative and she has no recognised genetic polymorphism associated with a periodic fever syndrome. 20 months since admission she has made a good recovery and is stable on colchicine 1mg od with no recurrence of the pleural and pericardial effusions. She has had one further episode of diarrhoea for 2 weeks associated with a raised faecal calprotectin and red macular rashes on her torso. Anticoagulation for the PE has been stopped. Polyserositis in the absence of infection or malignancy is uncommon and autoinflammatory and autoimmune aetiologies should be considered. This case highlights the importance of keeping a broad mind in patients presenting in an unusual way or when they do not respond to initial treatment. Autoinflammatory conditions are characterised by marked inflammation affecting the skin, serosal surfaces and synovium among others. Her history of colitis is intriguing. There are case reports of patients with familial Mediterranean fever presenting with serositis but she was negative for FMF on further genetic testing. Similarly she did not prove to have any of the genes that have so far been recognised as causing a periodic fever syndrome. We did consider Behçet’s but she denied a history of recurrent oral or genital ulceration or iritis and there was no family history. However Behçet’s can lead to inflammatory bowel disease and this remains an intriguing possibility. The dramatic response to colchicine was remarkable. Within 48 hours a re-accumulated pericardial effusion causing right ventricular strain had disappeared as had clinically apparent ascites. Most autoimmune, infective or malignant aetiologies would not be expected to respond in such a way and a yet undiscovered autoinflammatory syndrome remains a tantalising possibility. Key learning points Main learning points so far: Maintain a wide differential in patients presenting with polyserositis, especially when there is a poor response to typical management approaches. Consider autoinflammatory disease and take a detailed past medical and family history. Colchicine is cheap and generally well tolerated at lower doses. Colchicine is first line therapy for recurrent pericarditis. Learning points from the conference: What’s her risk of developing secondary amyloid or other secondary sequelae? How long will she need treatment for and how/ when you would you wean the colchicine? Is her partial Greek genetic heritage relevant? How common are wild type genetic polymorphisms occurring in autoinflammatory conditions? How frequently is colitis a feature of autoinflammatory diseases? Abstract: The increasing demand for water and energy in South Africa, and the capacity constraints and restrictions of both resources, have led to a rapid increase in their cost. The manufacturing industry remains South Africa’s third-largest consumer of water and second- largest consumer of national energy. The improvement of water and energy efficiency is becoming an increasingly important theme for both organisational success and national economic sustainability. This paper presents the ‘lean based water and energy stream mapping framework’ developed for the manufacturing industry, with the specific objective of decreasing its water and energy intensity. As with the traditional value stream mapping tool, the water and energy stream mapping focuses on eliminating water- and energy-specific wastes within a process. Water and energy waste categories that will be used in conjunction with the framework will also be discussed. The key objective of this paper is to detail the process of creating the water and energy stream mapping, and the statistical forecasting methodology used to develop the baseline water and energy demand data. The outcome of the implementation of the framework is the future state water and energy stream mapping, which is effectively a blueprint for increased water and energy efficiency within a studied process.

Abstract: Feminism and transgender, as social factions or collective subjectivities, have historically evaded, vilified or negated each other's philosophy and subjectivities. In particular, separatist feminist theorists have portrayed the two ‘sides'as consisting of mutually incompatible aims and subjectivities. These portrayals have worked to the detriment of both feminism and transgender. Third Wave Feminism and Transgender considers what positive outcomes on society in general, and the law as it pertains to gender in particular, may emerge from the identification of and cooperation between third wave feminism and transgender. Abstract: We report a silicon MEMS optical accelerometer based on the Fabry–Perot interferometer (FPI) principle in which the displacement of the proof mass is mechanically amplified by a V-beam structure prior to transduction. Mechanical amplification allows the sensitivity obtained with a given readout system to be increased without compromising the sensor bandwidth. The FPI cavity in our device is formed between a mirror situated on the V-beam and reflections from the end surface of a cleaved optical fibre. Simple analytical expressions have been derived for the amplification factor of the V-beam structure, in terms of its geometrical parameters, and for its mechanical stiffness which affects the resonant frequency. These were used to design a series of five accelerometers with different mechanical amplification factors which were fabricated and tested. A device having a V-beam angle of 1.9° was capable of detecting accelerations over a dynamic range of 103 between 0.01 g rms and 10 g rms, while a 1.33°angled device achieved the largest amplification of 18.6 ± 6.4.

Abstract: We present the development of superstructure fiber gratings (SFG) in Ge-doped, silica optical fiber using femtosecond laser inscription. We apply a simple but extremely effective single step process to inscribe low loss, sampled gratings with minor polarization dependence. The method results in a controlled modulated index change with complete suppression of mode coupling associated with the overlapping LPG structure leading to highly symmetric superstructure spectra, with the grating reflection well within the Fourier design limit. The devices are characterized and compared with numerical modeling by solving Maxwell's equations and calculating the back reflection spectrum using the bidirectional beam propagation method (BiBPM). Experimental results validate our numerical analysis, allowing for the estimation of inscription parameters such as the ac index modulation change, and the wavelength, position and relative strength of each significant resonance peak. We also present results on temperature and refractive index measurements showing potential for sensing applications.

In terms of visual appeal, no source from late-colonial Latin America rivals the Codice Trujillo del Peru, a nine-volume encyclopedic work compiled between 1782 and 1785. Conserved at the Real Biblioteca del Palacio Real in Madrid and in partial copies at libraries in Colombia and Peru, this lavish work depicts the natural world as well as the fabric of colonial society through nearly 1,400 anonymous watercolor images, with little corresponding text. Known also as the Codice Martinez Companon, it was commissioned by Baltasar Jaime Martinez Companon (1737-1797), a highly-educated religious authority of peninsular Spanish origin, who remitted it to King Charles IV of Spain following fieldwork undertaken while serving as Bishop of Trujillo. An exemplar of the Enlightenment prelate, Martinez Companon advocated for the affairs of indigenous peoples, promoted education throughout his diocese, and supported other relatively progressive projects within the framework of the late-colonial Bourbon reforms. Whereas volumes 3 through 8 of the codex document the plant life, animals, birds, and fish of Peru, volumes 1, 2, and 9 concentrate on the viceroyalty's cultures. The images encompass professionally-drawn maps, portraits of colonial officials, scenes of everyday life at all social levels, and archeological ruins. Amazingly, twenty pieces of dance music also appear in the codex between folios 176 and 194 of the second volume, within a section that contains illustrations of dancers and performing musicians in various settings. Neatly written in score format, seventeen of the pieces are songs or sung dances and three of them are instrumental. Most of the scores contain music for violin, a bass instrument, and up to four voices, which are essentially tunes with simple accompaniments in standard tonal harmony. One of the pieces (number 6, Tonada de el Chimo) contains unpitched notation for percussion, and one other (number 5, Baile de danzantes con pifano y tamboril) seems to give notation for percussion, although the line is labeled "Bass." Comprising three genres, bailes (dances), tonadas (Spanish lyric song of the late eighteenth century), and cachuas or qhachwas (historical duple-meter round dances of Peru), the pieces often feature syncopated rhythms, some of which are local in origin (Diana Fernandez Calvo, "La musica en el codice del obispo Baltasar Jaime Martinez Companon" in El obispo Martinez Companon: Vida y obra de un Navarro ilustrado en America, ed. Since the musical notation is descriptive rather than prescriptive, and succinct in form, a certain amount of improvisation would be expected if performing from the scores, including embellishing the melodic lines, adding or changing instruments, and repeating musical sections. It is argued that for polemical reasons the General Scholium, written twenty-six years after the first edition, substantially misrepresented the methodology of the main body of the text. The article discusses papers of Wallis, Wren and Huygens that use the third law of motion as set out by Newton in Book 1. It also argues that Newton's use of induction is quite different from and subtler than the ‘logical’ and ‘probabilistic’ notions of induction discussed and then rejected by a number of twentieth-century philosophers.

Abstract: Using finite element analysis (FEA), a model has been constructed to predict the thermo-fluidic and optical properties of a microstructure optical fibre (MOF). The properties under study include external temperature, input water velocity and optical fibre geometry. Under laminar flow the steady-state temperature is dependent on the water channel radius while independent of the input velocity. A critical channel radius is observed below which the steady-state temperature of the water channel is constant, while above, the temperature decreases. The MOF has been found capable of supporting multiple modes whose response to temperature was dominated by the thermo-optic coefficient of glass, despite the larger thermo-optic coefficient of water. This is attributed to the majority of the light being confined within the glass, which increased with increasing external temperature due to a larger difference in the refractive index between the glass core and the water channel.

Abstract: It is expected that 40% to 60% of initial alveolar bone volume will be lost up to 6 months after tooth extraction. OsteoScaf(TM) (TRT, Toronto, ON, Canada) (poly (DL-lactide-co-glycololide/calcium phosphate [PLGA/CaP] scaffold) is a novel bone substitute material and represents a promising alternative for maintaining alveolar bone integrity in this clinical scenario.Here it was hypothesized that OsteoScaf would reduce alveolar bone lost after tooth extraction in patient, acting as a clot-retention device.A total of 10 patients (32 sockets) were included in the study, of which 16 sockets were grafted with OsteoScaf and 16 were used as control (coagulum alone). Cone beam computed tomography (CBCT) was performed both immediately following extraction and also at 120 days postoperatively, at which time biopsy samples were also harvested for histological analyses.Quantitative analysis of CBCT showed less bone resorption in the OsteoScaf groups, being 10.5% to 14.4% less bone lost in the center of the socket, 15.4% in the buccal region, and 12.6% in the palatal. Qualitative histological analysis showed new bone tissue in direct apposition to the scaffold - demonstrating its osteoconductive nature.OsteoScaf diminished the expected bone lost during the postextraction remodeling of the alveolar bone ridge at 120 days postextraction.

Abstract: We describe the spectrum of a non-self-adjoint elliptic system on a finite interval. Under certain conditions we find that the eigenvalues form a discrete set and converge asymptotically at infinity to one of several straight lines. The eigenfunctions need not generate a basis of the relevant Hilbert space, and the larger eigenvalues are extremely sensitive to small perturbations of the operator. We show that the leading term in the spectral asymptotics is closely related to a certain convex polygon, and that the spectrum does not determine the operator up to similarity. Two elliptic systems which only differ in their boundary conditions may have entirely different spectral asymptotics. While our study makes no claim to generality, the results obtained will have to be incorporated into any future general theory.
