Purpose: The aim of this study was to evaluate the impact of the LSVT-BIG ® (Lee Silverman Voice Treatment Big) technique on functional mobility, dexterity, and quality of life in Parkinson's disease (PD).Materials and Methods: Fifteen patients with Parkinson's disease (9 males, and 6 females, aged 40-75 years; Hoehn & Yahr Stages I-III) completed a 4-week LSVT-BIG training program for 1 hour/session, 4 sessions per week. Parkinson's Disease Quality of Life Questionnaire-39 (PDQ-39) was used in measuring patients' quality of life at the beginning (t0) and at the end (t1) of the rehabilitation program. Nine-Hole Peg Test (NHPT) was used for dexterity. 10 Meter Walking Test (10MWT), Timed-Up-and-Go Test (TUG), and Five-Times Sit-to-Stand Test (5XSST) were used for assessing functional mobility. After 4 weeks, all participants were retested.Results: After 4 weeks of LSVT-BIG therapy, a statistically significant improvement was found in PDQ-39 mobility, activities of daily living, emotional state, stigma, cognition, communication, physical discomfort, and general scores after exercise compared to before. There was no statistically significant change in the PDQ-39 social support score after exercise compared to before. In addition, functional mobility improved as indicated by the TUG test, 5XSST, 10MWT (and dexterity by NHPT on the left.Conclusion: LSVT-BIG training may be a new therapeutic option for better walking, functional mobility, and manual capability performance and for enhancing the quality of life in PD. Randomized controlled trials with bigger sizes are needed to evaluate the effect of the LSVT-BIG rehabilitation program on reducing motor and non-motor impairments in patients with PD., Amaç: Bu çalışmanın amacı, Parkinson hastalığında (PH) LSVT-BIG ® (Lee Silverman Voice Treatment Big) tekniğinin fonksiyonel hareketlilik, el becerisi ve yaşam kalitesi üzerindeki etkisini değerlendirmekti.Gereç ve Yöntem: Parkinson hastalığı olan 15 hasta (9 erkek ve 6 kadın, yaşları 40-75; Hoehn & Yahr Evre I-III), haftada 4 seans, 1 saat/seans olmak üzere 4 haftalık bir LSVT-BIG eğitim programını tamamladı. Hastaların rehabilitasyon programının başında (t0) ve sonunda (t1) yaşam kalitelerinin ölçülmesinde Parkinson Hastalığı Yaşam Kalitesi Anketi-39 (PDQ-39) kullanıldı. El becerisi için Dokuz Delikli Çivi Testi (NHPT) kullanıldı. Fonksiyonel mobilitenin değerlendirilmesinde 10 Metre Yürüme Testi (10MWT), Zamanlı Kalk ve Yürü Testi (TUG) ve Beş Kez Oturup Kalkma Testi (5XSST) kullanıldı. 4 hafta sonra, tüm katılımcılar yeniden test edildi.Bulgular: 4 haftalık LSVT-BIG tedavisinden sonra, PDQ-39 mobilite, günlük yaşam aktiviteleri, duygusal durum, stigma, kognisyon, iletişim, bedensel rahatsızlık ve genel skorlarda egzersiz sonrası öncesine kıyasla istatistiksel olarak anlamlı bir iyileşme bulundu. Egzersiz sonrası bakılan PDQ-39 sosyal destek puanında öncesine göre istatistiksel olarak anlamlı bir değişiklik olmadı. Ayrıca TUG testi, 5XSST, 10MWT’de ve sol el için NHPT ile bakılan beceri testinde iyileşme gelişti.Sonuç: LSVT-BIG eğitimi, PH'da daha iyi yürüme, fonksiyonel hareketlilikve el becerisi performansı ile yaşam kalitesini arttıran yeni bir tedavi seçeneği olabilir. LSVT-BIG rehabilitasyon programının Parkinson hastalarında motor ve motor olmayan bozuklukları azaltma üzerindeki etkisini değerlendirmek için daha büyük boyutlarda randomize kontrollü çalışmalara ihtiyaç vardır.

Bu araştırmada, öncelikle MS hastalarında sigara içiminin yorgunluk ve özürlülük üzerine etkisinin araştırılması amaçlanmıştır. Araştırma prospektif olarak Gaziantep Üniversitesi Hastanesi Nöroloji Kliniğinde yapılmıştır. Araştırma kapsamına üç grup alınmıştır. Gruplar, revize Mc Donald tanı kriterlerine göre MS tanısı almış sigara içen 40 MS hastası ve sigara içmeyen 40 MS hastası ile 50 sağlıklı bireyden oluşmaktadır.Hastaların yorgunluğunun tespitinde MS hastalığı yorgunluğu için kullanılan Nörolojik Yorgunluk İndeksi (NFI-MS) kullanılmıştır. Çalışmaya alınan tüm olguların sosyodemografik özellikleri, yaş, cinsiyet, medeni durum, eğitim durumu, çalışma durumu, hastalık süresi ve anket sonuçları oluşturulan hasta takip formuna kaydedilmiştir. Hastaların özürlülük durumlarının tespiti Genişletilmiş Özürlülük Durum Ölçeği (EDSS) ile belirlenmiştir. Sigara içen hastaların bağımlılık düzeyleri Fagerstrom Nikotin Bağımlılık Testi ile belirlenmiştir. Fagerstrom Nikotin Bağımlılık testi sigaraya olan fiziksel bağımlılık düzeyinin saptaması amacıyla Fagerstrom tarafından geliştirilmiştir. Tüm olguların depresyon durumları BECK Depresyon Ölçeği (BDÖ) ile belirlenmiştir. Araştırma bulguları SPSS'20 istatistik programı ile analiz edilmiştir. İstatiksel değerlendirme, tanımlayıcı istatistik testleri, İndependent Sample T Testi, Tek Yönlü Varyans Analizi, Korelasyon testleri ile yapılmıştır.Araştıma sonucunda, sigara içmenin yorgunluğa neden olmadığı, MS hastalarında yorgunluğun sağlıklı kişilere göre daha fazla olduğu saptanmıştır. Ancak sigara içen ve içmeyen MS hastalarının yorgunluk düzeyleri arasında anlamlı bir farklılık olmadığı saptanmıştır. Bunun yanı sıra literatürle uyumlu olarak MS hastalarında orta ve hafif düzeyli depresyon olduğu MS hastalarında sigara içiminin depresyonu olumsuz yönde arttırdığı belirlenmiştir. In this study first of all, the effect of smoking on MS patients' fatigue and disability is aimed to be searched. The study was prospective and performed at Gaziantep University Department of Neurology. Research coverage has been taken into three groups.The groups were consisted of 40 smoker MS patient, 40 nonsmokerMS patients according to the McDonald diagnostic criteria and 50 healthy individuals without MS. Neurological fatigue index (NF-MS) test is used to determine the fatigue of MS patients. Sociodemographic characteristics of all patients in the study, age, gender, marital status, education level, employment status, has been created in the form of patient follow-up duration of the disease and the survey results.The detection of patient disability were determined by the Expanded Disability Status Scale (EDSS). Addictive levels of smoking of patient was determined by Fagerstrom Test for Nicotine Dependence. Depression status of all cases was determined with BECK Depression Inventory (BDI). The research findings were analyzed with software SPSS'20. Statistical analyed were done with determination Statistic test, İndependent Sample T test, ANOVA and corelation tests.As a result, the fatigue wasnot correlated with smoking in MS but fatigue frequency was higher than in healthy people. However, it was found that there was no significant difference between smoker and non-smoker fatigue levels in MS patients. In addition to, as in coherent with the literature medium and light depression has been determined in MS patients and this finding was getting worse with smoking. 129
