1. New processing routes for Zr-based ODS ferritic steels

Abstract

In this work, Zr addition is proposed to refine the nanoparticle dispersion in an ODS RAF steel of composition Fe-14Cr-2W-0.3Zr-0.24Y (wt.%). Three batches of material are obtained using pre-alloyed atomized powder, where yttrium is directly introduced in the melt, and manufactured through three different processing routes. First route is based on the newly developed STARS route that aims to avoid subsequent mechanical alloying. The second route explores the impact of mechanical alloying in pre-oxidized powders. The third route uses mechanically alloyed powders without the pre-oxidation process. The ODS-powders were individually consolidated by hot isostatic pressing and later hot rolled. The obtained materials were characterized by small-angle neutron scattering (SANS) and X-ray absorption spectroscopy (XAS) techniques. SANS and XAS analysis point out the absence of oxide nanoparticles in the material based on the STAR route. SANS analysis confirms that the mechanically alloyed materials do exhibit the presence of nanoparticles. These are identified as Zr-O-rich nanoprecipitates by XAS and the calculated A-ratio by SANS is linked with the phase Y2Zr2O7. Their radii are in the range of 3–3.6 nm. XAS results show that mechanical alloying minimizes the initial differences regarding the oxidation state between the ODS powders with and without pre-oxidation.

Abstract

An ODS steel with nominal composition Fe–14Cr–2W–0.4Ti-0.3Y2O3 (wt.%) was produced by mechanical alloying and compacted by hot isostatic pressing (HIP) followed by hot cross rolling (HCR). To check the effects of thermal aging at relevant temperatures of operation in fusion power plants, the alloy was thermally aged at 873 K for 2000 h. In this work, small-angle neutron scattering (SANS) and X-ray absorption spectroscopy (XAS) techniques are used for the advanced characterization of secondary phases and the oxide nanoparticle dispersion. SANS results show that the oxide nanoparticles remain stable after the thermal aging treatment. Composition of the oxide nanoparticles was identified as Y2TiO5 or Y2Ti2O7 by SANS, while non-stoichiometry was found by XAS analysis. Laves phase precipitation after the thermal aging treatment is further confirmed by SANS, from the magnetic anisotropic contribution to the scattering intensity associated to this metallic phase, and by XANES.

3. Validation of a score to identify inpatients at risk of a drug-related problem during a 4-year period

Abstract

Objective: Drug-related problems (DRP) produce high morbidity and mortality. It is therefore essential to identify patients at higher risk of these events. This study aimed to validate a DRP risk score in a large number of inpatients. Material and methods: Validation of a previously designed score to identify inpatients at risk of experiencing at least one DRP in a tertiary university hospital from 2010 to 2013. DRP were detected by a pharmacy warning system integrated in the electronic medical record. The score included the following variables associated with a higher risk of DRP: prescription of a higher number of drugs, greater comorbidity, advanced age, specific ATC groups and certain major diagnostic categories. Results: The study included a total of 52,987 admissions; of these, at least one DRP occurred in 14.9%. After validation of the score (period range, 2010–2013: 0.746–0.764), the area under the curve (AUC) was 0.751 (95% CI: 0.745–0.756). Conclusions: This value is higher than those reported in other studies describing validation of risk scores. The score showed good capacity to identify those patients at higher risk of DRP in a much larger sample of inpatients than previously described in the literature. This tool allows optimization of drug therapy monitoring in admitted patients. 4. RELACIONES NITRATO/ AMONIO/ UREA Y CONCENTRACIONES DE POTASIO EN LA PRODUCCIÓN DE PLÁNTULAS DE TOMATE

Abstract

El uso de amonio y urea como fuente de nitrógeno en la producción de plántulas para trasplante es muy limitado, debido a la carencia de información al respecto y al posible riesgo de toxicidad provocado por ambas formas de nitrógeno. Debido a ello, el objetivo del presente estudio fue evaluar cuatro relaciones nitrato/ amonio/ urea (100/0/0, 85/15/0, 85/0/15 y 85/7.5/7.5) y dos concentraciones de potasio (7 y 9 mol.m-3) en la solución nutritiva sobre el crecimiento y composición mineral de plántulas de tomate producidas en invernadero. Se utilizó un diseño completamente al azar con un arreglo de tratamientos factorial completo 4x2 con cuatro repeticiones. A 46 días después de la siembra se tomaron datos del crecimiento y se determinó la concentración nutrimental en hojas, tallos y raíces. Con la relación 85/0/15 y 7 mol K+·m-3 se logró el mayor diámetro de tallo; mientras que con 85/7.5/7.5 y 9 mol K+·m-3 se obtuvo el mayor volumen radical. Con 85/0/15 la relación vástago/raíz y el peso fresco de plántula se incrementaron 16.3 y 14.6 %, respectivamente, con respecto a 100/0/0. La interacción nitrato/amonio/urea-K tuvo efecto altamente significativo en la concentración de nitrógeno en hojas, tallos y raíces; de fósforo en hojas y raíces; de calcio y magnesio en raíces. La sustitución de un 15 % del total de nitrógeno nítrico presente en la solución nutritiva con un porcentaje similar de nitrógeno amoniacal y ureico o una mezcla 7.5 % N-amoniacal-7.5 % N-ureico incrementó algunas variables de calidad de las plántulas de tomate.

5. Cavity Design via Entrapment of Tetrapyrrole Macrocycles in Sol–Gel Matrices for Catalytic, Optical or Sensoring Functions

Abstract

The physicochemical and luminescent properties of tetrapyrrole macrocycles, such as the porphyrins (H 2 P) and phthalocyanines (H 2 Pc), were preserved by trapping or bonding these species in silica matrices. The method involved used hydroxy-aluminium tetrasulphophthalocyanine OH(Al)TSPc as a probe to find optimal conditions for the entrapment of tetrapyrrole molecules. This methodology made possible the trapping or fixing of macrocyclic species or their respective complexes in the interior of pores existing in monolithic, translucent, normal, or organo-modified silica xerogels. The average pore sizes ranged from 2.0 to 3.6 nm in these systems and depended on the structure, the nature of the cation in the complex and on the identity and position of the substituents at the periphery of the macrocyclic species. Under appropriate conditions, the tetrapyrrolic species can be trapped or bonded to the pore network in stable and monomeric form; however, the interactions with Si–OH groups on the pore walls inhibit the efficient displaying of its properties. To avoid this deleterious effect, some strategies are used, such as to place the macrocycle far from the pore walls through long bridges or by substituting Si–OH groups with alkyl or aryl species. Average pore diameters vary from 3.5 to 9.4 nm when long unions are established between the macrocycle and the pore walls or when more of one macrocyclic species are trapped inside each pore. The spectroscopic properties of the macrocycles trapped in these systems are similar to those displayed by the same species in solution. When phthalocyanines or porphyrins are trapped or bonded to the pore walls of organo-modified silica, the spectroscopic properties are better preserved and their intensities are a function of the chain length of the alkyl group present in the silica matrix. This last result suggests the possibility of tuning the pore size and polarity inside them by choosing the tetrapyrrole species that can be trapped or bonded; in this way, it is possible to create more efficient systems for catalytic, optical, sensoring and medical applications.

Abstract

Medical students need to understand core neuroscience principles as a foundation for their required clinical experiences in neurology. In fact, they need a solid neuroscience foundation for their clinical experiences in all other medical disciplines also, because the nervous system plays such a critical role in the function of every organ system. Due to the rapid pace of neuroscience discoveries, it is unrealistic to expect students to master the entire field. It is also unnecessary, as students can expect to have ready access to electronic reference sources no matter where they practice. In the pre-clerkship phase of medical school, the focus should be on providing students with the foundational knowledge to use those resources effectively and interpret them correctly. This article describes an organizational framework for teaching the essential neuroscience background needed by all physicians. This is particularly germane at a time when many medical schools are re-assessing traditional practices and instituting curricular changes such as competency-based approaches, earlier clinical immersion, and increased emphasis on active learning. This article reviews factors that should be considered when developing the pre-clerkship neuroscience curriculum, including goals and objectives for the curriculum, the general topics to include, teaching and assessment methodology, who should direct the course, and the areas of expertise of faculty who might be enlisted as teachers or content experts. These guidelines were developed by a work group of experienced educators appointed by the Undergraduate Education Subcommittee (UES) of the American Academy of Neurology (AAN). They were then successively reviewed, edited, and approved by the entire UES, the AAN Education Committee, and the AAN Board of Directors.

Abstract

The austenite phase transformations under continuous cooling from intercritical austenitization at 800 °C and full austenitization at 910 °C were investigated in a cold rolled advanced high strength dual phase steel (AHSS-DP) strip. Experimental continuous cooling transformation (CCT) diagrams were obtained using induction-quenching dilatometry and the corresponding microstructures were characterized by scanning electron microscopy (SEM) analysis and Vickers microhardness measurements. In general, it was shown that the experimental behavior of phase transformations in current DP steels depends on the dynamic changes that occur in the hardenability of austenite during continuous cooling at different rates. The main results reveal that the martensite-start transformation temperature (Ms) depends on the cooling rate, particularly when an intercritical austenitization and slow cooling rates conditions coincide. The unusual behavior of Ms temperature is explained in terms of solute distribution, which changes the chemical composition of metastable austenite (local enrichment in carbon content) during cooling, causes changes in hardenability, lowers both the Ms temperature and the critical cooling rate at which martensite is formed. In general terms, the behavior of Ms at slow cooling rates is relevant information that has not been reported yet in the specific literature because it is generally known that Ms does not depend on the cooling rate when quenching is performed from the austenite phase field.

INTRODUCCIÓN: Conocer los predictores de mala evolución en pacientes con Enfermedad por Coronavirus 2019 (COVID-19) permite identificar de forma temprana a los pacientes con peor pronóstico, aportando mejores herramientas a la hora de tomar decisiones clínicas. Se presenta el protocolo de un estudio de cohorte cuyo objetivo principal es identificar factores de riesgo de infección severa, critica y mortalidad en pacientes con COVID-19 internados en el Servicio de Clínica Médica del Hospital Durand (Buenos Aires, Argentina). MÉTODOS: Estudio de cohorte prospectivo con base en un único centro. Se incluirá a todos los pacientes que ingresen al servicio de Clínica Médica con diagnóstico de COVID-19 durante el periodo de estudio. Se recolectarán las características epidemiológicas, clínicas, de laboratorio, radiológicas y los datos de tratamiento, al ingreso y al momento del alta o muerte hospitalaria. El evento final primario es la muerte en la internación; los eventos secundarios son el desarrollo de enfermedad grave y enfermedad crítica, la internación en unidad cerrada y el requerimiento de asistencia respiratoria mecánica.

Desde una perspectiva lacaniana, las intervenciones en casos de autismo deberían apuntar a introducir una ausencia simbólica que movilice el circuito del deseo. Se presenta el caso de un niño de dos años (Q) diagnosticado con trastorno del espectro autista y que asistía a un tratamiento lacaniano. Se introdujo el paradigma del fort-da (que usa un par de significantes para representar la presencia-ausencia) para abordar los problemas de vínculo, intención comunicativa y juego simbólico. Después del tratamiento, Q aprendió a usar su propio lenguaje y juego simbólico para regular y expresar la ansiedad y otras emociones en ausencia de su madre.

Abstract

Background: Omega-3 fatty acids are critical for neuropsychological functioning. Adolescence is increasingly believed to entail brain vulnerability to dietary intake. The potential benefit on adolescent neurodevelopment of consuming walnuts, a source of omega-3 alpha-linolenic acid (ALA), remains unclear. Methods: We conducted a 6-month multi-school-based randomised controlled nutrition intervention trial to assess whether walnut consumption has beneficial effects on the neuropsychological and behavioural development of adolescents. The study took place between 04/01/2016 and 06/30/2017 in twelve different high schools in Barcelona, Spain (ClinicalTrials.gov Identifier: NCT02590848). A total of 771 healthy teenagers aged 11–16 years were randomised into two equal groups (intervention or control). The intervention group received 30 g/day of raw walnut kernels to be incorporated into their diet for 6 months. Multiple primary endpoints concerning neuropsychological (working memory, attention, fluid intelligence, and executive function) and behavioural (socio-emotional and attention deficit hyperactivity disorder [ADHD] symptoms) development were assessed at baseline and after intervention. Red blood cell (RBC) ALA status was determined at baseline and 6 months as a measure of compliance. Main analyses were based on intention-to-treat using a linear mixed-effects model. A per-protocol effect of the intervention was analysed using inverse-probability weighting to account for post-randomisation prognostic factors (including adherence) using generalised estimating equations. Abstract

Background and aims: The American Heart Association proposed 7 ideal cardiovascular health metrics (Life's Simple 7 [LS7]) namely, not smoking, body mass index <25 kg/m2, healthy diet, moderate physical activity ≥150 min/week, total blood cholesterol <200 mg/dL, blood pressure <120/80 mmHg and fasting blood glucose <100 mg/dL. Our objective was to assess the association between these LS7 metrics and the incidence of atrial fibrillation (AF). Methods and results: A total of 6,479 participants of the PREDIMED study were included. We calculated the participants’ baseline LS7 index ranging 0–7 points to categorize them according to their adherence to these LS7 health metrics. Multivariable Cox regression models were used to estimate Hazard Ratios (HR) and their 95% Confidence Intervals (95% CI). After a median follow-up of 4.8 years, we identified 250 incident cases of AF. After adjusting for potential confounders, adherence to LS7 index was not associated with the incidence of AF (adjusted HR 0.90 [95% CI: 0.56–1.45] for highest vs. lowest LS7 categories). Body mass index <25 kg/m2 was the only health metric individually associated with a lower risk of AF (HR 0.36 [95% CI: 0.16–0.78]). Conclusions: In a high cardiovascular risk Spanish population, adherence to American Heart Association's LS7 metrics was not associated with the risk of incident AF. 24. Anuran tadpoles inhabiting a fluoride-rich stream: diets and morphological indicators

Abstract

We in situ assessed the influence of natural fluoride concentrations in lotic freshwater ecosystems on diet and morphology of Boana cordobae tadpoles. Two streams were sampled in Argentina: Los Vallecitos stream (LF-LV) and Los Cerros Negros stream (HF-CN) with low and high fluoride levels, respectively. We captured tadpoles of B. cordobae in each stream using nets. Body weight (BW), total length (TL) and body condition (BC) of tadpoles was registered. Food items were identified to genus level and assigned to functional traits. Tadpoles showed significant differences in TL between streams, with smaller individuals in HF-CN, while did not show differences in BW and BC. The diet of tadpoles consisted mostly of microalgae. In tadpoles from HF-CN stream the proportion of cyanobacteria was lower than tadpoles from LF-LV. In relation to functional traits, small algae, high profile and colonial algae were more abundant in HF-CN. Algae attached by pads showed a higher proportion in HF-CN diets and stalked algae were more abundant in LF-LV. The differences in TL and diet of tadpoles can be attributed to differences in algal community composition, with genera that are affected by high concentrations of natural fluoride; for example cyanobacteria. The low algal richness registered in HF-CN stream does not affect the physiological state of the tadpoles, possibly because of a higher algal density in HF-CN diets. However, in further studies it would be important to evaluate the population status of B. cordobae from the HF-CN, because a small body length of tadpoles could have consequences at the population level.

25. Extreme mitochondrial reduction in a novel group of free-living metamonads

Metamonads are a diverse group of heterotrophic microbial eukaryotes adapted to living in hypoxic environments. All metamonads but one harbour metabolically altered ‘mitochondrion-related organelles’ (MROs) with reduced functions relative to aerobic mitochondria, however the degree of reduction varies markedly over the metamonad tree. To further investigate metamonad MRO diversity, we generated high quality draft genomes, transcriptomes, and predicted proteomes for five recently discovered free-living metamonads. Phylogenomic analyses place these organisms in a clade sister to the Fornicata – a group of metamonads that includes parasitic and free-living diplomonads andCarpediemonas-like organisms. Extensive bioinformatic analyses of the manually curated gene models showed that these organisms have extremely reduced MROs in comparison to other free-living metamonads. Loss of the mitochondrial iron-sulfur cluster (ISC) assembly system in some organisms in this group appears to be linked to the acquisition in their common ancestral lineage of a SUF-like minimal system (SMS) Fe/S cluster pathway through lateral gene transfer (LGT). One of the isolates, named ‘RC’, appears to have undergone even more drastic mitochondrial reduction losing almost all other detectable MRO-related functions. The extreme mitochondrial reduction observed within this free-living anaerobic protistan clade is unprecedented and demonstrates that mitochondrial functions, under some conditions, can be almost completely lost even in free-living organisms.

Abstract

Poor communication within healthcare teams occurs commonly, contributing to inefficiency, medical errors, conflict, and other adverse outcomes. Interprofessional bedside rounds (IBR) are a promising model that brings two or more health professions together with patients and families as part of a consistent, team-based routine to share information and collaboratively arrive at a daily plan of care. The purpose of this systematic scoping review was to investigate the breadth and quality of IBR literature to identify and describe gaps and opportunities for future research. We followed an adapted Arksey and O'Malley Framework and PRISMA scoping review guidelines. PubMed, CINAHL, PsycINFO, and Embase were systematically searched for key IBR words and concepts through June 2020. Seventy-nine articles met inclusion criteria and underwent data abstraction. Study quality was assessed using the Mixed Methods Assessment Tool. Publications in this field have increased since 2014, and the majority of studies reported positive impacts of IBR implementation across an array of team, patient, and care quality/delivery outcomes. Despite the preponderance of positive findings, great heterogeneity, and a reliance on quantitative non-randomized study designs remain in the extant research. A growing number of interventions to improve safety, quality, and care experiences in hospital settings focus on redesigning daily inpatient rounds. Limited information on IBR characteristics and implementation strategies coupled with widespread variation in terminology, study quality, and design create challenges in assessing the effectiveness of models of rounds and optimal implementation strategies. This scoping review highlights the need for additional studies of rounding models, implementation strategies, and outcomes that facilitate comparative research.

The study included two groups of mice, one of which received levofloxacin. The composition and structure of the microbiota were determined in the mice's stool using 16S rRNA sequencing; the differential taxa and metabolic pathway between mice treated with levofloxacin and control mice were also defied. The effect of levofloxacin was evaluated in the intestines, hind paws, and spines of mice through DNA microarray transcriptome and histopathological analyses; systemic inflammation was measured by flow cytometry. Levofloxacin decreased the pro-inflammatory bacteria, including Prevotellaceae, Odoribacter, and Blautia, and increased the anti-inflammatory Muribaculaceae in mice's stool. Histological analysis confirmed the intestinal inflammation in control mice, while in levofloxacin-treated mice, inflammation was reduced; in the hind paws and spines, levofloxacin also decreased the inflammation. Microarray showed the downregulation of genes and signaling pathways relevant in spondyloarthritis, including several cytokines and chemokines. Levofloxacin-treated mice showed differential transcriptomic profiles between peripheral and axial joints and intestines. Levofloxacin decreased the expression of TNF-α, IL-23a, and JAK3 in the three tissues, but IL-17 behaved differently in the intestine and the joints. Serum TNF-α was also reduced in levofloxacin-treated mice. Our results suggest that the microbiota modification aimed at reducing pro-inflammatory and increasing anti-inflammatory bacteria could potentially be a coadjuvant in treating inflammatory arthropathies.

28. A unique symbiosome in an anaerobic single-celled eukaryote

Abstract

Symbiotic relationships drive evolutionary change and are important sources of novelty. Here we demonstrate a highly structured syntrophic symbiosis between species of the anaerobic protistAnaeramoeba(Anaeramoebae, Metamonada) and bacterial ectosymbionts. We dissected this symbiosis with long-read metagenomics, transcriptomics of host and symbiont cells coupled with fluorescent in situ hybridization (FISH), and microscopy. Genome sequencing, phylogenomic analyses and FISH show that the symbionts belong to theDesulfobacteraceaeand were acquired independently in two differentAnaeramoebaspecies. We show that ectosymbionts likely reside deep within cell surface invaginations in a symbiosomal membrane network that is tightly associated with cytoplasmic hydrogenosomes. Metabolic reconstructions based on the genomes and transcriptomes of the symbionts suggest a highly evolved syntrophic interaction. Host hydrogenosomes likely produce hydrogen, acetate, and propionate that are consumed by the symbionts dissimilatory sulfate reduction, Wood-Ljungdahl and methylmalonyl pathways, respectively. Because the host genome sequences encode several vitamin B12-dependent enzymes but appear to lack the ability to biosynthesize this vitamin, we hypothesize that the symbionts supply their hosts with B12. We detected numerous lateral gene transfers from diverse bacteria toAnaeramoeba, including genes involved in oxygen defense and anaerobic metabolism. Gene families encoding membrane-trafficking components that regulate the phagosomal maturation machinery are notably expanded inAnaeramoebaspp. and may be involved in organizing and/or stabilizing the symbiosomal membrane system. Overall, the Anaeramoebae have evolved a dynamic symbiosome comprised of a vacuolar system that facilitates positioning and maintenance of sulfate-reducing bacterial ectosymbionts.

Abstract

The standard neurology clinical experience in medical school focuses primarily on bedside patient encounters; however, the limitations of the clinical environment due to the current COVID-19 pandemic have accelerated the need for virtual curriculum development. To provide guidance to Neurology clerkship directors during this unprecedented time, the American Academy of Neurology (AAN) Undergraduate Education Subcommittee (UES) formed a workgroup to develop an outline for a virtual curriculum, provide recommendations, and describe models of integrating virtual curricula into the neurology clerkship. In this overview, we discuss different methods of virtual instruction, hybrid models of clerkship training and the challenges to its implementation, professionalism issues, and modification of feedback and assessment techniques specific to the virtual learning environment. We also offer suggestions for implementation of a hybrid virtual curriculum into the neurology clerkship. The virtual curriculum is intended to supplement the core neurology in-person clinical experience and should not be used for shortening or replacing the required neurology clinical clerkship.

Abstract

This research work reports the effect of boron micro-additions (180 and 470 ppm) on the solidification structure, magnetic properties and hot ductility behavior of an advanced low-carbon highly alloyed twinning-induced plasticity (TWIP) steel. For this purpose, three experimental TWIP steels were fabricated by melting commercial raw materials and casting into metallic molds. Solidification structure was characterized by means of optical and scanning electron microscopy techniques, and a statistical study was carried out to measure dendrite features. A vibrating sample magnetometer was used, at room temperature, to determine magnetic properties, and a X-ray diffraction analysis was performed in order to identify the related phases during magnetic measurements. Finally, the hot ductility in the as-cast condition was evaluated at 700, 900 and 1100 °C, under a constant strain rate of 0.001 s−1. The results indicate that boron micro-additions cause an overall refining solidification structure and austenitic grain size. However, as the boron content increases, segregation of this element promotes formation of ferrite and ε-martensite, leading to ferromagnetic behavior. Nonetheless, with subsequent hot- and cold-rolling, the single austenitic phase is achieved, and this behavior is eliminated. Hot tensile tests revealed that boron micro-addition is beneficial to the hot ductility behavior. The greatest influence was observed for the higher concentration of boron (470 ppm). In comparison with the steel without boron content, the reduction of area (RA) is more than the triple of the hot workability during straining at 900 °C. Thus, present results demonstrate that boron micro-addition has an excellent potential for refining dendritic microstructure and improving the hot-deformation behavior of present low-carbon highly alloyed TWIP steel.

Abstract

Funder: Canadian Institutes of Health Research (Grant: FRN-142349) Natural Sciences and Engineering Research Council of Canada (Grant: RGPIN 05871-2014), Cells replicate and segregate their DNA with precision. Previous studies showed that these regulated cell-cycle processes were present in the last eukaryotic common ancestor and that their core molecular parts are conserved across eukaryotes. However, some metamonad parasites have secondarily lost components of the DNA processing and segregation apparatuses. To clarify the evolutionary history of these systems in these unusual eukaryotes, we generated a genome assembly for the free-living metamonad Carpediemonas membranifera and carried out a comparative genomics analysis. Here, we show that parasitic and free-living metamonads harbor an incomplete set of proteins for processing and segregating DNA. Unexpectedly, Carpediemonas species are further streamlined, lacking the origin recognition complex, Cdc6 and most structural kinetochore subunits. Carpediemonas species are thus the first known eukaryotes that appear to lack this suite of conserved complexes, suggesting that they likely rely on yet-to-be-discovered or alternative mechanisms to carry out these fundamental processes.

Abstract

BackgroundIn response to the cancellation of clinical clerkships due to COVID-19, the Johns Hopkins (JH) Neurology Education Team developed a virtual elective to enhance medical students’ clinical telemedicine skills and foster community between academic institutions.MethodsThis two-week clinical elective, entitled “Virtual Patient Rounds in Neurology,” was administered once in April 2020 and once in May 2020. The curriculum included attending/fellow-led Virtual Rounds, Student Presentations, and Asynchronous Educational Activities. We also developed a new lecture series entitledJHNeuroChats, which consisted of live synchronous lectures presented by JH faculty and Virtual Visiting Professors. Trainees and faculty from outside institutions were invited to participate in theJHNeuroChats. Students and faculty completed pre- and post-elective surveys to assess the educational impact of the elective. Student’st-tests were used to compare scores between pre- and post-elective surveys.ResultsSeven JH medical students enrolled in each iteration of the elective, and an additional 337 trainees and faculty, representing 14 different countries, registered for theJHNeuroChats. We hosted 48 uniqueJHNeuroChats, 32 (66.7%) of which were led by invited Virtual Visiting Professors. At the end of the elective, students reported increased confidence in virtually obtaining a history (P P P = 0.15).ConclusionsDespite the constraints imposed by COVID-19, this virtual Neurology elective increased medical students’ confidence in certain telemedicine skills and successfully broadened our learning community to encompass learners from around the world. As virtual medical education becomes more prevalent, it is important that we are intentional in creating opportunities for shared learning across institutions. We believe that this elective can serve as a model for these future educational collaborations.

Abstract

The proteolytic enzymes from V. cundinamarcensis latex, (P1G10), display healing activity in animal models following various types of lesions. P1G10 or the purified isoforms act as mitogens on fibroblast and epithelial cells by stimulating angiogenesis and wound healing in gastric and cutaneous ulcers models. Based on evidence that plant proteinases act as antitumorals, we verified this effect on a murine melanoma model. The antitumoral effect analyzed mice survival and tumor development after subcutaneous administration of P1G10 into C57BL/6J mice bearing B16F1 low metastatic melanoma. Possible factors involved in the antitumoral action were assessed, i.e., cytotoxicity, cell adhesion and apoptosis in vitro, haemoglobin (Hb), vascular endothelial growth factor (VEGF), tumor growth factor-β (TGF-β), tumor necrosis factor-α (TNF-α) content and N-acetyl-glucosaminidase (NAG) activity. We observed that P1G10 inhibited angiogenesis measured by the decline of Hb and VEGF within the tumor, and TGF-β displayed a non-significant increase and TNF-α showed a minor non-significant reduction. On the other hand, there was an increase in NAG activity. In treated B16F1 cells, apoptosis was induced along with decreased cell binding to extracellular matrix components (ECM) and anchorage, without impairing viability.

Abstract

Coronavirus disease 2019 (Covid-19) active cases continue to demand the development of safe and effective treatments. This is the first clinical trial to evaluate the safety and efficacy of oral thymic peptides. ; We conducted a nonrandomized phase 2 trial with a historic control group to evaluate the safety and efficacy of a daily 250-mg oral dose of thymic peptides in the treatment of hospitalized Covid-19 patients. Comparisons based on standard care from registry data were performed after propensity score matching. The primary outcomes were survival, time to recovery, and number of participants with treatment-related adverse events or side effects by day 20. ; A total of 44 patients were analyzed in this study: 22 in the thymic peptide group and 22 in the standard care group. There were no deaths in the intervention group compared to 24% mortality in standard care by day 20 (log-rank P=0.02). Kaplan-Meier analysis showed a significantly shorter time to recovery by day 20 in the thymic peptide group than in the standard care group (median, 6 days vs. 12 days; hazard ratio for recovery, 2.75 [95% confidence interval, 1.34 to 5.62]; log-rank P=0.002). No side effects or adverse events were reported. ; In patients hospitalized with Covid-19, the use of thymic peptides resulted in no side effects, adverse events, or deaths by day 20. Compared with the registry data, a significantly shorter time to recovery and mortality reduction were measured.

Abstract

Objectives In 2015, the National Academy of Medicine IOM estimated that 12 million patients were misdiagnosed annually. This suggests that despite prolonged training in medical school and residency there remains a need to improve diagnostic reasoning education. This study evaluates a new approach. Methods A total of 285 medical students were enrolled in this 8 center, IRB approved trial. Students were randomized to receive training in either abdominal pain (AP) or loss of consciousness (LOC). Baseline diagnostic accuracy of the two different symptoms was assessed by completing a multiple-choice question (MCQ) examination and virtual patient encounters. Following a structured educational intervention, including a lecture on the diagnostic approach to that symptom and three virtual patient practice cases, each student was re-assessed. Results The change in diagnostic accuracy on virtual patient encounters was compared between (1) baseline and post intervention and (2) post intervention students trained in the prescribed symptom vs. the alternate symptom (controls). The completeness of the student’s differential diagnosis was also compared. Comparison of proportions were conducted using χ 2-tests. Mixed-effects regressions were used to examine differences accounting for case and repeated measures. Compared with baseline, both the AP and LOC groups had marked post-intervention improvements in obtaining a correct final diagnosis; a 27% absolute improvement in the AP group (p Conclusions The study showed that a virtual patient platform combined with a diagnostic reasoning framework could be used for education and diagnostic assessment and improved correct diagnosis compared with baseline performance in a simulated platform.

47. Observations on Working Psychoanalytically with a Profoundly Amnesic Patient

Abstract

Individuals with profound amnesia are markedly impaired in explicitly recalling new episodic events, but appear to preserve the capacity to use information from other sources. Amongst these preserved capacities is the ability to form new memories of an emotional nature – a skill at the heart of developing and sustaining interpersonal relationships. The psychoanalytic study of individuals with profound amnesia might contribute to the understanding the importance of each memory system, including effects on key analytic processes such as transference and countertransference. However, psychoanalytic work in the presence of profound amnesia might also require important technical modifications. In the first report of its kind, we describe observations from a long term psychoanalytic process (72 sessions) with an individual (JL) who has profound amnesia after an anoxic episode. The nature of therapy was shaped by JL’s impairment in connecting elements that belong to distant (and even relatively close) moments in the therapeutic process. However, we were also able to document areas of preservation, in what appears to be a functioning therapeutic alliance. As regards transference, the relationship between JL and his analyst can be viewed as the evolution of a narcissistic transference, and case material is provided that maps this into three phases: (i) rejecting; (ii) starting to take in; and (iii) full use of the analytic space – where each phase exhibits differing degrees of permeability between JL and the analyst. This investigation appears to have important theoretical implications for psychoanalytic practice, and for psychotherapy in general – and not only with regard to brain injured populations. We especially note that it raises questions concerning the mechanism of therapeutic action in psychoanalysis and psychotherapy, and the apparent unimportance of episodic memory for many elements of therapeutic change.

Objective: We sought to explore whether obstetrics and gynecology (Ob-Gyn) ambulatory clinic preceptors can maintain their clinical productivity with a learner (medical student) present. Methods: We studied the productivity of five exemplary Ob-Gyn faculty over the 2016-2017 academic year. We used paired two-tailed t tests to compare physician productivity with and without a student. Faculty were interviewed and qualitative analyses were performed on faculty interview data to identify themes used to create sample best-practice workflows for student involvement in the clinic. Abstract

In the past decade, there have been dramatic changes in all aspects of neurologic care, and along with this, neurology education has transformed. These changes have affected all aspects of education across the educational continuum, including learners, teachers, educators, content, delivery methods, assessments, and outcomes. Health systems science, health humanities, diversity, equity, and inclusion and health disparities are becoming core components of neurology curricula, and, in the future, will be integrated into every aspect of our educational mission. The ways in which material is taught and learned have been influenced by technologic innovations and a growing understanding of the science of learning. We forecast that this trend will continue, with learners choosing from an array of electronic resources to engage with fundamental topics, allowing front-line clinical teachers to spend more time supporting critical reasoning and teaching students how to learn. There has been a growing differentiation of educational roles (i.e., teachers, educators, and scholars). We forecast that these roles will become more distinct, each with an individualized pattern of support and expectations. Assessment has become more aligned with the work of the learners, and there are growing calls to focus more on the impact of educational programs on patient care. We forecast that there will be an increased emphasis on educational outcomes and public accountability for training programs. In this article, we reflect on the history of medical education in neurology and explore the current state to forecast the future of neurology education and discuss ways in which we can prepare.

Abstract

Study question Does the functionally active microbial composition at the receptive phase endometria differ between women with recurrent implantation failure (RIF) and healthy controls? Summary answer Women with RIF have different functionally active microbiota in the receptive-phase endometria that are involved in distinct metabolical pathways when compared to healthy women. What is known already Recurrent implantation failure (RIF) is a common and burdening diagnosis in infertility treatment. Despite scientific advances in the field, the cause of RIF remains unclear and the treatment options ambiguous. Microorganisms have important role in human physiology and pathophysiology, and their potential involvement in endometrial functions and health are gaining support. Our previous work of microbiota mapping of healthy endometrium identified over 5000 functionally active microorganisms (bacteria, viruses, fungi, archaea) that participate in key metabolic pathways for embryo implantation. In the present work we focus on microbiota mapping in RIF patients, where endometrial dysfunction is suspected. Study design, size, duration A case-control age-match study of 20 RIF and 20 healthy controls was performed. Endometrial samples were obtained at the mid-secretory phase (LH + 7/8) using Pipelle curette. Participants/materials, setting, methods Total RNA sequencing (RNA-seq) was performed. R package MetagenomeSeq was used for metataxonomic microorganismal analyses and HUMAnN2 algorithm was used for functional metagenome assessment. The microbial maps were created using Kraken metagenomic sequence classification. With this approach we were able to identify and map the ‘active’ microorganisms in samples. Main results and the role of chance We identified a total of 4175 microorganisms in the human endometrial samples including different bacteria, fungi, viruses, and archaea. At the taxonomic level, RIF patients demonstrated a total of 180 microorganism with differential abundance when compared to controls (FDR Limitations, reasons for caution Poly A enrichment could avoid the detection of some microorganism. However, the resolution power in microbial identification was comparable to previous metatranscriptomics published studies. Wider implications of the findings Our study findings are first to demonstrate that alive microbes are involved in the uterine pathology as is RIF, where different metabolic pathways could be dysregulated. This knowledge could help to develop pre-/probiotic strategies to modulate uterine environment for improving endometrial functions in RIF. Trial registration number not applicable
