Abstract: ABSTRACT BACKGROUND: Laparoscopic Nissen fundoplication fails to control the gastroesophageal reflux in almost 15% of patients, and most of them must be reoperated due to postoperative symptoms. Different surgical options have been suggested. AIMS: This study aimed to present the postoperative outcomes of patients submitted to three different procedures: redo laparoscopic Nissen fundoplication alone (Group A), redo laparoscopic Nissen fundoplication combined with distal gastrectomy (Group B), or conversion to laparoscopic Toupet combined with distal gastrectomy with Roux-en-Y gastrojejunostomy (Group C). METHODS: This is a prospective study involving 77 patients who were submitted initially to laparoscopic Nissen fundoplication and presented recurrence of gastroesophageal reflux after the operation. They were evaluated before and after the reoperation with clinical questionnaire and objective functional studies. After reestablishing the anatomy of the esophagogastric junction, a surgery was performed. None of the patients were lost during follow-up. RESULTS: Persistent symptoms were observed more frequently in Group A or B patients, including wrap stricture, intrathoracic wrap, or twisted fundoplication. In Group C, recurrent symptoms associated with this anatomic alteration were infrequently observed. Incompetent lower esophageal sphincter was confirmed in 57.7% of patients included in Group A, compared to 17.2% after Nissen and distal gastrectomy and 26% after Toupet procedure plus distal gastrectomy. In Group C, despite the high percentage of patients with incompetent lower esophageal sphincter, 8.7% had abnormal acid reflux after surgery. CONCLUSIONS: Nissen and Toupet procedures combined with Roux-en-Y distal gastrectomy are safe and effective for the management of failed Nissen fundoplication. However, Toupet technique is preferable for patients suffering from mainly dysphagia and pain.

2. High Plasma Levels of Fibroblast Growth Factor 23 Are Associated with Increased Risk of COVID-19 in End-Stage Renal Disease Patients on Hemodialysis: Results of a Prospective Cohort

Abstract: End-stage renal disease (ESRD) patients are a population with high rates of COVID-19 and mortality. These patients present a low response to anti-SARS-CoV-2 immunization, which is associated with immune dysfunction. ESRD patients also present high plasma titers of Fibroblast Growth Factor 23 (FGF23), a protein hormone that reduces immune response in vivo and in vitro. Increased FGF23 levels associate with higher infection-related hospitalizations and adverse infectious outcomes. Thus, we evaluated whether ESRD patients with high FGF23 titers have an increased rate of SARS-CoV-2 infection. Methods: We performed a prospective cohort of ESRD patients in hemodialysis who had measurements of plasma intact FGF23 in 2019. We determined COVID-19 infections, hospitalizations, and mortality between January 2020 and December 2021. Results: We evaluated 243 patients. Age: 60.4 ± 10.8 years. Female: 120 (49.3%), diabetes: 110 (45.2%). During follow-up, 45 patients developed COVID-19 (18.5%), 35 patients were hospitalized, and 12 patients died (mortality rate: 26.6%). We found that patients with higher FGF23 levels (defined as equal or above median) had a higher rate of SARS-CoV-2 infection versus those with lower levels (18.8% versus 9.9%; Hazard ratio: 1.92 [1.03–3.56], p = 0.039). Multivariate analysis showed that increased plasma FGF23 was independently associated with SARS-CoV-2 infection and severe COVID-19. Discussion: Our results suggest that high plasma FGF23 levels are a risk factor for developing COVID-19 in ESRD patients. These data support the potential immunosuppressive effects of high circulating FGF23 as a factor implicated in the association with worse clinical outcomes. Further data are needed to confirm this hypothesis.

Abstract: Heart failure with preserved ejection fraction (HFpEF) is a growing public health problem in nearly 50% of patients with heart failure. Therefore, research on new strategies for its diagnosis and management has become imperative in recent years. Few drugs have successfully improved clinical outcomes in this population. Therefore, numerous attempts are being made to find new pharmacological interventions that target the main mechanisms responsible for this disease. In recent years, pathological mechanisms such as cardiac fibrosis and inflammation, alterations in calcium handling, NO pathway disturbance, and neurohumoral or mechanic impairment have been evaluated as new pharmacological targets showing promising results in preliminary studies. This review aims to analyze the new strategies and mechanical devices, along with their initial results in pre-clinical and different phases of ongoing clinical trials for HFpEF patients. Understanding new mechanisms to generate interventions will allow us to create methods to prevent the adverse outcomes of this silent pandemic.

4. Humoral Immune Response of BNT162b2 and CoronaVac Vaccinations in Hemodialysis Patients: A Multicenter Prospective Cohort

Abstract: The CoronaVac vaccine is the most used anti-SARS-CoV-2 vaccine worldwide. Previous data indicate that this vaccine produces a lower immune response than RNA vaccines such as BNT162b2. End-stage renal disease (ESRD) patients have an increased rate of COVID-19 and a reduced immune response to vaccinations. Currently, there is little data on this population’s immune response induced by CoronaVac. Methods: This study involved a prospective cohort of ESRD patients in chronic hemodialysis who received a two-dose immunization scheme of either CoronaVac (Sinovac Biotech) or BNT162b2 vaccines (Pfizer-BioNTech). We measured the plasma levels of anti-SARS-CoV-2 IgG antibodies. We determined antibody titers before immunization, 2 and 4 months after two doses, plus 4 months after a booster dose. Results: We evaluated 208 patients in three hemodialysis centers. The mean age was 62.6 ± 15.6 years, of whom 91 were female (41.75%). Eighty-one patients (38.94%) received the BNT162b2 vaccine and 127 (61.06%) received the CoronaVac vaccine. Patients who received the BNT162b2 vaccine had a higher humoral response compared to those who received the CoronaVac vaccine (4 months after the second dose: BNT162b2: 88.89%, CoronaVac: 51.97%, p < 0.001; 4 months after the booster: BNT162b2: 98.77%, CoronaVac: 86.61%, p < 0.001). Conclusions: Our results suggest that the CoronaVac vaccine induced a lower humoral response than the BNT162b2 vaccine in ESRD patients on hemodialysis.

Abstract: Objetivo: Determinar cómo las exportaciones de commodities inciden en los ingresos tributarios del Gobierno Central y el tipo de relación que existe entre de los impuestos directos e indirectos. Método: Se aplicó el método descriptivo correlacional, estructurándose una matriz de correlación y líneas de regresión, para ello se empleó datos oficiales de la SUNAT, INEI, BCRP. Resultados: En el estudio se evidencia el mayor aporte del IGV en la recaudación tributaria, seguido del impuesto a la renta, pero por el grado de correlación alcanzado este último impuesto tiene mayor incidencia en la recaudación fiscal. Sin embargo, al contrastar las exportaciones mineras con los impuestos directos se aprecia una débil, pero negativa relación de dependencia lo cual implica que la recaudación tributaria es débilmente afectada por las exportaciones de minerales. Conclusiones: Se concluye, por el grado y tipo de correlación obtenido, que el aumento de las exportaciones mineras no ejerce mayores cambios en la recaudación tributaria siendo además el impacto de tipo inverso.

Abstract: Aim: Amazonia hosts more tree species from numerous evolutionary lineages, both young and ancient, than any other biogeographic region. Previous studies have shown that tree lineages colonized multiple edaphic environments and dispersed widely across Amazonia, leading to a hypothesis, which we test, that lineages should not be strongly associated with either geographic regions or edaphic forest types. Location: Amazonia. Taxon: Angiosperms (Magnoliids; Monocots; Eudicots). Methods: Data for the abundance of 5082 tree species in 1989 plots were combined with a mega-phylogeny. We applied evolutionary ordination to assess how phylogenetic composition varies across Amazonia. We used variation partitioning and Moran's eigenvector maps (MEM) to test and quantify the separate and joint contributions of spatial and environmental variables to explain the phylogenetic composition of plots. We tested the indicator value of lineages for geographic regions and edaphic forest types and mapped associations onto the phylogeny. Results: In the terra firme and várzea forest types, the phylogenetic composition varies by geographic region, but the igapó and white-sand forest types retain a unique evolutionary signature regardless of region. Overall, we find that soil chemistry, climate and topography explain 24% of the variation in phylogenetic composition, with 79% of that variation being spatially structured (R2 = 19% overall for combined spatial/environmental effects). The phylogenetic composition also shows substantial spatial patterns not related to the environmental variables we quantified (R2 = 28%). A greater number of lineages were significant indicators of geographic regions than forest types. Main Conclusion: Numerous tree lineages, including some ancient ones (>66 Ma), show strong associations with geographic regions and edaphic forest types of Amazonia. We used variation partitioning and Moran’s eigenvector maps (MEM) to test and quantify the separate and joint contributions of spatial and environmental variables to explain the phylogenetic composition of plots. We tested the indicator value of lineages for geographic regions and edaphic forest types and mapped associations onto the phylogeny. Results: In the terra firme and várzea forest types, phylogenetic composition varies by geographic region, but the igapó and white-sand forest types retain a unique evolutionary signature regardless of region. Overall, we find that soil chemistry, climate, and topography explain 24% of the variation in phylogenetic composition, with 79% of that variation being spatially structured (R 2 = 19% overall for combined spatial/environmental effects). Phylogenetic composition also shows substantial spatial patterns not related to the environmental variables we quantified (R 2 = 28%). A greater number of lineages were significant indicators of geographic regions than forest types. Main conclusions: Numerous tree lineages, including some ancient ones (>66 Ma), show strong associations with geographic regions and edaphic forest types of Amazonia. Una mezcla se define como un material formado por dos o más componentes unidos pero que no se combinan químicamente, estas mezclas se clasifican en dos tipos; homogénea y heterogénea. Con la elaboración de este trabajo pretendemos que sea más fácil para el lector comprender los distintos conceptos y las diferencias que tienen uno con otro y que pueda poner en practica estos conocimientos en diferentes situaciones

Abstract: In a time of rapid global change, the question of what determines patterns in species abundance distribution remains a priority for understanding the complex dynamics of ecosystems. The constrained maximization of information entropy provides a framework for the understanding of such complex systems dynamics by a quantitative analysis of important constraints via predictions using least biased probability distributions. We apply it to over two thousand hectares of Amazonian tree inventories across seven forest types and thirteen functional traits, representing major global axes of plant strategies. Results show that constraints formed by regional relative abundances of genera explain eight times more of local relative abundances than constraints based on directional selection for specific functional traits, although the latter does show clear signals of environmental dependency. Abstract: Funding: Floristic identification in plots in the RAINFOR forest monitoring network have been supported by the Natural Environment Research Council (grants NE/B503384/1, NE/ D01025X/1, NE/I02982X/1, NE/F005806/1, NE/D005590/1 and NE/I028122/1) and the Gordon and Betty Moore Foundation. In a time of rapid global change, the question of what determines patterns in species abundance distribution remains a priority for understanding the complex dynamics of ecosystems. The constrained maximization of information entropy provides a framework for the understanding of such complex systems dynamics by a quantitative analysis of important constraints via predictions using least biased probability distributions. We apply it to over two thousand hectares of Amazonian tree inventories across seven forest types and thirteen functional traits, representing major global axes of plant strategies. Results show that constraints formed by regional relative abundances of genera explain eight times more of local relative abundances than constraints based on directional selection for specific functional traits, although the latter does show clear signals of environmental dependency. These results provide a quantitative insight by inference from large-scale data using cross-disciplinary methods, furthering our understanding of ecological dynamics. Publisher PDF

Abstract: Background Brain metastases (BM) are the most frequent intracranial tumours in adults. In patients with solitary BM, surgical resection (SR) or stereotactic radiosurgery (SRS) is performed. There is limited evidence comparing one treatment over the other. Objective To compare SR versus SRS on patients with solitary BMs, regarding local recurrence (LR) and overall survival (OS) conducting a systematic review and meta-analysis. Methods Systematic review of literature following PRISMA guidelines, using the databases of Medline, Clinicaltrials.gov, Embase, Web of Science, Sciencedirect, CINAHL, Wiley Online Library, Springerlink and LILACS. Following study selection based on inclusion and exclusion criteria, data extraction and a critical analysis of the literature was performed according to the GRADE scale. For quantitative analysis, a random effects model was used. Data were synthetized and evaluated on a forest plot and funnel plot. Results Two randomized clinical trials, four cohort studies and one case-control studies met our inclusion criteria for the qualitative analysis. None was excluded subsequently. Overall, 614 patients with single metastasis were included. Studies had high heterogeneity. Multiple significant variables affecting the outcome were signalized. Meta-analysis showed no significant differences for survival (HR, 1.10; 95% CI, 0.75-1.45) or LR (HR, 0.81; 95% CI, 0.42-1.20). Conclusions According to current evidence, in patients with a single small metastasis there is no statistically significant difference in OS or LR among the chosen techniques (SR or SRS). Multiple significant co-variables may affect both outcomes. Different outcomes better than OS should be evaluated in further randomized studies.

Abstract: Background and purpose Rheumatoid meningitis (RM) is a neurological complication of rheumatoid arthritis (RA). Current evidence is based on case reports and partial reviews. Methods This is a systematic review and meta-analysis following the PRISMA statement. The aim is to describe the characteristics of the disease, including clinical, imaging and laboratory findings, treatment, outcomes and prognosis reported in the literature. Results In all, 103 studies with 130 cases were included. RM affected adults with an average age of 62 years, with or without a previous RA diagnosis. RA activity and time with the disease were associated with a worse prognosis. Most common clinical manifestations were transient focal neurological signs (64.6%), systemic symptoms (51.3%), episodic headache (50.4%) and neuropsychiatric alterations (47.7%). Joint manifestations were present in only 27.4% of cases. Brain magnetic resonance imaging showed unilateral or bilateral involvement, predominantly frontoparietal. Both pachymeninges and leptomeninges were affected, the latter more frequently (82.88%). The laboratory findings included increased levels of rheumatoid factor (89.71%), anti-cyclic citrullinated peptide (89.47%), C-reactive protein (82.54%) and erythrocyte sedimentation rate (81.81%). Cerebrospinal fluid analysis showed an increase in the protein level (76.14%), with pleocytosis (85.19%) of mononuclear predominance (89.19%). Biopsy was performed in 72.52% of the patients. Corticosteroid pulse therapy was the main induction therapy. Disease relapse occurred in 31.17% of patients, whilst 54.54% had a full recovery. Conclusions Rheumatoid meningitis must be considered in adult patients with or without RA diagnosis, high-dose corticosteroid induction therapy should be installed and maintenance therapy plays a key role. It is not recommended to use anti-TNF as an induction therapy. Nowadays, RM has a significantly better outcome. These findings may aid clinicians in timely RM diagnosis and treatment, thus improving its outcomes.

Abstract: BACKGROUND AND AIMS Transplantation (KTx) is considered the best renal replacement therapy nephrologists can offer and improving its outcomes remains a primary challenge in our field. KTx ureteral JJ stenting has been used to prevent urological complications, but there is no consensus about its elective removal timing and literature regarding routine US imaging after EJJR to detect complications is lacking. Our aim was to define the incidence of urological complications diagnosed by routine US after EJJR in KTx, determine US utility and best time interval to perform it. METHOD We retrospectively analyzed all routine KTx US performed in our Unit from 2016 until 2020 by an experienced interventional nephrologist. US post EJJR findings were compared with previous patient US. KTx characteristics, treatment and outcomes were recorded. RESULTS CONCLUSION Routine US after EJJR allowed a timely diagnosis and early treatment of urological complications, a key factor for successful transplantation. KTx US is a cost-effective and reproducible test that provides crucial information to guide clinical decisions, being most efficient when performed 10 days post elective removal. Interventional nephrologists could do this examination promptly.

Abstract: The South American species of the myrmecomorphic spider genus Mazax O. Pickard-Cambridge, 1898 (Castianeirinae, Corinnidae) are revised for the first time, increasing the number of known species from South America from two to six and the number of species in the genus from seven to 11. Here, we reevaluate the identity of Apochinomma acanthaspis Simon, 1896, propose its transfer to Mazax, and consider M. akephaloi Perger & Pett, 2022 as a junior synonym of M. acanthaspis comb. nov. Additional documentation of M. ramirezi Rubio & Danişman, 2014 is provided. The species M. pax Reiskind, 1969 and M. spinosa (Simon, 1898) are recorded from South America for the first time. Three new species are proposed, all based on both sexes: Mazax mokana sp. nov. and M. leonidas sp. nov. from Colombia, and M. tembe sp. nov. from Brazil. Emended diagnoses of the genus and of all six known South American species, as well as a key to males and females for all 11 species in the genus, are provided.

Abstract: Introduction: Deep brain stimulation (DBS) is a standard surgical procedure for the treatment of advanced Parkinson Disease (PD) with motor complications that cannot be adequately managed by medical treatment. Currently available literature can guide physicians on basic aspects of patients’ selection and indications for DBS. However, there is a range of real-world clinical settings where the indications of DBS for Parkinson disease are debatable. Objective: to present the experience on PD patients with complex clinical manifestations treated with DBS in our hospital. Method: Report of four PD cases treated with DBS. Case 1: 63-year old woman with advanced PD and severe motor complications; Case 2: 60-year old man with 5 years of disease duration and mild motor complications; Case 3: 67-year old man with severe ventriculomegaly that may have precluded direct electrode passage to the surgical target; Case 4: 67- year-old woman with putative severe axial disability. Results: After one year of follow-up, all patients showed improvement on motor symptoms as well as quality of life. Discussion: We provide a brief rationale for the patient selection in each case to support the decision-making in the management of PD patients with complex clinical cases.

29. Development of portable microgrids to increase flexibility in grid operations

Abstract: I-DE has assessed the feasibility of a ‘Portable Microgrid’, a system that aims to reduce the environmental impacts of diesel generators (DG) operation in cities, whilst maintaining the cost-efficiency of the system. A range of scenarios that combine service duration, uses per year and i-DE normalised transformer's rating have been defined. The HOMER software has been used to run these scenarios’ simulations to design the optimal microgrid configuration. The resulting microgrid design costs are compared against current operation costs of DG used for i-DE. Additionally, a maximum DG energy quota has been set for the microgrid design in order to make sure it is cleaner than the current equipment. Afterwards, the sensitivity analysis of the results against battery costs has been analysed attending to current, 2022 and 2030 prices. Results have shown that the optimal portable microgrid includes a DG that will provide the baseload and run at maximum efficiency. It will be complemented by a fully-charged Li-ion battery. The system will be cost-competitive when the annual uses are equal or higher than 60 and intervention duration are equal or shorter than 8 h. Finally, 2030 battery costs make portable microgrids feasible in every case.

Abstract: ResumoO presente trabalho busca explorar, de forma breve e limitada a seu escopo, as relações existentes entre direitos humanos, dignidade e a assistência aos idosos desempenhada por robôs. Com a apresentação de noções introdutórias e básicas de tais elementos e de sua previsão em instrumentos internacionais, busca-se destacar algumas das novas tecnologias que vêm sendo desenvolvidas no âmbito da robótica, em conjunto com a inteligência artificial, para o cuidado com aqueles que se encontram nas fases mais avanças de suas vidas, apresentando exemplos do atual estágio evolutivo dessas tecnologias. Posteriormente, serão apresentados alguns dos questionamentos éticos formulados sobre o uso de tais tecnologias e as preocupações que devem ser consideradas para seu desenvolvimento, atreladas à privacidade, autonomia, identidade e liberdade dos idosos, tentando avaliar a repercussão sobre sua dignidade. Ao final, pretende-se reconhecer, como resultado, qual o impacto do uso de robôs no cuidado com idosos, especial e notadamente sob a perspectiva dos direitos humanos, fazendo a contraposição às críticas formuladas a sua implementação. Quanto à metodologia, o presente estudo foi realizado através de abordagem teórica, exploratória e descritiva, através de pesquisa com observância dos procedimentos bibliográficos, nas plataformas oficiais referenciadas e em repositórios diversos.Palavras-chave: Direitos Humanos. AbstractThe present work seeks to explore, in a brief way and limited to its scope, the existing relationships between Human Rights, Dignity and Assistance to the elderly performed by robots. With the presentation of introductory and basic concepts of such elements and its prediction in international instruments, it seeks to highlight some of the new technologies that have been developed in the scope of Artificial Intelligence and Robotics for the care of those who are in the most advanced stages of their lives, presenting examples of the current stage of such technology. Subsequently, it aims to portray some of the ethical questions formulated about the use of such technology and the concerns that must be considered regarding the privacy, autonomy, identity and freedom of the elderly, trying to assess the repercussions on their Dignity. At the end, it is intended to recognize, as a result, the impact of the use of robots, combined with artificial intelligence, in the care of the elderly, especially and notably from the perspective of human rights, contrasting the criticisms made to their implementation. As for the methodology, the present study was carried out using a theoretical, exploratory and descriptive approach, through research with observance of bibliographic procedures, in the official platforms referenced and in various repositories.Keywords: Human Rights. Abstract: BACKGROUND: Laparoscopic Nissen fundoplication fails to control the gastroesophageal reflux in almost 15% of patients, and most of them must be reoperated due to postoperative symptoms. Different surgical options have been suggested. AIMS: This study aimed to present the postoperative outcomes of patients submitted to three different procedures: redo laparoscopic Nissen fundoplication alone (Group A), redo laparoscopic Nissen fundoplication combined with distal gastrectomy (Group B), or conversion to laparoscopic Toupet combined with distal gastrectomy with Roux-en-Y gastrojejunostomy (Group C). METHODS: This is a prospective study involving 77 patients who were submitted initially to laparoscopic Nissen fundoplication and presented recurrence of gastroesophageal reflux after the operation. They were evaluated before and after the reoperation with clinical questionnaire and objective functional studies. After reestablishing the anatomy of the esophagogastric junction, a surgery was performed. None of the patients were lost during follow-up. RESULTS: Persistent symptoms were observed more frequently in Group A or B patients, including wrap stricture, intrathoracic wrap, or twisted fundoplication. In Group C, recurrent symptoms associated with this anatomic alteration were infrequently observed. Incompetent lower esophageal sphincter was confirmed in 57.7% of patients included in Group A, compared to 17.2% after Nissen and distal gastrectomy and 26% after Toupet procedure plus distal gastrectomy. In Group C, despite the high percentage of patients with incompetent lower esophageal sphincter, 8.7% had abnormal acid reflux after surgery. CONCLUSIONS: Nissen and Toupet procedures combined with Roux-en-Y distal gastrectomy are safe and effective for the management of failed Nissen fundoplication. However, Toupet technique is preferable for patients suffering from mainly dysphagia and pain. RESUMO RACIONAL: A fundoplicatura de Nissen laparoscópica falha em controlar o refluxo gastroesofágico em quase 15% dos pacientes e a maioria deles deve ser reoperada devido aos sintomas. Diferentes técnicas cirúrgicas têm sido sugeridas. OBJETIVOS: Apresentar os resultados pós-operatórios de pacientes submetidos a 3 procedimentos diferentes: Reoperação e fundoplicatura de Nissen laparoscópica apenas (Grupo A), reoperação e fundoplicatura de Nissen laparoscópica combinado com gastrectomia distal (Grupo B) ou reoperação e conversão para Toupet laparoscópico combinado com gastrectomia distal e reconstrução em Y-Roux (Grupo C). MÉTODOS: estudo prospectivo incluindo 77 pacientes submetidos inicialmente a fundoplicatura de Nissen laparoscópica que apresentaram recidiva do refluxo gastroesofágico após a operação. Eles foram avaliados antes e após a reoperação com questionário clínico e estudos funcionais específicos. Reestabelecida a anatomia da junção esôfago-gástrica, procedeu-se às referidas cirurgias. Nenhum paciente perdeu seguimento. RESULTADOS: Os sintomas persistentes foram observados com maior frequência nos pacientes dos Grupos A ou B, associados à estenose da fundoplicatura, fundoplicatura intratorácico ou fundoplicatura torcida. No Grupo C, os sintomas recorrentes associados a essa alteração anatômica foram observados com pouca frequência. Esfincter esofágico inferior incompetente foi confirmado em 57,7% dos pacientes pertencentes ao Grupo A em comparação com 17,2% após fundoplicatura de Nissen laparoscópica combinado com gastrectomia distal, e 26% após a Toupet laparoscópico combinado com gastrectomia distal e reconstrução em Y-Roux. Nesse grupo, apesar do alto percentual de pacientes com esfincter esofágico inferior incompetente, 8,7% apresentaram refluxo ácido anormal após a cirurgia. CONCLUSÕES: As técnicas de Nissen ou Toupet combinados com a gastrectomia distal em Y de Roux são seguras e eficazes para o manejo da falha da fundoplicatura a Nissen. A técnica de Toupet é preferível para pacientes que sofrem principalmente com disfagia e dor.

Abstract: The COVID-19 pandemic is a global public health problem. Patients with end-stage renal disease on hemodialysis are at a higher risk of infection and mortality than the general population. Worldwide, a vaccination campaign has been developed that has been shown to reduce severe infections and deaths in the general population. However, there are currently limited data on the clinical efficacy of vaccinations in the hemodialysis population.A national multicenter observational cohort was performed in Chile to evaluate the clinical efficacy of anti-SARS-CoV-2 vaccination in end-stage renal disease patients on chronic hemodialysis from February 2021 to August 2021. In addition, the BNT162b2 (Pfizer-BioNTech) and CoronaVac (Sinovac) vaccines were evaluated. The efficacy of vaccination in preventing SARS-CoV-2 infection, hospitalizations, and deaths associated with COVID-19 was determined.A total of 12,301 patients were evaluated; 10,615 (86.3%) received a complete vaccination (2 doses), 490 (4.0%) received incomplete vaccination, and 1196 (9.7%) were not vaccinated. During follow-up, 1362 (11.0%) patients developed COVID-19, and 150 died (case fatality rate: 11.0%). The efficacy of the complete vaccination in preventing infection was 18.1% (95% confidence interval [CI]:11.8-23.8%), and prevention of death was 66.0% (95% CI:60.6-70.7%). When comparing both vaccines, BNT162b2 and CoronaVac were effective in reducing infection and deaths associated with COVID-19. Nevertheless, the BNT162b2 vaccine had higher efficacy in preventing infection (42.6% vs. 15.0%) and deaths (90.4% vs. 64.8%) compared to CoronaVac.The results of our study suggest that vaccination against SARS-CoV-2 in patients on chronic hemodialysis was effective in preventing infection and death associated with COVID-19.

Surgical options are to redo LNF, distal gastrectomy alone or combination of both procedures. Redoing LNF, is associated with high rate of postoperative complications and re-failure. Purpose To present the early and medium-term postoperative outcomes of patients submitted to Laparoscopic Toupet Fundoplication combined with Distal Gastrectomy with Roux-en-Y gastrojejunostomy. Methods prospective study including 23 patients submitted to Laparoscopic Nissen Fundoplication who presented recurrence of gastroesophageal reflux after the operation. They were studied with endoscopy, barium swallow, manometry and 24 h pH monitoring in order to determine presence of esophagitis, anatomical deformities, defective Lower Esophageal Sphincter and pathologic acid reflux. Surgical process consisted on re-establishing the anatomy of the esophago-gastric junction, and then perform laparoscopic Toupet fundoplication combined with distal gastrecvtomy and Roux Y gastrojejunostomy. Results After surgery statistically significant improvement regarding heartburn (p Conclusion The proposed procedure is safe option to treat successfully patients after failed Nissen fundoplication. Reflux symptoms, esophagitis, presence oh hiatal hernia and reflux score improved after the procedure.

We aimed to study the effectiveness of SES combined with a sensory discrimination task in a well-controlled virtual environment in healthy participants, setting a foundation for its potential application in stroke rehabilitation. We employed electroencephalography (EEG) to gain a better understanding of the underlying neural mechanisms and dynamics associated with sensory training and SES. We conducted a single-session experiment with 26 healthy participants who explored a set of three visually identical virtual textures—haptically rendered by a robotic device and that differed in their spatial period—while physically guided by the robot to identify the odd texture. The experiment consisted of three phases: pre-intervention, intervention, and post-intervention. Half the participants received subthreshold whole-hand SES during the intervention, while the other half received sham stimulation. We evaluated changes in task performance—assessed by the probability of correct responses—before and after intervention and between groups. We also evaluated differences in the exploration behavior, e.g., scanning speed. EEG was employed to examine the effects of the intervention on brain activity, particularly in the alpha frequency band (8–13 Hz) associated with sensory processing. We found that participants in the SES group improved their task performance after intervention and their scanning speed during and after intervention, while the sham group did not improve their task performance. However, the differences in task performance improvements between groups only approached significance. Furthermore, we found that alpha power was sensitive to the effects of SES; participants in the stimulation group exhibited enhanced brain signals associated with improved touch sensitivity likely due to the effects of SES on the central nervous system, while the increase in alpha power for the sham group was less pronounced. Our findings suggest that SES enhances texture discrimination after training and has a positive effect on sensory-related brain areas. Further research involving brain-injured patients is needed to confirm the potential benefit of our solution in neurorehabilitation.

Abstract: Objetivo: Determinar cómo las exportaciones de commodities inciden en los ingresos tributarios del Gobierno Central y el tipo de relación que existe entre de los impuestos directos e indirectos. Método: Se aplicó el método descriptivo correlacional, estructurándose una matriz de correlación y líneas de regresión, para ello se empleó datos oficiales de la SUNAT, INEI, BCRP. Resultados: En el estudio se evidencia el mayor aporte del IGV en la recaudación tributaria, seguido del impuesto a la renta, pero por el grado de correlación alcanzado este último impuesto tiene mayor incidencia en la recaudación fiscal. Sin embargo, al contrastar las exportaciones mineras con los impuestos directos se aprecia una débil, pero negativa relación de dependencia lo cual implica que la recaudación tributaria es débilmente afectada por las exportaciones de minerales. Conclusiones: Se concluye, por el grado y tipo de correlación obtenido, que el aumento de las exportaciones mineras no ejerce mayores cambios en la recaudación tributaria siendo además el impacto de tipo inverso.

Abstract: In a time of rapid global change, the question of what determines patterns in species abundance distribution remains a priority for understanding the complex dynamics of ecosystems. The constrained maximization of information entropy provides a framework for the understanding of such complex systems dynamics by a quantitative analysis of important constraints via predictions using least biased probability distributions. We apply it to over two thousand hectares of Amazonian tree inventories across seven forest types and thirteen functional traits, representing major global axes of plant strategies. Results show that constraints formed by regional relative abundances of genera explain almost ten times more of local relative abundances then constraints based on either directional or stabilizing selection for specific functional traits, although the latter does show clear signals of environmental dependency. These results provide a quantitative insight by inference from large-scale data using cross-disciplinary methods, furthering our understanding of ecological dynamics.

Abstract: Andamios impresos en 3D con poros heterogéneos, surge como estrategia para la regeneración de tejidos. En este estudio, se evaluó la regeneración ósea en defectos críticos de ratas Wistar, debido a la osteoconducción de andamios de poliácido-láctico (PAL), impresos en 3D con diferentes tamaños de poros; 250-300 µm en la periferia, seguido de 350-400 µm y en el centro 400-740 µm, debido a que los pequeños promueven adhesión celular, mientras que los grandes la angiogénesis. Los andamios se imprimieron en 3D con PLA, un material termoplástico, biocompatible, biorreabsorbible, aprobado por la Administración de Alimentos y Medicamentos de los Estados Unidos (FDA, por sus siglas en inglés), evaluando tamaño de poro y porosidad, in vivo, en defectos de 9 mm de diámetro en calvarias de ratas, calculando el tejido mineralizado por la radiodensidad de las unidades Hounsfield (UH) en imágenes microtomográficas a 8, 30, 60 y 90 días. Los resultados demostraron rango de poros de 200-800 µm (como el diseño), la porosidad fue del 98%, favoreciendo el flujo de nutrientes, oxígeno y eliminación de desechos. Se observó in vivo tejido radiodenso al día 30, evidentemente al 90, concordando con las UH 93.66 y 118.31, respectivamente. Los andamios 3D con poros heterogéneos, demostraron su capacidad osteoconductora en la regeneración ósea, abriendo alternativas en la bioingeniería tisular.

42. Exploring the use of CO2-expanded ionic liquids as solvents in microextraction of 3,3′,4,4′-tetrachlorobiphenyl from aqueous solutions

Abstract: This paper explores the effect of adding supercritical carbon dioxide (scCO2) to Ionic Liquids (ILs) for the microextraction of very diluted pollutants from aqueous matrices. The proposed system uses an IL, trihexyl-tetradecyl-phosphonium bis(trifluoromethylsulfonyl)imide ([P6,6,6,14][Tf2N]) and 1-hexyl-3-methyl-imidazolium tris(pentafluoroethyl)trifluorophosphate ([hmim][FAP]), as extractant, and scCO2 as diluent that may change the solvent capacity by changing the solvent polarity. The study was first carried out for the extraction of 3,3′,4,4′-tetrachlorobiphenyl (PCB-77) from aqueous solutions using a designed microextraction cell. Different ILs, sample volumes and kinetics of extraction were investigated. Results show that within 15 min the maximum extraction percentage was achieved employing a scCO2 partial pressure of 80 bar. To investigate the influence of the polarity change by the presence of carbon dioxide more broadly, other pollutants with different water solubilities were used. It was observed that scCO2 (partial pressure) reduced the recoveries for benzophenone and PCB-77 but increased the extraction of triclosan. This is due to the change on solvent's polarity and viscosity of the mixture. This was corroborated through a ternary system simulation including IL-pollutant-scCO2. All these results establish that the combination of CO2 and IL can in some cases enhance the extraction, but the affinity between the pollutant and scCO2 is a key parameter that discerns whether expanding the IL with scCO2 is beneficial for the extraction.

Abstract: Rheumatoid meningitis (RM) is a rare complication of rheumatoid arthritis (RA). RM mimics many other conditions such as subdural empyema, unsteady gait, focal brain dysfunction, stroke, relapsing-remitting motor signs, headache, neuropsychiatric disorders, seizures, parkinsonism, and meningeal tumors. RM is considered a disease with poor prognosis. However, cases reported in the last decade show a good outcome. We report two cases with a favorable outcome. A 48-year-old man with a three-year history of RA admitted for headache, sensory disturbances, and speech difficulties. Brain magnetic resonance imaging (MRI) showed a left parietal subdural laminar lesion with restricted diffusion and a small left superior frontal acute infarction. A subdural empyema was originally suspected, and antimicrobials were prescribed. A follow-up MRI did not show progression of the subdural lesion and the patient was discharged 14 days after admission without focal deficits. A 44-year-old female patient with two years of seronegative RA was admitted for severe headache, confusion, nausea and vomiting. Brain MRI showed subtle supra and infratentorial leptomeningeal involvement and a left cerebellar acute infarct. A meningoencephalitis due to etanercept was initially thought and treated with dexamethasone. The patient was discharged but had to be admitted again and a new MRI showed a progression of the leptomeningeal involvement. She worsened and required endotracheal intubation. Cyclophosphamide was started and the patient became asymptomatic three months later. We propose that treatment should not be delayed waiting a biopsy when a diagnosis of RM is made and after a cerebrospinal fluid infection has been ruled out.

Abstract: The antimicrobial properties of a series of anionic bis(carbene) silver complexes Na3[Ag(NHCR)2] were investigated (2a–2g and 2c′, where NHCR is a 2,2′-(imidazol-2-ylidene)dicarboxylate-type N-heterocyclic carbene). The complexes were synthesized by the interaction of imidazolium dicarboxylate compounds with silver oxide in the presence of aqueous sodium hydroxide. Complexes 2f,g were characterized analytically and spectroscopically, and the ligand precursor 1f and complexes 2c and 2g were structurally identified by X-ray diffraction methods. The anions of 2c and 2g, [Ag(NHCR)2]3−, showed a typical linear disposition of Ccarbene-Ag-Ccarbene atoms and an uncommonly eclipsed conformation of carbene ligands. The antimicrobial properties of complexes 2a–g, which contains chiral (2b–2e and 2c′) and non-chiral derivatives (2a,f,g), were evaluated against Gram-negative bacteria, Escherichia coli and Pseudomonas aeruginosa, and a Gram-positive bacterium, Staphylococcus aureus. From the observed values of the minimal inhibitory concentration and minimal bactericidal concentration, complexes 2a and 2b showed the best antimicrobial activity against all strains. An interesting chirality–antimicrobial relationship was found, and eutomer 2c′ showed better activity than its enantiomer 2c against the three bacteria. Furthermore, these complexes were investigated experimentally and theoretically by 109Ag nuclear magnetic resonance, and the electronic and steric characteristics of the dianionic carbene ligands were also examined.

50. Update in the Peruvian Caponiidae: New records and new species of Nyetnops Platnick & Lise, 2007 (Araneae: Caponiidae)

Abstract: An update on the diversity and distribution of Caponiidae from Peru is presented. New distributional records for Caponina cajabamba Platnick, 1994 are given and the male is described for the first time. Additionally, three new species of the genus Nyetnops Platnick & Lise, 2007 are described: Nyetnops alexanderi Villarreal & Martínez sp. nov. (♂♀), Nyetnops madre Villarreal & Martínez sp. nov. (♂♀) and Nyetnops josei Villarreal & Martínez sp. nov. (♂). The taxonomic key proposed by Sánchez-Ruiz et al. (2020) for Nyetnops is updated to include all the described species as well as the new ones proposed herein. Maps showing the distribution of the records of Caponiidae species in Peru, mainly focused on the genus Nyetnops is included.
