2. Simple Schizophrenia or Neurotic Disorder? Case report

Introduction The diagnosis of simple schizophrenia remains an unusual and controversial diagnosis today. The presentation of nonspecific symptoms shared by other nosological entities make differential diagnosis a challenge. Objectives The main objective of this case report is to review the diagnosis of simple schizophrenia and its differential diagnosis. Methods Case report and literature review. We present the case of a 52-year-old man who was admitted to a medium stay unit for psychosocial rehabilitation with the diagnosis of simple schizophrenia after his debut at 49 years of age with clinical manifestations of progressive self-care abandonment and personality change. Results Given the psychosocial deterioration observed and lack of response to pharmacological and psychotherapeutic treatments, the possible diagnoses of dementia praecox and simple schizophrenia were considered. Several individual and family interviews, neuropsychological and projective tests (HTP test, figure 1-3) were performed in order to define the diagnosis. The results revealed age-appropriate cognitive functioning and the absence of data suggestive of an underlying psychotic disorder. On the other hand, it was observed that the patient was able to establish some social relationships and participate in group activities in the medium stay unit. These findings suggest the influence of factors related to the socio-familial environment and cast doubt on the initial diagnostic hypothesis. Conclusions The diagnosis of simple schizophrenia continues to present itself as a complex diagnosis that requires a careful review of the differential diagnosis. 3. A case report of inhibition and severe desnutrition: negative symptoms in resistant schizophrenia

Introduction The appearance of inhibitory symptoms encompassed in what are known as negative symptoms is part of the usual symptoms of schizophrenia. Sometimes this inhibition reaches a significant severity, so it is essential to know its approach. Objectives Case report and literature review regarding the treatment of resistant schizophrenia with a predominance of negative symptoms Methods We present the clinical case of a 28-year-old man diagnosed with schizophrenia at 23 years old, whose onset was characterized by delusional ideas of harm (poisoning) and delusions with a mystic-religious theme that lead him to reduce his intake until requiring a first admission for severe desnutrition. Subsequently, after two more admissions, the patient presents selective reduction in food intake, decrease in daily activity and apathy without positive symptoms. Results Throughout the treatment, several lines of antipsychotic treatments have been tried at the maximum tolerated dose (haloperidol, oral paliperidone and depot, aripiprazole and clozapine up to a dose of 600 mg). Clozapine resistance required testing various augmentation strategies (Venlafaxine, Lamotrigine and Electroconvulsive therapy) with low results. Finally, to complement the treatment, the patient was transferred to a mid-stay unit where psychosocial treatment with a multidisciplinary approach was started. This has allowed more continuous follow-up and thus a partial improvement of the clinic. Conclusions Numerous studies describe numerous augmentation strategies for clozapine-resistant schizophrenia with negative symptoms. However, the results are still inconclusive, needing more research. Meanwhile, we want to highlight the importance of complementing the treatment with psychosocial approaches. El Campo Casabe está formado por una estructura anticlinal asimétrica con buzamiento moderado hacia el este, afectado por fallas transpresionales que involucran secuencias sedimentarias del Cretáceo y Paleógeno. Como consecuencia, se produjeron fallas normales e inversas que definen 8 bloques operativos en el campo. Se resaltan como características relevantes la alta heterogeneidad de yacimientos no consolidados producidos en conjunto, la limitada continuidad areal de los yacimientos y las desfavorables relaciones de movilidad. Estas condiciones motivaron al equipo técnico del campo a realizar análisis integrados y multidisciplinarios que comprenden estudios de geología, yacimientos e ingeniería de subsuelo, validados con la experiencia operativa obtenida en el campo, con el fin de explotar más eficientemente los hidrocarburos de cada una de las 23 capas productoras.Este análisis integrado incorpora cambios en los volúmenes de inyección de agua por arena en función de los volúmenes remanentes de petróleo, calculados a partir de las eficiencias volumétrica y de desplazamiento y de los estudios de conectividad hidráulica. A partir de estos volúmenes, se definieron pronósticos de producción incremental usando curvas de flujo fraccional. También se determinaron perfiles de inyección por pozo que generen una condición de balance en el sistema. Todo esto, con el fin de incrementar el factor de recobro en el campo, mejorar la utilización del agua de inyección y disminuir el índice de falla relacionado a desbalances en el sistema producción–inyección. Los resultados a la fecha indican que en las zonas de estudio se ha obtenido una reducción del 25% en el índice de falla, un incremento de la producción del 21%, una reversión de la tendencia de la declinación del campo del 15% al 7% y una reducción del 30% de los costos asociados a consumo de energía y química para tratamiento de agua.

Las infecciones en adultos, fuera del embarazo, son poco frecuentes. No se conoce la frecuencia de complicaciones neurológicas en adultos con infección invasiva por este microorganismo. Hemos estudiado la frecuencia y las características de la afectación del sistema nervioso central (SNC) en pacientes adultos con infección invasiva por S. agalactiae. Pacientes y métodos: Se revisó a todos los pacientes adultos con infección invasiva por S. agalactiae en un hospital terciario entre 2003 y 2011. Resultados: En 75 pacientes se aisló S. agalactiae en sangre, líquido cefalorraquídeo o líquido articular. De ellos, 7 (9,3%) tuvieron afectación neurológica: 5 hombres y 2 mujeres no embarazadas, con edades entre 20 y 62 años. Los diagnósticos fueron: absceso epidural secundario a espondilodiscitis con compresión medular, meningitis bacteriana aguda, ictus isquémico como presentación de endocarditis bacteriana (2 pacientes cada uno) y meningoventriculitis tras neurocirugía y derivación ventricular. Un paciente con endocarditis por S. agalactiae y S. aureus falleció en la fase aguda y otra a los 3 meses por neoplasia metastásica. El resto se recuperó sin secuelas. En todos los casos, hubo factores predisponentes sistémicos para la infección y 5 (71,4%) tenían rotura de barrera mucocutánea como posible origen de la infección. Conclusiones: La afectación del SNC es relativamente frecuente en pacientes adultos con infección invasiva por S. agalactiae. El aislamiento de S. agalactiae debe hacer investigar causas predisponentes sistémicas y causas de rotura de barrera mucocutánea, sobre todo en meningitis. 4 Pág., Cetacean morbillivirus (CeMV) causes illness and death in cetaceans worldwide; the CeMV strains circulating in the Southern Hemisphere are poorly known. We detected a pilot whale CeMV strain in 3 short-finned pilot whales (Globicephala macrorhynchus) stranded in Brazil during July-October 2020. Motor vehicle collisions (MVCs) are a severe threat to wildlife biodiversity worldwide and most vertebrate species are at risk. However, there is a considerable knowledge gap on the traumatic features and potential patterns of MVCs in wildlife. We investigated traumatic injuries (TIs) caused by MVCs (MVCs-TIs) in 430 neotropical wild mammals representing 44 species from Brazil. Injuries were classified topographically into four categories: abdomen/pelvis (AP), chest (TX), head/neck (HN) and extremities (EX). We also determined the prevalence of pathological changes in MVC fatalities. AP (n = 381; 89%) was the most affected body segment, followed by TX (n = 372; 87%), HN (n = 363; 84%) and EX (n = 288; 67%). The most prevalent gross pathological findings were single or multiple bone fractures (n = 397; 92%), visceral organ rupture (n = 371; 86%), haemothorax (n = 220; 51%) and pulmonary haemorrhage (n = 212; 49%). Microscopically, pulmonary oedema (n = 324; 82%) and haemorrhage (n = 272; 69%) were the most prevalent lesions. No distinct TI patterns were evident across the various taxonomic groups, although trends were found in some taxa, such as armadillos. These results may help clinicians performing emergency care on MVC wildlife patients and may be of value in pathological and forensic investigations where a MVC has been deemed a likely contributory factor to death.

Characterizing microorganisms and enzymes involved in lignin biodegradation in thermal ecosystems can identify thermostable biocatalysts. We integrated stable isotope probing (SIP), genome-resolved metagenomics, and enzyme characterization to investigate the degradation of high-molecular weight, 13C-ring-labeled synthetic lignin by microbial communities from moderately thermophilic hot spring sediment (52 °C) and a woody “hog fuel” pile (53 and 62 °C zones). 13C-Lignin degradation was monitored using IR-GCMS of 13CO2, and isotopic enrichment of DNA was measured with UHLPC-MS/MS. Assembly of 42 metagenomic libraries (72 Gb) yielded 344 contig bins, from which 125 draft genomes were produced. Fourteen genomes were significantly enriched with 13C from lignin, including genomes of Actinomycetes (Thermoleophilaceae, Solirubrobacteraceae, Rubrobacter sp.), Firmicutes (Kyrpidia sp., Alicyclobacillus sp.) and Gammaproteobacteria (Steroidobacteraceae). We employed multiple approaches to screen genomes for genes encoding putative ligninases and pathways for aromatic compound degradation. Our analysis identified several novel laccase-like multi-copper oxidase (LMCO) genes in 13C-enriched genomes. One of these LMCOs was heterologously expressed and shown to oxidize lignin model compounds and minimally transformed lignin. 26. NAD pool as an antitumor target against cancer stem cells in head and neck cancer

SummaryHead and neck squamous cell carcinoma (HNSCC) is a heterogeneous group of tumors that affect different anatomical locations. Despite this heterogeneity, HNSCC treatment depends on the anatomical location, TNM stage and resectability of the tumor. Classical chemotherapy is based on platinum-derived drugs (cisplatin, carboplatin and oxaliplatin), taxanes (docetaxel, paclitaxel) and 5-fluorouracil1. Despite advances in HNSCC treatment, the rate of tumor recurrence and patient mortality remain high. Therefore, the search for new prognostic identifiers and treatments targeting therapy-resistant tumor cells is vital. Our work demonstrates that there are different subgroups with high phenotypic plasticity within the CSC population in HNSCC. CD10, CD184, and CD166 may identify some of these CSC subpopulations with NAMPT as a common metabolic gene for the resilient cells of these subpopulations. We observed that NAMPT reduction causes a decrease in tumorigenic and stemness properties, migration capacity and CSC phenotype through NAD pool depletion. However, NAMPT-inhibited cells can acquire resistance by activating the NAPRT enzyme of the Preiss-Handler pathway. We observed that coadministration of the NAMPT inhibitor with the NAPRT inhibitor cooperated inhibiting tumor growth. The use of an NAPRT inhibitor as an adjuvant improved NAMPT inhibitor efficacy and reduced the dose and toxicity of these inhibitors. Therefore, it seems that the reduction in the NAD pool could have efficacy in tumor therapy. This was confirmed by in vitro assays supplying the cells with products of inhibited enzymes (NA, NMN or NAD) and restoring their tumorigenic and stemness properties. In conclusion, the coinhibition of NAMPT and NAPRT improved the efficacy of antitumor treatment, indicating that the reduction in the NAD pool is important to prevent tumor growth.

Bacterial catabolic pathways have considerable potential as industrial biocatalysts for the valorization of lignin, a major component of plant-derived biomass. Here, we describe a pathway responsible for the catabolism of acetovanillone, a major component of several industrial lignin streams. Rhodococcus rhodochrous GD02 was previously isolated for growth on acetovanillone. A high-quality genome sequence of GD02 was generated. Transcriptomic analyses revealed a cluster of eight genes up-regulated during growth on acetovanillone and 4-hydroxyacetophenone, as well as a two-gene cluster up-regulated during growth on acetophenone. Bioinformatic analyses predicted that the hydroxyphenylethanone (Hpe) pathway proceeds via phosphorylation and carboxylation, before β-elimination yields vanillate from acetovanillone or 4-hydroxybenzoate from 4-hydroxyacetophenone. Consistent with this prediction, the kinase, HpeHI, phosphorylated acetovanillone and 4-hydroxyacetophenone. Furthermore, HpeCBA, a biotin-dependent enzyme, catalyzed the ATP-dependent carboxylation of 4-phospho-acetovanillone but not acetovanillone. The carboxylase’s specificity for 4-phospho-acetophenone ( k cat / K M = 34 ± 2 mM −1 s −1 ) was approximately an order of magnitude higher than for 4-phospho-acetovanillone. HpeD catalyzed the efficient dephosphorylation of the carboxylated products. GD02 grew on a preparation of pine lignin produced by oxidative catalytic fractionation, depleting all of the acetovanillone, vanillin, and vanillate. Genomic and metagenomic searches indicated that the Hpe pathway occurs in a relatively small number of bacteria. This study facilitates the design of bacterial strains for biocatalytic applications by identifying a pathway for the degradation of acetovanillone.

Purpose: High mortality and a limited performance of valvular surgery are typical features of infective endocarditis (IE) in octogenarians, even though surgical treatment is a major determinant of a successful outcome in IE. Methods: Data from the prospective multicentre ESC EORP EURO-ENDO registry were used to assess the prognostic role of valvular surgery depending on age. Results: As compared to < 80yo patients, ≥ 80yo had lower rates of theoretical indication for valvular surgery (49.1% vs. 60.3%, p < 0.001), of surgery performed (37.0% vs. 75.5%, p < 0.001), and a higher in-hospital (25.9% vs. 15.8%, p < 0.001) and 1-year mortality (41.3% vs. 22.2%, p < 0.001). By multivariable analysis, age per se was not predictive of 1-year mortality, but lack of surgical procedures when indicated was strongly predictive (HR 2.98 [2.43–3.66]). By propensity analysis, 304 ≥ 80yo were matched to 608 < 80yo patients. Propensity analysis confirmed the lower rate of indication for valvular surgery (51.3% vs. 57.2%, p = 0.031) and of surgery performed (35.3% vs. 68.4%, p < 0.0001) in ≥ 80yo. Overall mortality remained higher in ≥ 80yo (in-hospital: HR 1.50[1.06–2.13], p = 0.0210; 1-yr: HR 1.58[1.21–2.05], p = 0.0006), but was not different from that of < 80yo among those who had surgery (in-hospital: 19.7% vs. 20.0%, p = 0.4236; 1-year: 27.3% vs. 25.5%, p = 0.7176). Conclusion: Although mortality rates are consistently higher in ≥ 80yo patients than in < 80yo patients in the general population, mortality of surgery in ≥ 80yo is similar to < 80yo after matching patients. These results confirm the importance of a better recognition of surgical indication and of an increased performance of surgery in ≥ 80yo patients.

La presente investigación busca conocer las necesidades, gustos y preferencias de las mujeres adultas mayores en cuanto al calzado. La información inicial fue obtenida de 12 grupos focales y 42 entrevistas. Luego se aplicó un formulario estructurado con objetivo no encubierto a 144 mujeres de 60 años y más. Se hizo un análisis uní y bivariado con aplicación de prueba F de Snedecor para establecer diferencias significativas. Igualmente, se ejecutó una red neuronal artificial de clasificación.

International audience; AIMS: To evaluate the current management and survival of patients with left-sided infective endocarditis (IE) complicated by congestive heart failure (CHF) in the ESC-EORP European Endocarditis (EURO-ENDO) registry. METHODS AND RESULTS: Among the 3116 patients enrolled in this prospective registry, 2449 (mean age: 60 years, 69% male) with left-sided (native or prosthetic) IE were included in this study. Patients with CHF (n~=~698, 28.5%) were older, with more comorbidity and more severe valvular damage (mitro-aortic involvement, vegetations >10\,mm and severe regurgitation/new prosthesis dehiscence) than those without CHF (all p\,≤q\,0.019). NAD+ is an important metabolite in cell homeostasis that acts as an essential cofactor in oxidation-reduction (redox) reactions in various energy production processes, such as the Krebs cycle, fatty acid oxidation, glycolysis and serine biosynthesis. Furthermore, high NAD+ levels are required since they also participate in many other nonredox molecular processes, such as DNA repair, posttranslational modifications, cell signalling, senescence, inflammatory responses and apoptosis. In these nonredox reactions, NAD+ is an ADP-ribose donor for enzymes such as sirtuins (SIRTs), poly-(ADP-ribose) polymerases (PARPs) and cyclic ADP-ribose (cADPRs). Therefore, to meet both redox and nonredox NAD+ demands, tumour cells must maintain high NAD+ levels, enhancing their synthesis mainly through the salvage pathway. NAMPT, the rate-limiting enzyme of this pathway, has been identified as an oncogene in some cancer types. Thus, NAMPT has been proposed as a suitable target for cancer therapy. NAMPT inhibition causes the depletion of NAD+ content in the cell, leading to the inhibition of ATP synthesis. This effect can cause a decrease in tumour cell proliferation and cell death, mainly by apoptosis. Therefore, in recent years, many specific inhibitors of NAMPT have been developed, and some of them are currently in clinical trials. espanolEn este estudio se propone el diseno de un videojuego basado en la metodologia game-based learning como medio de comunicacion para facilitar la formacion de habitos alimentarios saludables en ninos con diagnostico de Enfermedad Celiaca (EC). La necesidad surge de lo observado en la practica profesional en la consulta de nutricion, donde madres de pacientes celiacos manifiestan como preocupacion principal el hecho de que el nino, expuesto a diversas actividades sociales y academicas, en la cuales no necesariamente se encuentra con su compania, sea capaz de cumplir la dieta libre de gluten, unico tratamiento para la EC. La investigacion se fundamenta en el concepto y manifestaciones clinicas de la EC, el uso del juego como recurso para la ensenanza (game-based learning), y la vision teorica del paradigma constructivista del aprendizaje. Por sus caracteristicas, esta investigacion resulta congruente con el Proyecto Factible, en un nivel descriptivo y se apoya en la investigacion documental utilizando la tecnica de analisis y el registro documental como instrumento para la obtencion de informacion. Se desarrollo en dos fases, una fase preparatoria donde se obtuvo la informacion documental necesaria para el cumplimiento de los objetivos propuestos y una fase de diseno que consistio en el desarrollo de los componentes instruccionales del videojuego y la aproximacion a un prototipo. El videojuego disenado es entonces un juego educativo (serious game) llamado El Mundo de Celia, que se compone de un mundo en forma de tablero, compuesto por tres niveles, donde el participante debe superar nueve retos en total, y mediante la practica del juego, puede lograr una mayor comprension de la enfermedad y finalmente aplicar estos conocimientos en la vida diaria logrando la consolidacion de habitos alimentarios saludables adaptados a la condicion celiaca que permitan mejorar su estado de salud. EnglishThis study proposes the design of a video game based on the game-based learning methodology as a means of communication to facilitate the formation of healthy eating habits in children with a diagnosis of Celiac Disease (CD). The need arises from what is observed in the professional practice in the nutrition consultation, where mothers of celiac patients express as their main concern the fact that the child, exposed to various social and academic activities, in which they are not necessarily with their company, be able to comply with the gluten-free diet, the only treatment for CD. The research is based on the concept and clinical manifestations of CD, the use of games as a teaching resource (game-based learning), and the theoretical vision of the constructivist paradigm of learning. Due to its characteristics, this research is congruent with the Feasible Project, at a descriptive level and is supported by documentary research using the analysis technique and the documentary record as an instrument for obtaining information. It was developed in two phases, a preparatory phase where the necessary documentary information was obtained to fulfill the proposed objectives and a design phase that consisted of the development of the instructional components of the video game and the approach to a prototype. The video game designed is then an educational game (serious game) called El Mundo de Celia, which consists of a world in the form of a board, composed of three levels, where the participant must overcome nine challenges in total, and by practicing the game, you can achieve a greater understanding of the disease and finally apply this knowledge in daily life, achieving the consolidation of healthy eating habits adapted to the celiac condition that allow you to improve your health.

Cytochrome P450 enzymes have tremendous potential as industrial biocatalysts, including in biological lignin valorization. Here, we describe P450s that catalyze the O-demethylation of lignin-derived guaiacols with different ring substitution patterns. Bacterial strains Rhodococcus rhodochrous EP4 and Rhodococcus jostii RHA1 both utilized alkylguaiacols as sole growth substrates. Transcriptomics of EP4 grown on 4-propylguaiacol (4PG) revealed the up-regulation of agcA, encoding a CYP255A1 family P450, and the aph genes, previously shown to encode a meta-cleavage pathway responsible for 4-alkylphenol catabolism. The function of the homologous pathway in RHA1 was confirmed: Deletion mutants of agcA and aphC, encoding the meta-cleavage alkylcatechol dioxygenase, grew on guaiacol but not 4PG. By contrast, deletion mutants of gcoA and pcaL, encoding a CYP255A2 family P450 and an ortho-cleavage pathway enzyme, respectively, grew on 4-propylguaiacol but not guaiacol. CYP255A1 from EP4 catalyzed the O-demethylation of 4-alkylguaiacols to 4-alkylcatechols with the following apparent specificities (k(cat)/K(M)): propyl > ethyl > methyl > guaiacol. This order largely reflected AgcA’s binding affinities for the different guaiacols and was the inverse of GcoA(EP4)’s specificities. The biocatalytic potential of AgcA was demonstrated by the ability of EP4 to grow on lignin-derived products obtained from the reductive catalytic fractionation of corn stover, depleting alkylguaiacols and alkylphenols. By identifying related P450s with complementary specificities for lignin-relevant guaiacols, this study facilitates the design of these enzymes for biocatalytic applications. We further demonstrated that the metabolic fate of the guaiacol depends on its substitution pattern, a finding that has significant implications for engineering biocatalysts to valorize lignin.

This research reports ticks on wild raptors in Brazil. Between the years 1936 and 2019, 127 larvae, 230 nymphs and 34 adult ticks were collected on 92 raptors (among 27 different species) from 35 localities in distinct Brazilian biomes. Additionally, an extensive literature review on ticks on wild raptors has been carried out, demonstrating that from 1993 to 2016, 29 larvae, 81 nymphs, 29 adults and 186 indeterminate immature ticks (larvae or nymphs) were collected on 41 raptors (16 different species) in 17 distinct localities in the Brazilian territory. The following tick species were identified on wild raptors in the country: Amblyomma aureolatum (Pallas, 1772), Amblyomma auricularium (Conil, 1878), Amblyomma brasiliense Aragao, 1908, Amblyomma cajennense (Fabricius, 1787) sensu stricto, Amblyomma calcaratum Neumann, 1899, Amblyomma coelebs Neumann, 1899, Amblyomma dubitatum Neumann, 1899, Amblyomma longirostre (Koch, 1844), Amblyomma nodosum Neumann, 1899, Amblyomma ovale Koch, 1844, Amblyomma parkeri Fonseca & Aragao, 1952, Amblyomma sculptum Berlese, 1888, Haemaphysalis juxtakochi Cooley, 1946, Rhipicephalus microplus (Canestrini, 1888), and Rhipicephalus sanguineus (Latreille, 1806) sensu lato. This study is therefore a significant contribution to our knowledge of the ticks associated with Brazilian raptors.

Resumen El Grupo de Trabajo de Metabolismo y Nutricion de la Sociedad Espanola de Medicina Intensiva, Critica y Unidades Coronarias (SEMICYUC) ha revisado y actualizado las recomendaciones del tratamiento nutrometabolico en el paciente critico (previamente publicadas por el grupo en el ano 2011) con la finalidad de ayudar a la toma de decisiones en la practica clinica diaria. Para su elaboracion, llevada a cabo entre marzo de 2016 y febrero de 2019, se ha contado con un panel de expertos dedicados a la Medicina Intensiva con amplia experiencia en el tratamiento nutricional de los pacientes criticos. Para cada una de las recomendaciones se ha establecido un nivel de evidencia basado en la metodologia del grupo GRADE (Grading of Recommendations Assessment, Development and Evaluation Working Group) y un grado de recomendacion que ha tenido en cuenta el impacto clinico de la recomendacion, independientemente del nivel de evidencia establecido por la escala GRADE.

The Metabolism and Nutrition Working Group of the Spanish Society of Intensive and Critical Care Medicine and Coronary Units (SEMICYUC) has reviewed and updated the recommendations for specialized nutritional and metabolic support in critically ill patients published by the Group in 2011, with the primary aim of helping decision making in daily clinical practice. The recommendations have been formulated by an expert panel with broad experience in nutritional and metabolic support in critically ill patients, and were drafted between March 2016 and February 2019. A level of evidence has been provided for each of the recommendations, based on the GRADE methodology (Grading of Recommendations Assessment, Development and Evaluation Working Group). A grade of recommendation has also been produced, taking into account the clinical impact of the recommendation, regardless of the level of evidence established by the GRADE scale.

Aims Infective endocarditis (IE) is a life-threatening disease associated with high mortality and morbidity worldwide. We sought to determine how socioeconomic factors might influence its epidemiology, clinical presentation, investigation and management, and outcome, in a large international multicentre registry. Methods and results The EurObservational Programme (EORP) of the European Society of Cardiology EURO-ENDO (European Infective Endocarditis) registry comprises a prospective cohort of 3113 adult patients admitted for IE in 156 hospitals in 40 countries between January 2016 and March 2018. Patients were separated in three groups, according to World Bank economic stratification [group 1: high income (75.6%); group 2: upper-middle income (15.4%); group 3: lower-middle income (9.1%)]. Group 3 patients were younger [median age (interquartile range, IQR): group 1, 66 (53–75) years; group 2, 57 (41–68) years; group 3, 33 (26–43) years; P < 0.001] with a higher frequency of smokers, intravenous drug use, and human immunodeficiency virus infection (all P < 0.001) and presented later [median (IQR) days since symptom onset: group 1, 12 (3–35); group 2, 19 (6–54); group 3, 31 (12–62); P < 0.001] with a higher likelihood of developing congestive heart failure (13.6%, 11.1%, and 22.6%, respectively; P < 0.001) and persistent fever (9.8%, 14.2%, and 27.9%, respectively; P < 0.001). Among 2157 (69.3%) patients with theoretical indication for cardiac surgery, surgery was performed less frequently in group 3 patients (75.5%, 76.8%, and 51.3%, respectively; P < 0.001), who also demonstrated the highest mortality (15.0%, 23.0%, and 23.7%, respectively; P < 0.001). Conclusion Socioeconomic factors influence the clinical profile of patients presenting with IE across the world. Despite younger age, patients from the poorest countries presented with more frequent complications and higher mortality associated with delayed diagnosis and lower use of surgery.

Objective:To investigate a cluster of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections in employees working on 1 floor of a hospital administration building.Methods:Contact tracing was performed to identify potential exposures and all employees were tested for SARS-CoV-2. Whole-genome sequencing was performed to determine the relatedness of SARS-CoV-2 samples from infected personnel and from control cases in the healthcare system with coronavirus disease 2019 (COVID-19) during the same period. Carbon dioxide levels were measured during a workday to assess adequacy of ventilation; readings >800 parts per million (ppm) were considered an indication of suboptimal ventilation. To assess the potential for airborne transmission, DNA-barcoded aerosols were released, and real-time polymerase chain reaction was used to quantify particles recovered from air samples in multiple locations.Results:Between December 22, 2020, and January 8, 2021, 17 coworkers tested positive for SARS-CoV-2, including 13 symptomatic and 4 asymptomatic individuals. Of the 5 cluster SARS-CoV-2 samples sequenced, 3 were genetically related, but these employees denied higher-risk contacts with one another. None of the sequences from the cluster were genetically related to the 17 control sequences of SARS-CoV-2. Carbon dioxide levels increased during a workday but never exceeded 800 ppm. DNA-barcoded aerosol particles were dispersed from the sites of release to locations throughout the floor; 20% of air samples had >1 log10 particles.Conclusions:In a hospital administration building outbreak, sequencing of SARS-CoV-2 confirmed transmission among coworkers. Transmission occurred despite the absence of higher-risk exposures and in a setting with adequate ventilation based on monitoring of carbon dioxide levels.

Several recent reports have raised concern that infected coworkers may be an important source of severe acute respiratory coronavirus virus 2 (SARS-CoV-2) acquisition by healthcare personnel. In a suspected outbreak among emergency department personnel, sequencing of SARS-CoV-2 confirmed transmission among coworkers. The suspected 6-person outbreak included 2 distinct transmission clusters and 1 unrelated infection.

The valorization of lignin, a major component of plant-derived biomass, is essential to sustainable biorefining. We identified the major monoaromatic compounds present in black liquor, a lignin-rich stream generated in the kraft pulping process, and investigated their bacterial transformation. Among tested solvents, acetone extracted the greatest amount of monoaromatic compounds from softwood black liquor, with guaiacol, vanillin, and acetovanillone, in an approximately 4:3:2 ratio, constituting ~90% of the total extracted monoaromatic content. 4-Ethanol guaiacol, vanillate, and 4-propanol guaiacol were also present. Bacterial strains that grew on minimal media supplemented with the BL extracts at 1mM total aromatic compounds included Pseudomonas putida KT2442, Sphingobium sp. SYK-6, and Rhodococcus rhodochrous EP4. By contrast, the extracts inhibited the growth of Rhodococcus jostii RHA1 and Rhodococcus opacus PD630, strains extensively studied for lignin valorization. Of the strains that grew on the extracts, only R. rhodochrous GD01 and GD02, isolated for their ability to grow on acetovanillone, depleted the major extracted monoaromatics. Genomic analyses revealed that EP4, GD01, and GD02 share an average nucleotide identity (ANI) of 98% and that GD01 and GD02 harbor a predicted three-component carboxylase not present in EP4. A representative carboxylase gene was upregulated ~100-fold during growth of GD02 on a mixture of the BL monoaromatics, consistent with the involvement of the enzyme in acetovanillone catabolism. More generally, quantitative RT-PCR indicated that GD02 catabolizes the BL compounds in a convergent manner via the β-ketoadipate pathway. Overall, these studies help define the catabolic capabilities of potential biocatalytic strains, describe new isolates able to catabolize the major monoaromatic components of BL, including acetovanillone, and facilitate the design of biocatalysts to valorize under-utilized components of industrial lignin streams.

Objective To determine factors related to limitations on life support within 48 h of intensive care unit (ICU) admission. Study design Prospective multicenter study. Setting Eleven ICUs. Patients All patients who died and/or had limitations on life support after ICU admission during a four-month period. Variables Patient characteristics, hospital characteristics, characteristics of limitations on life support. Time-to-first-limitation was classified as early ( Results 3335 patients were admitted; 326 (9.8%) had limitations on life support. A total of 344 patients died; 247 (71.8%) had limitations on life support (range among centers, 58.6%–84.2%). The median (p25–p75) time from admission to initial limitation was 2 (0–7) days. CHAID analysis found that the modified Rankin score was the variable most closely related with early limitations. Among patients with Rankin >2, early limitations were implemented in 71.7% (OR = 2.5; 95% CI: 1.5–4.4) and lung disease was the variable most strongly associated with early limitations (OR = 12.29; 95% CI: 1.63–255.91). Among patients with Rankin ≤2, 48.8% had early limitations; patients admitted after emergency surgery had the highest rate of early limitations (66.7%; OR = 2.4; 95% CI: 1.1–5.5). Conclusion Limitations on life support are common, but the practice varies. Quality of life has the greatest impact on decisions to limit life support within 48 h of admission.
