BACKGROUND: Nutritional status is a predictor of the prognosis of cardiovascular diseases. The association between the Prognostic Nutritional Index (PNI), which is an immunonutritional parameter, and cardiovascular diseases has been extensively studied in the literature. OBJECTIVES: The aim of this study was to investigate whether PNI is associated with coronary collateral development. METHODS: This retrospective study included 172 patients with chronic total occlusion. The patients were diagnosed with stable coronary artery disease, and all patients underwent coronary angiography. PNI was calculated using serum albumin level and lymphocyte count. Collateral circulation was classified according to Rentrop grade. RESULTS: There was a positive correlation between PNI and Rentrop grade (r = 0.168, p = 0.026) and a negative correlation between C-reactive protein and PNI (r = -0.353, p < 0.001). Multivariate logistic regression analysis showed that uric acid and PNI were independent predictors of Rentrop grade (p = 0.008 and p = 0.037, respectively). CONCLUSIONS: This study showed that PNI, which can easily be calculated using serum albumin level and lymphocyte count, was a predictor of coronary collateral development in terms of Rentrop grade.

Bu çalışmada problemli pornografi kullanımı deneyiminde psikolojik iyi oluşta duygu düzenleme ve ahlaki tutarsızlığın rolü araştırılması amaçlanmıştır. Bu amaçla karma modellerden keşfedici sıralı desen kullanılmıştır. Araştırmanın nitel aşamasında üç kişiyle derinlemesine görüşmeler yapılmış ve veriler Giorgi’nin betimleyici fenomenolojik analiz yöntemiyle analiz edilmiştir. Buna göre problemli porno kullanımını deneyiminin psikolojik yapısı çıkarılmıştır. Bu bileşenler 1) porno ile ilk tanışma ve kullanımın başlaması, 2) pornoya karşı tutumu şekillendiren perspektifler, 3) kullanım motivasyonları, 4) kullanıma etki eden faktörler, 5) porno kullanımını problemli olarak görmeye başlama ve 6) problemli pornografi kullanımına karşı yapılan girişimler olarak bulunmuştur. Araştırmanın nicel aşamasında ise problemli porno kullanımının açıklanmasında son yıllarda öne çıkmış bir model olan Ahlaki Tutarsızlığa Bağlı Pornografi Problemleri Modeli (Pornography Problems Due to Moral Incongruence Model) yapısal eşitlik modeline göre incelenmiştir. Bunun için nitel aşamadan elde edilen verilerden ve literatürden yararlanılarak problemli porno kullanımına dair bir soru havuzu oluşturulmuştur. Uzman görüşünün ardından son hali verilen ölçek açımlayıcı ve doğrulayıcı faktör analizlerinin ardından 13 maddelik Problemli Porno Kullanım Ölçeği oluşturulmuştur. Benzer şekilde dört maddelik Pornoya Karşı Ahlaki Onaylamama Ölçeği geliştirilmiştir. Geliştirilen ölçekler, duygu düzenlemede öz yeterlilik ve psikolojik iyi oluş ölçeği, kullanıma ve demografik özelliklere dair sorularla birlikte 18 yaş üstündeki 143 kadın, 249 erkek, 3 diğer ve 3 de cinsiyetini belirtmek istemiyorum diyen toplam 398 kişiye uygulanmıştır. Böylece ahlaki uyumsuzluğa bağlı pornografi sorunları modelinin üç bileşeni olan duygu disregülasyonu ve dürtü kontrolündeki bireysel farklar, kullanım alışkanlıkları ve ahlaki onaylamamanın psikolojik iyi oluş üzerindeki etkilerine bakılmıştır. Kullanım süresiyle oluşturulan yapısal eşitlik modeli sonuçlarına göre duygu düzenlemede öz yeterliliğin, porno kullanım süresine, problemli porno kullanımına ve psikolojik iyi oluşa etki ettiği görülmüştür. Yine porno kullanım süresinin de hem pornoya karşı ahlaki onaylamamaya hem de problemli porno kullanımına etki ettiği görülmüştür. Pornoya karşı ahlaki onaylamamanın da problemli porno kullanımına etki ettiği bulunmuştur. Problemli porno kullanımının da psikolojik iyi oluş üzerinde etkisinin olduğu gözlenmiştir. Porno kullanım sıklığıyla oluşturulan yapısal eşitlik modelinde ise ilk modelden farklı olarak duygu düzenlemenin porno kullanım sıklığı üzerindeki anlamlı bir etkisi bulunmamıştır. Bu haliyle bakıldığında birinci modelin de ahlaki tutarsızlığa bağlı pornografi problemleri modelini onayladığı ikinci modelin ise kısmen onayladığı görülmektedir. Problemli porno kullanımı ile diğer ilişkili nicel bulgulara bakıldığında erkek, bekar ve cinsel hayatı olmayanlarda problemli porno kullanım puanları daha yüksektir. Yine yaş ve eğitim düzeyi düştükçe PPK puanları artmaktadır. Benzer şekilde son altı ayda pornografi kullanımları dolayısıyla sıkıntı yaşama puanı erkek, bekar, cinsel hayatı olmayan, öğrenci ve çalışmayan kişilerde daha yüksektir. Genel olarak daha genç yaş gruplarında sıkıntı yaşayan kişilerin oranları diğer gruplarındakinden anlamlı derecede yüksektir. Gruplara ayrılmamış yaş değişkeniyle son altı ayda sıkıntı yaşama puanıyla negatif yönde zayıf bir korelasyonu söz konusudur. Yine eğitim olarak yaş gruplarının içerisinde sıkıntı yaşadım diyenlerin oranlarının en yüksek olduğu grup lise mezunu ve altı olan gruptur. Bu oran eğitim seviyesi daha yüksek gruplarda eğitim düzeyinin artışına paralel olarak düşmektedir. In this study, it is aimed to investigate the role of emotion regulation and moral incongruence in psychological well-being in the experience of problematic pornography use. For this purpose, exploratory sequential design of mixed models were conducted. In the qualitative part of the research, in-depth interviews were conducted with three people and the data were analysed by Giorgi's descriptive phenomenological analysis method. Accordingly, the psychological structure of the experience of problematic porn use is extracted. These components were found as 1) the first exposure with porn and the beginning of its use, 2) the perspectives that shape the attitude towards porn, 3) the motivations for use, 4) the factors affecting the use, 5) the beginning of perceiving porn use as problematic and 6) the attempts against the problematic use of pornography.In the quantitative part of the research, the Pornography Problems Due to Moral Incongruence Model, which is a prominent model in recent years in explaining problematic porn use, was examined according to the structural equation model. For this, a pool of questions on problematic porn use was created by using the data obtained from the qualitative stage and the literature. Following the expert opinion, the 13-item Problematic Porn Use Scale was created after the exploratory and confirmatory factor analyses. Similarly, the 4-item Scale of Moral Disapproval of Pornography was developed. The scales developed, regulatory emotional self-efficacy and psychological well-being scale, together with questions about use and demographic characteristics, were applied to a total of 398 people over the age of 18 consist of 143 women, 249 men, 3 other and 3 people not defined their gender. Thus, the effects of the three components of the Pornography Problems Due to Moral Incongruence Model -emotional regulation, habits of use, and the moral disapproval- on psychological well-being were examined. According to the results of the structural equation model created with the duration of use, it was seen that self-efficacy in emotion regulation had an effect on porn use, problematic porn use and psychological well-being. Again, the duration of porn use has been shown to affect both moral disapproval against porn and problematic porn use. Moral disapproval of porn has also been found to have an impact on problematic porn use. Problematic porn use has also been observed to have an effect on psychological well-being. In the structural equation model created by the frequency of porn use, unlike the first model, emotion regulation did not have a significant effect on the frequency of porn use. As such, it is seen that the first model approves the model of pornography problems due to moral inconsistency, while the second model partially approves.The other quantitative findings associated with problematic porn use, problematic porn use scores are higher in male, single with non-sexual life. Again, as the age and education level decreases, PPU scores increase. Similarly, in the last six months, the score of experiencing distress due to pornography use is higher in male, single, non-sexual life, student and non-working people. In general, the proportion of people experiencing distress in younger age groups is significantly higher than in other groups. There is a weak and negative correlation with the ungrouped age variable with the score of experiencing distress in the last six months. Again, the group with the highest rates of those who admit they have experienced difficulties was the group with high school graduation and below. The rate of having experienced distress decreases in parallel with the increase in the level of education in groups with higher levels of education.

Diophantine set theory has an importence role in Mathematics.In this paper,we consider prime number p=+53 and give some Diophantine P+53 triples. Some of the such sets are extended but others not. We give several of them with proofs. Also, some types of elements of the Diophantine P+53 m-tubles are determined. One can be work on other Diophantine P+53 - m tubles and discover extendibility of them.

Turizm sektörünün gelişmesi ile insanların istek ve ihtiyaçları da değişmiştir. Bununla birlikte sağlık turizmi de bu istek ve ihtiyaçlarla birlikte gelişme göstermiştir. Sağlık turizminin gelişmesindeki en büyük etki, insanların yüksek kalite düşük fiyat politikası izleyen sağlık hizmetlerini tercih etmesi olmuştur. Sağlık turizmi genel kabul edilen görüşe göre dört başlığa ayrılmaktadır. Medikal turizm, termal turizm, Selus Per Aqua (SPA) - Welness turizmi ve ileri yaş turizmi/engelli turizmi olarak gruplandırılmaktadır. Medikal turizm ile sağlık turizmi birbiriyle karıştırılsa da farklı öneme sahip iki kavramdır. Sağlık turizmi, kişinin zihin ve beden sağlığını korumak, geliştirmek veya yenilemek amacıyla yaşadığı ülkenin dışına çıkmasıdır. Medikal turizm ise tıbbi müdahale amaçlı yapılan sağlık iyileştirilmesidir. Bu çalışmada, sağlık turizminin alt dalı olan ve son yıllarda gittikçe önemli hale gelen medikal turizm ele alınmıştır. Literatür incelemesi sonucu Türkiye'de medikal turizm sektöründe belirsizliklerin bulunduğu sonucuna varılmış ve bu bağlamda Türkiye'de medikal turizmin sektörel yapısı, aktörleri, işleyişi ve sorunları işlenmiştir. Araştırmada iki aşamada veri elde edilerek bulgular ortaya koyulmuştur. İlk aşamada; Joint Commission International (JCI) akreditasyonu almış sağlık kuruluşları ve Kamu Hastaneleri Genel Müdürlüğü tarafından önceliklendirilmiş hastanelerin 2015-2021 yılları arasında tedavi olmaya gelen yabancı hastalarının dijital ortamda paylaştıkları şikâyetleri ele alınmıştır. Şikâyetlerin analizinde tematik analiz yaklaşımı kullanılmıştır. Araştırmanın ikinci aşamasında ise; Türkiye'de medikal turizm alanında hizmet veren kurumların yöneticileri ile yarı yapılandırılmış mülakat gerçekleştirilmiştir. Elde edilen veriler tematik analiz yaklaşımıyla analiz edilmiş, bulgular, betimsel öğeler de kullanılarak ve tablolaştırılarak sunulmuştur. Araştırmanın sonucunda, eğitimli ve yabancı dil bilen personel eksikliğinin bulunduğu, paydaşlar arası kopuklukların bulunduğu, denetimlerin yapılamadığı, altyapı sorununun, tanıtım ve pazarlama konusunda eksikliklerin, medikal turizm alanında sorunların olduğu, hastaların personel davranışı konusunda şikâyetçi oldukları, sağlık sisteminde iyileştirmeler yapılması gerektiği, Uluslararası Sağlık Hizmetleri Anonim Şirketi (USHAŞ) hakkında fikir ayrılıklarının olduğu, mevzuatın yetersiz kaldığı ortaya çıkmıştır. Medikal turizm alanında kurumlar arası iş birliğinin olmadığı ve düzenleyici kuruma bağlı algılanan sorunlar bulunduğu saptanmış ve bazı önerilerde bulunulmuştur. With the development of the tourism sector, people's wishes and needs have also changed. However, health tourism has also developed with these demands and needs. The biggest impact on the development of health tourism has been people's preference for health services that follow a high quality low price policy. Health tourism is divided into four headings according to the generally accepted view. Medical tourism is grouped as thermal tourism, Selus Per Aqua (SPA) - Wellness tourism and advanced age tourism/disabled tourism. Although medical tourism and health tourism are confused with each other, they are two concepts with different importance. Health tourism is a person's going out of the country they live in in order to protect, improve or renew their mental and physical health. Medical tourism, on the other hand, is the improvement of health for the purpose of medical intervention. In this study, medical tourism, which is a sub-branch of health tourism and has become increasingly important in recent years, was discussed. As a result of the literature review, it was concluded that there are uncertainties in the medical tourism sector in Turkey, and in this context, the sectoral structure, actors, functioning and problems of medical tourism in Turkey are studied. In the study, data were obtained in two stages and the findings were revealed. First stage; The complaints shared by foreign patients who came to be treated between 2015-2021 in the digital environment of the hospitals that have received Joint Commission International (JCI) accreditation and the hospitals prioritized by the General Directorate of Public Hospitals are discussed. Thematic analysis approach was used in the analysis of complaints. In the second stage of the research; Semi-structured interviews were conducted with the managers of institutions serving in the field of medical tourism in Turkey. The data obtained were analyzed with thematic analysis approach, and the findings were presented by using descriptive items and tabulated. As a result of the research, it was concluded that there are problems in the field of medical tourism, there are disconnections between stakeholders, there is a lack of trained and foreign language staff, there are no inspections, there is an infrastructure problem, the patients complain about the personnel behavior, there is a need for improvements in the health system, there are deficiencies in promotion and marketing, there are differences of opinion about International Health Services Incorporated (USHAŞ). It turned out that the legislation was insufficient. It was determined that there was no interagency cooperation in the field of medical tourism, there were perceived problems related to the regulatory institution and some recommendations were made.

Background: A sizeable proportion of patients have discordant low-density lipoprotein cholesterol (LDL-C) and non-high-density lipoprotein cholesterol (non-HDL-C). Objectives: We assessed the relationship between discordance of LDL-C and non-HDL-C and coronary artery disease (CAD) severity. Methods: We retrospectively evaluated the data of 574 consecutive patients who underwent coronary angiography. Fasting serum lipid profiles were recorded, SYNTAX and Gensini scores were calculated to establish CAD complexity and severity. We determined the medians for LDL-C and non-HDL-C to examine the discordance between LDL-C and non-HDL-C. Discordance was defined as LDL-C greater than or equal to the median and non-HDL-C less than median; or LDL-C less than median and non-HDL-C greater than or equal to median. A p value < 0.05 was accepted as statistically significant. Results: LDL-C levels were strongly and positively correlated with non-HDL-C levels (r = 0.865, p < 0.001) but 15% of patients had discordance between LDL-C and non-HDL-C. The percentage of patients with a Gensini score of zero or SYNTAX score of zero did not differ between discordant or concordant groups (p = 0.837, p = 0.821, respectively). Mean Gensini and SYNTAX scores, percentage of patients with Gensini score ≥20 and SYNTAX score >22 were not different from group to group (p = 0.635, p = 0.733, p = 0.799, p = 0.891, respectively). Also, there was no statistically significant correlation between LDL-C and Gensini or SYNTAX scores in any of the discordant or concordant groups. Additionally, no correlation was found between non-HDL-C and Gensini or SYNTAX score. Conclusions: While there was discordance between LDL-C and non-HDL-C (15% of patients), there is no difference regarding CAD severity and complexity between discordant and concordant groups. Resumo Fundamento: Uma proporção considerável de pacientes apresenta níveis discordantes de colesterol de lipoproteína de baixa densidade (LDL) e de não alta densidade (não HDL). Objetivos: Avaliar a relação da discordância entre colesterol LDL e não HDL com a gravidade da doença arterial coronariana (DAC). Métodos: Avaliamos retrospectivamente os dados de 574 pacientes submetidos consecutivamente à angiografia coronariana. Foram registrados os perfis lipídicos séricos em jejum, e depois foram calculados os escores SYNTAX e Gensini para estabelecer a complexidade e a gravidade da DAC. Determinamos as medianas para colesterol LDL e não-HDL para examinar a discordância entre ambos. Discordância foi definida como LDL maior ou igual à mediana e não-HDL menor que mediana; ou LDL menor que a mediana e não-HDL maior ou igual à mediana. Valor de p < 0,05 foi aceito como estatisticamente significante. Resultados: Os níveis de colesterol LDL estiveram forte e positivamente correlacionados com os níveis de colesterol não-HDL (r = 0,865, p < 0,001), mas 15% dos pacientes apresentaram discordância entre LDL e não-HDL. A porcentagem de pacientes com escore Gensini ou SYNTAX zero não diferiu entre os grupos discordantes ou concordantes (p = 0,837, p = 0,821, respectivamente). Escores médios de Gensini e SYNTAX, porcentagem de pacientes com escore Gensini ≥ 20 e SYNTAX > 22 não foram diferentes de grupo para grupo (p = 0,635, p = 0,733, p = 0,799, p = 0,891, respectivamente). Além disso, não houve correlação estatisticamente significativa entre os escores de cholesterol LDL e Gensini ou SYNTAX em nenhum dos grupos discordantes ou concordantes. Também não foi encontrada correlação entre cholesterol não HDL e escore Gensini ou SYNTAX. Conclusões: Embora tenha havido discordância entre colesterol LDL e não-HDL (15% dos pacientes), não há diferença quanto à gravidade e complexidade da DAC entre os grupos discordantes e concordantes.
