Abstract

Aims: A substantial proportion of patients with heart failure have preserved left ventricular ejection fraction (HF-PEF). Previous studies have reportedmixed resultswhether survival is similar to those patientswith heart failure and reduced EF (HF-REF). Methods and results: We compared survival in patients with HF-PEF with that in patients with HF-REF in a meta-analysis using individual patient data. Preserved EF was defined as an EF ≥ 50%. The 31 studies included 41 972 patients: 10 347 with HF-PEF and 31 625 with HF-REF. Compared with patients with HF-REF, those with HF-PEF were older (mean age 71 vs. 66 years), were more often women (50 vs. 28%), and have a history of hypertension (51 vs. 41%). Ischaemic aetiology was less common (43 vs. 59%) in patients with HF-PEF. There were 121 [95% confidence interval (CI): 117, 126] deaths per 1000 patient-years in those with HF-PEF and 141 (95% CI: 138, 144) deaths per 1000 patient-years in those with HF-REF. Patients with HF-PEF had lower mortality than those with HF-REF (adjusted for age, gender, aetiology, and history of hypertension, diabetes, and atrial fibrillation); hazard ratio 0.68 (95% CI: 0.64, 0.71). The risk of death did not increase notably until EF fell below 40%. Conclusion: Patients with HF-PEF have a lower risk of death than patients with HF-REF, and this difference is seen regardless of age, gender, and aetiology of HF. However, absolute mortality is still high in patients with HF-PEF highlighting the need for a treatment to improve prognosis.

Abstract

With the entry of the Ballast Water Management Convention into force, ballast water discharged from ships must meet standards limiting the concentrations of living organisms. Monitoring devices to confirm compliance with these standards would ideally be portable, easy-to-use instruments capable of rapid and accurate shipboard analysis of ballast water. Following a framework established for the validation of such potential devices, six devices were evaluated in a series of laboratory and field tests at three contrasting coastal locations. Devices were designed to quantify organisms in the = 10 and < 50 µm size class. In all cases, the compliance monitoring devices were compared to the agreed-upon performance standard for quantifying living organisms: microscopy-based, vital fluorophore approach. Specific results from these validations are available elsewhere, although examples are shown to demonstrate the analytical and statistical approaches used for gauging--with data analysis and statistical approaches--each device's performance. Each metric used to evaluate devices (e.g., linearity, precision, and accuracy) was informative. However, linearity between the microscopy-based method and the compliance devices, especially along a large range of organism concentrations, would not be suitable for establishing performance criteria. Concentrations well below or above the limit for this size class (10 living organisms mL-1) would be easily categorized as meeting or exceeding discharge standard and their values do not need to be well constrained and pinpointed. Precision, when measured as the coefficient of variation, was sensitive to the dimensions and scale of the devices' measurements, as certain devices calculated and reported cell concentrations, whereas other devices reported non-dimensional values along a wide dynamic range. Accuracy, defined as the agreement between the compliance device and the standard approach as to whether the sample met or exceeded the discharge standard, was measured by logistic regression analysis. Following this analysis, the likelihoods of detecting exceedances based upon cell concentration were calculated for each field site and cultured test organism. Accuracy was useful in defining the likelihood of correctly identifying an exceedance, and these likelihoods could be calculated for a range of cell concentrations. The concurrent testing of multiple compliance devices minimized the analysis burden as well as the logistical hurdles associated with field testing at multiple--for this study, three--locations. Eventually, the test procedures could be modified to measure variation among different units of the same device or applied to actual measurements of ballast water rather than communities of ambient organisms or cultured microalgae. Abstract

Background: Anaemia is common among patients with heart failure (HF) and is an important prognostic marker. Aim: We sought to determine the prognostic importance of anaemia in a large multinational pooled dataset of prospectively enrolled HF patients, with the specific aim to determine the prognostic role of anaemia in HF with preserved and reduced ejection fraction (HF-PEF and HF-REF, respectively). Design: Individual person data meta-analysis. Methods: Patients with haemoglobin (Hb) data fromthe MAGGIC dataset were used. Anaemia was defined as Hb < 120 g/l in women and <130 g/l inmen. HF-PEF was defined as EF ≥ 50%; HF-REF was EF < 50%. Cox proportional hazardmodelling, with adjustment for clinically relevant variables, was undertaken to investigate factors associated with 3-year all-causemortality. Results: Thirteen thousand two hundred and ninety-five patients with HF from 19 studies (9887 with HF-REF and 3408 with HF-PEF). The prevalence of anaemia was similar among those with HF-REF and HF-PEF (42.8 and 41.6% respectively). Compared with patients with normal Hb values, those with anaemia were older, were more likely to have diabetes, ischaemic aetiology, New York Heart Association class IV symptoms, lower estimated glomerular filtration rate and were more likely to be taking diuretic and less likely to be taking a beta-blocker. Patients with anaemia had higher all-cause mortality (adjusted hazard ratio [aHR] 1.38, 95% confidence interval [CI] 1.25-1.51), independent of EF group: aHR 1.67 (1.39-1.99) in HFPEF and aHR 2.49 (2.13-2.90) in HF-REF. Conclusions: Anaemia is an adverse prognostic factor in HF irrespective of EF. The prognostic importance of anaemia was greatest in patients with HF-REF.

Abstract

A substantial proportion of patients with heart failure have preserved left ventricular ejection fraction (HF-PEF). Previous studies have reported mixed results whether survival is similar to those patients with heart failure and reduced EF (HF-REF). We compared survival in patients with HF-PEF with that in patients with HF-REF in a meta-analysis using individual patient data. Preserved EF was defined as an EF epsilon 50. The 31 studies included 41 972 patients: 10 347 with HF-PEF and 31 625 with HF-REF. Compared with patients with HF-REF, those with HF-PEF were older (mean age 71 vs. 66 years), were more often women (50 vs. 28), and have a history of hypertension (51 vs. 41). Ischaemic aetiology was less common (43 vs. 59) in patients with HF-PEF. There were 121 [95 confidence interval (CI): 117, 126] deaths per 1000 patient-years in those with HF-PEF and 141 (95 CI: 138, 144) deaths per 1000 patient-years in those with HF-REF. Patients with HF-PEF had lower mortality than those with HF-REF (adjusted for age, gender, aetiology, and history of hypertension, diabetes, and atrial fibrillation); hazard ratio 0.68 (95 CI: 0.64, 0.71). The risk of death did not increase notably until EF fell below 40. Patients with HF-PEF have a lower risk of death than patients with HF-REF, and this difference is seen regardless of age, gender, and aetiology of HF. However, absolute mortality is still high in patients with HF-PEF highlighting the need for a treatment to improve prognosis.

Abstract

Westinghouse has developed a thin film electroluminescent (TFEL) edge emitting imaging station for use in electrophotographic printing. The physical and electrical drive characteristics offer unique opportunities for gray-scale printing and resolution enhancement. The physical dimension of the light-emitting element in the process direction is less than to micrometers . In addition, the light output of the edge-emitting array is not Lambertian but, rather, is highly directed. The majority of light is emitted in a wedge with a 30 deg half angle measured form the plane of the device. The array is activated by an ac waveform. Light emission occurs as the field across the device changes direction. The rise time to peak light emission is a few microsecond(s) and the decay is a few hundred microsecond(s) . This allows an array element to be turned on and off several times during the exposure of a single 300 dot-per-inch pixel in a low-speed printer. This paper provides a detailed description of the physical dimensions, electronic drive system, and light emission characteristics of edge-emitting array. Abstract

A full size 400 dpi Edge Emitter array was designed and constructed. The light-emitting edge is 8. 18 inches long capable of printing across the full width of the page. An Edge Emitter head was assembled consisting of three components: a 400 dpi Edge Emitter array a circuit board containing the channel driver ICs and a SELFOC R lens array in a frame. The frame also provided the means of mounting in a 6 ppm electrophotographic printer. Abstract

The Alliance for Coastal Technologies (ACT) has been established to support innovation and to provide the information required to select the most appropriate tools for studying and monitoring coastal and ocean environments. ACT is a consortium of nationally prominent ocean science and technology institutions and experts who provide credible performance data of these technologies through third-party, objective testing. ACT technology verifications include laboratory and field tests over shortand long-term deployments of commercial technologies in diverse environments to provide unequivocal, unbiased confirmation that technologies meet key performance requirements. ACT demonstrations of new technologies validate the technology concept and help eliminate performance problems before operational introduction. ACT's most recent demonstration of pCO2 sensors is an example of how ACT advances the evolution of ocean observing technologies, in this case to address the critical issue of ocean acidification, and promotes more informed decision making on technology capabilities and choices.
