A 14-month-old boy with swelling around the eyes and low protein levels in the blood was found to have an intolerance for cow's milk. He did not present any of the common symptoms for milk intolerance, such as diarrhea or vomiting. Aside from the swelling around his eyes, his physical examination was normal. His serum levels of protein and albumin were abnormally low at 31 g/L and 260.8 micromoles/L. Normal ranges for these levels are 61-79 g/L for protein, and 565.1-724.5 micromoles/L for albumin. He consumed 24 to 30 oz of cow's milk per day, and his solid diet consisted largely of dairy products. Within 3 months of eliminating milk from his diet, his protein and albumin level became normal, and his eye swelling went down. One should suspect milk intolerance in patients with low blood-protein levels.

Ameloblastoma is a true neoplasm of odontogenic epithelial origin. It is the second most common odontogenic neoplasm, and only odontoma outnumbers it in reported frequency of occurrence. Vaginal hydroceles are very common in the northeastern part of India at the residential area of our patient, but abdominoscrotal hydrocele is a relatively rare condition. Most of the abdominoscrotal hydroceles reported in literature are single case reports, because surgeons come across one or two cases in their career. Here we report a case of unilateral huge abdominoscrotal swelling extending up to the umbilical level, with constriction at the middle, causing varicosity of the veins of the left lower limb with atrophy of testis, containing chocolate coloured fluid within. Burkitt's lymphoma is Burkitt's an aggressive non-Hodgkin's lymphomas (NHL). It occurs most often in immunocompromised patients. It typically affects those patients with a high CD4 count and patients often have a high serum lactate dehydrogenase (LDH) levels. The patient is a 33 year old male who presented with progressive left thigh swelling. He has been treated with highly active antiretroviral therapy (HAART) therapy for several years .Patient's cat scan (CT)showed evidence of a soft tissue mass which was confirmed by an incisional biopsy. The patient once diagnosed underwent intensive chemotherapy including hyper-CVAD chemotherapy regimen. His thigh swelling responded well to this treatment. Patient's hospital course was complicated by tumor lysis syndrome which required four days of dialysis treatment. Pathogenesis, diagnosis and therapy of AIDS associated Burkitt's lymphoma are reviewed for this unusual presentation of Burkitt's lymphoma. Vulvar edema is the accumulation of fluid in the tissues of the vulva, the external portion of the vagina. It may result from injury, infection, or the blockage of the lymphatic or blood vessels. Vulvar edema may also be associated with conditions that cause edema in the feet, such as heart, kidney, and liver failure or cancer; complications of pregnancy, such as preeclampsia or prolonged labor; or the use of a birthing chair, in which body position and resulting pressure may contribute to swelling. A case is described of a pregnant 20-year-old woman who developed vulvar edema during tocolysis, the inhibition of uterine contractions. The patient went into premature labor at 34 weeks of pregnancy and was given the drugs ritodrine and magnesium sulfate intravenously to prevent uterine contractions. Vulvar edema developed on the right side of the vulva five days after starting drug therapy, and spread to the left side of the vulva within the following 24 hours. The patient did not develop a fever, maintained a normal blood pressure, and had no evidence of infection or general edema. The vulvar edema did not improve after treatment with antibiotics or care of the affected skin. The edema had blocked off the opening of the vagina, preventing examination, nine days after admission to the hospital. A cesarean section was performed and a live female infant was delivered. The patient recovered without complications and the vulvar edema resolved within four days. In this case, the vulvar edema was thought to result from the prolonged use of tocolytic agents, which caused fluid retention. In addition, the position of the patient during prolonged bed rest most likely contributed to the fluid accumulation at the vulva. (Consumer Summary produced by Reliance Medical Information, Inc.)
