Resumo Fundamento Ainda não temos informações acerca do impacto da pandemia da COVID-19 sobre a atividade médica assistencial no Brasil. Objetivo Descrever as repercussões da pandemia da COVID-19 na rotina de atendimentos em um hospital terciário, referência regional em cardiologia e oncologia. Métodos Estudo de corte transversal. Foi realizado levantamento dos atendimentos no período de 23/03/2020 (fechamento do comércio local) até 23/04/2020 (P20) e comparado com o mesmo período em 2019 (P19).Resultados: Detectamos redução no número de consultas cardiológicas, teste ergométrico, Holter, monitorização ambulatorial da pressão arterial, eletrocardiograma e ecocardiograma (90%, 84%, 94%, 92%, 94% e 81%, respectivamente). Em relação à cirurgia cardíaca e cateterismo cardíaco, houve redução de 48% e 60%, respectivamente. Aumento no número de angioplastia transluminal coronária (33%) e de implante de marca-passo definitivo (29%). Houve 97 internamentos na UTI em P19, contra 78 em P20, redução de 20%. Diminuição dos atendimentos no pronto-socorro cardiológico (45%) e nos internamentos na enfermaria de cardiologia (36%). Houve diminuição nas consultas oncológicas de 30%. Sessões de quimioterapia reduziram de 1.944 para 1.066 (45%). Sessões de radioterapia diminuíram 19%. Conclusão A COVID-19 provocou redução considerável no número de consultas nos ambulatórios de cardiologia, oncologia e demais especialidades. Houve uma preocupante diminuição no número de cirurgias cardíacas e nas sessões de quimioterapia e radioterapia nas semanas iniciais da pandemia. A procura por atendimento no pronto-socorro cardiológico, assim como as internações na UTI e enfermaria cardiológicas, também reduziram, gerando preocupação acerca da evolução e prognóstico destes pacientes portadores de outras patologias, que não a COVID-19, nestes tempos de pandemia. OBJECTIVES: This observational, cross-sectional study based aimed to test whether heart failure (HF)-disease management program (DMP) components are influencing care and clinical decision-making in Brazil. METHODS: The survey respondents were cardiologists recommended by experts in the field and invited to participate in the survey via printed form or email. The survey consisted of 29 questions addressing site demographics, public versus private infrastructure, HF baseline data of patients, clinical management of HF, performance indicators, and perceptions about HF treatment. RESULTS: Data were obtained from 98 centers (58% public and 42% private practice) distributed across Brazil. Public HF-DMPs compared to private HF-DMP were associated with a higher percentage of HF-DMP-dedicated services (79% vs 24%; OR: 12, 95% CI: 94-34), multidisciplinary HF (MHF)-DMP [84% vs 65%; OR: 3; 95% CI: 1-8), HF educational programs (49% vs 18%; OR: 4; 95% CI: 1-2), written instructions before hospital discharge (83% vs 76%; OR: 1; 95% CI: 0-5), rehabilitation (69% vs 39%; OR: 3; 95% CI: 1-9), monitoring (44% vs 29%; OR: 2; 95% CI: 1-5), guideline-directed medical therapy-HF use (94% vs 85%; OR: 3; 95% CI: 0-15), and less B-type natriuretic peptide (BNP) dosage (73% vs 88%; OR: 3; 95% CI: 1-9), and key performance indicators (37% vs 60%; OR: 3; 95% CI: 1-7). In comparison to non- MHF-DMP, MHF-DMP was associated with more educational initiatives (42% vs 6%; OR: 12; 95% CI: 1-97), written instructions (83% vs 68%; OR: 2: 95% CI: 1-7), rehabilitation (69% vs 17%; OR: 11; 95% CI: 3-44), monitoring (47% vs 6%; OR: 14; 95% CI: 2-115), GDMT-HF (92% vs 83%; OR: 3; 95% CI: 0-15). In addition, there were less use of BNP as a biomarker (70% vs 84%; OR: 2; 95% CI: 1-8) and key performance indicators (35% vs 51%; OR: 2; 95% CI: 91,6) in the non-MHF group. Physicians considered changing or introducing new medications mostly when patients were hospitalized or when observing worsening disease and/or symptoms. Adherence to drug treatment and non-drug treatment factors were the greatest medical problems associated with HF treatment. CONCLUSION: HF-DMPs are highly heterogeneous. New strategies for HF care should consider the present study highlights and clinical decision-making processes to improve HF patient care.

In relation to cardiac surgery and cardiac catheterization, there were 48% and 60% decreases, respectively. There was an increase in the number of percutaneous transluminal coronary angioplasties (33%) and definitive pacemaker implantations (29%). There were 97 admissions to the ICU during P19, in contrast with 78 during P20, a 20% decrease. Visits to the cardiac emergency room (45%) and admissions to the cardiology ward (36%) also decreased. The decrease in oncology consultations was 30%. Chemotherapy sessions decreased from 1,944 to 1,066 (45%), and radiotherapy sessions decreased by 19%. Conclusion COVID-19 has led to a considerable decrease in the number of consultations in outpatient clinics for cardiology, oncology, and other specialties. There was a concerning decrease in the number of cardiac surgeries, chemotherapy sessions, and radiotherapy sessions during the initial weeks of the pandemic. The number of people seeking care in the cardiac emergency room and the number of admissions to the cardiology ward and ICU also decreased, generating concern regarding the evolution and prognosis of these patients with pathologies other than COVID-19 during this pandemic time. IntroductionRecently, we demonstrated that the polynomial interpolation method can be used to accurately calculate the daily acceleration of cases and deaths by Covid-19. The acceleration of new cases is important for the characterization and comparison of epidemic curves. The objective of this work is to measure the diversity of epidemic curves and understand the importance of socioeconomic variables in the acceleration, peak cases and deaths by Covid-19 in Brazilian states.MethodsThis is an ecological study with time series analysis of new cases and deaths by Covid-19 in Brazil and its 27 federation units. Using the polynomial interpolation method, we calculated the daily cases and deaths with the measurement of the respective acceleration. We calculated the correlation coefficient between the epidemic curve data and socioeconomic data.ResultsThe combination of daily data and acceleration determined that the states of Brazil are in different stages of the epidemic. Maximum acceleration of peak cases, peak of cases, maximum acceleration of deaths and peak of deaths are associated with the Gini index and population density, but did not correlate with HDI and per capita income.ConclusionBrazilian states showed heterogeneous data curves. Density population and socioeconomic inequality are associated with worse control of the epidemic.

INTRODUCTION: The acceleration of new cases is important for the characterization and comparison of epidemic curves. The objective of this study was to quantify the acceleration of daily confirmed cases and death curves using the polynomial interpolation method. METHODS: Covid-19 epidemic curves from Brazil, Germany, the United States, and Russia were obtained. We calculated the instantaneous acceleration of the curve using the first derivative of the representative polynomial. RESULTS: The acceleration for all curves was obtained. CONCLUSIONS: Incorporating acceleration into an analysis of the Covid-19 time series may enable a better understanding of the epidemiological situation.

e12617 Background: The combination of doxorubicin (DOX) with paclitaxel (PTX) is effective in the treatment of breast cancer (BC). However, DOX-associated cardiotoxicity (CTX) is aggravated by the use of PTX. There´s no consensus on which sequence is most safe. Methods: Prospective study of women with primary BC. All of participants received four cycles of DOX and 12 infusions of PTX. Participants were divided into two groups, at the discretion of the oncologist: Group 1-PTX before DOX and Group 2-DOX before PTX. Background: Heart failure is a severe complication associated with doxorubicin (DOX) use. Strain, assessed by two-dimensional speckle tracking (2D-STE), has been shown to be useful in identifying subclinical ventricular dysfunction. Objectives: a) To investigate the role of strain in the identification of subclinical ventricular dysfunction in patients who used DOX; b) to investigate determinants of strain response in these patients. Methods: Cross-sectional study with 81 participants: 40 patients who used DOX ±2 years before the study and 41 controls. All participants had left ventricular ejection fraction (LVEF) ≥55%. Total dose of DOX was 396mg (242mg/ms2). The systolic function of the LV was evaluated by LVEF (Simpson), as well as by longitudinal (εLL), circumferential (εCC), and radial (εRR) strains. Multivariate linear regression (MLR) analysis was performed using εLL (model 1) and εCC (model 2) as dependent variables. Results: Systolic and diastolic blood pressure values were higher in the control group (p < 0.05). εLL was lower in the DOX group (-12.4 ±2.6%) versus controls (-13.4 ± 1.7%; p = 0.044). The same occurred with εCC: -12.1 ± 2.7% (DOX) versus -16.7 ± 3.6% (controls; p < 0.001). The S’ wave was shorter in the DOX group (p = 0.035). On MLR, DOX was an independent predictor of reduced εCC (B = -4.429, p < 0.001). DOX (B = -1.289, p = 0.012) and age (B = -0.057, p = 0.029) were independent markers of reduced εLL. Conclusion: a) εLL, εCC and the S’ wave are reduced in patients who used DOX ±2 years prior to the study despite normal LVEF, suggesting the presence of subclinical ventricular dysfunction; b) DOX was an independent predictor of reduced εCC; c) prior use of DOX and age were independent markers of reduced εLL. Fundamento: A insuficiência cardíaca é uma complicação grave associada ao uso da doxorrubicina (DOX). O strain, avaliado por speckle tracking bidimensional (2D-STE), tem se mostrado útil na identificação de disfunção ventricular subclínica. Objetivos: a) Investigar o comportamento do strain na identificação de disfunção ventricular subclínica em pacientes que usaram DOX; b) investigar determinantes do comportamento do strain nestes pacientes. Métodos: Estudo transversal com 81 participantes: 40 pacientes que usaram DOX ± 2 anos antes do estudo e 41 controles. Todos tinham fração de ejeção do ventrículo esquerdo (FEVE) ≥ 55%. A dose total de DOX foi de 396 mg (242 mg/m2). A função sistólica do VE foi avaliada pela FEVE (Simpson), assim como pelo strain longitudinal (εLL), circunferencial (εCC) e radial (εRR). Realizamos análise de regressão linear multivariada (RLM) usando como variáveis dependentes o εLL (modelo 1) e o εCC (modelo 2). Resultados: Os valores da pressão arterial sistólica e diastólica foram maiores no grupo controle (p < 0,05). O εLL foi menor no grupo DOX (-12,4 ± 2,6%) versus controle (-13,4 ± 1,7%; p = 0,044). O mesmo ocorreu em relação ao εCC: -12,1 ± 2,7% (DOX) versus -16,7 ± 3,6% (controles; p < 0,001). A onda S' foi menor no grupo DOX (p = 0,035). Na RLM, a DOX foi preditora independente de redução do εCC (B = -4,429, p < 0,001). DOX (B = -1,289, p = 0,012) e idade (B = -0,057, p = 0,029) foram marcadores independentes de redução do εLL. Conclusões: a) O εLL, o εCC e a onda S' estão reduzidos nos pacientes que usaram DOX ±2 anos antes do estudo, apesar da FEVE ser normal, sugerindo presença de disfunção ventricular subclínica; b) a DOX foi preditora independente de redução do εCC; c) o uso prévio da DOX e a idade foram marcadores independentes de redução do εLL.

The latest Heart Failure Guidelines by the Department of Heart Failure of the Brazilian Society of Cardiology (DEIC/SBC) were finalized on March 2018. Since then, a significant number of therapeutic interventions and diagnostic approaches has arisen or consolidated their position in international clinical practice and in clinical research. In addition, the COVID-19 pandemic has taught us much about the pathophysiological model of myocardial damage and raised many questions about the continuity and safety of medication use in patients with chronic HF suffering from an acute manifestation of this new and complex clinical entity. In the last few months, we have been working quickly and collaboratively, and for the first time in 20 years, DEIC used digital platforms to discuss, deliberate, and draft this important document, opting for a focused update instead of a full-text guideline. We were inspired by the 2020 Canadian Heart Failure Guidelines,1 but had the benefit of observing the impact on clinical practice and the consolidation of this new knowledge, in addition to new results from clinical trials published over the last 12 months. In order to report on these developments, we hosted a pioneering scientific conference on September 19, 2020, the I Heart Failure Summit Brazil 2020 (digital), with approximately 900 participants, many of them DEIC associates. The leadership of the Science Board was key in organizing the various working groups and developing a secure and practical method for discussions and votes. With social distancing and the use of digital technology, the conference enabled wide-ranging debates from various perspectives, based on the best available scientific evidence. In this document, DEIC/SBC provides reviews and detailed updates to its Chronic Heart Failure Guidelines. The work started in July 2020, with the choice of the Editorial Board, which established priorities, divided the 52 participants into working groups, and developed a schedule of activities. These working groups, each consisting of five to seven participants, began intense online discussions that led to the elaboration of preliminary tables, widely circulated before their subsequent review by the 11-member Review Board. The final discussions took place during a virtual plenary session on December 4, 2020, with all collaborators, who had the opportunity to vote on the main recommendations. Decisions regarding classes of recommendation required a three-quarters supermajority vote. Class of Recommendation and Level of Evidence follow the same definitions used in the last guideline, as established by SBC/CONDir. See below. The therapeutic recommendations proposed in this document are based on the latest available scientific evidence, considering not only the aspects of clinical efficacy from large clinical trials. We have sought to summarize the primary recommendations in flowcharts and algorithms that are easy to understand and to apply in clinical practice, proposing approaches for the diagnosis and treatment of heart failure. Our commitment to the scientific community, linked to research and assistance to heart failure patients, public and private managers, and policy-makers, will certainly have the benefit of a document that sought to present scientific interventions in an accessible format, facilitating its implementation in the various spheres where heart failure patients receive care. Classes of Recommendation Class I Conditions for which there is conclusive evidence or, if not, there is general agreement that the procedure is safe and useful/effective. Class II Conditions for which there is conflicting evidence and/or divergence of opinion about the safety and usefulness/efficacy of the procedure. Class IIA Weight or evidence/opinion in favor of the intervention. Most approve. Class IIB Less well-established safety and usefulness/efficacy, without predominance. Class III Conditions for which there is evidence and/or general agreement that the procedure is not useful/effective, and in some cases may be harmful. Levels of Evidence Level A Data derived from multiple consistent randomized controlled clinical trials, and/or a robust meta-analysis of randomized clinical trials. Level B Data derived from a less robust meta-analysis, one single randomized trial or non-randomized trials (observational). Level C Data derived from consensual expert opinion.
