Operating the fluid catalytic cracking unit (FCCU) in afterburning conditions can increase the regenerator temperatures above the metallurgical design leading to mechanical failures of the cyclones and plenum chamber. This paper presents the methodology applied in a commercial FCCU to investigate the afterburning causes and the technical solutions that can be implemented to reduce the afterburning. Thus, by evaluating the regenerator temperature profile, regenerator as-build design, and the internals mechanical status, it was concluded that the main cause of afterburning was the non-uniform distribution and mixing of air and catalyst. The industrial results showed that optimizing the catalyst bed level, stripping steam, reaction temperature, and equilibrium catalyst (e-cat) activity reduced the afterburning by 39%. Other process parameters such as feed preheat temperature, slurry recycling, and excess oxygen did not significantly influence afterburning because of air and catalyst maldistribution. Revamping the regenerator to assure a symmetrical layout of cyclones reduced the afterburning by 86%, increased the fines retention in FCCU inventory, and provided a better regeneration of the spent e-cat. The reduction of operating temperatures at around 701°C removed the risk of catalyst thermal deactivation, and therefore the e-cat activity was increased by 10.2 wt.%.

Introduction. Fractures of the distal radius epiphysis are the most common fractures of the upper limb, present both in the general population active following major trauma and in the elderly population in minimal trauma due to osteoporosis. Among the adjuvant therapies for orthopedic treatment of distal radius epiphyseal fracture we can list Super Inductive System (SIS), a therapy based on the interaction between the electric field and the human body with the improvement of the healing process by acting on the pathophysiological stages of bone callus. Material and method. A clinical case study was performed on a 28-year-old patient, hospitalized and treated in the neurosurgery department of the Constanta County and Emergency Hospital for a polytrauma by road accident (passenger) with amyelotic cervical vertebral trauma, thoracic trauma and trauma to the right upper limb, subsequently performing 12 SIS therapy sessions at the Balneal and Rehabilitation Sanatorium of Techirghiol. CT examination of the cervical spine reveals fractures of C4 vertebra (the blade and pedicle) and C5 vertebra (vertebral body, lamina and pedicle). Right forearm radiography reveals fracture of the right radial styloid. After conservative treatment of the cervical injury and orthopedic treatment of the upper limb injury, the clinical evolution is favorable, allowing the patient to be discharged and allowed to do 12 sessions of SIS therapy, 3 times a week, within 4 weeks. The subsequent clinical and paraclinical evolution was favorable for the outpatient orthopedic ambulatory reevaluation performed at 5 weeks. Results and discussions. Due to the type of fracture of the radial distal epiphysis (linear fracture without displacement), absence of comorbidities and young age, led to the indication of orthopedic treatment with immobilization in the antebrachio-palmarcast, which allowed subsequent physiotherapy.

We report a case of an unusual congenital anomaly congenital perineal lipoma occurred in a full-term female neonate. Physical examination showed one soft perineal mass located in the right side of perineum between the vulva and the anus. No abnormalities of vulva or of anus were detected. The patient underwent ultrasound examination confirming a homogeneous fat tissue matter in its structure. Traumatic kidney injuries are an important life threatening situation following blunt or penetrating abdominal lesions. At every age, the goal of renal trauma management is preservation of renal function with minimal morbidity. It is imperative that these patients are treated urgently and efficiently to prevent renal damage. We present a series of patients (adults and children), treated in our department for kidney traumatic lesions on a period of 10 years. Introduction: Despite the fact that almost 1/6 of the total male population of the world is circumcised it has been a long time since it has been forgotten the origin of this intervention. The procedure was practiced for religious beliefs, cultural or medical, the latter being fashionable in modern surgery in the last century. Material and method: The trial was conducted in the Clinic of Pediatric and Orthopaedic Surgery of "St. Andrew" Emergency County Hospital of Constantza, between 2011 and 2015. During this period we verified the cases of phimosis and separated them in two age groups. We looked at those who were circumcised and those who had conservative surgery of the foreskin. Results: During those five years, we had 1,196 cases of phimosis. Some of them were circumcised, and at others the foreskin was kept. For circumcision, there were different aspects: * Biological circumcision has been practiced for different pathologies that needed this type of intervention; * Psychological: presence or absence of the foreskin perceived like a state of well-being; * Social: in terms of cultural, religious or social or when an individual wishes to be circumcised. Conclusions: Surgery of the foreskin, unless required by medical reasons, should not be performed in the first 4 years of life, taking into account the development of the glansforeskin system during this period. Western trends about the integrity of the human body have their justification in the light of new investigations conducted (microscopic anatomy, immunology, etc). For circumcision it must be considered a multitude of factors that are not only medical, but also social, cultural, religious, etc. Regarding statistics in our country and especially in Dobrogea, there isn't an accuracy because of the inconsistent practice of the intervention in hospital. Trauma is the leading cause of death in pediatric patients between 1 and 18 years old. Abdominal lesions in children are mostly due to car accidents, falls from heights or sport-related injuries. In this study it is discussed the therapeutic management of abdominal parenchymal organ injuries in polytrauma in children. The material used for this 5 year retrospective statistical study represents two lots of polytrauma children who associate abdominal trauma and solid organ injuries, hospitalized in the Pediatric Surgery and Orthopedics Constanta County Hospital, one group treated surgically and the other group with conservative treatment. Asthma is still one of the most common chronic inflammatory disease of the airways who determines a high rate of morbidity and mortality in children population. This paper aims to highlight the main risk factors for asthma in children. It is a prospective study and was conducted on a sample of 199 cases with obstructive respiratory disease admitted in the Pediatric Clinic Emergency Hospital Constanta. After a four years period, we identified 126 cases of children that developed asthma. Assessment of risk factors for asthma showed high frequency for the following risk factors: male gender (57.1%), urban (60%), prematurity (29.51%), family atopy (53.2), artificial nutrition, exposure to cigarette smoke. Cutaneous hemangiomas are the most common tumors of infant and children. Risk or complicated hemangiomas are considered those hemangiomas located in specific anatomic areas, which can interfere with vital functions and can cause serious deformities, irreversible. Various methods of treatment are described for these complicated hemangiomas. In our clinic we used bleomycin. Article refers to three cases of patients treated with intralesional bleomycin injection which shows hemangiomas located in risk areas: nose, tongue, and ear. After these injections we obtained a significant reduction of vascular tumors and improve organs function. Post operatory pain assessment study monitored a group of patients using assessment scales according to the protocol used painkiller category, minor and major surgery. The study group consisted of 47 patients aged 0 - 192 months, divided into four age groups and two categories according to surgical complexity. The measurement scales used in the post operatory pain evaluation study are: DAN, EVA, and CHEOPS. At the age category, the assessment shows DAN lower score averages in children treated with meperidine and paracetamol compared to those treated with paracetamol, statistically significant differences due to surgical category. For the categories A, B, and C age group there were found statistically significant differences in pain scores according to the protocol used analgesic, but no clinical differences were found due to different type of surgical procedures (major, minor, plastic). Pain scores were significantly lower with epidural analgesia than the others following the analgesic protocol, in the D age group. [ABSTRACT FROM AUTHOR]

Spontaneous bacterial peritonitis (SBP) is a complication of liver cirrhosis with an increased in-hospital mortality rate. For this reason, the aim of the present study was to examine the main predictors of mortality in order to be able to identify high-risk patients in time and to guide the optimal treatment for prognosis improvement. We retrospectively collected demographic, clinical, laboratory and treatment data as well as data regarding length of stay and cost of hospitalization from 72 patients diagnosed with SBP between January 2010 and December 2019 in the Emergency Clinical Hospital St. Apostle Andrew, Constanta, Romania. Patients were divided into two groups: Those who survived and those who died. Logistic regression was used to identify a possible association between these factors and the increased risk of mortality. Univariate analysis revealed that clinical factors (fever, chills, and hepatic encephalopathy), biological factors such as serum and ascites leukocyte value, polymorphonuclear percentage (PMN), erythrocyte sedimentation rate (ESR) value, previous SBP episodes, and the presence of complications such as acute kidney injury (AKI), sepsis, and systemic inflammatory response syndrome (SIRS) were significantly associated with in-hospital mortality in patients with SBP. Multivariate analysis revealed that SIRS (P=0.0010) and fever (P=0.0258) were significantly associated with in-hospital mortality in patients with SBP. In the last years, the commercial drone/unmanned aerial vehicles market has grown due to their technological performances (provided by the multiple onboard available sensors), low price, and ease of use. Being very attractive for an increasing number of applications, their presence represents a major issue for public or classified areas with a special status, because of the rising number of incidents. Our paper proposes a new approach for the drone movement detection and characterization based on the ultra-wide band (UWB) sensing system and advanced signal processing methods. This approach characterizes the movement of the drone using classical methods such as correlation, envelope detection, time-scale analysis, but also a new method, the recurrence plot analysis. The obtained results are compared in terms of movement map accuracy and required computation time in order to offer a future starting point for the drone intrusion detection.

Aim: We observed the most vulnerable parts of the human body in traffic accidents., Materials and Methods: The study was made on the cases of death in Timis region in the last 3 years (2005-2007). Out of a total of 2314 forensic autopsies, 407 were consequences of traffic accidents., Results: In Romania, it is compulsory to perform the forensic autopsy on traffic accident (TA) victims. In our study concerning the mortality in TA, cranio-cerebral trauma represents 33.58%, vertebro-medullar trauma--8.09%, thoracic trauma--2.89%, and singular abdominal trauma--1.73%. The topographic distribution shows: axial regions--79.19% (skull 60%, thoracic 9.19%, rachis 5%, pelvis 4%, and abdomen 2%); limbs 20.80%. The gender distribution shows a bigger proportion of male drivers as well as pedestrians (3:1 ratio). The analysis of alcohol abuse in TA shows that 35% of the deceased drivers were under the influence of alcohol., Conclusions: The forensic expertise in traffic accidents is very useful in identifying the liable party, in establishing the level of alcohol intoxication, as well as in identifying the injuries with lethal potential.

After classifying the phases of blood clot retraction within the mechanism of blood coagulation and the factors on which this phenomenon is depending, the authors briefly review the methods employed for its investigation. An improvement based on the determination of plasma clot retraction, performed in a plasma with a standard count of platelets (100,000/mm3) is presented. The authors have employed it for the investigation of 100 normal subjects and 25 patients with platelet dysfunctions, and have obtained very good results.
