Este documento presenta una aproximación analítica a la relación del sistema de monedas digitales (criptomonedas) y la economía digital en el Ecuador. Se trata de demostrar la importancia de una relación entre los elementos, que eventualmente permitiría lograr una apertura comercial y económica del Ecuador a un mercado cada vez más digitalizado. Se reseña la historia de la moneda como instrumento válido para el comercio y como base de la economía hasta llegar a las criptomonedas y un breve detalle del desarrollo de la economía digital; luego se pasa a un análisis y reflexión del autor sobre la relación de estas dos variables y la oportunidad que tienen dentro de la economía ecuatoriana, que al final sustentarán las conclusiones finales del trabajo. Se trabaja bajo un modelo de investigación documental, analítico y reflexivo, seleccionando fuentes de consulta que permiten canalizar artículos científicos e informes relevantes y sitios web especializados en la temática planteada, que a su vez permiten observar no solo la realidad local, sino también identificar particularidades de lo ocurrido en otras latitudes al respecto.

The cancer syndrome polymerase proofreading-associated polyposis results from germline mutations in the POLE and POLD1 genes. Mutations in the exonuclease domain of these genes are associated with hyper- and ultra-mutated tumors with a predominance of base substitutions resulting from faulty proofreading during DNA replication. When a new variant is identified by gene testing of POLE and POLD1, it is important to verify whether the variant is associated with PPAP or not, to guide genetic counseling of mutation carriers. In 2015, we reported the likely pathogenic (class 4) germline POLE c.1373A > T p.(Tyr458Phe) variant and we have now characterized this variant to verify that it is a class 5 pathogenic variant. For this purpose, we investigated (1) mutator phenotype in tumors from two carriers, (2) mutation frequency in cell-based mutagenesis assays, and (3) structural consequences based on protein modeling. Whole-exome sequencing of two tumors identified an ultra-mutator phenotype with a predominance of base substitutions, the majority of which are C > T. A SupF mutagenesis assay revealed increased mutation frequency in cells overexpressing the variant of interest as well as in isogenic cells encoding the variant. Moreover, exonuclease repair yeast-based assay supported defect in proofreading activity. Lastly, we present a homology model of human POLE to demonstrate structural consequences leading to pathogenic impact of the p.(Tyr458Phe) mutation. The three lines of evidence, taken together with updated co-segregation and previously published data, allow the germline variant POLE c.1373A > T p.(Tyr458Phe) to be reclassified as a class 5 variant. That means the variant is associated with PPAP.

Este trabajo ha volcado su mirada a la necesidad de encontrar un nuevo posible escenario de lo que es la educación ubicua, haciendo que como parte de ese modelo se integre a los sistemas cognitivos, para orientar un entorno mucho más ajustado a las necesidades de los beneficiarios (estudiantes y profesores). Se inicia analizando el efecto de la pandemia del COVID-19 en el sistema académico, sin dejar de considerar el hecho cierto de que la tecnología está presente en este ámbito desde hace mucho tiempo, pero las condiciones presentadas desde el primer trimestre del año 2020, ha hecho que este sea un tema de creciente relevancia. La problemática se resume en el conflicto que poseen los alumnos universitarios, en cuanto a realizar las diligencias académicas en un ambiente on-line, al no poseer un modelo tecnológico que les brinde aptitudes para la elaboración de las actividades tomando en cuenta sus existentes necesidades, asimismo considerando la autonomía de elección de tiempo y lugar para atender sus responsabilidades de una forma mucho más eficiente y efectiva para el sistema. De ahí se desprende la revisión documental desarrollada y el planteamiento de una aproximación de solución, eventualmente viable.

En la actualidad, la IoT o Internet de las cosas, un concepto tecnológico que ha tomado gran fuerza, permite mitigar problemas en distintos sectores económicos. Con la constante evolución de las tecnologías de comunicación inalámbrica y la creciente tendencia de conectar cualquier elemento a Internet, y también la necesidad presente de tener controlado cualquier entorno en tiempo real, el Internet de las cosas permite brindar soluciones innovadoras utilizando tecnologías y protocolos eficientes y de última generación. LoRa, es una de las nuevas tecnologías de transmisión inalámbrica en el ámbito IoT, brinda un consumo bastan te reducido de energía permitiendo obtener un gran alcance de transmisión que supera a tecnologías como WiFi y redes celulares. Estas características combinadas con la posibilidad actual que se tiene de almacenar y centralizar cantidad de datos en servidores que estén conectados a Internet, permite el desarrollo de sistemas de monitoreo eficientes y robustos. En la presente investigación se realizó el estudio de la tecnología LoRa implementada en una solución IoT de monitoreo remoto, las ventajas que presente su implementación combinada con el uso de un servidor privado virtual y el protocolo estándar para IoT, denominado MQTT.

El sistema político es el resultado de las acciones políticas, económicas y sociales que fueron acogidas por una sociedad determinada en un momento determinado. Así, el régimen político, demuestra la materialización coyuntural de las diversas relaciones de poder, tanto políticas como económicas en diversas sociedades. Igualmente, el sistema político no sólo aborda a la totalidad de fenómenos vinculados al adjetivo político, sino a la generalidad del fenómeno. Así, entre los aspectos a considerar están los espacios donde se toman decisiones que requiere y aspira una comunidad política, los fenómenos sociales que trascienden en la conformación del poder político, las relaciones de entrada y retroalimentación, entre la clase política y su lucha por el poder, finalmente, los procesos políticos interactuantes y dependientes entre sí, por medio de reglas, así como la movilidad de las reglas siempre sujetas a cambios y externalidades que se van modificando en el tiempo. Todo esto nos conlleva a verificar que el sistema político es una forma de adaptación a la sociedad y eso lo hemos palpado en cada uno de los gobiernos de los diferentes Estados, en los cuales se han ido adoptando diversos sistemas políticos según su necesidad.

Objectives The assessment of physical function is fundamental in the management of patients with idiopathic inflammatory myopathies (IIMs). We aimed to investigate the physical function of patients with IIMs compared with those with non-IIM autoimmune rheumatic diseases (AIRDs) utilizing Patient-Reported Outcome Measurement Information System (PROMIS) Physical Function (PF) data obtained in the COVAD study, an international self-reported e-survey assessing the safety of COVID-19 vaccines in AIRDs. Methods Demographics, AIRD diagnosis, disease activity, and PROMIS PF short form-10a data were extracted from the COVAD database. PROMIS PF-10a scores were compared between disease categories and stratified by disease activity. Factors affecting PROMIS PF-10a scores other than disease activity were identified by multivariable regression analysis in patients with inactive disease. Results A total of 1057 IIM patients, 3635 non-IIM AIRD patients and 3981 healthy controls (HCs) responded to the COVAD e-survey from April to August 2021. Using a binomial regression model, the predicted mean of PROMIS PF-10a scores was significantly lower in IIM patients compared with non-IIM AIRD patients or HCs [36.3 (95% CI 35.5, 37.1) vs 41.3 (95% CI 40.2, 42.5) vs 46.2 (95% CI 45.8, 46.6), P Conclusion Physical function is significantly impaired in IIMs compared with non-IIM AIRDs or HCs, even in patients with inactive disease. Our study highlights a critical need for better strategies to minimize functional disability in patients with IIMs.

ObjectivesDisease flares in the post COVID-19 vaccination period represent a prominent concern, though risk factors are poorly understood. We studied these flares among patients with idiopathic inflammatory myopathies (IIMs) and other autoimmune rheumatic diseases (AIRDs).MethodsThe COVAD-1 and -2 global surveys were circulated in early 2021 and 2022 respectively, and we captured demographics, comorbidities, AIRDs details, COVID-19 infection history, and vaccination details.Flares of IIMs were defined as a. patient self-reported, b. immunosuppression (IS) denoted, c. clinical sign directed, and d. with >7.9-point MCID worsening of PROMISPF10a score. Risk factors of flares were analyzed using regression models.ResultsOf 15165 total respondents, 1278 IIMs (age 63 years, 70.3% female, 80.8% Caucasians), and 3453 AIRDs were included. Flares of IIM were seen in 9.6%, 12.7%, 8.7%, and 19.6% patients by definitions a-d respectively with a median time to flare of 71.5 (10.7-235) days, similar to AIRDs. Patients with active IIMs pre-vaccination (OR:1.2; 95%CI:1.03-1.6, p = 0.025) were prone to flares, while those receiving Rituximab (OR:0.3; 95%CI:0.1-0.7, p = 0.010) and Azathioprine (OR:0.3, 95%CI:0.1-0.8, p = 0.016) were at lower risk. Female gender and comorbidities predisposed to flares requiring changes in immunosuppression. Asthma (OR: 1.62; 95%CI: 1.05-2.50, p = 0.028) and higher pain VAS (OR: 1.19; 95%CI: 1.11-1.27, p ConclusionA diagnosis of IIMs confers an equal risk of flares in the post COVID-19 vaccination period to AIRDs, with active disease, female gender, and comorbidities conferring a higher risk. Disparity between patient and physician reported outcomes represents a future avenue for exploration.

Resumen La terapia basada en incretinas lleva al control glucemico de una manera dependiente de glucosa con un bajo riesgo de hipoglucemia, por lo que resulta atractiva para su uso hospitalario. El objetivo de esta revision sistematica fue evaluar los beneficios de la terapia basada en incretinas en pacientes con diabetes tipo 2 hospitalizados fuera de la unidad de cuidados intensivos. Se buscaron estudios publicados hasta agosto de 2021 en las bases de datos PubMed y Scopus. Se seleccionaron los ensayos clinicos que comparaban la terapia basada en incretinas (sola o en combinacion con insulina) versus el regimen con insulina. Los resultados de los estudios incluidos mostraron que la terapia basada en incretinas registro un promedio de glucosa sanguinea, un porcentaje de registros dentro de meta terapeutica y un porcentaje de falla al tratamiento similar al manejo con insulina, particularmente en pacientes con hiperglucemia leve a moderada. Ademas, el tratamiento basado en incretinas se asocio con una menor dosis total de insulina y una menor incidencia de hipoglucemia. En conclusion, la terapia basada en incretinas logro un control glucemico similar al tratamiento con insulina en los pacientes con diabetes tipo 2 hospitalizados fuera de la unidad de cuidados intensivos, con la ventaja de disminuir el requerimiento insulinico y con menor riesgo de hipoglucemia.

Los robots SCARA (Selective Compliance Assembly Robot Arm) se utilizan para trabajos de paletizado ya que son adecuados para el montaje y el posicionamiento en plano. En este artículo, nuestro objetivo es obtener un modelo dinámico de un robot SCARA de 3GDL haciendo uso de redes neuronales recurrentes (RNN). A partir de los antecedentes, se optó por utilizar un modelamiento no paramétrico, para evitar las pérdidas de algunas características del modelo obtenido tales como la fricción y el desgaste en los actuadores con tal de mejorar la precisión del robot. El flujo de datos se obtendrá a partir del simulador robótico CoppeliaSim. Luego, se deben tener los datos de entrada y salida para la red neuronal, efectuar un preprocesamiento y normalización de los datos para el entrenamiento. Se hará uso de las librerías existentes en el lenguaje de programación de alto nivel Python para desarrollar un algoritmo de identificación por redes neuronales recurrentes basado en el tipo de aprendizaje por retropropagación. Además, se realizará la validación y prueba correspondiente. Como producto final se espera tener un modelo que pueda representar el sistema físico con gran precisión, de tal forma que se puedan diseñar estrategias de control con buen rendimiento.

While multiple alternative and synergistic approaches including heterologous vaccination, booster doses, and antiviral drugs have been developed, equitable vaccine uptake remains the foremost strategy to manage pandemic. Although none of the currently approved vaccines are live-attenuated, several reports of disease flares, waning protection, and acute-onset syndromes have emerged as short-term adverse events after vaccination. Hence, scientific literature falls short when discussing potential long-term effects in vulnerable cohorts. The COVAD-2 survey follows on from the baseline COVAD-1 survey with the aim to collect patient-reported data on the long-term safety and tolerability of COVID-19 vaccines in immune modulation. The e-survey has been extensively pilot-tested and validated with translations into multiple languages. Anticipated results will help improve vaccination efforts and reduce the imminent risks of COVID-19 infection, especially in understudied vulnerable groups. Hyperglycemia in hospitalized patients, either with or without diabetes, is a common, serious, and costly healthcare problem. Evidence accumulated over 20 years has associated hyperglycemia with a significant increase in morbidity and mortality, both in surgical and medical patients. Based on this documented link between hyperglycemia and poor outcomes, clinical guidelines from professional organizations recommend the treatment of hospital hyperglycemia with a therapeutic goal of maintaining blood glucose (BG) levels less than 180 mg/dL. Insulin therapy remains a mainstay of glycemic management in the inpatient setting. The use of non-insulin antidiabetic drugs in the hospital setting is limited because little data are available regarding their safety and efficacy. However, information about the use of incretin-based therapy in inpatients has increased in the past 15 years. This review aims to summarize the different treatment strategies for hyperglycemia in hospitalized noncritical patients that are supported by observational studies or clinical trials with insulin and non-insulin drugs. In addition, we propose a protocol to help with the management of this important clinical problem.

Numerous repositioned drugs have been sought to decrease the severity of SARS-CoV-2 infection. It is known that among its physicochemical properties, Ursodeoxycholic Acid (UDCA) has a reduction in surface tension and cholesterol solubilization, it has also been used to treat cholesterol gallstones and viral hepatitis. In this study, molecular docking was performed with the SARS-CoV-2 Spike protein and UDCA. In order to confirm this interaction, we used Molecular Dynamics (MD) in “SARS-CoV-2 Spike protein-UDCA”. Using another system, we also simulated MD with six UDCA residues around the Spike protein at random, naming this “SARS-CoV-2 Spike protein-6UDCA”. Finally, we evaluated the possible interaction between UDCA and different types of membranes, considering the possible membrane conformation of SARS-CoV-2, this was named “SARS-CoV-2 membrane-UDCA”. In the “SARS-CoV-2 Spike protein-UDCA”, we found that UDCA exhibits affinity towards the central region of the Spike protein structure of − 386.35 kcal/mol, in a region with 3 alpha helices, which comprises residues from K986 to C1032 of each monomer. MD confirmed that UDCA remains attached and occasionally forms hydrogen bonds with residues R995 and T998. In the presence of UDCA, we observed that the distances between residues atoms OG1 and CG2 of T998 in the monomers A, B, and C in the prefusion state do not change and remain at 5.93 ± 0.62 and 7.78 ± 0.51 Å, respectively, compared to the post-fusion state. Next, in “SARS-CoV-2 Spike protein-6UDCA”, the three UDCA showed affinity towards different regions of the Spike protein, but only one of them remained bound to the region between the region's heptad repeat 1 and heptad repeat 2 (HR1 and HR2) for 375 ps of the trajectory. The RMSD of monomer C was the smallest of the three monomers with a value of 2.89 ± 0.32, likewise, the smallest RMSF was also of the monomer C (2.25 ± 056). In addition, in the simulation of “SARS-CoV-2 membrane-UDCA”, UDCA had a higher affinity toward the virion-like membrane; where three of the four residues remained attached once they were close (5 Å, to the centre of mass) to the membrane by 30 ns. However, only one of them remained attached to the plasma-like membrane and this was in a cluster of cholesterol molecules. We have shown that UDCA interacts in two distinct regions of Spike protein sequences. In addition, UDCA tends to stay bound to the membrane, which could potentially reduce the internalization of SARS-CoV-2 in the host cell.

Este trabajo muestra al lector una aproximación de la relación entre la computación ubicua y la educación, misma que define al modelo de educación ubicua, un paradigma académico que aprovecha la omnipresencia de la tecnología puesta al servicio de las personas. El horizonte que de una u otra manera se plantea, muestra una problemática que pudiera ser considerada para un proceso de investigación que profundice en las capacidades de los sistemas, entre ellos informáticos, computacionales, académicos, cognitivos y dentro de estos últimos, la integración de la inteligencia artificial, la minería de datos académicos, todo en comunión con sistemas de gestión del aprendizaje. Metodológicamente, este documento se basa en la revisión de un amplio compendio de documentos que luego de una valoración cualitativa, han sido considerados como relevantes, se llega al planteamiento de una discusión que deberá desembocar en una o varias líneas de investigación al respecto. Al final se plantean conclusiones, que desde la perspectiva del autor son prudentes de considerar.

Objectives The COVID-19 vaccination in autoimmune diseases (COVAD) study aimed to assess short-term COVID-19 vaccination-related adverse events (AEs) in RA patients. Methods An online self-reported questionnaire (March–December 2021) was used to capture data related to COVID-19 vaccination-related AEs in RA, other autoimmune rheumatic diseases (AIRDs) (excluding RA and inflammatory myositis), non-rheumatic autoimmune diseases (nrAIDs) and healthy controls (HCs). Descriptive and multivariable regression analyses were performed. Results Of the 9462 complete respondents, 14.2% (n = 1347) had been diagnosed with RA; they had a mean (s.d.) age of 50.7 (13.7) years, 74.2% were women and 49.3% were Caucasian. In total, 76.9% and 4.2% of patients with RA reported minor and major AEs, respectively. Patients with active and inactive RA had similar AE and hospitalization frequencies. Overall, AEs were reported more frequently by BNT162b2 and mRNA-1273 recipients and less frequently by BBV152 recipients compared with the rest. Major AE and hospitalization frequencies were similar across recipients of different vaccines. Patients receiving methotrexate and hydroxychloroquine reported fewer minor AEs than those patients not on them. Compared with HCs and patients with other AIRDs, patients with RA reported similar total AEs, overall minor AEs, and hospitalizations. Compared with nrAIDs, patients with RA reported lower frequencies of overall AEs, minor AEs (both odds ratio [OR] = 0.7; 95% CI: 0.5, 0.9), and injection site pain (OR = 0.6; 95% CI: 0.5, 0.8) with similar major AE and hospitalization frequencies. Conclusion Despite the differences in AE frequency across different COVID-19 vaccines, all were well tolerated in patients with RA and were comparable to HCs, providing reassurance as to the safety of COVID-19 vaccination.
