In lower gastrointestinal bleeding (LGIB), it is very important to stratify the risk of LGIB for a proper management.Identity the independent risk factors to mortality and severity (require critical care, prolonged hospitalization, reebleding, re hospitalization, politrasfusion, surgery for bleeding control) in LGIB.It is an analytic prospective cohort study, performed between June 2016 and April 2018 in a tertiary care hospital. Independent factors were determined using binomial logistic regression.A total of 98 patients were included, of which 13 patients (13,3%) died, and 56 (57,1%) met severity criteria. The independent risk factor for mortality was Glasgow scale under 15, and for severe bleeding were: Systolic blood pressure under 100 mm Hg, albumin lower than 2,8 g/dL.The frequency of mortality and severe LGIB is high in our population, the principal risk factors were systolic blood pressure under than 100 mm Hg, Glasgow score lower than 15, albumin lower than 2,8 g/dL. Identifying these associated factors would improve the management of LGB in the emergency room.

Gastric hemangioma as a cause of upper gastrointestinal bleeding (UGIB) is a rare event. We present the case of an 83 years old male with a history of abdominal pain, vomiting and melena, along with an 8 Kg weight loss. The upper gastrointestinal endoscopy showed an elevated, ulcerated lesion in the gastric antrum with a visible vessel, for which he receives endoscopic therapy. In the abdominal computed tomography, a contrast enhancing, well-circumscribed mass attached to the gastric wall is observed. Due to the persistence of the UGIB, the patient suffers hemodynamic decompensation and undergoes exploratory laparotomy, where a vascularized mass is found. The pathology report informs a gastric cavernous hemangioma.

We report the case of a male patient of 75 years old who presents with abdominal pain, hyporexia, early satiety, general malaise and watery stools, admitted in emergency for an episode of syncope. On physical examination, hepatomegaly of 6cm below the right costal margin was detected. CT scan showed multiple liver metastases. An upper endoscopy found multiple hyperpigmented lesions on the second portion of the duodenum. Histology and immunohistochemistry studies concluded it was duodenal melanoma. Skin and ocular examination did not reveal associated neoplastic lesions.

The following is a case report involving a 16 year old female with trichotillomania as an antecedent. This patient presented to the Emergency Room with a chief complaint of early satiety and persistent abdominal pain for the past 3 months. However, recently her abdominal pain has worsened and it is now complicated by nausea and vomiting. The physical exam was notable for epigastric pain on deep palpation. The biochemical analysis and abdominal ultrasound were otherwise unremarkable. An esophagogastroduodenoscopy was subsequently performed where a trichobezoar was discovered. It extended from the gastric fundus to the third portion of the duodenum. A surgical extraction of the trichobezoar was then performed. The trichobezoar was found to be 130 cm in length and 8 cm wide at its most cephalad aspect. It is important to note that they also found five mall perforations throughout the duodenum and jejunum. The patient was discharged with outpatient follow up with psychiatry. In this report we describe the case of a patient with Rapunzel syndrome that was complicated by small bowel perforation and we provide a review of the salient literature concerning this syndrome and its associated complications.

We present the case of a 30-year old female with a history of abdominal pain, fever, poor oral tolerance and weight loss for 6 months. An abdominal CT scan showed marked gastric dilatation due to extrinsic compression from lymphadenopathies around the second portion of the duodenum. The upper endoscopy revealed the presence of a penetrating gastric ulcer in the greater curvature. Biopsies of the lesions showed hystiocytes with granulomatous features and Acid Fast Bacilli (AFB) positive, and the cultures grew Mycobacterium tuberculosis sensitive to Isonazid and Rifampin. Subsequently anti-TB regimen was initiated achieving great clinical and endoscopic improvement.

Inflammatory bowel disease is associated with extraintestinal manifestations. Among these manifestations is the venous tromboembolism which presents a risk three times more than that presented in general population. We report the case of a 61-year-old male with a history of abdominal pain, chronic diarrhea and fever, with leukocytosis, and fecal samples containing leukocytes, partial ileal stenosis with multiple ulcers in the enteroscopy, with histologic findings compatible with Crohn's disease. The patient has a good outcome with prednisone and maintenence azathioprine, presenting at the fifth month deep venous thrombosis of both lower extremities that resolvewith anticoagulation treatment.

We present a descriptive analysis of our cases of sphincterotomy followed by papillary large balloon dilation in a single session (ES-LBD) in the management of difficult to extract calculi, with the objective of assessing rates of therapeutic success and complications in local experience.ERCP procedures with ES-LBD performed for choledocholithiasis between January 2009 and December 2014 in patients older than 18 years and without preexistent sphincterotomy were selected from records of the Gastroenterology Service at Cayetano Heredia National Hospital. A descriptive analysis of therapeutic success and complications was performed.73 procedures in 73 patients were included (65.8% female, 34.2% male). Patient's average age was 59.4±19.8 years. Average diameter of calculi was 14.6±3.3mm. Average diameter of dilations was 14.6±2.27 mm. In 8 cases mechanical lithotripsy was performed. Complete calculi extraction was achieved in 56 procedures (76.7%). Complications occurred in 4 cases (5.5%). There were no deaths.Our figures of therapeutic success and complications with ES-LBD for difficult to extract calculi are similar to those reported in literature. ES-LBD is an effective and safe technique for management of choledocholithiasis.

To validate the Rockall score in elderly patients with non variceal upper gastrointestinal bleeding, in terms of mortality and recurrent bleeding at 30 days follow-up.Patients older than 60 year-old, with non variceal upper gastrointestinal bleeding demonstrated by endoscopy, who were attended in a third level general hospital from June 2009 to June 2013, were included. Data was analyzed with the ROC (Receiver Operating Characteristic) curve. The area under curve was obtained to assess the Rockall score.One hundred ninety patients were included, 64.2% were males, with an average age of 74 years old. Overall mortality was 16.8% and respiratory failure with second bleeding episode was the most common cause of death (34.3% and 31.3% respectively). 5.52% of patients presented a second bleeding episode. A transfusion of more than 2 blood-packs was needed in the 24.7% of the patients. The area under ROC curve using the Rockall score was 0,76 (IC: 0.68-0.84) for mortality risk, 0.71 (IC: 0.55-0.88) for the risk of rebleeding and 0.66 (IC: 0.58-0.74) for needing a more than 2 blood-packs transfusion.Rockall score is a good predictor for mortality and rebleeding during the 30 day-period after a non-variceal bleeding episode in elderly patients. The best sensibility and specificity was obtained with the scores of 5 and 6, respectively.

To assess the BISAP and APACHE II scores in predicting severity according to the 2012 Atlanta classification and whether the obesity factor added to these scores improves prediction.A prospective study between January 2013 and April 2014 including all patients with acute pancreatitis was performed according to the new Atlanta 2012 classification. ROC curves were fabricated for BISAP, BISAP-O, APACHE-II scores and Apache O and appropriate cutoffs were selected to the sensitivity, specificity, PPV, NPV, RPP and RPN.We studied 334 patients. 65.27% were overweighted or obese. The biliar etiology was 86.53%. Only 8.38% had severe pancreatitis and 1.5% died. Areas under the ROC curve and cut points selected were: BISAP: 0.8725, 2; BISAP-O: 0.8246, 3; APACHE-II: 0.8547, 5; APACHE-O: 0.8531, 6. Using these cutoffs the sensitivity, specificity, PPV, NPV, RPP and RPN were BISAP: 60.71%, 91.83%, 40.48%, 96.23 %, 7.43, 0.43; BISAP-O: 60.71%, 86.93%, 29.82%, 96.03%, 4.76, 0.45; APACHE-II: 85.71%, 76.14%, 24.74%, 98.31%, 3.6, 0.19; APACHE-O: 82.14%, 79.41%, 26.74%, 97.98%, 4, 0.22.BISAP, BISAP-O, APACHE-II and APACHE-O systems can be used to identify patients at low risk of severity because of its high NPV, however their use should be cautious considering that the RPP and RPN do not reach optimal levels indicating that their value in predicting severity is limited. On the other hand adding the obesity factor did not improve their predictive ability.

During the first 6 weeks after a variceal hemorrhage there is a 30-40% of probability of recurrence and those who rebleed 20- 30% die. Passed this period, the risk of rebleeding is of 60% and reaches a mortality of 60-70% in two years without treatment.Describe the frequency of rebleeding and mortality due to rebleeding in cirrhotic patients treated for variceal hemorrhage at Endoscopic Centers of Hospital Nacional Cayetano Heredia, Lima, Peru and Hospital Nacional Arzobispo Loayza, Lima, Peru during the years 2009-2011.The study type is a transversal, periodic and retrospective one in which were included 176 cirrhotic patients older than 14 years who have bleed for esophageal varices and that have received endoscopic therapy. The instruments used were a data sheet with all the information obtained from the clinical chart of each patient, the CHILD score to assess severity of hepatic disease, endoscopic informs and phone calls.The frequency of rebleeding before 6 weeks was 32.20% (56 patients). Also, the frequency of rebleeding after that time was 22.56% (37 patients). There was a mortality rate of 5.70% (10 patients) and a mortality rate due to rebleeding of 13.33% (6 patients).Variceal hemorrhage is an important cause of mobimortality in peruvian people. The frequency of rebleeding and mortality due to rebleeding resulted slightly lower than in other countries.

To validate SODA (severity of dyspepsia assessment) questionnaire in our population for evaluating symptoms severity in patients with dyspepsia.Content and appearance validity were measured, and then a modified questionnaire was developed. A pilot test was made and reliability, construct validity and responsiveness were measured.An adequate content and appearance validity were obtained. On the pilot test, reliability of the complete questionnaire and by components had Cronbach alpha values over 0,7. Construct validity of SODA was evaluated by correlating modified SODA results with SF-36 scores using Pearson test: -0.72 (p0.001). When we compared results of modified SODA questionnaire and the score of a question about subjective perception of the disease using Spearman test we obtained similar results: 0.72 (p0.001). Two means of SODA questionnaire scores, before and after treatment, were compared using Student's T test, and a significant difference was found, (p0.001) with a drop mean of 5.70 ±5.33.Modified SODA questionnaire meets all criteria of validity and it could be used to measure dyspepsia severity and its evolution in a Peruvian population.

Peptic ulcer disease is the main cause of upper gastrointestinal bleeding and Helicobacter pylori is its principal etiology. The sensitivity of the diagnostics tests is low for the detection of H. pylori en the setting of bleeding peptic ulcer. In addition there are different results among them.To validate the rapid urease test (RUT) in patients with bleeding peptic ulcer.We prospectively included patients older than 14 years old who presented with bleeding peptic ulcer and performed diagnostic studies of RUT and histology for the detection of H. pylori. Two biopsies were taken (one from the antrum and another one from the corpus) for RUT and four biopsies (two from both antrum and corpus) for histology. RESULTS Ninety-three patients were included, gastric ulcer was the most frequent site of the bleeding. 48 patients were positive for H. pylori by histology studies and 55 patients were positive to H. pylori by RUT. The sensitivity and specificity of the RUT were 89.6% (IC 77.3-96.5) y 73.3% (IC 58.0-85.4) respectively.The RUT has a high sensitivity for the detection of H. pylori en the setting of bleeding peptic ulcer.

Sequential therapy is used as an alternative to growing antibiotic resistance of Helicobacter pylori to the standard triple therapy. Despite the success it had in Europe, we have no information regarding this therapy in our region.To evaluate the eradication rate of Helicobacter pylori using sequential therapy and show its adverse effects.We performed a prospective, observational, open descriptive study. 31 patients were evaluated who were treated with sequential therapy in the following way: the first 5 days omeprazole 20 mg and amoxicillin 1 g every 12 hours and following 5 days omeprazole 20 mg, clarithromycin 500 mg and tinidazole 500 mg every 12 hours. After four weeks of treatment, each patient had a C13 urea breath test to check for eradication.31 patients were included, one patient was excluded from the protocol due to adverse drug react to amoxicillin. Of the remaining 30 patients who completed treatment, 22 (73%) were negative to breath test and 8 (27%) were positive. Of the patients who completed treatment, 10 had minor adverse events to treatment, the main symptoms were epigastralgia and nausea.Sequential therapy had an eradication rate of 73% which is much lower than that reported in European studies. However, therapy is easily accesible with lower cost and fewer side effects tan standard therapy.

Prospective cohort study. Including patients with the diagnosis of UGIB, older than 18 years old, with an endoscopic diagnostic showing the peptic ulcer as the cause of the bleeding and with studies for H. Pylori and other microorganisms. We excluded patients who took NSAIDS in the last 4 weeks. To determinate H. Pylori infection, 2 biopsies of antrum and 2 of gastric body were taken for histologic study, and 1 biopsy of antrum and one of gastric body were taken to do a rapid urease test. H. Pylori was determined with the presence of it in any of the two tests, and was negative when it was not found in neither of two tests. We followed up patients within 30 days after their hospital admission. CONCLUSION
