3. Early And Long Term Outcomes Of Aortic Valve Surgery Combined Or Not With Cabg Surgery

Abstract

Objective: The number of patients doing aortic valve surgery with or without CABG (Coronary Artery Bypass Graft) is increasing continuouslyin our country. The goal of this study is to evaluate the early and long-term results of aortic valve surgery alone or combinedwith CABG surgery. Methods: This is a retrospective and prospective study. We included the patients underwent aortic valve surgery (replacement or anotherprocedure) with or without CABG from January 2007 to January 2013 . The population of 330 patients is divided into two groups: Group I included 81 patients combined surgery; Group II included 249 patients with isolated aortic valve surgery. In the early results wereferred hospital mortality and perioperative complications. The follow-up time is from 2 to 73 months for 303 cases that were dischargedfrom hospital. The long-term results were evaluated in terms of mean survival ,quality of life and prosthetic-related complications. Results: The hospital mortality was 3.6 % in general. The hospital mortality: group I is 4. 9% and group II is 3. 2 %. The difference isnot statistically significant. Low cardiac output, conduction disturbances, stroke, pulmonary complications, renal complications, bleeding,atrial fibrillation, wound infections, ventricular arrhythmias are respectively14. 8% vs. 9. 6%, 3. 7% vs. 6. 9%, 3. 7% vs. 0. 4%, 9. 9 % vs.2. 8%, 3. 7% vs. 1. 2%, 6. 3% vs. 2.0%, 14. 8 % vs. 19.7%, 11. 3% vs. 1. 6 %, 6. 2% vs. 5. 6 % respectively for the group I and II. The differenceswere statistically significant only for low cardiac output, stroke, pulmonary and wound complications. The long-term outcomes: overall mortality 3. 9%, bleeding 1. 9%, prosthetic endocarditis and thrombosis 1.3%. Endocarditis and thrombosis are more frequent ingroup I. During the follow-up survival was 93% and 96% respectively for group I and II, with no significant difference between groups. Conclusions: Simultaneous coronary artery by-pass with aortic valveincreases slightly the operative mortality and perioperative complications.CABG surgery does not influence long–term results of aortic valve surgery.

9. Early Results of Ascending Aorta and Aortic Arch Surgery in Albania

Abstract

Background: Treatment of aneurysms of the ascending aorta, arch aorta, or both are surgically challenging and has traditionally carrieda high hospital mortality rate. The use of refined operative techniques has resulted in reduced hospital mortality rates. Patients and Methods: We conducted a prospective analysis of consecutive patients who underwent 74 surgical procedures betweenJanuary 2011 and January 2014, for graft replacement of the ascending aorta or transverse aortic arch. There were 58 men (78.4%)and 16 women (21.6%). The mean age was 55.3±9.8 years (range 30 – 74 years). The etiology was medial degeneration in 44 patients(59.5%), bicuspid aortic pathology in 28 patients (37.8%) and aortic dissection in 2 patients (2.7%). Fifteen patients (20.3%) were operatedon an emergency basis for acute aortic dissection. Results: The ascending aorta was replaced in all 69 patients (93,2%) and plicated in five patients (6.8%). Ascending aorta and aorticarch were replaced in 9 patients (12.1%). 17 patients had only ascended aortic procedure, 35 patients had separate aortic valve andascending aorta replacement (47.3%), and 18 patients (24.3%) received a valved conduit (Modified Bentall procedure). Concomitantbypass grafting was performed in nine patients. Mean cross-clamp and bypass times were 115.38±41.19 min and 143.26±55.79 min respectively. The Early hospital mortality rate was 3.38% (2 out of 59 patients) in the elective surgery group and 26.67% in the emergency group (4 out of 15). Conclusions: Surgery of the ascending aorta and aortic arch can be performed with low morbidity and mortality rates at our clinicin Albania.

Abstract

Results Overall, 6 leads were removed from 4 patients. The commonest indication for extraction was infection (endocarditis) in three patients (75 %). The other patient was operated because of massive thrombus on the leads. Removal was complete and safe for all the patients. All the patients received epicardial lead with pacemaker pocket located in the rectus abdominis muscle. The postoperative period was free of complications in all the patients.

Abstract

Methods A 30-year-old female patient presented with a history of three months complaining of interscapular chest pain and progressive dyspnea on exertion. Last two weeks she had nocturnal dyspnea and excessive chest pain. The patient had no clinical signs of myasthenia gravis. Radiological investigations revealed a mass lesion of 82x55x90 mm located in the anterior mediastinum without any calcifications or mediastinal extension. Echocardiography showed compression of the right ventricle with dilatation of the inferior vena cava.

Abstract

Carnitine palmitoyltransferase II (CPT2) deficiency is a rare inherited disorder affecting fatty acid metabolism. This enzymatic defect presents with a broad clinical spectrum, from severe neonatal forms that can be fatal, to milder myopathic variants characterized by myalgia and recurrent myoglobinuria in adolescence and adulthood. Herein, we report the case of a male patient who developed exertional rhabdomyolysis and acute kidney injury due to CPT2 deficiency. This case underscores the importance of considering genetic disorders in the differential diagnosis of patients presenting with recurrent exercise intolerance and metabolic crises. Early recognition and diagnosis enable prompt implementation of dietary and lifestyle modifications aimed at mitigating potential complications such as renal impairment. Abstract

Introduction: Mediastinitis remains one of the most serious complications of cardiac surgery. The reported incidence is 1-4%, while the related mortality varies from 10-47%., Case Presentation: A patient with triple vessel disease (TVD) was hospitalized at our clinic for coronary artery bypass graft (CABG) surgery. The preoperative examination results were normal. We performed standard CABG under extracorporeal circulation. The patient had a favorable postoperative course. On the fifth postoperative day, the wound showed seropurulent drainage. The treatment of the patient's wound continued with open dressing, negative wound pressure device, debridement, minimal muscle plasticity, and total bilateral muscle pectoral flap plasticity. The infecting microorganism was identified as multidrug-resistant Acinetobacter baumani, and systemic antibiotic therapy was initiated. The patient had "per secundum closure" of the wound after all these efforts. The wound healed completely 2 months after discharge, and the patient was in good health., Conclusions: Mediastinitis is associated with high mortality and high financial and human costs. 14. Giant Aneurysm of Ascending Aorta and Aortic Arch: A Report of a Rare Case.

Abstract

A thoracic aortic aneurysm is considered giant when its diameter exceeds 10 cm. We report a rare case of a giant aneurysm involving the ascending aorta and aortic arch in a 40-year-old man, initially diagnosed as an acute aortic dissection. The patient underwent emergency surgery, during which the ascending aorta and aortic arch were replaced under deep hypothermia and circulatory arrest with selective antegrade cerebral perfusion. Severe functional mitral valve regurgitation should be treated in patients undergoing myocardial revascularization. When replacement is considered the best therapeutic option, preservation of the mitral subvalvular apparatus is crucial, especially in the emergency setting, because of its primary role in preserving geometry and function of left and right ventricles. Here we present a simple and quick technique, where subvalvular apparatus is preserved in toto in patients undergoing mitral valve replacement with a bioprosthesis.

16. Surgery Resection of a Massive Thymic Carcinoma during Urgent Coronary Artery Bypass Grafting.

Abstract

Background: Thymic carcinoma is a very rare tumour. It is classified as thymoma type C according to World Health Organization classification. There are not many publications of simultaneous surgical treatment of thymoma during cardiac surgery interventions., Case Report: We present a case of simultaneous surgical treatment of incidentally discovered thymic carcinoma during an urgent coronary artery by-pass operation. A 55-year-old man with diagnosis three coronary vessel diseases indicated urgent by-pass surgery. The patient underwent triple coronary bypass surgery. During the intervention, it was discovered incidentally a strong mass 15 x 12 cm located in the right pleural space. A tumour was excised totally, and biopsy referred thymoma type C or thymic carcinoma. The patient did very well early postoperatively. He was referred to oncologist clinicians for further treatment. The patient was clinically very good for at least 1.5 years after surgery., Conclusion: We think that simultaneous surgical treatment of thymoma, whenever it is encountered during cardiac surgery procedures, is the recommended solution.

17. A Very Rare Case of Cor Triatriatum with Severe Mitral Regurgitation.

Abstract

Background: Cor triatriatum sinister is rare congenital heart disease. It is mainly presented in childhood and often accompanied with other congenital anomalies. The cases with cor triatriatum treated surgically in adults and accompanied with severe mitral regurgitation are very rare., Case Report: We present a case with diagnosed cor triatriatum and severe mitral regurgitation. The diagnose was made by echocardiography. She was a female 25 years that was hospitalised with signs of heart failure NYHA II-III., Conclusion: We performed the resection of the membrane in the left atrium and repair of a mitral valve according to Alfieri. The patient did very well after the surgery.

Abstract

Introduction: The saphenous vein is commonly used as a vascular graft in peripheral artery surgery but rarely used for vascular access. The literature on straight configuration saphenous vein transposition to the popliteal artery is scarce. Here we present two cases of straight configuration saphenous vein transposition to the popliteal artery for vascular access, the surgical technique and respective follow-up., Case Report: Two young men, aged 29 and 36 years, were chosen for lower-limb vascular access for hemodialysis. The first patient was paraplegic since birth. He used his arms to move so upper extremity vascular access was avoided. The second patient presented with an infected upper extremity arteriovenous graft (AVG) and after multiple closed AVFs he had no more available arm veins. Both patients received autologous lower extremity straight configuration saphenous vein transpositions to the popliteal artery under spinal anesthesia in May and October 2012, respectively. Cannulation of the fistula was allowed after one month. There were no early complications. Slight swelling on the leg appeared in one of the patients. Both fistulas were still functional after 36 and 32 months, respectively., Conclusions: The straight configuration saphenous vein transposition to popliteal artery is simple to perform, offers a long and straight segment for cannulation and may be a suitable autologous vascular access in selected patients.

19. Aortic Annulus Enlargement: Early and Long-Terms Results.

Abstract

Aim: Patient-prosthesis mismatch (PPM) is a common occurrence in aortic valve surgery. Even the discussions about the impact of this phenomenon on the results of aortic valve surgery, the management of this problem remain one of the main topics in this kind of surgery. One of the ways of a solution is aortic annulus enlargement. The main topic of this study is to evaluate the early and longterm results of this technique in our country., Methods: During the period January 2010 -January 2015, 641 patients performed aortic valve surgery. In ten patients we performed aortic annulus enlargement according to Manouguian technique to avoid severe patient-prothesis mismatch. Operative mortality and perioperative complications (low cardiac output, pulmonary complications, etc..) were considered the indicators of the early results. Survival, clinical presentation according to NYHA, quality of life were the indicators to evaluate long-term results. Preoperative and postoperative echocardiographic data were also used to evaluate our results. We collected the data from hospital registrations and periodical clinical visit and echographic examination after hospital discharge., Results: In our group, 6 of 10 patients were diagnosed with stenotic aortic valve, two patients had aortic valve regurgitation and two mixed valve pathology. Four patients had concomitant cardiac surgery procedure, mitral or CABG. In all cases, aortic valve pathology was the primary diagnose. In the preoperative echocardiographic examination mean transvalvular gradient was 54.3 ± 6.42. We had no death during early or late postoperative period. Only one patient had pulmonary complications and long time of respiratory assistance because of his pulmonary pathology. The same patient had low cardiac output and wound infection. Early after surgery mean transprostethic gradient was 16.2 ± 3.44 and late postoperative was 15.9 ± 4.3. No patient had the severe patient-prothesis mismatch. Mean follow-up was 49 ± 20.26 months. During follow-up, we had no death, and all patients had very good quality of life., Conclusions: Aortic valve annulus enlargement can be used with very good early and late results with the final goal to increase the potential benefit of the patient from surgery of aortic valve.

Abstract

In the surgery of aortic valve replacement is always attempted, as much as possible, to implant the larger prosthesis with the mains goals to enhance the potential benefits, to minimise transvalvular gradient, decrease left ventricular size and avoid the phenomenon of patient-prosthesis mismatch. Implantation of an ideal prosthesis often it is not possible, due to a small aortic annulus. A variety of aortic annulus enlargement techniques is reported to avoid patient-prosthesis mismatch. We present the case that has submitted four three times open heart surgery. We used Manouguian technique to enlarge aortic anulus with excellent results during the fourth time of surgery.

Abstract

Introduction: The aim of this study was to evaluate ventricular geometry, its relationship with the inflammatory markers, and mortality of patients with end-stage renal disease on peritoneal and hemodialysis treatment., Materials and Methods: We enrolled adult patients on long-term dialysis (hemodialysis and peritoneal dialysis) for more than 3 months. Two-dimensional echocardiography was performed by an experienced cardiologist who was blinded to all clinical details of patients. Cardiovascular mortality was assessed during a 2-year follow-up period., Results: There were 129 participants, of whom 86 (66%) were on hemodialysis. Left ventricular hypertrophy was present in 86.7%; concentric hypertrophy was found in 64 (49.1%) and eccentric hypertrophy in 48 patients (37.2%). Patients with left ventricular hypertrophy were further divided into tertiles according to their left ventricular mass index. Logistic regression found pulse pressure as an independent risk factor associated with left ventricular mass index (odds ratio [OR], 1.04; 95% confidence interval (CI), 1.01 to 1.19; P = .047). Cardiovascular mortality rate was 15.5%. Multivariable analysis showed that C-reactive protein (OR, 1.06; 95% CI, 1.01 to 1.10; P = .01), pulse pressure (OR, 1.01; 95% CI, 1.0 to 1.26; P = .046), and left ventricular mass index (OR, 1.03; 95% CI, 1.01 to 1.21; P = .03) were independent risk factors for cardiovascular mortality., Conclusions: Concentric hypertrophy is the most frequent left ventricular geometry model in patients with chronic kidney disease. Inflammation, pulse pressure, and  left ventricular hypertrophy are interrelated and all contribute to mortality and cardiovascular death risk among dialysis patients.

22. Surgical Treatment of Anomalous Origin of Right Coronary Artery in a Patient with Mitral Stenosis.

Abstract

Background: An anomalous origin of the right coronary artery is rarely observed, with a reported incidence between 0.026% and 0.25%. This condition is often completely asymptomatic and is found incidentally during angiographic evaluation for other cardiac diseases. However some patients present with exertion angina or sudden death. Surgical treatment in patients with anomalous RCA is still controversial. Treatment can be conservative, angioplasty or surgery., Case Presentation: A 59-year-old man was admitted with severe mitral stenosis. He complained exertion and rest dyspnea, NYHA III class. He had sequels of embolic stroke, results of left atrial thrombus. Echocardiography showed calcified severe mitral stenosis with mitral orifice area of 1.1 square centimeters with PSPAP 60 mmHg and normal LV function. Routine coronary angiography before surgery showed aberrant origin of RCA from the left sinus of Valsalva with 90% stenosis at his origin. Multi-slice computed tomography proved the diagnosis of anomalous RCA arising from the left sinus of Valsalva and taking an inter-arterial course between the aorta and pulmonary artery. The patient underwent mitral valve replacement with mechanical St. Jude prosthesis No 29 and saphenous vein graft to RCA. We chose by-pass grafting techniques because after aortotomy, RCA was too close to LMCA, intramural course was too short and stenosis of RCA was outside of aortic wall. The patient's perioperative course was without complications and patient was discharged on the seventh postoperative day., Conclusion: Correction of anomalous of the origin of right coronary artery is mandatory in cases where patient has to be operated for other cardiac causes.

23. Prosthetic Subclavian-Aortic Bypass as a Safe Surgical Technique for the Coarctation of the Aorta in Adults.

Abstract

Background: Coarctation represents 5-8% of congenital heart disease. Residual hypertension remains the main problem after late correction. Surgical treatment in the adult remains a challenge for the surgeon. Our prefered method used in this category is the Subclavian-aortic bypass., Material and Methods: We have reviewed our registry for the period of 12 years (1998- 2010) and we found a group of 18 adult patients being operated for coarctation of the aorta. The mean age of this group of patients was 24.7 ± 8.43 years (range 16-42 years). 13 were males and 5 females., Results: Sugical technique: Most of the patients (13 pts, 72%) which were obviously treated with subclavian-aortic bypass with a Dacron prostheses. Mean preoperative and postoperative pressure gradients measured by echocardiography were 77.7 ± 20.16 mmHg and 22.3 ± 9.14 mmHg respectively. No mortality was observed in this series of patients. Chylothorax was the only complication observed in one patient in the early postoperative period., Conclusion: Coarctation of the aorta in adults is treated with optimal early results at our surgical centre. Subclavian-aortic bypass grafting requires less aortic dissection, can be performed with a partially occluding clamp, and does not compromise the spinal cord vascularization.

24. Incidence and Impact of Patient-Prosthesis Mismatch in Isolated Aortic Valve Surgery.

Aim: The mains topics of this work are the incidence of patient-prosthesis mismatch and the influence in the early results of isolated aortic valve surgery., Methods: In 193 patients isolated aortic valve surgery was performed. The study population was divided in three subgroups: 20 patients with severe, 131 patients with moderate and 42 patients without patient-prosthesis mismatch. The indexed effective orifice area was used to define the subgroups. Operative mortality and perioperative complications were considered the indicators of the early results of aortic valve surgery., Results: The incidence of severe and moderate patient-prosthesis mismatch was respectively 10.3% and 67.8%. Hospital mortality and perioperative complications were: mortality 5% vs. 3.1% vs. 2.4% (p = 0.855), low cardiac output 5% vs. 6.9% vs. 4.8% (p = 0.861); pulmonary complications 5% vs. 3.1 vs. 0.0% (p = 0.430); exploration for bleeding 5% vs. 0.8% vs. 2.4% (p = 0.319); atrial fibrillation 30% vs. 19.8% vs. 11.9% (p = 0.225); wound infection 5% vs. 0.8% vs. 0.00% (p = 0.165), respectively for the group with severe, moderate and without patient-prosthesis mismatch., Conclusions: Patient-prosthesis mismatch is a common occurrence in aortic valve surgery. This phenomenon does not affect the early results of aortic valve surgery.

Chylopericardium is a rare disorder that may be primary (idiopathic) or secondary to injury of the thoracic duct or thymus gland. Pediatric cardiac operations are more commonly related to this complication because thymus gland is very active in this population and atrophies in the adult patients. We present a case of chylopericardium after mitral valve repair for rheumatic disease, due to thymus gland tributaries injury.

Abstract

Introduction: Cardiac valve calcification (CVC) has long been regarded as a consequence of abnormal calcium-phosphate metabolism in uremic patient associated with increased cardiovascular mortality in this population. We evaluated the association between residual renal function (RRF), phosphate level and valve calcification in peritoneal dialysis (PD) and hemodialysis (HD) patients., Methods: We studied 30 stable PD patients (60 % males; mean age 57 ± 12.36 years) and 34 HD patients (58.8 % males; mean age 50.8 ± 10.4 years) on renal replacement therapy (RRT) from 6 up to 36 months. The presence of CVC was assessed by standard bi-dimensional echocardiography. RRF was calculated by standard technique., Results: Valve calcification was more frequently found in HD compared to PD patients (70.6 vs 29.4 %, p = 0.007). Significantly lower phosphate [1.38 ± 0.41 versus 1.99 ± 0.35 mmol/L (p < 0.0001)], a higher RRF [4.09 ± 2.09 ml/min vs 0.62 ± 0.89 ml/min (p < 0.0001)], and older age [57 ± 12.36 years vs 50.8 ± 10.4 years (p = 0.033)] were observed in PD as compared to HD patients. The logistic regression analysis for the presence of valve calcification when adjusted for age and diabetes, with type of therapy, serum phosphate, RRF, CRP, and serum albumin as variables in the model, revealed significant association between the presence of valve calcification and age and RRF. The correlation between phosphate levels and RRF was even stronger in PD patients than in HD patients (r = -0.704; p = 0.0001) vs (r = -0.502; p = 0.02)., Conclusions: Our study shows that the residual renal function in PD patients contributes significantly to the maintenance of phosphate balance and may explain the lower prevalence of valve calcification in PD patients compared with HD patients in the period up to first 3 years under renal replacement therapy.

Abstract

Lactic acidosis is a rare but severe complication in patients with type 2 diabetes treated with metformin. Patients with lactic acidosis show commonly signs of shock, tissue hypoxia, acute hepatic or renal failure and the link between metformin therapy and lactic acidosis may be coincidental, associated or causal. Excessive plasma metformin concentrations show that lactic acidosis is due to a toxicological mechanism. We report a case of severe multiorganic failure in a subject after treatment with high doses of metformin.

Abstract

Background: Techniques of reduction aortoplasty are widely published in the literature with conflicting results. External support seems to be an important factor in preventing recurrence but, in some cases, this technique caused erosion of the aorta because of the wrinkles the prosthesis creates in the rear side of the aorta., Case Presentation: A 73 year old patient with aortic valve stenosis and borderline dilated ascending aorta had aortic valve replacement and simple wrapping without aortoplasty. To avoid the formation of wrinkles, the dacron external support was tailored appropriately to obtain a curved, custom-made prosthesis. This custom-made prosthesis had the same diameter as the dilated aorta and, after valve replacement, fitted it properly. After 18 months neither computerized axial tomography nor ecocardiography detected wrinkles or dilatation recurrence., Conclusion: A safe, simple and probably new way to prepare an external wrapping is presented, which in this patient respected the shape of the aorta and prevented the formation of wrinkles in the prosthesis and possible complications such as wall erosion.
