Hospitalized patients receiving treatment at intensive care units (ICU) usually show poor oral hygiene, and may have the mouth and oropharingeal region colonized by pathogens involved in nosocomial pneumonia. The presence of these pathogens may increase the risk for respiratory diseases. The aim of this study was to investigate the presence of respiratory pathogens in the oral cavity of hospitalized patients at ICU.Were included in the study 30 patients from Hospital Raul Sertã, Nova Friburgo, with the diagnostic of nosocomial pneumonia, and tracheal aspirate samples were cultured to identify the causing microorganisms. In addition, microbiological samples from supragingival dental plaque, tongue and respiratory tube were cultured for the presence of a panel of respiratory pathogens.The most frequently found bacteria in the tracheal aspirate were S. Pneumoniae 23.3% (7), P. aeruginosa 20% (6), S. aureus 13.3% (4), K. pneumoniae 13.3% (4), C. albicans 6.6% (2), a-hemolytic streptococcus 6.6% (2), Staphylococcus sp. 6.6% (2), A. calcoaceticus 3.3% (1), E. coli 3.3% (1) and E. cloacae 3.3% (1). 70% (21) of these microorganisms were found in the dental biofilm, 63.33% (19) in tongue samples; 73.33% (22) in the respiratory tube; and 43.33% (13) in all sampling sites simultaneously. No differences in proportions could be observed between the sampling sites (p0.05) CONCLUSIONS: The results of this study show that respiratory pathogens associated with nosocomial pneumonia are present in the oral biofilm of hospitalized patients in ICU, which may serve as a reservoir for these microorganisms.

Ci. méd. biol., v.2, n.1-2003.pdf: 154141 bytes, checksum: 46f8098f2b6b449491b23cfe756a93ba (MD5) Previous issue date: 2003-01 O objetivo deste estudo foi determinar os níveis de MMP-9 (gelatinase B) presentes no fluido crevicular gengival de sítios com destruição tecidual (PP) de pacientes com periodontite crônica generalizada e compará-los aos níveis de MMP-9 de sítios com inflamação gengival, mas sem sinais de perda de inserção, destes mesmos pacientes (GP) e de pacientes afetados apenas por gengivite (GG). O grupo teste constituiu-se de 24 pacientes com periodontite crônica, sendo 7 homens e 17 mulheres, com média de idade de 48,2 anos (DP ± 7,4) e o grupo controle, de 22 pacientes, 10 homens e 12 mulheres, com média de idade de 47,5 anos (DP ± 9,4), sem sinais clínicos de perda de inserção. Os parâmetros clínicos analisados foram Índice de Placa (IP), Índice Gengival (IG), Profundidade de Bolsa à Sondagem (PBS) e Nível de Inserção Clínica (NI). As amostras de fluido gengival foram coletadas com o método de lavagem intra-sulcular. Os níveis de MMP-9 nas amostras coletadas foram determinados através da técnica de ELISA. Os resultados mostraram que a quantidade de MMP-9 nos sítios PP (318,5 DP ± 304.9) é significativamente maior do que em sítios GG (média 110 DP ± 121) (p < 0,003) e GP (106 DP ± 118) (p < 0,001), e nenhuma diferença significativa foi observada entre GG e GP, apesar de todos os sítios apresentarem níveis de IG e IP similares. Concluindo, a elevação dos níveis de MMP-9 nos sítios PP sugere que essa enzima pode estar envolvida no complexo processo de destruição tecidual que ocorre na periodontite. Salvador

Objective: To map the participation of the elastic system fibers of human gingiva. Methods: To conduct this study, fragments of human gingiva from ten individuals aged between 18 and 60 years, removed after periodontal surgery for prosthetic purposes, were analyzed by the histochemical and immuno-histochemical methods, to evaluate the participation of the elastic system fibers in this tissue. Results: The results demonstrated the presence of three type of elastic system fibers, that is to say, oxitalan, elaunin and elastic fibers,distributed as follows: 1) the oxitalan fibers form a network of thin fibers, located close to the basal membrane, at the level of the conjunctive tissue papillae; 2) elaunin fibers are found in close contact with the oxitalan fibers in the papillary and submaxillary regions, following the collagen fiber bundles; 3) a small quantity of elastic fibers were observed, dispersed throughout the deeper conjunctive tissue and around the blood vessels. Conclusion: The three types of elastic system fibers, that is, oxitalan, elaunin and elastic fibers are normal constituents of the extracellular matrixof human gingiva conjunctive tissue.

O objetivo deste ensaio-clínico, aleatório, duplo-cego foi avaliar as alterações clínicas e no número de bactérias obtidas com o uso do laser de Nd:YAG associado à raspagem em furcas grau II de pacientes com periodontite crônica. Num modelo de boca dividida, sessenta e duas furcas (62) grau II foram selecionadas de trinta e um (31) pacientes que receberam tratamento periodontal prévio da boca toda com exceção dos sítios experimentais. As trinta e uma (31) furcas do grupo Controle receberam duas sessões, com intervalo semanal, de raspagem, alisamento e polimento corono-radicular manual associado ao ultrassom. O grupo Teste recebeu o mesmo tratamento que o Controle, seguido da aplicação do laser de Nd:YAG (100mJ/pulso; 15Hz; 1,5W; 60seg, 141,50J/cm2). Os parâmetros clínicos avaliados antes, um, três, e seis meses depois do tratamento foram: Índice Gengival (IG), Índice de Placa (IP), Distância Junção Esmalte Cemento - Margem Gengival (JEC-MG), Profundidade Clínica de Sondagem do Componente Horizontal (FH) e Vertical (FV) da Furca e Nível Clínico de Inserção (NCI). Os parâmetros microbiológicos coletados antes, imediatamente após, um, três, e seis meses depois do tratamento foram: número de Unidades Formadoras de Colônias (UFCs) de bactérias totais; número total de UFCs de bactérias pigmentadas de preto; porcentagem de pacientes com Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis e Prevotella intermedia Os resultados demonstraram uma redução significativa das UFCs de bactérias totais em ambos os grupos, sendo esta maior no grupo Teste imediatamente após o tratamento. Após um e três meses, houve um aumento desse número, porém não alcançando os níveis iniciais para ambos os grupos. Já aos seis meses houve um novo aumento do número de UFCs de bactérias totais alcançando níveis próximos dos inicias no grupo Teste, enquanto o grupo Controle manteve-se semelhante aos três meses. As UFCs de bactérias pigmentadas de preto e a porcentagem de pacientes com as bactérias estudadas, se comportaram da mesma forma para ambos os grupos, tendo uma redução imediatamente após o tratamento, e um retorno à níveis próximos dos iniciais após seis meses. Para os valores de, IP, IG, FV, FH e NCI foi observado uma redução ao longo do experimento, porém sem diferença entre os grupos, e para distância JEC-MG houve um aumento ao longo do estudo também sem diferença entre os grupos. Conclui-se que o laser de Nd:YAG associado à raspagem promoveu redução bacteriana nas furcas grau II imediatamente após o tratamento, porém essa redução adicional não foi observada durante os demais tempos experimentais e que ambos os tratamento promoveram melhoras na maioria dos parâmetros clínicos avaliados ao longo do estudo. The purpose of this clinical trial, randomized, double-blinded was to evaluated the bacterial reduction and the clinical improvement achieved with the Nd:YAG associated to conventional treatment on furcation sites of patients with chronic periodontitis. In a split mouth design study, sixty two (62) class II furcations were selected from thirty one (31) patients that received previous full mouth periodontal treatment, except for the experimental sites. The thirty one (31) furcations of the Control group underwent twice manual and ultrasonic root debridment in weekly intervals. The Test group received the same treatment as the Control group followed by the Nd:YAG laser irradiation (100mJ/pulse; 1.5W; 15Hz; 60sec, 141.50J/cm2). The clinical parameters evaluated before, one, three, and six months after the treatment were: Gingival Index (GI), Plaque Index (PI), Clinical Pocket Depth of Furcation Horizontal (HF) and Vertical (VF) Components, Cement Enamel Junction Gingival Margin distance (CEJ-GM) and Clinical Attachment Level (CAL). The microbiological parameters evaluated before, immediately, one, three and six months after the treatment were: Colony Forming Units (CFU) of total bacteria number; CFU of dark pigmented bacteria and the percentage values of patients with Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis and Prevotella intermedia. The results showed a CFU total bacteria number significant reduction for both groups immediately after the treatment, but it was significant higher for the Test group. After one and three months, an increase of this number was observed, however not reaching the initial levels for both groups. At the six months an increase almost equal to baseline levels in Test group was showed, while the Control group still similar to the three months results. The CFU of dark pigmented bacteria and the percentage values of patients with the studied bacteria, reduced after the treatment and returned to values close to the initial ones six months after the baseline for both groups. The VF, HF, CAL, PI and GI values reduced throughout the study, however with no difference between then, and for CEJGM distance an increase was noted throughout the study without significant difference between the groups. It was concluded that the Nd: YAG laser associated with scaling and root planning promoted bacterial reduction on class II furcation immediately after its application, although this reduction was not observed six months after and that both treatments promoted improvements of most clinical parameters throughout the study.
