1. Essential Points in the Emergency Management in Transport Accidents which Can Lead to a BLEVE-Fireball

Abstract

Huge amounts of hazardous materials are transported by rail and road, being from time to time involved in traffic accidents. In these cases, flammable materials such as, for example, LPG, can originate a severe accident if a loss of containment takes place: a BLEVE explosion usually followed by a fireball. This type of accident, which often follows the domino effect sequence of fire?explosion, has caused the death of many people, including firefighters and spectators, the mechanical and thermal effects reaching significant distances. A historical survey has been performed on 167 accidents obtained from diverse databases. The results thus obtained have been used, together with the adequate mathematical models, to analyze the time to failure that can be expected and to estimate the lethality reach of the diverse effects –overpressure, ejected fragments, thermal radiation. Finally, a set of considerations concerning the safety and emergency measures that should be adopted in these accidents are commented.

Abstract

Even though BLEVEs have been studied by a number of authors, there are still significant gaps in their knowledge and in the prediction of their physical effects. Diverse methodologies have been proposed to calculate the peak overpressure of the explosion, but their results show an important scattering. Most of them assume a reversible, isentropic phenomenon, not at all logical for an explosion. Instead, some recent papers have assumed an irreversible process, much closer to the real phenomenon. This communication analyses these aspects, comparing the results obtained by applying the diverse methodologies to data obtained from a real case. The uncertainties found (vessel failure mode, directional effects) are also commented. Finally, some conclusions are derived on the best way to estimate the overpressure.

Abstract

Achieving high-performance conditions and maximizing the fusion yield of plasma discharges have been one of the main goals of recent Joint European Torus (JET) campaigns in preparation for its second deuterium–tritium (D–T) campaign. The simulations shown in this work delve into the role of external heating using neutral beam injection (NBI) and radio-frequency waves in the ion cyclotron range of frequencies (ICRF) in order to optimise high fusion performance in the JET tokamak. A baseline discharge with record neutron rate is used as a reference in order to perform a D–T prediction, which considers the NBI+RF synergy. In this work, our focus is on JET’s two main minority schemes, H and 3He. This study tackles the heating mechanisms by which these schemes reach high-performance conditions. The H scheme typically boosts the ICRF fusion enhancement through the second D harmonic heating, whereas 3He minority is characterised by its strong bulk ion heating. Both features are beneficial for increasing the fusion yield. Nevertheless, the minority concentration is a relevant parameter, which needs to be assessed to understand in which concentration ranges the benefits of each particular minority scheme are met. Therefore, the main objective of this work is to assess in which concentration range the ICRF fusion enhancement and bulk ion heating are optimal, for H and 3He, respectively. Under these conditions, our prediction suggests 3He concentration should stay above 1.2% and H should remain below 2.2%.

Abstract

Dexketoprofen (S(+)-2-(3-benzoylphenyl)propionic acid) is the active enantiomer of ketoprofen, a racemic nonsteroidal anti-inflammatory drug with proven analgesic activity in the treatment of pain, including acute pain after dental surgery. The aim of this randomised, double-blind study was to compare the analgesic efficacy of single oral doses (5, 10 and 20mg) of the tromethamine salt of dexketoprofen (DKR.TRIS) [equivalent to 3.5, 7 and 14mg of dexketoprofen, respectively] with that of placebo in the dental pain model. Ibuprofen (400mg) was used to validate the model. A total of 206 patients experiencing moderate to severe pain after surgical removal of the impacted third molar tooth under local anaesthesia were entered in this trial at 2 centres. Patients rated their pain intensity and pain relief on verbal rating and visual analogue scales at regular intervals during the 6-hour period after drug administration. Patients were also requested to provide a global evaluation of treatment efficacy at the end of the observation period. DKR.TRIS (10 and 20mg) and ibuprofen were superior to placebo as determined by significantly higher values for the sum of the pain intensity difference scores and total pain relief scores from baseline. Furthermore, the time to reduce baseline pain by at least 50% was significantly more rapid in patients treated with DKR.TRIS (10 and 20mg) and ibuprofen than in placebo recipients. The mean time to remedication with rescue analgesia was significantly longer in all active treatment groups compared with placebo. DKR.TRIS was well tolerated, with a reported incidence of adverse events similar to that of placebo. No serious adverse events were reported in any treatment group. The results of this study suggest that DKR.TRIS is an effective analgesic with a favourable tolerability profile for the treatment of acute pain states.

The evolution of the experiment computing models resulting from the higher amounts of data expected after the restart of the LHC are also described.

Material y métodos: 130 entrevistas a niños entre 5 y 10 años recogiendo: sexo, edad, entorno familiar, horas de pantallas, horas de deporte, somatometría, puntuación del grado de responsabilidad y sociabilidad y test de Goodenough (como estimación del cociente intelectual). Resultados: los modelos de regresión lineal múltiple mostraron una asociación estadísticamente significativa entre el índice de masa corporal (IMC) y las horas de televisión y vídeos (B = 0,06; p = 0,05) y entre el IMC y las horas de ordenador y videojuegos (B = 0,34; p < 0,01), así como entre la estimación del cociente intelectual y las horas de ordenador y video-juegos (B = -1,67; p = 0,02). Conclusiones: deben desarrollarse estrategias para concienciar a las familias sobre los efectos perjudiciales del abuso de pantallas, tanto de los asociados al sedentarismo y a los contenidos negativos, como del daño sobre el desarrollo intelectual que produce el abuso de los juegos electrónicos.

Abstract

espanolSe realizo una intervencion en el ambito escolar para administrar consejo breve antitabaco a escolares de la Educacion Secundaria Obligatoria (ESO) en Zaragoza durante los cursos 2006-2007 y 2007-2008. Los objetivos del estudio fueron medir la efectividad del consejo breve apoyado por material escrito y estudiar la progresion del tabaquismo. El estudio incluyo una encuesta autoadministrada y una coximetria, asi como una reevaluacion un ano despues. Se estudiaron 15 centros escolares, 8 del grupo intervencion y 7 de control. Se administro consejo apoyado con un folleto frente al control en el que no se utilizaba el folleto. La asignacion a los grupos no fue aleatoria. Se evaluaron 1.720 estudiantes entre los dos cursos 2006-07 y 2007-08 de los cuales 51,95% fueron hombres y 48,5% mujeres. La edad media fue 14,6 anos (desviacion estandar [DE] = 1,36) y fueron fumadores el 19,7%. The study was implemented in the 2006-2007 and 2007- 2008 courses respectively. The study�s objectives were to asses the effectiveness of smoking counselling with the aid of a specifically designed leaflet. The study also included an evaluation of expired CO. The authors surveyed on smoking behaviour as well as on personal details of the students. The study includes 8 intervention schools and 7 control schools. Allocation wasn�t randomized due to logistic reasons. The interventional group received advice against smoking by means of spoken counsel from a professional supported by a leaflet. The control group only received the spoken counsel. The expired CO was assessed in each group. The counsel was repeated in each group one year later to the same students. Abstract

The Large Hadron Collider (LHC) experiments will collect unprecedented data volumes in the next Physics run, with high pile-up collisions resulting in events that require a complex processing. Hence, the collaborations have been required to update their Computing Models to optimize the use of the available resources and control the growth of resources, in the midst of widespread funding restrictions, without penalizing any of the Physics objectives. The changes in computing for Run2 represent significant efforts for the collaborations, as well as significant repercussions on how the WLCG sites are built and operated.This paper focuses on these changes, and how they have been implemented and integrated in the Spanish WLCG Tier-1 centre at Port d'Informació Cientifica (PIC), which serves the ATLAS, CMS and LHCb experiments. The approach to adapt a multi-VO site to the new requirements, while maintaining top reliability levels for all the experiments, is as well presented. Additionally, a description of work done to reduce the operational and maintenance costs of the Spanish Tier-1 centre, in agreement with the expectations from WLCG, is provided.

Abstract

To assess the efficacy of metamizol 1 g and 2 g in the relief of pain after surgical extraction of the lower third molar, and to compare the therapeutic effect with that of ibuprofen 600 mg or placebo.A total of 253 patients aged between 18 years and 60 years who had undergone extraction of the lower third molar (types II-IV) under local anaesthesia, up to a maximum of 108 mg of mepivacaine, were randomly assigned to a single oral dose of a new galenic form (drinkable vials) of metamizol 1 g (n = 75), metamizol 2 g (n = 72), ibuprofen 600 mg (n = 74) or placebo (n = 32). Pain intensity was evaluated by a 100-mm visual analogue scale. To enter the study, a pain level of 50 mm or more was required. The duration of the trial was 1 h. Assessments were carried out at 15, 30 and 60 min after treatment.The analgesic efficacy of metamizol 2 g was significantly better than ibuprofen and placebo with regard to all evaluated parameters. The values of the pain intensity difference at 15 min, the percentage of patients with a decrease of 50% or more on the visual analogue scale at 60 min and the sum of pain intensity differences at 60 min showed metamizol 2 g to be significantly more effective than metamizol 1 g. In general, metamizol 1 g was as effective as ibuprofen 600 mg. The analgesic efficacy of placebo was significantly lower than that of all active treatments. A lower number of patients treated with metamizol 1 g (n = 1) or metamizol 2 g (n = 1) needed rescue medication than those given ibuprofen (n = 7) or placebo (n = 5). No serious adverse effects developed and none of the patients had to leave the study for this reason.The model of the lower third molar, for which the analgesic outcome referred to the first hour after drug administration, demonstrated that the analgesic efficacy of oral metamizol 2 g was significantly higher than that of ibuprofen 600 mg or placebo. Metamizol 1 g and ibuprofen 600 mg showed a similar therapeutic effect. All regimens were as well tolerated as placebo.

Abstract

Background In a previous study of 54 patients with biopsy-proven giant-cell arteritis (GCA) we found that 22.2% (12 patients) had significant aortic structural damage (ASD) (aneurysm/dilatation) after a median follow-up of 5.4 years. Development of ASD was not related to persistence of clinically apparent disease or elevation of acute-phase reactants or pro-inflammatory cytokines during follow-up (1) The outcome of ASD after longer follow-up has never been prospectively evaluated. Objectives To assess the development of new ASD and the outcome of previously detected abnormalities after longer follow-up. Methods Patients from the initial cohort who continued their follow-up visits in our department were subjected to the same screening protocol every four years approximately. The screening protocol consisted of a chest X-ray and a CT scan if aortic aneurysm was suspected or changes respect to the baseline X-ray were observed. The abdominal aorta was evaluated by ultrasonography (US). ASD was defined when the ascending aorta measured >4 cm in diameter, the aortic arch or the descending aorta ≥4cm, the abdominal aorta >3 cm or, when the aorta exhibited a fusiform dilatation. Results were compared to those obtained from the previous screening. Results Eighteen of the 54 patients from the original cohort had died or were lost tofollow-up before they could be re-evaluated. The remaining 36 patients were subjected to a new screening (median follow-up at the time of second screening 8.5 years, range 8-12). In 7 of these patients ASD had been detected in the previous evalutation (1 involving the abdominal aorta and 6 the thoracic aorta). In the second screening the abdominal aneurysm remained stable whereas thoracic ASD increased in diameter. Three patients had newly discovered thoracic ASD not detected in the first screening. One of them died of an aortic dissection 9 years after the diagnosis of GCA. The remaining 26 patients did not show changes in the chest X-ray when compared to the baseline X-ray, or, if changes were suspected, the CT scan did not evidence significant aortic abnormalities. No new abdominal aneurysms were observed by US. To date, 7patients have been subjected to a third screening after a follow-up of 13 years (range 11.6-15), with no evidence of new ASD in any of them. Overall, 27.7% of patients from the original cohort developed ASD after a median follow-up of 10 years (range 4.3-15). Conclusions Our study suggests that GCA-patients develop ASD mainly but not exclusively, within the first 5 years of follow-up. Once ASD occurs, dilatation increases over time, particularly in the thoracic aorta underlining the need for periodic evaluation. These results should be confirmed in a larger series of patients. Abstract

This paper describes a study of the risk factors associated with disease outbreaks in fish species of fish farms and rivers of north-east Spain. We focused our work on the isolation of fish pathogens (bacteria, virus), the water quality (physicochemical and microbiological quality) and management characteristics. We have observed 2 important viral diseases, infectious pancreatic necrosis and spring viraemia of carp, and 2 important bacterial ones, furunculosis (Aeromonas salmonicida) and bacterial kidney disease (BKD) (Renibacterium salmoninarum). Our preliminary results show that there are some potential risk factors associated with the main diseases of fish, such as fish age, fish species, production system, season and water temperature, but their role depends on the disease.

Abstract

Scientific experiments are producing huge amounts of data, and the size of their datasets and total volume of data continues increasing. These data are then processed by researchers belonging to large scientific collaborations, with the Large Hadron Collider being a good example. The focal point of scientific data centers has shifted from efficiently coping with PetaByte scale storage to deliver quality data processing throughput. The dimensioning of the internal components in High Throughput Computing (HTC) data centers is of crucial importance to cope with all the activities demanded by the experiments, both the online (data acceptance) and the offline (data processing, simulation and user analysis). This requires a precise setup involving disk and tape storage services, a computing cluster and the internal networking to prevent bottlenecks, overloads and undesired slowness that lead to losses cpu cycles and batch jobs failures. In this paper we point out relevant features for running a successful data storage and processing service in an intensive HTC environment.

Abstract

Recent studies have demonstrated a negative correlation between the cholesterol content of the HDL fraction of serum lipoproteins and the incidence of arteriosclerosis. In this study the limits of normality of HDL-cholesterol have been determined in a male and female population, followed by an analysis of the HDL-cholesterol in patients with myocardial infarction. Normal values for total cholesterol in males and females were, respectively, 194.7 +/- 61.8 mg/dl and 188 +/- 62.8 mg/dl, and those of HDL-cholesterol, 63.1 +/- 14.6 mg/dl and 70.3 +/- 24.4 mg/dl. Total cholesterol in the patients with myocardial infarction was not significantly different from that of the control group, but HDL-cholesterol was significantly lower (p0.05).

Abstract

Although iron-mediated mechanisms are important in experimental brain injury after carotid occlusion, their clinical role in acute ischemic stroke has not been determined. We evaluated the influence of iron stores, measured as serum ferritin, on the outcome of acute cerebral infarct. Admission and fasting glycemia, glycosylated hemoglobin, serum cortisol, serum ferritin, and 24-hour urinary free cortisol levels were measured on the first day of hospitalization in 67 patients admitted with an acute ischemic stroke of less than 24 hours' duration. Patients were classified into two groups according to their Canadian Stroke Scale (CSS) score on day 30: good outcome group (alive and CSS score > 7 points) and poor outcome group (dead or CSS score < or = 7 points). Thirty-three patients (49%) had good outcome and 34 (51%) poor outcome. Fasting glycemia (P = .001), serum cortisol (P < .001), and urinary free cortisol (P = .001) but not admission glycemia and glycosylated hemoglobin had higher levels in patients with poor outcome. Serum ferritin values were greater in the poor outcome group (218 +/- 156 micrograms/L versus 133 +/- 125 micrograms/L; P = .004), and a correlation between ferritin values and degree of worsening or improvement of the CSS score on day 30 was found (P = .002). Serum cortisol (odds ratio [OR], 6.7; 95% confidence interval [CI], 1.7 to 26), fasting glycemia (OR, 5.4; 95% CI, 1.2 to 24), and serum ferritin (OR, 4.6; 95% CI, 1.1 to 19) were independently related to poor outcome in a logistic regression analysis. High serum ferritin levels within the first 24 hours of hospitalization for an acute ischemic stroke are related to a poor prognosis, independent of the stress response. More research is needed to determine the origin of increased serum ferritin levels and the therapeutic implications.

Abstract

The atmospheric dispersion of virus was simulated using a computer model which had been developed for predicting the dispersion of toxic gases from chemical engineering plants. The results were compared with data from four outbreaks in which virus was believed to have been transported by air: two outbreaks of foot-and-mouth disease in the United Kingdom in 1967 and outbreaks of Aujeszky's disease in Yorkshire in 1981 to 1982 and Indiana in 1988. There was relatively good agreement with most of these data. The paper shows that the model could be useful in an emergency because the risk of virus spread could be predicted in real time. Abstract

Phosphoinositides are lipids that play a critical role in processes such as cellular signalling, ion channel activity and membrane trafficking. When mutated, several genes that encode proteins that participate in the metabolism of these lipids give rise to neurological or developmental phenotypes. PI4KA is a phosphoinositide kinase that is highly expressed in the brain and is essential for life. Here we used whole exome or genome sequencing to identify 10 unrelated patients harbouring biallelic variants in PI4KA that caused a spectrum of conditions ranging from severe global neurodevelopmental delay with hypomyelination and developmental brain abnormalities to pure spastic paraplegia. Some patients presented immunological deficits or genito-urinary abnormalities. Functional analyses by western blotting and immunofluorescence showed decreased PI4KA levels in the patients’ fibroblasts. Immunofluorescence and targeted lipidomics indicated that PI4KA activity was diminished in fibroblasts and peripheral blood mononuclear cells. In conclusion, we report a novel severe metabolic disorder caused by PI4KA malfunction, highlighting the importance of phosphoinositide signalling in human brain development and the myelin sheath., Verdura et al. report a novel rare brain metabolic disorder caused by recessive mutations in PI4KA, which encodes an enzyme with a pivotal role in phosphoinositide metabolism at the cell membrane. The description of this syndrome will simplify the identification of undiagnosed cases with similar clinical features.

Abstract

Purpose: Cervical cancer is a viral-associated tumor caused by the infection with the human papilloma virus. Cervical cancer is an immunogenic cancer that expresses viral antigens. Despite being immunogenic, cervical cancer does not fully respond to immune checkpoint inhibitors (ICI). LIF is a crucial cytokine in embryo implantation, involved in maternal tolerance that acts as an immunomodulatory factor in cancer. LIF is expressed in cervical cancer and high levels of LIF is associated with poor prognosis in cervical cancer., Experimental Design: We evaluated the impact of LIF on the immune response to ICI using primary plasmocytoid dendritic cells (pDC) and macrophage cultures, syngeneic animals and patient-derived models that recapitulate the human tumor microenvironment., Results: We found that the viral proteins E6 and E7 induce the expression of LIF via the NFκB pathway. The secreted LIF can then repress type I interferon expressed in pDCs and CXCL9 expressed in tumor-associated macrophages. Blockade of LIF promotes the induction of type I interferon and CXCL9 inducing the tumor infiltration of CD8 T cells. This results in the sensitization of the tumor to ICI. Importantly, we observed that patients with cervical cancer expressing high levels of LIF tend to be resistant to ICI., Conclusions: Our data show that the HPV virus induces the expression of LIF to provide a selective advantage to the tumor cell by generating local immunosuppression via the repression of type I interferon and CXCL9. Abstract

T-cell bispecific antibodies (TCB) are engineered molecules that bind both the T-cell receptor and tumor-specific antigens. Epidermal growth factor receptor variant III (EGFRvIII) mutation is a common event in glioblastoma (GBM) and is characterized by the deletion of exons 2-7, resulting in a constitutively active receptor that promotes cell proliferation, angiogenesis, and invasion. EGFRvIII is expressed on the surface of tumor cells and is not expressed in normal tissues, making EGFRvIII an ideal neoantigen target for TCBs. We designed and developed a novel 2+1 EGFRvIII-TCB with optimal pharmacologic characteristics and potent antitumor activity. EGFRvIII-TCB showed specificity for EGFRvIII and promoted tumor cell killing as well as T-cell activation and cytokine secretion only in patient-derived models expressing EGFRvIII. Moreover, EGFRvIII-TCB promoted T-cell recruitment into intracranial tumors. EGFRvIII-TCB induced tumor regression in GBM animal models, including humanized orthotopic GBM patient-derived xenograft models. Abstract

The tumor immune microenvironment is a main contributor to cancer progression and a promising therapeutic target for oncology. However, immune microenvironments vary profoundly between patients, and biomarkers for prognosis and treatment response lack precision. A comprehensive compendium of tumor immune cells is required to pinpoint predictive cellular states and their spatial localization. We generated a single-cell tumor immune atlas, jointly analyzing published data sets of >500,000 cells from 217 patients and 13 cancer types, providing the basis for a patient stratification based on immune cell compositions. Projecting immune cells from external tumors onto the atlas facilitated an automated cell annotation system. To enable in situ mapping of immune populations for digital pathology, we applied SPOTlight, combining single-cell and spatial transcriptomics data and identifying colocalization patterns of immune, stromal, and cancer cells in tumor sections. Abstract

Brain metastases are the most common tumor of the brain with a dismal prognosis. A fraction of patients with brain metastasis benefit from treatment with immune checkpoint inhibitors (ICI) and the degree and phenotype of the immune cell infiltration has been used to predict response to ICI. However, the anatomical location of brain lesions limits access to tumor material to characterize the immune phenotype. Here, we characterize immune cells present in brain lesions and matched cerebrospinal fluid (CSF) using single-cell RNA sequencing combined with T cell receptor genotyping. Tumor immune infiltration and specifically CD8 + T cell infiltration can be discerned through the analysis of the CSF. Consistently, identical T cell receptor clonotypes are detected in brain lesions and CSF, confirming cell exchange between these compartments. The analysis of immune cells of the CSF can provide a non-invasive alternative to predict the response to ICI, as well as identify the T cell receptor clonotypes present in brain metastasis.
