Abstract: Aim: The SAMe-TT2R2 score helps identify patients with atrial fibrillation (AF) likely to have poor anticoagulation control during anticoagulation with vitamin K antagonists (VKA) and those with scores >2 might be better managed with a target-specific oral anticoagulant (NOAC). We hypothesized that in clinical practice, VKAs may be prescribed less frequently to patients with AF and SAMe-TT2R2 scores >2 than to patients with lower scores. Methods and results: We analyzed the Phase III dataset of the Global Registry on Long-Term Oral Antithrombotic Treatment in Patients with Atrial Fibrillation (GLORIA-AF), a large, global, prospective global registry of patients with newly diagnosed AF and ≥1 stroke risk factor. We compared baseline clinical characteristics and antithrombotic prescriptions to determine the probability of the VKA prescription among anticoagulated patients with the baseline SAMe-TT2R2 score >2 and ≤ 2. Among 17,465 anticoagulated patients with AF, 4,828 (27.6%) patients were prescribed VKA and 12,637 (72.4%) patients an NOAC: 11,884 (68.0%) patients had SAMe-TT2R2 scores 0-2 and 5,581 (32.0%) patients had scores >2. The proportion of patients prescribed VKA was 28.0% among patients with SAMe-TT2R2 scores >2 and 27.5% in those with scores ≤2. Conclusions: The lack of a clear association between the SAMe-TT2R2 score and anticoagulant selection may be attributed to the relative efficacy and safety profiles between NOACs and VKAs as well as to the absence of trial evidence that an SAMe-TT2R2-guided strategy for the selection of the type of anticoagulation in NVAF patients has an impact on clinical outcomes of efficacy and safety. The latter hypothesis is currently being tested in a randomized controlled trial. Abstract: Abstract Introduction The standard treatment for patients with focal drug-resistant epilepsy (DRE) who are not eligible for open brain surgery is the continuation of anti-seizure medication (ASM) and neuromodulation. This treatment does not cure epilepsy but only decreases severity. The PRECISION trial offers a non-invasive, possibly curative intervention for these patients, which consist of a single stereotactic radiotherapy (SRT) treatment. Previous studies have shown promising results of SRT in this patient population. Nevertheless, this intervention is not yet available and reimbursed in the Netherlands. We hypothesize that: SRT is a superior treatment option compared to palliative standard of care, for patients with focal DRE, not eligible for open surgery, resulting in a higher reduction of seizure frequency (with 50% of the patients reaching a 75% seizure frequency reduction at 2 years follow-up). Methods In this waitlist-controlled phase 3 clinical trial, participants are randomly assigned in a 1:1 ratio to either receive SRT as the intervention, while the standard treatments consist of ASM continuation and neuromodulation. After 2-year follow-up, patients randomized for the standard treatment (waitlist-control group) are offered SRT. Patients aged ≥ 18 years with focal DRE and a pretreatment defined epileptogenic zone (EZ) not eligible for open surgery will be included. The intervention is a LINAC-based single fraction (24 Gy) SRT treatment. The target volume is defined as the epileptogenic zone (EZ) on all (non) invasive examinations. The seizure frequency will be monitored on a daily basis using an electronic diary and an automatic seizure detection system during the night. Potential side effects are evaluated using advanced MRI, cognitive evaluation, Common Toxicity Criteria, and patient-reported outcome questionnaires. In addition, the cost-effectiveness of the SRT treatment will be evaluated. Discussion This is the first randomized trial comparing SRT with standard of care in patients with DRE, non-eligible for open surgery. The primary objective is to determine whether SRT significantly reduces the seizure frequency 2 years after treatment. The results of this trial can influence the current clinical practice and medical cost reimbursement in the Netherlands for patients with focal DRE who are not eligible for open surgery, providing a non-invasive curative treatment option. Abstract: Objectives: The presence of intracranial lesions or epilepsy may lead to functional reorganization and hemispheric lateralization. We applied a clinical magnetoencephalography (MEG) protocol for the localization of the contralateral and ipsilateral S1 and M1 of the foot and hand in patients with non-lesional epilepsy, stroke, developmental brain injury, traumatic brain injury and brain tumors. We investigated whether differences in activation patterns could be related to underlying pathology. Methods: Using dipole fitting, we localized the sources underlying sensory and motor evoked magnetic fields (SEFs and MEFs) of both hands and feet following unilateral stimulation of the median nerve (MN) and posterior tibial nerve (PTN) in 325 consecutive patients. The primary motor cortex was localized using beamforming following a self-paced repetitive motor task for each hand and foot. Abstract: Background: Colchicine reduces risk of cardiovascular events in patients post–myocardial infarction and in patients with chronic coronary disease. It remains unclear whether this effect is related to the time of onset of treatment following an acute coronary syndrome (ACS). Objectives: This study investigates risk for major adverse cardiovascular events in relation to history and timing of prior ACS, to determine whether the benefits of colchicine are consistent independent of prior ACS status. Methods: The LoDoCo2 (Low-Dose Colchicine 2) trial randomly allocated patients with chronic coronary disease to colchicine 0.5 mg once daily or placebo. The rate of the composite of cardiovascular death, spontaneous myocardial infarction, ischemic stroke, or ischemia-driven coronary revascularization was compared between patients with no prior, recent (6-24 months), remote (2-7 years), or very remote (>7 years) ACS; interaction between ACS status and colchicine treatment effect was assessed. Results: In 5,522 randomized patients, risk of the primary endpoint was independent of prior ACS status. Colchicine consistently reduced the primary endpoint in patients with no prior ACS (incidence: 2.8 vs 3.4 events per 100 person-years; hazard ratio [HR]: 0.81; 95% confidence interval [CI]: 0.52-1.27), recent ACS (incidence: 2.4 vs 3.3 events per 100 person-years; HR: 0.75; 95% CI: 0.51-1.10), remote ACS (incidence: 1.8 vs 3.2 events per 100 person-years, HR: 0.55; 95% CI: 0.37-0.82), and very remote ACS (incidence: 3.0 vs 4.3 events per 100 person-years, HR: 0.70; 95% CI: 0.51-0.96) (P for interaction = 0.59). Conclusions: The benefits of colchicine are consistent irrespective of history and timing of prior ACS. Abstract: In South-West Uganda, manure is highly valued for sustaining yields of East African Highland Banana, but it is in short supply. As a result, banana growers import manure from rangelands up to 50 km away. We aimed to explore the potential of this regional banana-livestock integration to meet crop nutrient requirements for sustainable intensification of banana cropping systems. We used a mixed-methods approach supported by detailed data collection. Multiple spatial levels were integrated: field-level modelling to determine long-term nutrient requirements, a household-level survey to characterize farmer practices, and a regional-level spatial analysis to map banana production and manure source areas. For median to 90th percentile banana yields (37-52 t FW/ha/year), minimum K requirements were 118–228 kg/ha/year. To supply this with manure, 10.5–20.5 t DM manure/ha/year would be needed, requiring 47–91 tropical livestock units and 27–52 ha of rangeland, far more than what is potentially available currently. However, using only manure to satisfy potassium requirements increases the risk of N losses due to nutrient imbalances likely to result from large manure applications. For sustainable intensification, manure supplemented with K-based fertilizers is a better option than manure alone, as it is more cost-effective and reduces potential N losses.

Abstract: Many interventions are assuming that introduced seeds diffuse. However, the details of this diffusion among farmers are poorly understood. This article presents data from eight sites in four on the diffusion of seed and associated information given to farmers involved in N2Africa’s demonstration trials. The study showed that 2–3 years after the trials had been organised, more than 90% of the farmers who had participating in the trial activities and were given a seed-input package with 1–5 kg of legume seed had shared this seed, on average with four other farmers. The farmers who received this seed from these directly involved farmers shared their seed less frequently. Eighty per cent of all the seed sharings were of 1–2 kg of seed given as a gift. Only 5% of the sharings involved a cash transaction. More than half of the seed sharings were with family members and around a third were between friends. Men shared at least as often as women and both men and women shared most with persons of their own sex. Information about rhizobium as an associated input for soya was shared by more than one-third of farmers, almost exclusively by farmers who had participated in the demonstration trials themselves. Extrapolation of data suggest that in addition to the 250,000 farmers who participated directly in the N2Africa demonstration trials, another 1,400,000 farmers may have received seed of a new legume crop or variety. The results show that knowing about the character of the seed sharing mechanisms may offer opportunities to influence the diffusion of seeds. Providing farmers with somewhat larger amounts of seeds, emphasise the importance of sharing seeds and information with relatives and friends could be an important factor in achieving a high multiplier effect.

Abstract: SUMMARYThe success of scaling out depends on a clear understanding of the factors that affect adoption of grain legumes and account for the dynamism of those factors across heterogeneous contexts of sub-Saharan Africa. We reviewed literature on adoption of grain legumes and other technologies in sub-Saharan Africa and other developing countries. Our review enabled us to define broad factors affecting different components of the scaling out programme of N2Africa and the scales at which those factors were important. We identified three strategies for managing those factors in the N2Africa scaling out programme: (i) testing different technologies and practices; (ii) evaluating the performance of different technologies in different contexts; and (iii) monitoring factors that are difficult to predict. We incorporated the review lessons in a design to appropriately target and evaluate technologies in multiple contexts across scales from that of the farm to whole countries. Our implementation of this design has only been partially successful because of competing reasons for selecting activity sites. Nevertheless, we observe that grain legume species have been successfully targeted for multiple biophysical environments across sub-Saharan Africa, and to social and economic contexts within countries. Rhizobium inoculant and legume specific fertiliser blends have also been targeted to specific contexts, although not in all countries. Relatively fewer input and output marketing models have been tested due to public–private partnerships, which are a key mechanism for dissemination in the N2Africa project.

8. 'Functional connectivity' is a sensitive predictor of epilepsy diagnosis after the first seizure.

Abstract: BACKGROUND: Although epilepsy affects almost 1% of the world population, diagnosis of this debilitating disease is still difficult. The EEG is an important tool for epilepsy diagnosis and classification, but the sensitivity of interictal epileptiform discharges (IEDs) on the first EEG is only 30-50%. Here we investigate whether using 'functional connectivity' can improve the diagnostic sensitivity of the first interictal EEG in the diagnosis of epilepsy. METHODOLOGY/PRINCIPAL FINDINGS: Patients were selected from a database with 390 standard EEGs of patients after a first suspected seizure. Patients who were later diagnosed with epilepsy (i.e. > or = two seizures) were compared to matched non-epilepsy patients (with a minimum follow-up of one year). The synchronization likelihood (SL) was used as an index of functional connectivity of the EEG, and average SL per patient was calculated in seven frequency bands. In total, 114 patients were selected. Fifty-seven patients were diagnosed with epilepsy (20 had IEDs on their EEG) and 57 matched patients had other diagnoses. Epilepsy patients had significantly higher SL in the theta band than non-epilepsy patients. Furthermore, theta band SL proved to be a significant predictor of a diagnosis of epilepsy. When only those epilepsy patients without IEDs were considered (n = 74), theta band SL could predict diagnosis with specificity of 76% and sensitivity of 62%. CONCLUSION/SIGNIFICANCE: Theta band functional connectivity may be a useful diagnostic tool in diagnosing epilepsy, especially in those patients who do not show IEDs on their first EEG. Our results indicate that epilepsy diagnosis could be improved by using functional connectivity.

Abstract: We evaluated the usefulness of a co-design process to generate a relevant basket of options for climbing bean cultivation in the context of a large-scale project. The aim was to identify a range of options sufficiently diverse to be of interest for farmers of widely-different resource endowment. The co-design process consisted of three cycles of demonstration, evaluation and re-design in the eastern and southwestern highlands of Uganda in 2014–2015. Evaluations aimed to distinguish preferences of farmers between the two areas, and among farmers of different gender and socio-economic backgrounds. Farmers, researchers, extension officers and NGO staff re-designed treatments for demonstrations in the next season. Climbing bean yields and evaluation scores varied between seasons and sites. Evaluation scores were not always in line with yields, revealing that farmers used multiple evaluation criteria next to yield, such as marketability of varieties, availability of inputs and ease of staking methods. The co-design process enriched the basket of options, improved the relevance of options demonstrated and enhanced the understanding of preferences of a diversity of users. Developing options for resource-poor farmers was difficult, however, because they face multiple constraints. The basket of options developed in this study can be applied across the East-African highlands, with an ‘option-by-context’ matrix as a starting point for out-scaling. The study also showed, however, that consistent recommendations about the suitability of technologies for different types of farmers were hard to identify. Abstract: SUMMARYThe large diversity of farms and farming systems in sub-Saharan Africa calls for agricultural improvement options that are adapted to the context in which smallholder farmers operate. The socio-ecological niche concept incorporates the agro-ecological, socio-cultural, economic and institutional dimensions and the multiple levels of this context in order to identify which options fit best. In this paper, we illustrate how farming systems analysis, following the DEED cycle of Describe, Explain, Explore and Design, and embedding co-learning amongst researchers, farmers and other stakeholders, helps to operationalize the socio-ecological niche concept. Examples illustrate how farm typologies, detailed farm characterization and on-farm experimental work, in combination with modelling and participatory approaches inform the matching of options to the context at regional, village, farm and field level. Recommendation domains at these gradually finer levels form the basis for gradually more detailed baskets of options from which farmers and other stakeholders may choose, test and adjust to their specific needs. Tailored options identified through the DEED cycle proof to be more relevant, feasible and performant as compared to blanket recommendations in terms of both researcher and farmer-identified criteria. As part of DEED, on-farm experiments are particularly useful in revealing constraints and risks faced by farmers. We show that targeting options to the niches in which they perform best, helps to reduce this risk. Whereas the conclusions of our work about the potential for improving smallholders’ livelihoods are often sobering, farming systems analysis allows substantiating the limitations of technological options, thus highlighting the need for enabling policies and institutions that may improve the larger-scale context and increase the uptake potential of options.

Abstract: Soybean yields could benefit from the use of improved varieties, phosphate-fertilizer and rhizobium inoculants. In this study, we evaluated the results of widespread testing of promiscuous soybean varieties with four treatments: no inputs (control); SSP fertilizer (P); inoculants (I) and SSP plus inoculants (P + I) among smallholder farmers in northern Nigeria in 2011 and 2012. We observed a strong response to both P and I, which significantly increased grain yields by 452 and 447 kg ha−1 respectively. The additive effect of P + I (777 kg ha−1) resulted in the best average yields. Variability in yield among farms was large, which had implications for the benefits for individual farmers. Moreover, although the yield response to P and I was similar, I was more profitable due to its low cost. Only 16% of the variability in control yields could be explained by plant establishment, days to first weeding, percentage sand and soil exchangeable magnesium. Between 42% and 61% of variability in response to P and/or I could be explained by variables including year, farm size, plant establishment, total rainfall and pH. The predictive value of these variables was limited, however, with cross-validation R2 decreasing to about 15% for the prediction between Local Government Areas and 10% between years. Implications for future research include our conclusion that averages of performance of technologies tell little about the adoption potential for individual farmers. We also conclude that a strong agronomic and economic case exists for the use of inoculants with promiscuous soybean, requiring efforts to improve the availability of good quality inoculants in Africa.

Abstract: Indicators for sustainability are a hot and debated topic. Sustainable intensification of agriculture is also widely debated due to the divergent views on the future of agriculture and the wide variety of indicators used. Legumes are seen as a key option for sustainable intensification of smallholder farming systems in sub-Saharan Africa (SSA). We developed a framework for assessing the sustainability of contrasting farming systems to illustrate the complex balancing act involved, using a case study of the N2Africa project. N2Africa offers legume options to farmers in SSA (www.N2Africa.org). We worked at farm household level and used a hierarchical framework of principles and criteria to select the indicators for sustainability. One of the main outcomes is a list of questions and hurdles we ran into when developing the framework. This can be used by others as guidance both when choosing indicators and to critically evaluate existing sustainability assessments. We illustrate that many of the decisions made in developing an indicator framework are subjective and that they include important but easily overlooked details. We conclude that, only by being explicit about the steps taken and the assumptions and decisions made, one can develop a sustainability framework that results in meaningful outcomes.

Abstract: Climbing beans offer potential for sustainable intensification in the East-African highlands, but their introduction requires a major change in the cropping system compared with the commonly grown bush bean. We explored farm-level opportunities, constraints and trade-offs for climbing bean cultivation in the eastern highlands of Uganda. We established current food self-sufficiency, income, investment costs and labour, and assessed the ex-ante, farm-level impact of four climbing bean options on these indicators. Input for this assessment were a detailed characterization of 16 farms of four types, and on-farm, experimental data of adaptation trials of climbing bean. Climbing beans generally improved food self-sufficiency and income, but often required increased financial investment and always demanded more labour than current farm configurations. Opportunities for integration of climbing beans on small farms were limited. Although some of the poorest farmers accrued the largest absolute benefits from climbing beans, their ability to make the necessary investments is questionable. The analysis was translated into a simple-to-use modelling tool to enable participatory analysis of the outcomes with farmers of the four farm types to understand their perspectives and decision-making. The discussions revealed a recent increase in market prices for climbing bean resulting in growing interest in their cultivation in the eastern highlands. A lack of seed and stakes was limiting climbing bean cultivation, and a sufficient amount of climbing bean seed needs to be ensured through strengthening of farmer cooperatives and improved storage.

Abstract: Background— The aim of this study is to examine the relationship between time in the therapeutic range (TTR) and clinical outcomes in heart failure patients in sinus rhythm treated with warfarin. Methods and Results— We used data from the Warfarin versus Aspirin in Reduced Cardiac Ejection Fraction (WARCEF) trial to assess the relationship of TTR with the WARCEF primary outcome (ischemic stroke, intracerebral hemorrhage, or death), with death alone, ischemic stroke alone, major hemorrhage alone, and net clinical benefit (primary outcome and major hemorrhage combined). Multivariable Cox models were used to examine how the event risk changed with TTR and to compare the high TTR, low TTR, and aspirin-treated patients, with TTR being treated as a time-dependent covariate. A total of 2217 patients were included in the analyses; among whom 1067 were randomized to warfarin and 1150 were randomized to aspirin. The median (interquartile range) follow-up duration was 3.6 (2.0–5.0) years. Mean (±SD) age was 61±11.3 years, with 80% being men. The mean (±SD) TTR was 57% (±28.5%). Increasing TTR was significantly associated with reduction in primary outcome (adjusted P P =0.001), and improved net clinical benefit (adjusted P P =0.082 for ischemic stroke and adjusted P =0.109 for major hemorrhage). Conclusions— In patients with heart failure in sinus rhythm, increasing TTR is associated with better outcome and improved net clinical benefit. Patients in whom good quality anticoagulation can be achieved may benefit from the use of anticoagulants. Clinical Trial Registration— URL: http://www.clinicaltrials.gov . Abstract: BACKGROUND: The field of acute coronary syndromes is characterised by an increasing tendency towards early invasive catheter-based diagnostics and therapeutics-a practice based on observational and retrospective data. OBJECTIVE: To compare immediate versus deferred angioplasty in patients with non-ST-segment elevation acute coronary syndromes (NSTE-ACS) METHODS: A randomised, prospective multicentre trial was performed in patients admitted with NSTE-ACS, eligible for percutaneous coronary intervention (PCI). Interim analysis was performed after enrolment of 251 patients; PCI was appropriate in 142 patients. These patients were randomised to immediate PCI (n = 73) or deferred PCI (24-48 h) (n = 69). Patients received protocol-driven glycoprotein IIb/IIIa blockers, aspirin and clopidogrel. The primary end point was a composite of death, non-fatal myocardial infarction (MI) or unplanned revascularisation, at 30 days. After hospital discharge outpatient follow-up was performed at 30 days and 6 months. RESULTS: The incidence at 30 days of the primary end point was 60% in the group receiving immediate PCI and 39% in the group receiving deferred PCI (relative risk (RR) = 1.5, 95% CI 1.09 to 2.15; p = 0.004). No deaths occurred in either group. MI was significantly more common in the group receiving immediate PCI (60% vs 38%, RR = 1.6, 95% CI 1.12 to 2.28, p = 0.005). Unplanned revascularisation was similar in both groups. The observed difference was preserved over 6-months' follow-up. CONCLUSIONS: Immediate PCI was associated with an increased rate of MI in comparison with a 24-48 h deferred strategy, despite aggressive antithrombotic treatment. The results suggest that PCI for high-risk patients with non-refractory NSTE-ACS should be delayed for at least 24 h after hospital admission. TRIAL REGISTRATION NUMBER: ISRCTN80874637

Abstract: Survival in patients with high grade glioma has been extended in recent years as a result of more intensive therapy. As a consequence, more late term complications of treatment may be observed.A 69-year-old woman presented at the outpatient department of Neurology with headache and loss of strength in the left arm. She had been treated 7 years previously for a high grade glioma with resection and radiotherapy. One year later she had received chemotherapy for a local recurrence. Since then she was free of complaints. At investigation a left sided hemiparesis was found. As recurrence of the tumour was suspected, MR imaging of the brain was performed, which showed abnormalities suggestive for the so called 'stroke-like migraine attacks after radiotherapy' (SMART) syndrome. The further clinical course, with spontaneous recovery of strength within a few weeks and the regression of the cortical hyperintensity on MRI, confirmed the probable diagnosis.The SMART syndrome is a relatively unknown condition and should be included in the differential diagnosis in patients who present with new complaints long after cerebral radiotherapy.

Abstract: Item does not contain fulltext BACKGROUND: The effect of intensified platelet inhibition for patients with unstable angina or myocardial infarction without ST-segment elevation who do not undergo revascularization has not been delineated. METHODS: In this double-blind, randomized trial, in a primary analysis involving 7243 patients under the age of 75 years receiving aspirin, we evaluated up to 30 months of treatment with prasugrel (10 mg daily) versus clopidogrel (75 mg daily). In a secondary analysis involving 2083 patients 75 years of age or older, we evaluated 5 mg of prasugrel versus 75 mg of clopidogrel. RESULTS: At a median follow-up of 17 months, the primary end point of death from cardiovascular causes, myocardial infarction, or stroke among patients under the age of 75 years occurred in 13.9% of the prasugrel group and 16.0% of the clopidogrel group (hazard ratio in the prasugrel group, 0.91; 95% confidence interval [CI], 0.79 to 1.05; P=0.21). Similar results were observed in the overall population. The prespecified analysis of multiple recurrent ischemic events (all components of the primary end point) suggested a lower risk for prasugrel among patients under the age of 75 years (hazard ratio, 0.85; 95% CI, 0.72 to 1.00; P=0.04). Rates of severe and intracranial bleeding were similar in the two groups in all age groups. There was no significant between-group difference in the frequency of nonhemorrhagic serious adverse events, except for a higher frequency of heart failure in the clopidogrel group. CONCLUSIONS: Among patients with unstable angina or myocardial infarction without ST-segment elevation, prasugrel did not significantly reduce the frequency of the primary end point, as compared with clopidogrel, and similar risks of bleeding were observed. Abstract: It is a major challenge to embed modern financial technology in the continuously changing organisational structure of a multinational company. The processes generating cash flow are to be controlled and proper forecasting models are to be implemented. Based on quarterly rolling forecasts the basic information for managing transaction exposures is provided. Clear guidelines from the “internal bank” and involvement of its financial experts in formulating currency policies at a decentralised Products Division level are key success factors in increasing awareness and knowledge at the level where the business decisions are taken.

23. Real world data on adjuvant treatment of older HER2-positive breast cancer patients – A single institution experience through 8 years

Abstract: Background: The large randomized trials on trastuzumab for primary breast cancer (BC) included few old patients. With exception of endocrine treatment, trials on adjuvant therapy for the old group specifically are scarce. Aims: To compare adjuvant treatment, recurrences and survival in old and older-old patients with primary HER2 positive BC. Material and methods: Patients ≥ 70 years with diagnose of primary HER2 positive BC from 2008 through 2015 were included in this retrospective non-randomized investigation. Standard clinical and biological data (age, surgery, tumor size, nodal status, histopathological grade, vascular invasion, expression of hormone receptors, recurrences and death) were extracted from patient's charts. Comparisons were performed according to age (old; 70–74 years vs older old; ≥ 75 years) and treatment with trastuzumab or not. Patients that initiated adjuvant trastuzumab but did not complete one year (n = 8) were included in the trastuzumab group in survival analyzes. Recurrence-free survival (RFS) and overall survival (OS) were calculated in uni- and multivariate analyses. Results: A total of 115 patients were registered, eleven patients had distant metastasis and seven were omitted from all treatment including primary surgery due to serious concomitant illness and a poor general condition leaving 97 patients for analysis. There were no differences between the groups (70–74; n = 40), (≥75; n = 57) in tumor size (p = 0.86), nodal status (p = 0.10), ER (p = 0.25), PgR (p = 1.0) or vascular invasion (p = 1.0). Abstract: The problem of estimating the slope of a linear relationship between two jointly normally distributed random variables is considered when outliers may occur in the explanatory variable. It will be studied as a special case of an errors-in-variables problem where the explanatory variable is measured which a nonnormally distributed error. In this more general model and under certain conditions a consistent estimator can be given with a normal limiting distribution. Applications to cases of outliers in the explanatory variable will be presented.

Abstract: The robustness literature provides many solutions to the problem of detecting and handling outliers. In this paper we study how to detect ‘influential points’ in a simultaneous linear model. The model describes the Dutch economy during the post-war period. Outliers in both explanatory and the dependent variables are located by using regression diagnostics. The (interactive) computerprogram lists influential points by methods due to Belsley, Kuh and Welsch. The estimation method is modified two stage least squares. Finally simultaneous predictions are presented and compared to the 2SLS predictions and the realisations.

Abstract: AIMS: This report describes the baseline characteristics of patients in the Reduction of Events with Darbepoetin alfa in Heart Failure trial (RED-HF) which is testing the hypothesis that anaemia correction with darbepoetin alfa will reduce the composite endpoint of death from any cause or hospital admission for worsening heart failure, and improve other outcomes. METHODS AND RESULTS: Key demographic, clinical, and laboratory findings, along with baseline treatment, are reported and compared with those of patients in other recent clinical trials in heart failure. Compared with other recent trials, RED-HF enrolled more elderly [mean age 70 (SD 11.4) years], female (41%), and black (9%) patients. RED-HF patients more often had diabetes (46%) and renal impairment (72% had an estimated glomerular filtration rate < 60 mL/min/1.73 m2). Patients in RED-HF had heart failure of longer duration [5.3 (5.4) years], worse NYHA class (35% II, 63% III, and 2% IV), and more signs of congestion. Mean EF was 30% (6.8%). RED-HF patients were well treated at randomization, and pharmacological therapy at baseline was broadly similar to that of other recent trials, taking account of study-specific inclusion/exclusion criteria. Median (interquartile range) haemoglobin at baseline was 112 (106-117) g/L. CONCLUSION: The anaemic patients enrolled in RED-HF were older, moderately to markedly symptomatic, and had extensive co-morbidity. Fil: McMurray, John J. V.. 35. Automated cardiac arrest detection using a photoplethysmography wristband: algorithm development and validation in patients with induced circulatory arrest in the DETECT-1 study.

Abstract: Background: Unwitnessed out-of-hospital cardiac arrest is associated with low survival chances because of the delayed activation of the emergency medical system in most cases. Automated cardiac arrest detection and alarming using biosensor technology would offer a potential solution to provide early help. We developed and validated an algorithm for automated circulatory arrest detection using wrist-derived photoplethysmography from patients with induced circulatory arrests., Methods: In this prospective multicentre study in three university medical centres in the Netherlands, adult patients (aged 18 years or older) in whom short-lasting circulatory arrest was induced as part of routine practice (transcatheter aortic valve implantation, defibrillation testing, or ventricular tachycardia induction) were eligible for inclusion. Exclusion criteria were a known bilateral significant subclavian artery stenosis or medical issues interfering with the wearing of the wristband. After providing informed consent, patients were equipped with a photoplethysmography wristband during the procedure. Invasive arterial blood pressure and electrocardiography were continuously monitored as the reference standard. Development of the photoplethysmography algorithm was based on three consecutive training cohorts. For each cohort, patients were consecutively enrolled. When a total of 50 patients with at least one event of circulatory arrest were enrolled, that cohort was closed. Validation was performed on the fourth set of included patients. The primary outcome was sensitivity for the detection of circulatory arrest., Findings: Of 306 patients enrolled between March 14, 2022, and April 21, 2023, 291 patients were included in the data analysis. In the development phase (n=205), the first training set yielded a sensitivity for circulatory arrest detection of 100% (95% CI 94-100) and four false positive alarms; the second training set yielded a sensitivity of 100% (94-100), with six false positive alarms; and the third training set yielded a sensitivity of 100% (94-100), with two false positive alarms. In the validation phase (n=86), the sensitivity for circulatory arrest detection was 98% (92-100) and 11 false positive circulatory arrest alarms. The positive predictive value was 90% (95% CI 82-94)., Interpretation: The automated detection of induced circulatory arrests using wrist-derived photoplethysmography is feasible with good sensitivity and low false positives. 36. Evaluation of a novel cuffless photoplethysmography-based wristband for measuring blood pressure according to the regulatory standards.

Abstract: Aims: Elevated blood pressure (BP) is a key risk factor in cardiovascular diseases. However, obtaining reliable and reproducible BP remains a challenge. This study, therefore, aimed to evaluate a novel cuffless wristband, based on photoplethysmography (PPG), for continuous BP monitoring., Methods and Results: Predictions by a PPG-guided algorithm were compared to arterial BP measurements (in the sub-clavian artery), obtained during cardiac catheterization. Eligible patients were included and screened based on AAMI/European Society of Hypertension (ESH)/ISO Universal Standard requirements. The machine learning-based BP algorithm required three cuff-based initialization measurements in combination with ∼100 features (signal-derived and patient demographic-based). Ninety-seven patients and 420 samples were included. Mean age, weight, and height were 67.1 years (SD 11.1), 83.4 kg (SD 16.1), and 174 cm (SD 10), respectively. Systolic BP was ≤100 mmHg in 48 samples (11%) and ≥160 mmHg in 106 samples (25%). Diastolic BP was ≤70 mmHg in 222 samples (53%) and ≥85 mmHg in 99 samples (24%). The algorithm showed mean errors of ±3.7 mmHg (SD 4.4 mmHg) and ±2.5 mmHg (SD 3.7 mmHg) for systolic and diastolic BP, respectively. Similar results were observed across all genders and skin colours (Fitzpatrick I-VI)., Conclusion: This study provides initial evidence for the accuracy of a PPG-based BP algorithm in combination with a cuffless wristband across a range of BP distributions. 37. Acute Coronary Syndrome Subphenotypes Based on Repeated Biomarker Measurements in Relation to Long-Term Mortality Risk.

Abstract: Background: We aimed to identify patients with subphenotypes of postacute coronary syndrome (ACS) using repeated measurements of high-sensitivity cardiac troponin T, N-terminal pro-B-type natriuretic peptide, high-sensitivity C-reactive protein, and growth differentiation factor 15 in the year after the index admission, and to investigate their association with long-term mortality risk., Methods and Results: BIOMArCS (BIOMarker Study to Identify the Acute Risk of a Coronary Syndrome) was an observational study of patients with ACS, who underwent high-frequency blood sampling for 1 year. Biomarkers were measured in a median of 16 repeated samples per individual. Cluster analysis was performed to identify biomarker-based subphenotypes in 723 patients without a repeat ACS in the first year. Patients with a repeat ACS (N=36) were considered a separate cluster. Differences in all-cause death were evaluated using accelerated failure time models (median follow-up, 9.1 years; 141 deaths). Three biomarker-based clusters were identified: cluster 1 showed low and stable biomarker concentrations, cluster 2 had elevated concentrations that subsequently decreased, and cluster 3 showed persistently elevated concentrations. The temporal biomarker patterns of patients in cluster 3 were similar to those with a repeat ACS during the first year. Clusters 1 and 2 had a similar and favorable long-term mortality risk. Cluster 3 had the highest mortality risk. The adjusted survival time ratio was 0.64 (95% CI, 0.44-0.93; P =0.018) compared with cluster 1, and 0.71 (95% CI, 0.39-1.32; P =0.281) compared with patients with a repeat ACS., Conclusions: Patients with subphenotypes of post-ACS with different all-cause mortality risks during long-term follow-up can be identified on the basis of repeatedly measured cardiovascular biomarkers. Patients with persistently elevated biomarkers have the worst outcomes, regardless of whether they experienced a repeat ACS in the first year.

A comprehensive meta-analysis of studies on home telemonitoring systems (hTMS) in HF and the effect on clinical outcomes are provided., Methods and Results: A systematic literature search was performed in four bibliographic databases, including randomized trials and observational studies that were published during January 1996-July 2022. A random-effects meta-analysis was carried out comparing hTMS with standard of care. All-cause mortality, first HF hospitalization, and total HF hospitalizations were evaluated as study endpoints. Sixty-five non-invasive hTMS studies and 27 invasive hTMS studies enrolled 36 549 HF patients, with a mean follow-up of 11.5 months. In patients using hTMS compared with standard of care, a significant 16% reduction in all-cause mortality was observed [pooled odds ratio (OR): 0.84, 95% confidence interval (CI): 0.77-0.93, I2: 24%], as well as a significant 19% reduction in first HF hospitalization (OR: 0.81, 95% CI 0.74-0.88, I2: 22%) and a 15% reduction in total HF hospitalizations (pooled incidence rate ratio: 0.85, 95% CI 0.76-0.96, I2: 70%)., Conclusion: These results are an advocacy for the use of hTMS in HF patients to reduce all-cause mortality and HF-related hospitalizations. 39. Consistency, variability, and predictability of on-farm nutrient responses in four grain legumes across East and West Africa.

Abstract: Grain legumes are key components of sustainable production systems in sub-Saharan Africa, but wide-spread nutrient deficiencies severely restrict yields. Whereas legumes can meet a large part of their nitrogen (N) requirement through symbiosis with N 2 -fixing bacteria, elements such as phosphorus (P), potassium (K) and secondary and micronutrients may still be limiting and require supplementation. Responses to P are generally strong but variable, while evidence for other nutrients tends to show weak or highly localised effects. Here we present the results of a joint statistical analysis of a series of on-farm nutrient addition trials, implemented across four legumes in four countries over two years. Linear mixed models were used to quantify both mean nutrient responses and their variability, followed by a random forest analysis to determine the extent to which such variability can be explained or predicted by geographic, environmental or farm survey data. Legume response to P was indeed variable, but consistently positive and we predicted application to be profitable for 67% of farms in any given year, based on prevailing input costs and grain prices. Other nutrients did not show significant mean effects, but considerable response variation was found. This response heterogeneity was mostly associated with local or temporary factors and could not be explained or predicted by spatial, biophysical or management factors. An exception was K response, which displayed appreciable spatial variation that could be partly accounted for by spatial and environmental covariables. Abstract: Aims: Evidence regarding the role of serial measurements of biomarkers for risk assessment in post-acute coronary syndrome (ACS) patients is limited. The aim was to explore the prognostic value of four, serially measured biomarkers in a large, real-world cohort of post-ACS patients., Methods and Results: BIOMArCS is a prospective, multi-centre, observational study in 844 post-ACS patients in whom 12 218 blood samples (median 17 per patient) were obtained during 1-year follow-up. The longitudinal patterns of high-sensitivity cardiac troponin T (hs-cTnT), N-terminal-pro-B-type natriuretic peptide (NT-proBNP), high-sensitivity C-reactive protein (hs-CRP), and growth differentiation factor 15 (GDF-15) were analysed in relation to the primary endpoint (PE) of cardiovascular mortality and recurrent ACS using multivariable joint models. Median age was 63 years, 78% were men and the PE was reached by 45 patients. The average biomarker levels were systematically higher in PE compared with PE-free patients. After adjustment for 6-month post-discharge Global Registry of Acute Coronary Events score, 1 standard deviation increase in log[hs-cTnT] was associated with a 61% increased risk of the PE [hazard ratio (HR) 1.61, 95% confidence interval (CI) 1.02-2.44, P = 0.045], while for log[GDF-15] this was 81% (HR 1.81, 95% CI 1.28-2.70, P = 0.001). These associations remained significant after multivariable adjustment, while NT-proBNP and hs-CRP were not. Abstract: Abstract: We performed bone scintigraphy in 6 patients with suspected cardiac amyloidosis. To evaluate feasibility of left ventricle function analysis, we additionally performed electrocardiographically gated SPECT acquisition. The cardiac-gated SPECT data confirmed adequate tracer uptake for automatic myocardial contour determination. LVEF estimations ranged between 24% and 54%. Comparison with LVEF estimations from prior echocardiography generally showed only small differences. In one patient, the LVEF measurements from both methods seemed discordant, probably reflecting actual LVEF worsening, which was confirmed at follow-up echocardiography. 42. Drivers of mortality in patients with chronic coronary disease in the low-dose colchicine 2 trial.

Abstract: Background: Low-dose colchicine significantly reduces the risk of cardiovascular events in patients with chronic coronary disease. An increase of non-cardiovascular death raised concerns about its safety. This study reports cause-specific mortality and baseline predictors of mortality in the Low-Dose Colchicine 2 (LoDoCo2) trial., Methods: Patients with chronic coronary disease were randomly allocated to colchicine 0.5 mg once daily or placebo on a background of optimal medical therapy. Cause-specific mortality data were analysed, stratified by treatment status. Multivariate analyses were performed to examine the predictors of mortality as well as cardiovascular and non-cardiovascular death., Results: After a median 28.6 months follow-up, 133 out of 5522 participants (2.4%) died. Forty-five deaths were cardiovascular (colchicine versus placebo: 20 [0.7%] versus 25 [0.9%], HR, 0.80; 95% CI, 0.44-1.44), while eighty-eight deaths were non-cardiovascular (53 [1.9%] versus 35 [1.3%]; HR, 1.51; 95% CI, 0.99-2.31). Forty-eight deaths were due to cancer (26 [0.9%] versus 22 [0.8%]), thirteen end-stage pulmonary disease (9 [0.3%] versus 4 [0.1%]), eight infection (4 [0.1%] versus 4 [0.1%]), five dementia (4 [0.1%] versus 1 [0.0%]) and five related multiple organ failure (3 [0.1%] versus 2 [0.1%]). Multivariable analysis demonstrated age > 65 years was the only independent baseline characteristic associated with non-cardiovascular death (HR, 3.65; 95% CI, 2.06-6.47)., Conclusions: During the LoDoCo2 trial, assignment to colchicine was not associated with an adverse effect on any specific causes of death. Abstract: Iron deficiency has been extensively researched and is associated with adverse outcomes in heart failure. However, to our knowledge, the temporal evolution of iron status has not been previously investigated in patients with acute coronary syndrome (ACS). Therefore, we aimed to explore the temporal pattern of repeatedly measured iron, ferritin, transferrin, and transferrin saturation (TSAT) in relation to prognosis post-ACS. BIOMArCS (BIOMarker study to identify the Acute risk of a Coronary Syndrome) is a prospective, multicenter, observational cohort study conducted in The Netherlands between 2008 and 2015. A total of 844 patients with post-ACS were enrolled and underwent high-frequency (median 17) blood sampling during 1 year follow-up. Biomarkers of iron status were measured batchwise in a central laboratory. We analyzed 3 patient subsets, including the case-cohort (n = 187). The primary endpoint (PE) was a composite of cardiovascular mortality and repeat nonfatal ACS, including unstable angina pectoris requiring revascularization. The association between iron status and the PE was analyzed using multivariable joint models. Mean age was 63 years; 78% were men, and >50% had iron deficiency at first sample in the case-cohort. After adjustment for a broad range of clinical variables, 1 SD decrease in log-iron was associated with a 2.2-fold greater risk of the PE (hazard ratio 2.19, 95% confidence interval 1.34 to 3.54, p = 0.002). Similarly, 1 SD decrease in log-TSAT was associated with a 78% increased risk of the PE (hazard ratio 1.78, 95% confidence interval 1.17 to 2.65, p = 0.006). Ferritin and transferrin were not associated with the PE. Repeated measurements of iron and TSAT predict risk of adverse outcomes in patients with post-ACS during 1 year follow-up. 44. Basket of options: Unpacking the concept.

Abstract: How to stimulate technological change to enhance agricultural productivity and reduce poverty remains an area of vigorous debate. In the face of heterogeneity among farm households and rural areas, one proposition is to offer potential users a 'basket of options' - a range of agricultural technologies from which potential users may select the ones that are best suited to their specific circumstances. While the idea of a basket of options is now generally accepted, it has attracted little critical attention. In this paper, we reflect on outstanding questions: the appropriate dimensions of a basket, its contents and how they are identified, and how a basket might be presented. We conceive a basket of options in terms of its depth (number of options related to a problem or opportunity) and breadth (the number of different problems or opportunities addressed). The dimensions of a basket should reflect the framing of the problem or opportunity at hand and the objective in offering the basket. We recognise that increasing the number of options leads to a trade-off by decreasing the fraction of those options that are relevant to an individual user. Farmers might try out, adapt or use one or more of the options in a basket, possibly leading to a process of technological change. We emphasise that the selection (or not) of specific options from the basket, and potential adaptation of the options, provide important opportunities for learning. Baskets of options can therefore be understood as important boundary concepts that invite critical engagement, comparison and discussion. Abstract: Pediatric chronic ear disease and its sequelae often necessitate surgical intervention, such as ear tube insertion, ossiculoplasty, tympanoplasty, and cholesteatoma removal. Although these procedures have traditionally been performed with the microscope, use of rigid endoscopes provides an alternative method for visualization. The endoscope offers improved visualization of the middle ear space and adjacent structures and can either be used alone to perform surgery through the ear canal or together with the microscope if mastoidectomy is required. Endoscopic ear surgery can reduce the need for a postauricular incision or mastoidectomy while resulting in equivalent hearing outcomes compared with those performed with the microscope. Abstract: Objective: Evaluate the impact of cochlear anomalies on hearing outcomes for pediatric patients with cochlear implants., Study Design: Retrospective chart review., Setting: Tertiary care center., Subjects and Methods: Charts were retrospectively reviewed for cases where pediatric cochlear implant surgery was performed between 2002 and 2018 at a single, tertiary care institution. Patients were divided into groups based on the presence or absence of radiological cochlear abnormalities, which were further classified as low or high risk anomalies. Hearing outcomes were evaluated by measuring pure tone averages and word recognition scores preoperatively, 3 and 12 months postoperatively, in addition to the most recent test results., Results: There were 154 ears implanted in our cohort of 100 patients. 107 ears had normal cochlear anatomy, 31 had low risk, and 16 had high risk abnormalities. The most common modality of preoperative imaging was CT scan. Postoperative mean pure tone average (PTA) was significantly higher in patients with inner ear anomalies compared to those with normal anatomy. No significant difference in PTA was noted between low versus high risk patients. <50% of patients had word recognition scores available within the first year following surgery., Conclusion: Abnormalities of the inner ear significantly influenced hearing outcomes over time following cochlear implant surgery when compared to pediatric patients with normal anatomy. 48. Tracing legume seed diffusion beyond demonstration trials: An exploration of sharing mechanisms.

Abstract: Many interventions are assuming that introduced seeds diffuse. However, the details of this diffusion among farmers are poorly understood. This article presents data from eight sites in four on the diffusion of seed and associated information given to farmers involved in N2Africa's demonstration trials. The study showed that 2-3 years after the trials had been organised, more than 90% of the farmers who had participating in the trial activities and were given a seed-input package with 1-5 kg of legume seed had shared this seed, on average with four other farmers. The farmers who received this seed from these directly involved farmers shared their seed less frequently. Eighty per cent of all the seed sharings were of 1-2 kg of seed given as a gift. Only 5% of the sharings involved a cash transaction. More than half of the seed sharings were with family members and around a third were between friends. Men shared at least as often as women and both men and women shared most with persons of their own sex. Information about rhizobium as an associated input for soya was shared by more than one-third of farmers, almost exclusively by farmers who had participated in the demonstration trials themselves. Extrapolation of data suggest that in addition to the 250,000 farmers who participated directly in the N2Africa demonstration trials, another 1,400,000 farmers may have received seed of a new legume crop or variety. The results show that knowing about the character of the seed sharing mechanisms may offer opportunities to influence the diffusion of seeds. Abstract: Background: Impaired renal function predicts mortality in acute coronary syndrome (ACS), but its evolution immediately following index ACS and preceding next ACS has not been described in detail. We aimed to describe this evolution using serial measurements of creatinine, glomerular filtration rate [eGFR Cr ] and cystatin C [CysC]., Methods: From 844 ACS patients included in the BIOMArCS study, we analysed patient-specific longitudinal marker trajectories from the case-cohort of 187 patients to determine the risk of the endpoint (cardiovascular death or hospitalization for recurrent non-fatal ACS) during 1-year follow-up. Study included only patients with eGFR Cr  ≥ 30 ml/min/1.73 m 2 . Survival analyses were adjusted for GRACE risk score and based on data >30 days after the index ACS (mean of 8 sample per patient)., Results: Mean age was 63 years, 79% were men, 43% had STEMI, and 67% were in eGFR stages 2-3. During hospitalization for index ACS (median [IQR] duration: 5 (3-7) days), CysC levels indicated deterioration of renal function earlier than creatinine did (CysC peaked on day 3, versus day 6 for creatinine), and both stabilized after two weeks. Higher CysC levels, but not creatinine, predicted the endpoint independently of the GRACE score within the first year after index ACS (adjusted HR [95% CI] per 1SD increase: 1.68 [1.03-2.74])., Conclusion: Immediately following index ACS, plasma CysC levels deteriorate earlier than creatinine-based indices do, but neither marker stabilizes during hospitalization but on average two weeks after ACS.
