The role of clinical researchers is vital to cancer progress. The teaching, research, and leadership roles that academic oncologists hold need to be accounted for and appropriately compensated. National metrics are currently inexistent, but are necessary to move the oncology research field forward. Clinical research and routine clinical care must be harmoniously integrated without competing. This article reviews the national landscape of clinical cancer research and proposes a call for action.

Abstract

ObjectiveLung-MAP (SWOG S1400) is a master platform trial assessing targeted therapies in squamous NSCLC. The objective of study C (S1400C) was to evaluate the response rate to palbociclib, a cyclin-dependent kinase 4 and cyclin-dependent kinase 6 inhibitor, in patients with cell cycle gene abnormalities.MethodsPatients with squamous NSCLC, a performance status of 0 to 2, and normal organ function who had progressed after at least one prior platinum-based chemotherapy with cyclin-dependent kinase 4 gene (CDK4) or cyclin D1 gene (CCND1), cyclin D2 gene (CCND2), or cyclin D3 gene (CCND3) amplifications on tumor specimens were eligible. The study was originally designed as a phase II/III trial comparing palbociclib with docetaxel, but it was modified to a single-arm phase II trial with the primary end point of response when immunotherapy was approved. If two or fewer responses were seen in the first 20 patients, then the study would cease enrollment.ResultsA total of 88 patients (9% of patients screened) were assigned to S1400C, and 53 patients enrolled (including 17 to receive docetaxel). One patient who had been registered to receive docetaxel was re-registered to receive palbociclib after progression while taking docetaxel. The frequencies of cell cycle gene alterations in the eligible patients taking palbociclib (n = 32) were as follows: CCND1, 81% (n = 26); CCND2, 9% (n = 3); CCND3, 6% (n = 2); and CDK4, 3% (n = 1). In all, 32 eligible patients received palbociclib. There were two partial responses (response rate 6% [95% confidence interval (CI): 0%-15%]), both with CCND1 amplification. Twelve patients had stable disease (38% [95% CI: 21%-54%]). The median progression-free survival was 1.7 months (95% CI: 1.6-2.9 months) and the median overall survival was 7.1 months (95% CI: 4.2-12.5).ConclusionPalbociclib as monotherapy failed to demonstrate the prespecified criteria for advancement to phase III testing.

Abstract

Daily and seasonal fluctuations in environmental conditions can significantly impact the survivorship and reproductive success of animals by altering energetic costs and predation risks. Behaviorally, animals often respond to changes in their environment through regulation of acthity and associated movement patterns. We used accelerometer-informed GPS telemetry to assess nightly activity and movement patterns in response to environmental conditions in a small endotherm, the eastern spotted skunk (Spilogale putorius). Nightly movements and activity of skunks were monitored in association with ambient temperature, precipitation, and moon illumination during late winter through summer in Alabama. Eastern spotted skunks were strictly nocturnal, exhibiting almost no daytime activity. Total time active and total distance moved each night increased with ambient temperature and rainfall. Variation in moon illumination, which may affect predation risk, did not impact skunk nightly activity or movement. Eastern spotted skunks likely reduce predation risk by being active between sunset and sunrise when they are less visible to predators. Lower activity and movement at cooler temperatures significantly reduces thermoregulatory costs for small endotherms. Increased activity of skunks during or shortly after precipitation may be driven by increased prey availability. Our findings indicate that small endotherms, like the eastern spotted skunk, rely extensively on behavioral thermoregulation, instead of physiological adaptation, to buffer themselves against changing environmental conditions. Abstract

Introduction: Aggressive end-of-life (EOL) care for cancer patients can lead to increased hospitalizations and worse quality of death, while goals of care (GOC) discussions including EOL care conversations are associated with fewer hospitalizations and increased hospice use. During hematology–oncology training, fellows should develop communication skills that include eliciting and documenting patients' GOC to provide quality care during EOL. We aimed to determine the frequency of documentation of GOC discussions in fellow's clinics as well as characteristics of EOL care. Methods: This study was conducted at an academic cancer center where year 1–3 fellows retrospectively reviewed patient medical records from July 2016 to June 2017 to identify patient deaths and collect information on hospitalizations, treatment, and place of death to analyze relationships with GOC discussions. Results: Out of 103 patient deaths, 48 (47%) had documented GOC discussions, 69 (67%) patients were enrolled on hospice, and 20 (19%) had an advance directive. GOC discussions were associated with higher hospice enrollment and advance directive documentation and lower hospitalizations. Conclusions: All fellows had at least one patient who died in their patient panels, but less than half of patients had documented GOC discussions. Fellowship programs should consider incorporating quality improvement measures and communication skills training to ensure fellows have competence in GOC communication and EOL care delivery. Abstract

This is the first study to investigate the interrelationship of social anxiety with the variables anxiety, depression, locus of control, ego strength and ways of coping in a sample of university students. There were high scores of social anxiety which were related to high scores on measures of anxiety and depression, low ego strength, external locus of control and emotion coping rather than problem focused coping. While the results are relational rather than being predictive of causality they raise a number of issues with regard to theory, prevention, intervention and promoting mental health within a university setting. Abstract

Non-small cell lung cancer constitutes 85% to 90% of lung cancer and is the most common cause of cancer death. Abstract

Neuroscientists studying the neural correlates of mouse behavior often lack access to the brain-wide activity patterns elicited during a specific task of interest. Fortunately, large-scale imaging is becoming increasingly accessible thanks to modalities such as Ca2+ imaging and functional ultrasound (fUS). However, these and other techniques often involve challenging cranial window procedures and are difficult to combine with other neuroscience tools. We address this need with an open-source 3D-printable cranial implant—the COMBO (ChrOnic Multimodal imaging and Behavioral Observation) window. The COMBO window enables chronic imaging of large portions of the brain in head-fixed mice while preserving orofacial movements. We validate the COMBO window stability using both brain-wide fUS and multisite two-photon imaging. Moreover, we demonstrate how the COMBO window facilitates the combination of optogenetics, fUS, and electrophysiology in the same animals to study the effects of circuit perturbations at both the brain-wide and single-neuron level. Overall, the COMBO window provides a versatile solution for performing multimodal brain recordings in head-fixed mice. Whole-brain optical imaging approaches are often applied through a cranial window but are limited by their low spatio-temporal resolution. 50. The Implementation of a Video-Enhanced Aikido-Based School Violence Prevention Training Program To Reduce Disruptive and Assaultive Behaviors among Severely Emotionally Disturbed Adolescents.

Abstract

The martial art of Aikido was used as an intervention with 15 middle and high school students with severe emotional disturbances in an alternative educational setting. Students with an extensive history of violently disruptive and assaultive behaviors were trained for 12 weeks in this nonviolent Japanese martial art in order to achieve the following outcomes: reduce disruptive classroom behaviors and verbally abusive and physically assaultive behaviors toward other students and staff members, and decrease the number of school-wide disciplinary referrals for violent behaviors. The training focused on peaceful conflict resolution, nonviolent self-defense, individual and group confrontation management, and autogenic relaxation training. Significant improvements were achieved in reducing violent behaviors during the training sessions, and these positive behaviors transferred into other classrooms. Parents and administrators also noted positive behaviors exhibited by student participants, including enhanced respect for authority and peers, improved conflict de-escalation skills, and enhanced feelings of confidence and self-worth. It was found that Aikido training can be implemented with a minimum of scheduling disruptions and financial expenditure. Appended is a 12-week behavioral summary chart. (Contains 46 references.)
